Abstract Background: Adverse drug reactions (ADRs) are a significant cause of morbidity, prolonged hospitalization, and increased healthcare expenditure. Tertiary care hospitals, due to the complexity of cases and polypharmacy, are particularly vulnerable to high ADR incidence and its associated economic burden. Objectives: This study aims to evaluate the economic impact of ADRs in a tertiary care hospital by estimating their direct and indirect costs, identifying high-risk drug classes, and assessing the influence of ADRs on length of stay and resource utilization. Methods: A prospective observational study will be conducted in selected departments of a tertiary care hospital over a defined period. Suspected ADRs will be identified using standard causality assessment tools such as the Naranjo scale and WHO-UMC criteria.
