Artificial Intelligence vs. Human Readers in Contrast-Enhanced Harmonic Imaging Endoscopic Ultrasound Interpretation of Solid Pancreatic Masses: A Multicenter Interobserver Study Nicoleta Podină, Lucian Gheorghe Gruionu, Anca Udriștoiu, Elena Codruța Gheorghe, Voicu Rednic, Alina Liliana Constantin, Maria Simona Badiu, Cristian George Țieranu, Nona Bejinariu, Cristina Pojoga, Claudia Hagiu, Andrada Seicean, Adrian Săftoiu Journal of Clinical Medicine, 2026 Background/Objectives: Contrast-enhanced harmonic imaging endoscopic ultrasound (CHI-EUS) is a valuable tool for characterizing solid pancreatic tumors. However, interobserver variability remains a significant limitation in clinical interpretation. Artificial intelligence (AI) may offer objective, reproducible assessments, potentially enhancing diagnostic performance. This study compared the diagnostic accuracy and interobserver agreement of nine physicians with varying CHI-EUS experience levels vs. a dedicated AI system and a general-purpose large language model (ChatGPT) on the same 118 histologically confirmed cases. Methods: We conducted a prospective, multicenter, observer-blinded study involving 118 CHI-EUS video cases of histologically confirmed (EUS-FNB) focal pancreatic masses from three tertiary care centers in Romania. Nine readers were stratified into three groups: trainees ( 10 years). All readers and two AI models received standardized, anonymized 2 min CHI-EUS video clips. A dedicated AI system used a convolutional neural network (CNN) for lesion segmentation and time–intensity curve (TIC) extraction, followed by a feedforward neural network (FNN) for classification. ChatGPT was separately evaluated on the same videos. Diagnostic metrics (accuracy, sensitivity, specificity, positive predictive value [PPV], negative predictive value [NPV], and AUROC) were calculated. Interobserver agreement was assessed using Fleiss’ and Cohen’s kappa statistics. Results: The dedicated AI system achieved an overall accuracy of 95.8% (sensitivity 96.6%; specificity 94.1%) in diagnosing pancreatic adenocarcinoma. Expert readers had a mean accuracy of 78.8% (sensitivity 86%, specificity 61%, and AUROC 0.74), intermediates 80.8% (sensitivity 83%, specificity 75%, and AUROC 0.84), and trainees had a mean accuracy of 67.2% (sensitivity 70%, specificity 60%, and AUROC 0.67). For the most-likely-diagnosis parameter, interobserver agreement was similar between intermediates (Fleiss’ κ = 0.407) and experts (κ = 0.389), while trainees showed lower agreement (κ = 0.203). ChatGPT correctly classified only 14.1% of PDAC cases. Conclusions: A specialized AI model for CHI-EUS video analysis can achieve expert-level performance and reduce diagnostic variability across experience levels. Integration of dedicated AI systems into CHI-EUS interpretation may enhance accuracy and serve as a valuable decision support tool in clinical and training settings.
TNF-alpha Promoter Single-Nucleotide Polymorphisms and Inflammatory Bowel Diseases in Romania: Association with Disease Susceptibility and Clinical Features Cristian George Țieranu, Luis Ovidiu Popa, Ioana Țieranu, Monica Irina Duțescu, Carmen Monica Preda, Andrei Ovidiu Olteanu, Cristian Valentin Toma, Adrian Săftoiu, Olivia Mihaela Popa Journal of Clinical Medicine, 2026 Background/Objectives: Tumor necrosis factor alpha (TNF-alpha) plays a key role in systemic inflammation in multiple disorders, including inflammatory bowel diseases (IBDs). Our purpose was to investigate the contribution of two promoter single-nucleotide polymorphisms (rs361525/–238G/A and rs1800629/–308G/A) to disease susceptibility, clinical features, and response to biologic therapy in a cohort of Romanian patients with IBDs. Methods: A total of 198 patients with IBDs, 106 with Crohn’s disease (CD) and 92 with ulcerative colitis (UC), as well as 160 healthy controls, all Caucasians of Romanian origin, were genotyped using TaqMan Allelic Discrimination Assays. Phenotypical and anti-TNF treatment characteristics of the patients with IBDs were recorded. Statistical analyses were performed using OpenEpi and PLINK v1.07 software. Results: We found a significantly higher frequency of the minor allele A of rs361525 in patients with CD than in the controls (6.6% vs. 2.2%, p = 0.01, OR = 3.16). Half of the patients with extraintestinal manifestations (EIMs) had at least one copy of the rs1800629 A allele compared with approximately 10% of patients without EIM (p = 1 × 10−4, OR 9.58 for UC and p = 9 × 10−4, OR 6.60 for CD). In the whole IBD group of patients, the carriers of the minor allele (AA+GA) for both SNPs studied (rs1800629 and rs361525) were significantly more likely to have EIM associated with IBDs (p = 3 × 10−7, OR 7.87; p = 0.03, OR 3.02, respectively). In patients with UC, the analysis according to disease extension revealed that the frequency of the minor allele of rs1800629 was significantly higher in the subgroup with the E2 phenotype compared to the E1 and E3 phenotypes (16.6% versus 5.6%, p = 0.02, OR 3.32). Conclusions: These findings highlight the role of genetic TNF-alpha variants in disease susceptibility, phenotype, and systemic involvement, supporting their potential relevance in understanding IBD heterogeneity.

Campion, Cesare Hassan, Helmut Messmann, Raf Bisschops, Lorenzo Fuccio, Antonio Facciorusso, Tony Tham, and Endoscopy, 2026 The European Society of Gastrointestinal Endoscopy (ESGE) has identified a critical need to establish structured training for safe and effective use of artificial intelligence (AI) in endoscopy. This manuscript presents the results of a formal Delphi consensus process and outlines the official ESGE position, offering a comprehensive curriculum for acquiring and maintaining the competence needed to exploit the benefit of using AI tools in endoscopy. The proposed framework defines the prerequisites in the preadoption phase, core training components, and requirements to maintain optimal implementation. Key recommendations include: (1) ensuring basic competency in standard endoscopy procedures; (2) acquiring foundational knowledge of AI principles; (3) implementing educational programs to enhance AI literacy; (4) recognizing and mitigating cognitive biases in human–AI interaction; (5) avoiding over-reliance on AI in clinical decision-making; and (6) continuous monitoring of key performance indicators throughout AI system integration.
Clinical impact of clonal hematopoiesis on patients with solid tumors: a systematic review and meta-analysis Vlad M. Croitoru-Cazacu Frontiers in Oncology, 2026 Introduction Clonal hematopoiesis (CH) is a common age-related phenomenon associated with an increased risk of hematologic malignancies and cardiovascular disease. Its prognostic significance in patients with solid tumors remains unclear. The aim of this meta-analysis was to evaluate the association between CH and clinical outcomes, including overall survival (OS), progression-free survival (PFS), cardiovascular events, and all-cause mortality in patients with solid tumors. Methods We conducted a meta-analysis according to PRISMA guidelines, including 21 studies comprising 4845 patients with CH and 63557 patients without CH. Hazard ratios (HRs) and odds ratios (ORs) with 95% confidence intervals were pooled using random-effects or fixed-effects models as appropriate, based on assessments of between-study heterogeneity. Results CH was not significantly associated with OS in patients with solid tumors (HR: 1.10, 95% CI: 0.92–1.32, p = 0.30). The pooled analysis using a random-effects model suggested a trend toward improved PFS in patients with CH compared with those without CH, although statistical significance was not reached (HR: 0.83, 95% CI: 0.67–1.02, p = 0.08). However, CH was associated with an increased mortality in patients with solid tumors, with nearly a twofold higher risk compared to those without CH (OR = 1.70, 95% CI: 1.34-2.16, p < 0.00001). Furthermore, CH was significantly associated with an increased risk of cardiovascular events (OR = 2.75, 95% CI: 1.38 to 5.47, p = 0.004). Conclusion Our meta-analysis indicated that CH mutations have a prognostic value and are associated with a clinically meaningful increased risk of overall mortality and cardiovascular events in patients with solid tumors.
Clinicopathologic relevance of EpCAM and CD44 in pancreatic cancer: insights from a meta-analysis Bogdan Silviu Ungureanu, Dan Ionut Gheonea, Adina Turcu-Stiolica, Michael Schenker, Daniel Pirici, Cristin-Constantin Vere, Andrei Fierut, Adrian Saftoiu Stem Cell Research and Therapy, 2025 Recent evidence suggests that EpCAM and CD44 could serve as diagnosis or prognosis markers in pancreatic cancer (PC). In this meta-analysis, we evaluated their associations with clinicopathologic features. Specifically, we compared immunohistochemical-positive and -negative PC patients for T stage (T3-T4 vs. T1-T2), N stage (N1 vs. N0), M stage (M1 vs. M0), tumor grade (well/moderately vs. poorly differentiated), UICC Stage (III, IV vs. I, II), and overall survival (OS). The diagnostic meta-analysis was performed analysing the pooled sensitivity and specificity and evaluating overall accuracy to indicate the diagnostic efficacy of the markers. The protocol of this systematic review and meta-analysis was registered on the PROSPERO website under the registration number of CRD42024568390. A systematic search of PubMed, Scopus, and ISI Web of Science was conducted on January 30th, 2025. The statistical analysis was performed using the Review Manager 5.4 software and R language (R package Mada and Metafor). The quality of the studies included was assessed using the Newcastle-Ottawa scale and the QUADAS-2 tool. Data from relevant studies were independently screened and extracted using Rayyan, by at least two authors. A total of 19 studies were eligible (9 studies for EpCAM, 9 studies for CD44, and 2 studies for both EpCAM and CD44), comprising a total of 1370 patients. The diagnostic meta-analysis demonstrated moderate accuracy for EpCAM (AUC, 95% CI of 0.802, 0.69-0.96). A statistically significant association was found for CD44 expression and T-status (OR = 2.04, 95%CI = 1.18-3.51), or N-stage (OR = 2.68, 95%CI = 1.86-3.85), or TNM stage (OR = 3.79, 95%CI = 2.14-6.71). CD44v6 overexpression predicted worse OS (HR = 2.33, p < 0.00001), while EpCAM + CD44 + co-expression was prognostic (HR = 2.02, p = 0.02). Heterogeneity was not observed among the studies included, but further research is warranted to better understand the clinical implications of these markers' positivity in PC diagnosis and prognosis.
Red Cell Distribution Width at 24 Hours as an Early Predictor of Mortality and Severity in Acute Pancreatitis: A Multicenter Retrospective Cohort Study Alina Florentina Pistrițu, Mihai Radu Pahomeanu, Hosein Nayyerani, Horațiu Herdeș, Mihaela Bîrligea, Andreea Daniela Bota, Cristian Valentin Toma, Carmen Monica Preda, Cristian George Țieranu, Elena Mirela Ionescu, Radu Bogdan Mateescu, Adrian Săftoiu, Lucian Negreanu Journal of Gastrointestinal and Liver Diseases, 2025 Background and Aims: Acute pancreatitis (AP) presents with variable severity, and early identification of patients at risk for poor outcomes remains challenging. Red cell distribution width (RDW) is a widely available marker with potential prognostic value. Methods: We analyzed 682 patients from the RO-API/BUC-API registry admitted with AP between 2015 and 2023. RDW measured at 24±6 hours post-admission was assessed for associations with severity (per Revised Atlanta Classification) and in-hospital mortality. ROC analysis and logistic regression were applied. Results: Median RDW was significantly higher in non-survivors and patients with severe disease. RDW ≥13.85% predicted mortality [area under the curve (AUC)=0.67], and ≥14.35% predicted severe AP (AUC=0.67). RDW remained an independent predictor after adjusting for potential confounders. Conclusions: RDW at 24 hours could be a useful early biomarker for predicting severity and mortality in AP.
Deep learning segmentation architectures for automatic detection of pancreatic ductal adenocarcinoma in EUS-guided fine-needle biopsy samples based on whole-slide imaging Anca Loredana Udriștoiu, Nicoleta Podină, Bogdan Silviu Ungureanu, Alina Constantin, Claudia Valentina Georgescu, Nona Bejinariu, Daniel Pirici, Daniela Elena Burtea, Lucian Gruionu, Stefan Udriștoiu, Adrian Săftoiu Endoscopic Ultrasound, 2025 Background EUS-guided fine-needle biopsy is the procedure of choice for the diagnosis of pancreatic ductal adenocarcinoma (PDAC). Nevertheless, the samples obtained are small and require expertise in pathology, whereas the diagnosis is difficult in view of the scarcity of malignant cells and the important desmoplastic reaction of these tumors. With the help of artificial intelligence, the deep learning architectures produce a fast, accurate, and automated approach for PDAC image segmentation based on whole-slide imaging. Given the effectiveness of U-Net in semantic segmentation, numerous variants and improvements have emerged, specifically for whole-slide imaging segmentation. Methods In this study, a comparison of 7 U-Net architecture variants was performed on 2 different datasets of EUS-guided fine-needle biopsy samples from 2 medical centers (31 and 33 whole-slide images, respectively) with different parameters and acquisition tools. The U-Net architecture variants evaluated included some that had not been previously explored for PDAC whole-slide image segmentation. The evaluation of their performance involved calculating accuracy through the mean Dice coefficient and mean intersection over union (IoU). Results The highest segmentation accuracies were obtained using Inception U-Net architecture for both datasets. PDAC tissue was segmented with the overall average Dice coefficient of 97.82% and IoU of 0.87 for Dataset 1, respectively, overall average Dice coefficient of 95.70%, and IoU of 0.79 for Dataset 2. Also, we considered the external testing of the trained segmentation models by performing the cross evaluations between the 2 datasets. The Inception U-Net model trained on Train Dataset 1 performed with the overall average Dice coefficient of 93.12% and IoU of 0.74 on Test Dataset 2. The Inception U-Net model trained on Train Dataset 2 performed with the overall average Dice coefficient of 92.09% and IoU of 0.81 on Test Dataset 1. Conclusions The findings of this study demonstrated the feasibility of utilizing artificial intelligence for assessing PDAC segmentation in whole-slide imaging, supported by promising scores.
Technical Feasibility of Microwave Ablation in Pancreatic Tumors: A Scoping Review of Procedural Efficacy and Safety Daniela Tabacelia, Carlos Robles-Medranda, Artsiom Klimko, Stephen P. Voermans, Adrian Săftoiu, Cristian George Tieranu, Cezar Stroescu Cancers, 2025 Background/Objectives: Pancreatic cancer remains one of the most aggressive and lethal malignancies, with limited effective treatment options for advanced stages. Microwave Ablation (MWA) has emerged as a minimally invasive therapeutic modality, offering potential benefits in tumor control. This review aims to critically assess the safety and efficacy of MWA in the treatment of pancreatic cancer, focusing on its application in various pancreatic lesions. Methods: We systematically reviewed studies published between 2010 and 2023 that evaluated the use of MWA in pancreatic tumors, including locally advanced pancreatic cancer (LAPC), pancreatic neuroendocrine tumors (PNETs), and pancreatic metastases from renal cell carcinoma (RCC). Due to limited data on survival rates and long-term outcomes, our analysis concentrated primarily on the technical aspects and immediate procedural outcomes of MWA. Results: MWA was technically feasible in all cases. The overall complication rate was approximately 16.7% (nine patients), with higher incidences in tumors located in the pancreatic head. Reported complications included pancreatitis and pseudocyst formation. Procedural parameters varied, with applied energy ranging from 20 to 80 watts and ablation times between 2 to 15 min, depending on the microwave generator type and approach (percutaneous, intraoperative or endoscopic). All cases demonstrated effective necrosis of the target tissue, and several studies reported notable tumor size reductions, averaging up to 70%. Conclusions: MWA shows promise as a therapeutic option for pancreatic cancer, achieving high technical success rates and significant tumor reductions. However, the procedure is associated with a moderate complication rate, particularly in tumors located in the pancreatic head.
