Black pepper (Piper nigrum L.) fruit extract ameliorates erectile dysfunction in alloxan-induced diabetic rats Exsa Hadibrata, Sutyarso Sutyarso, Hendri Busman, Syazili Mustofa, Wawan Abdullah Setiawan, et al. F1000research, 2026 Background Diabetes mellitus (DM) is a chronic metabolic disorder frequently associated with male sexual and reproductive dysfunction, including erectile dysfunction (ED), reduced libido, and impaired spermatogenesis. Black pepper ( Piper nigrum L.) fruit extract has been reported to possess antidiabetic and reproductive benefits, yet its effect on DM-related ED remains underexplored. Objective To evaluate the effects of Piper nigrum fruit extract on erectile function, libido, sperm parameters, and testicular histology in alloxan-induced diabetic male rats. Methods Thirty male Sprague Dawley rats were divided into five groups: normal control, diabetic control, diabetic rats treated with black pepper extract (122.5 or 245 mg/kg BW), and diabetic rats treated with sildenafil citrate. Erectile function was assessed via penile reflexes, libido by mating behavior, sperm quality by concentration, motility, and morphology, and testicular histology by Leydig cell and spermatogonia counts. Data were analyzed using ANOVA with significance at p<0.05. Results Alloxan-induced diabetic rats showed significant impairment in erectile function, libido, sperm quality, and testicular histology (p<0.001 vs control). Black pepper extract at 122.5 mg/kg BW significantly improved total penile reflexes compared with diabetic controls (9.33±1.03 vs 6.00±1.26, p=0.02). Libido parameters including courtship latency (5.50±0.55 vs 21.00±9.47, p=0.013), mount latency (19.00±10.81 vs 37.17±6.31, p=0.009), and mount frequency (18.05±5.99 vs 7.17±1.83, p=0.002) were significantly improved. Sperm analysis revealed increases in sperm concentration (19.2±6.7 vs 12.6±1.3, though not significant, p=0.877), motility (31.8±23 vs 27±30, p=0.697), and normal morphology (40.9±7.8 vs 35±10.8, p=0.04). Testicular histology showed restoration of Leydig cell count (59.33±4.0 vs 30.50±3.86, p=0.035) and spermatogonia number (319.4±64.59 vs 491±37.0, p<0.001). The 245 mg/kg BW dose primarily improved sperm concentration (62.95±29.4 vs 12.6±1.3, p=0.01) and motility (36.6±23 vs 27±30, p=0.436). Sildenafil citrate significantly enhanced most parameters compared with diabetic controls (p<0.05). Conclusion Piper nigrum fruit extract ameliorates sexual dysfunction and reproductive impairment in alloxan-induced diabetic rats, particularly at 122.5 mg/kg BW, with significant improvements in erectile function, libido (p<0.05), sperm quality (p≤0.04), and testicular histology (p=0.035). These findings suggest its potential as a natural therapeutic agent for DM-related male reproductive dysfunction.

F1000research, 2026 Background Diabetes mellitus (DM) is often associated with male sexual and reproductive dysfunction, including erectile impairment, reduced libido, hormonal disturbances, and poor semen quality. Black pepper ( Piper nigrum L.) fruit extract has shown antidiabetic and reproductive benefits in experimental studies, but its role in diabetes-related sexual dysfunction remains unclear. Objective To evaluate the effects of black pepper fruit extract on hormonal, erectile, libido, sperm, histological, and molecular parameters in alloxan-induced diabetic male rats. Methods Thirty male Sprague Dawley rats were assigned to five groups: normal control, diabetic control, diabetic rats treated with black pepper extract at 122.5 or 245 mg/kg body weight (BW) for 8 consecutive days, and diabetic rats treated with sildenafil citrate 1 mg/kg BW. Outcomes included intratesticular testosterone (ITT), erectile parameters (quick flip, long flip, erection, total penile reflexes [TPR]), libido parameters (courtship latency [CL], mount latency [ML], mount frequency [MF]), sperm parameters (concentration, progressive motility, normal morphology), Leydig cell and spermatocyte counts, and eNOS mRNA expression. Data were analyzed using the Kruskal-Wallis test followed by Dunn’s post hoc test. Results Compared with diabetic controls, the 122.5 mg/kg BW black pepper group significantly improved TPR, CL, ML, and MF. The 245 mg/kg BW group significantly improved ITT, sperm concentration, and normal sperm morphology. Both black pepper doses increased Leydig cell counts. Sildenafil significantly improved TPR, sperm concentration, progressive motility, and normal morphology. eNOS mRNA expression was higher in all treatment groups than in diabetic controls. Conclusion Black pepper fruit extract showed dose-dependent effects in alloxan-induced diabetic rats. The lower dose was more beneficial for erectile and libido-related parameters, whereas the higher dose had greater effects on ITT and selected sperm outcomes. These findings support its potential as a preclinical candidate for diabetic male sexual and reproductive dysfunction.
Influence of Rhizophora apiculata barks extract on Cholesterol, Triglyceride, LDL, and HDL Levels of Rattus norvegicus (Sprague Dawley) fed high-cholesterol diet Syazili Mustofa, Andi Nafisah Tendri Adjeng, Evi Kurniawaty, Lovina Ramadhita, Tania Tamara Research Journal of Pharmacy and Technology, 2024 Background:Hypercholesterolemia is a lipid metabolic condition associated with cardiovascular disease (CVD), the leading cause of death worldwide. Rhizophora apiculata is a plant that grows along the Indonesian coast and may provide significant support for using traditional medicine. Bioactive molecules extracted from Rhizophora apiculta can be used in supplementary medicines and considered to be an alternative way to address the ever-increasing requirements of novel drugs. More research is needed to substantiate the usage of Rhizophora apiculate astraditional medicine.This study aims to investigate the effects of Rhizophora apiculata barks (RAB) ethanolic extract on lipid metabolism, namely Total Cholesterol (TC), Triglyceride (TG), LDL, and HDL levels in mice fed a high-cholesterol diet (HCD). Methods: The study comprised 30 white male rats (Rattus norvegicus) of the Sprague Dawley strain, aged 2-3 months and weighing 200-250 grams. Six groups of mice (n = 5) were formed. Group 1 was fed a conventional diet; Group 2 was given the High-Cholesterol Diet (HCD); Group 3 was given HCD and simvastatin 40 mg/kgbwt (p.o. ); and Groups 4, 5, and 6 were treated with HCD and RAB extract 56.55 mg/kgbwt (p.o.), RAB 28.28 mg/kgbwt (p.o.), and RAB 14.14 mg/kgbwt. All of the treatments began on the same day and lasted 30 days. Results: RAB-treated animal groups (56.55; 28.28; and 14.14 mg/kgbwt) had markedly lessened TC levels by 41.18%, 35.57%, and 9.24%, respectively; TG levels declined by 45.85%; 43.77%; and 27.74%, respectively; LDL levels reduced by 26.08%; 16.55%; and 11.21%, respectively; and HDL levels increased by 19.37%; 15.12%; and 13.21%, respectively. Conclusion: RAB-treated animal groups (56.55; 28.28; and 14.14 mg/kg) had markedly lessened total cholesterol, triglyceride, LDL and increased HDL levels (p < 0.01) related to the HCD alone batch. These findings imply that the ethanolic extract of Rhizophora apiculata barks influences lipid metabolism and has potency as traditional medicine.
Decreased plasma epidermal growth factor (EGF) levels in patients with severe chronic obstructive pulmonary disease Retno AS Soemarwoto, Jamsari, Yanwirasti, Andika Chandra Putra, Syazili Mustofa Pneumologia, 2019 Background Chronic mucus hypersecretion is a common feature in chronic obstructive pulmonary disease (COPD) and is associated with epidermal growth factor (EGF) activity. Aberrant EGF and its receptor signalling can cause airway hyperproliferation, increase in mucous cell differentiation and mucus hyperproduction. Furthermore, it can also promote subepithelial fibrosis and excessive collagen deposition in COPD. The objective of this research was to investigate the plasma levels of EGF in smokers with COPD in comparison with clinically healthy smokers. In addition, the relationship between the plasma levels of EGF and clinical features was investigated. Methods A cross-sectional study included 82 clinically stable male patients with mild-to-very severe COPD (mean age: 64.5±8.6 years), and the control group consisted of 86 healthy male smokers (mean age: 61.6±9.5 years). To define COPD, we performed spirometry and classified COPD using Global Initiative for Chronic Obstructive Lung Disease (GOLD) classification. We analyzed the levels of EGF by enzyme-linked immunosorbent assay in plasma. Results The mean serum levels of EGF were significantly lower in smokers with COPD than those in controls (69.30 and 83.82 pg/mL, respectively, p = 0.046). The plasma levels of EGF were significantly different (p = 0.004) between mild COPD and moderate-to-very severe COPD. There were no significant differences between the levels of EGF in plasma of spontaneous sputum producers (COPD patients) vs. nonsputum producers (p = 0.101) and between nonexacerbated COPD and exacerbated COPD patients(p = 0.138). Conclusions There is a significant difference in the plasma levels of EGF in male smokers with COPD as compared with male healthy smokers. Our findings suggest that the plasma levels of EGF may contribute to the pathogenesis of COPD.
