  • Influence of Phosphatidyl Inositol (4,5) Bisphosphate,5-Phosphatase (PIP2), in Patients With Kidney Stones and Renal Failure

    Phosphatidylinositol 4,5-bisphosphate (PIP2) is an essential lipid involved in metabolic processes. It is integral to the cell membrane of all animal and plant cells and acts as a second messenger in various signaling pathways. Still, its hydrolysis can lead to increased calcium levels and kidney stones. The study aims to investigate the role of PIP2 enzyme levels in patients with renal failure and kidney stones. The enzyme-produced amount was measured by enzyme-linked immunosorbent assay (ELISA). Peripheral whole-blood samples were collected in gel tubes from eighty patients (40 with kidney stones and 40 with renal failure), and 40 were healthy individuals. The results of urea, creatinine, and uric acid levels were significantly different between renal failure patients (140.35±41.45, 8.70±3.22, 8.43±1.47) and the control group (27.07±7.21, 0.70±0.26, 4.53±0.94), with a p-value of 0.001. Uric acid levels in kidney stone patients were also significant (6.30±1.39) with a p-value of 0.001. The findings revealed significant differences in PIP2 levels among patients with kidney stones (6.63±1.40) and renal failure (9.13±1.76) compared to the control group (1.87±0.94), with p-values of 0.001 and 0.02, respectively. Serum Ca²⁺ levels also showed significant differences (9.55±1.63, 8.19±1.11) compared to the control group (9.47±0.28) with p-values of 0.94 and 0.001, respectively. To confirm the results of the p-value. Effect Size (Cohen's d) was conducted, and confidence intervals (95% CI) were calculated for all parameters, and the results were consistent with the p-value. This study demonstrated that PIP2 and vitamin D3 play distinct roles in Ca2+ homeostasis, underscoring their potential role in disease pathology.
  • The Polymorphism of OCRL Gene in Kidney Stones and Kidney Failure Patients

    Kidney disease is a kidney injury or disease that affects many people globally. The study aims to evaluate the effect of Phosphatidyl Inositol (4,5) Bisphosphate, 5-Phosphatase (PIP2) enzyme on calcium (Ca2+) levels and its relationship to oculocerebrorenal gene variations in kidney stone and kidney failure patients. The enzyme-produced amount was measured by enzyme-linked immunosorbent assay (ELISA). Genetic variations were studied through the polymerase chain reaction (PCR) technique, followed by the sequencing of fragments of the exons (9, 13, 15). Blood samples were collected from eighty patients (40 with kidney stones and 40 with kidney failure), and 40 were healthy individuals. Results showed a significant difference between the levels of PIP2 in patients with kidney stones (6.63±0.22) and kidney failure patients (9.13±0.27), at a p-value of 0.004, compared with the control group (1.87±0.14), at a p-value of 0.02. Also, the differences in the serum Ca2+ levels were highly significant differences (9.55±0.25, 8.19±0.17) and the control group (9.46±0.04) at p-values of 0.004 and 0.001, respectively. In addition to the results of urea, creatinine, and uric acid, there were highly significant differences between kidney failure 140.35±6.55, 8.69±0.50, and 8.42±0.23 versus the control group 27.07±1.13, 0.69±0.04, and 4.53±0.14, respectively. The p-value was 0.001. In patients with kidney stones, the uric acid level was significant at 6.30±0.21, with a p-value of 0.001. Sequencing revealed a variant A/G in exon 15 in a female patient suffering from kidney failure and two intronic substitutions (c.1359+262G>A) and (c.1359+251G>C) before exon 13 in a 55-year-old male patient with kidney failure.
  • Potential Role of miR-484 in Modulating VDR Gene Expression Level in Calcium Oxalate Kidney Stone Patients

    Calcium oxalate (CaOx) kidney stones, accounting for over 70% of all types, are the most common stone type. MicroRNAs (miRNAs) are post-transcriptional epigenetic regulators, involved in disease development. One of these miRNAs is miRNA 484 (miR-484). Vitamin D (VD), a fat-soluble vitamin important for calcium (Ca) metabolism, is primarily mediated by the Vitamin D Receptor (VDR), a nuclear transcription factor. This study aimed to investigate miR-484's regulatory role in modulating VDR gene expression, evaluate serum VD and biochemical parameters in CaOx kidney stone patients compared to healthy controls, and their potential as diagnostic biomarkers. Blood samples were collected from 50 CaOx patients and 40 controls. Serum VD, Ca, urea, and creatinine levels were measured. To assess miR-484 and VDR gene expression, absolute quantification via RT-qPCR was performed. Comparisons, correlations, and ROC curve analyses were performed. The results showed that patients exhibited non-significantly higher serum Ca, urea, and creatinine levels than controls. While VD levels were significantly lower in patients compared to controls. Gene expression analysis revealed a significant miR-484 downregulation and VDR upregulation in patients. Moderate negative correlation was observed between miR-484 and VDR expression in patients. ROC analysis showed that VD, VDR, and miR-484 had good diagnostic accuracy. In conclusion, the study reveals that the downregulation of miR-484 in CaOx patients may lead to overexpression of the VDR, potentially altering VD metabolism and leading to VD deficiency, which can disrupt Ca handling in bone and kidneys, contributing to hypercalciuria, one of the primary risk factors for CaOx stone formation.
  • The Impact of VDR-FokI Polymorphism in Iraqi Patients with Prostate Cancer and Prostate Benign Hyperplasia

    استخدم تعدد الطرز الوراثية لمورث مستقبل فيتامين د عند الموقع FokI لتقييم تاثيرتعدد الطرزالرواثية على مستويات فيتامين د وهرمون الذكورة وهرمون الحليب في امصال مرضى سرطان البروستات وتضخم البروستات الحميد مقارنة بالأفراد الأصحاء. تم تضخيم موقع الحصر FOKI لمورث مستقبل فيتامين د باستخدام تقنية TaqMan RT-PCR وجد أن الطراز الوراثيTT له تأثير حماية من الاصابة بسرطان البروستات وتضخم البروستات الحميد بنسبة 70% و50 % على التوالي، في حين كان الطراز الوراثي CC مرتبطا 100% بكل من سرطان البروستات و تضخم البروستات الحميد و لم يكن توزيع الطرز الوراثية TT و TC و CC متسقًا مع معادلة هاردي واينبرغ في مرضى سرطان البروستات حيث ظهر فرق معنوي بين القيم الملاحظة والمتوقعة باختبارمربع كاي عند مستوى معنوية P ≥0.05. ، و لم تظهر هذه الاختلافات في المرضى الذين يعانون من تضخم البروستات الحميد أو مجموعة السيطرة. بينم اتأثر مستوى فيتامين د بالطراز الوراثي CC لـمستقبل فيتامين د - FOKI بشكل ملحوظ في مرضى سرطان البروستات مقارنة بمستوياته في الطرز الوراثية TT وTC . ولم يكن هناك اختلاف في مستوى فيتامين د بين الطرز الوراثية الثلاثة في مرضى BPH ومجموعة السيطرة الاصحاء. لم تظهر الطرز الوراثية تأثيرا على مستويات هرموني الذكورة والحليب بين المجموعات المدروسة. ويمكن الاستنتاج أن تأثيرتعدد الطرز الوراثية لمستقبل فيتامين د- FOKI مرتبط بمرضى سرطان البروستات العراقيين أكثر من تضخم البروستات الحميد مع نقص فيتامين د في مصل الدم.
  • GRHPR gene variations in Iraqi patients infected with calcium oxalate kidney stones

    حصى الكلى هو مرض شديد الخطورة له تهديد خطير لكل من الصحة والاقتصاد العالميين. توجد عدة أنواع مختلفة من الحصى، ولكن حصوات أكسالات الكالسيوم هي الأكثر انتشارًا في العراق. لم يتم دراسة التغيرات في تركيزات اختزال الجليوكسيلات واختزال هيدروكسي البيروفات في المصل والتغيرات الجينية المرتبطة بحصوات أكسالات الكالسيوم في المرضى العراقيين سابقًا لذا تهدف هذه الدراسة إلى التركيز على هذه النقاط. اشتملت هذه الدراسة على 80 شخص, كانو 50 مريضاً مصابا بحصى أكسالات الكالسيوم مقارنة بـ 30 لمجموعة السيطرة. تم اجراء بعض الاختبارات البيوكيميائية لوظائف الكلى (الكرياتينين واليوريا وحمض البوليك) في مصل كلا المجموعتين، فضلا عن تحاليل تعداد الدم الكامل، وسكر الدم العشوائي، واختبارات فصائل الدم. كما تم جمع البول من أجل الفحص العام للبول، لتأكيد رؤية بلورات الأكسالات في بول المريض. بالإضافة إلى قياس تركيز إنزيم اختزال الجليوكسيلات واختزال هيدروكسي البيروفات في المصل كلا المجموعتين بطريقة الايلايزا. و تم استخلاص الدنا من الدم الكامل وتم التحري عن الطفرات الممرضة c.295C>T (rs119490108) وc.165G>A (rs180177314) و rs180177322في مورث GRHPRعن طريق تضخيم الحمض النووي المستهدف عن طريق تفاعل البلمرة المتسلسل وتحري التعاقبات المباشر للمنتجات ثم تم تحليل النتائج. وجدت هذه الدراسة أن تركيز الإنزيم في مجموعة السيطرة (4.78 ± 1.06 مجم / ديسيلتر) كان أعلى بكثير من تركيزه في مجموعة المرضى (0.411 ± 0.02 مجم / ديسيلتر). لم تظهر الطفرات الممرضة في كلا المجموعتين، ولكن وجد بان عدة مواقع ضمن القطع المدروسة كانت ذات تعدد بالأشكال: في اكسون 4: rs2768659 (A>G) و rs1294628807 (G>A)و rs2736664 (C>A)و اكسون 6: c.494-68A>G rs309459 (A>G)و rs309458 (A>G) واكسون 9 : .rs1057507 (A>G) و كأستنتاج وجد بان تكوين حصى اوكزالات الكالسيوم ارتبط بانخفاض انزيم اختزال الجليوكسيلات واختزال هيدروكسي البيروفات في المرضى مقارنة بمجموعة السيطرة والتي ربما حدثت بسبب الطفرات او الوراثة الفوقية الكابحة للتعبير الجيني.

    Polycystic syndrome (PCOS) is a considerable infertility disorder in adolescents and adult women in reproductive age. Obesity is a vigorous risk factor related to POCS. This study aims to evaluate the association of obesity and PCOS by investigating several parameters including: anthropological, biochemical (lipid profile, fasting blood sugar, glucose tolerance test, and hormone levels (LH, FSH, LH/FSH ratio, Estradiol2 and Testosterone),and genetic parameters (Fat mass and Obesity associated gene (FTO) polymorphism at rs17817449) in 63 obese and non-obese PCOS women. The biochemical tests were investigated by colorimetric methods while FTO gene polymorphism was detected by PCR–RFLP. Lipid profile, FBS, GTT, hormones (LH, LH/FSH ratio) in obese PCOS patients were significantly higher than non-obese non PCOS patients. It was found that the FTO variant TT risk genotype is a predisposing factor to obesity but not for PCOS. The study substantiated a possible familial risk factor for developing obesity among women in the same family.
