Digital patient education and its role in overcoming dental anxiety and barriers to endodontic care Iuliana Sofian-Pauliuc, Antonio Castaño-Séiquer, David Ribas-Perez, Ignacio Barbero-Navarro Saudi Dental Journal, 2026 Dental fear and anxiety (DFA) serve as a significant obstacle to oral healthcare, affecting approximately 15% of the adult population. In endodontics, misconceptions regarding procedural pain and complexity often lead to treatment avoidance or a preference for extraction over tooth-preserving therapy. Currently, there remains a notable information gap regarding the efficacy of modern educational modalities in mitigating these barriers. The purpose of this scoping review is to map the existing research on DFA, socioeconomic barriers to endodontic care, and the relative effectiveness of new educational tools, particularly multimedia and Artifical Intelligence (AI) -driven interventions, in raising patient awareness and lowering DFA. Following the Arksey & O'Malley guidelines and PRISMA-ScR (Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews), a systematic search was carried out for the years 2015-2025 using PubMed/MEDLINE, ScienceDirect, Cochrane, and Wiley. Two reviewers conducted independent data charting and study selection. Joanna Briggs Institute (JBI) critical assessment techniques were used to evaluate methodological quality. Notably, there was no pre-registered review protocol, which is recognised as a methodological limitation. Ten studies met the inclusion criteria, representing diverse geographic cohorts, including Saudi Arabia, India, Pakistan, Brazil, and Turkey. The findings indicate that previous negative dental experiences, fear of pain and limited knowledge regarding root canal treatment are major contributors to DFA. Educational interventions, particularly video-based and animated materials, were associated with improved patient understanding and reduced anxiety levels. Recent studies also suggest that AI-generated educational content may enhance clarity and trust while reducing patient anxiety, although further validation is required. Socioeconomic factors, including treatment cost and access to specialised care, were also identified as important determinants influencing treatment decisions. Emerging digital and AI-driven educational tools show promise in reducing DFA but require further expert validation and ethical oversight before widespread implementation to avoid misinformation and ensure clinical safety. Future research should prioritise longitudinal and multi-centre studies to evaluate the long-term impact of digital education on patient decision-making and treatment outcomes.
Current practices and perceived effectiveness of alternative behavioral management techniques for pediatric dental anxiety: a cross-sectional survey of dentists in Spain Daniela del Toro-Alcántara, Marcela Arenas González, Ángel Luis Formoso Veloso, Carolina Caleza Jiménez, María José Barra Soto, María Biedma Perea, David Ribas-Perez Frontiers in Dental Medicine, 2026 Objectives Fear of the dentist and associated anxiety in pediatric patients can negatively impact dental care and oral health. Traditional behavioral management techniques, such as physical restraint or sedation, are increasingly debated and rejected. The main objective of this study was to analyze the level of knowledge, use, and perceived effectiveness of alternative behavioral management techniques among practicing dentists in Spain. Materials and methods A cross-sectional study was conducted with 144 dental practitioners from Seville, Spain, predominantly female (77.8%) and aged 25–35 years (44.4%). Participants represented various specialties, with general dentistry being the most common (51.4%). Data were collected regarding the frequency of use, knowledge, and perceived effectiveness of behavioral management techniques for pediatric patients. Results Across all specialties, the most frequently reported behavioral management technique was tell-show-do, particularly among general dentists, orthodontists, and pediatric dentists. Endodontists primarily favored positive reinforcement, whereas oral surgeons/implantologists reported higher use of pharmacological management. Use of physical restraint was minimal and limited to pediatric dentistry. Conclusions The study highlights the need to enhance training in emotional management and alternative behavioral management techniques to prevent future trauma and reduce the likelihood that children will avoid dental care due to anxiety. Additionally, adapting the clinical environment to be more child-friendly is important to support effective behavioral management. Clinical relevance Improving dentists' knowledge and use of alternative behavioral management strategies can reduce pediatric dental anxiety, improve patient cooperation, and promote long-term oral health by encouraging regular dental visits.
Regenerative Endodontic Treatment in Permanent Incisors: Two Case Reports with 6 Years of Follow-Up María Biedma-Perea, Marcela Arenas-González, María José Barra-Soto, Carolina Caleza-Jiménez, David Ribas-Pérez Children, 2026 Background: Regenerative endodontic treatment (RET) has emerged as a biologically based alternative to traditional apexification for managing immature permanent teeth with pulp necrosis. By promoting tissue ingrowth and continued root development, RET aims not only to eliminate infection but also to reinforce structurally compromised roots. Although its clinical use has expanded, evidence regarding the long-term predictability and durability of RET remains limited, as most published studies provide only short- or mid-term follow-up. Case presentation: This report describes two pediatric cases involving regenerative procedures performed on three immature permanent maxillary incisors, each followed for more than six years. The first case involved a 7-year-old girl who developed pulp necrosis in a maxillary lateral incisor after acute dental trauma. Management followed a regenerative protocol using triple antibiotic paste (ciprofloxacin, metronidazole, and minocycline) as intracanal medication and mineral trioxide aggregate as the coronal barrier. The second case concerned an 8-year-old girl presenting with chronic infection and sinus tracts affecting both maxillary central incisors. These teeth were treated using a regenerative approach with calcium hydroxide as the intracanal medicament and Biodentine as the sealing material. Clinical, radiographic, and cone beam computed tomography evaluations demonstrated complete symptom resolution and periapical healing but incomplete progressive apical closure. All treated teeth developed a calcified apical barrier, and outcomes remained stable throughout the extended follow-up period. Conclusions: While inherently limited by the nature of case reports, these findings support RET as a reliable and durable therapeutic option for necrotic immature permanent teeth, including cases in which conventional apexification has not been successful.
Dental health as a determinant of operational readiness in military populations: Evidence from Ceuta (Spain) Alejandro Bel-Blesa, Marta Hernández-Donadeu, Javier Flores-Fraile, Juan Gómez-Salgado, Luis El Khoury-Moreno, Julio Torrejón-Martínez, Eva Rosel-Gallardo, David Ribas-Pérez, Antonio Castaño-Séiquer Medicine United States, 2025 The Spanish Armed Forces require optimal oral health to ensure operability during international missions. In a cross-sectional study with a sample of 691 military personnel, 17.66% were classified as temporarily unfit (NAT), predominantly among enlisted ranks (21.7%). Employment status and smoking showed significant associations with NAT ( P < .000), while gender, age, and alcohol consumption were not significant factors. The decayed, missing filled teeth index, particularly its decayed component, was strongly related to NAT ( P = .000). Periodontal disease presence and the need for multiple dental treatments significantly increased NAT prevalence ( P < .000). Use of dental floss correlated with higher fitness, whereas mouthwash use was associated with increased NAT ( P < .05). These findings highlight the importance of prevention and risk factor control to maintain oral fitness and ensure military personnel effectiveness during deployments.
Impact of dental aesthetics on self-esteem in students at the Polígono Sur education permanent center in Seville, Spain Adriana Dayaneira Fernández-Cevallos, David Ribas-Perez, Marcela Arenas-González, Luis Elkhoury-Moreno, Julio Torrejón-Martínez, Eva Rosel-Gallardo, Antonio Luis Castaño-Seiquer Scientific Reports, 2025 Self-esteem is a stable characteristic that is usually maintained from adolescence to adulthood. People with high self-esteem are mentally prepared to face different challenges in their lives with great confidence. Assess the influence of dental aesthetics on self-esteem in students at the Polígono Sur Education Permanent Center. A cross-sectional descriptive study with qualitative and quantitative variables in which 92 participants from the permanent education center located in the polígono sur of the city of Seville. To identify if there was a relationship between self-perception of dental aesthetics and self-esteem, two surveys were conducted in the educational center, one of them to determine a high or low psychosocial impact of dental aesthetics and the Rosenberg self-esteem scale, which evaluated the level of acceptance and respect that participants had for themselves. Demographic variables analyzed were gender, age, nationality, and occupation. To analyze categorical variables, contingency tables and the Pearson Chi-square test were used. Students did not have a low self-esteem in relation to a high psychosocial impact of dental aesthetics p > 0, 05 (0.069), but a high self-esteem indifferent to whether they had a high or low psychosocial impact of dental aesthetics. However, there were exceptions according to gender (p = 0.019) or nationality (p = 0.030). Self-esteem was not greatly affected by the psychosocial impact of dental aesthetics in the majority of students evaluated, but it was possible to state that there was a negative impact in the male gender and also by nationality.
Modified CAMBRA Protocol for Caries Risk Assessment in Children Aged 6 to 14 Years Esther Pérez de Mora, Ángel Luis Formoso-Veloso, Marcela Arenas-González, Asunción Mendoza-Mendoza, David Ribas-Pérez Dentistry Journal, 2025 Background/Objectives: This study aimed to propose a modified CAMBRA protocol for caries risk analysis in a pediatric population, adapted to their needs and habits. Methods: A descriptive and analytical observational study was conducted after obtaining approval from the Ethics Committee. Caries risk was determined using the CAMBRA Questionnaire and the modified University of Seville Questionnaire (CAMBRA-OP Questionnaire). Results: The final study sample consisted of 300 patients aged 6 to 14 years, of whom 54% were boys and 46% were girls. The distribution of caries risk according to CAMBRA was as follows: 33% low, 7% moderate, 48.6% high, and 11.3% extreme. The distribution of caries risk according to the CAMBRA-OP was as follows: 27.7% low, 12.3% moderate, 48.7% high, and 11.3% extreme. Correlating both tests, a statistically significant association was observed between the caries risk determined by the CAMBRA and CAMBRA-OP, observing a strong correlation between both systems (p = 0.001; Cramer’s V = 0.826). The two diagnostic models exhibited high concordance (0.815 [p = 0.001]) using Cohen’s Kappa index. Conclusions: This tool is focused on this age group and is easily interpretable by professionals, thus contributing to more effective and personalized prevention of dental caries in childhood.
Oral Health Status Among Children and Adolescents from Vulnerable Populations: A Cross-Sectional Study in Seville, Spain Rodolfo Esteban Reyes-Lara, Adrián Curto, David Ribas-Perez, Ignacio Barbero-Navarro, Diego Rodriguez-Menacho, Javier Flores-Fraile, Antonio Castaño-Séiquer Dentistry Journal, 2025 Background/Objectives: The aim of this study was to evaluate the oral health status of socially vulnerable children and adolescents in Seville and to examine its associations with behavioral and sociodemographic determinants. We hypothesized that greater social vulnerability and suboptimal behaviors would be associated with higher caries experience. Methods: A cross-sectional analytical study was conducted on 250 participants aged 2 to 17 years attending the Luis Séiquer Social Dentistry Foundation between January and March 2025. Clinical examinations followed the WHO Oral Health Surveys: Basic Methods (5th edition, 2013) and were performed by a calibrated dentist. Variables included dental caries indices (dft, DMFT), pulpal treatment needs, and oral hygiene practices. Statistical analyses included Kruskal–Wallis, Mann–Whitney U, Fisher’s exact tests, and multivariate regression models (significance level p < 0.05). Results: Caries prevalence was high across all age groups, particularly in primary dentition (mean dft = 3.05 ± 3.80; DMFT = 2.99 ± 3.66; p < 0.001). Pulp therapy needs were significantly higher among preschoolers (mean = 2.22 ± 2.31). Factors such as low parental education, migrant background, insufficient toothbrushing frequency, and frequent sugar intake were strongly associated with poorer oral health outcomes. Although fluoridated toothpaste use was widespread (>94%), dental floss use remained limited (34.8%). Multivariate analyses confirmed a strong association between social inequalities and oral disease burden. Conclusions: Children and adolescents from vulnerable groups in Seville experience a high prevalence of dental caries and substantial unmet treatment needs. Findings highlight marked oral health disparities linked to socioeconomic status, emphasizing the urgent need for early preventive programs, culturally adapted oral health education, and equitable access to dental care.
MIH and Cavities as Markers of Oral Health Inequality in Children from Southwest Andalusia (Spain) Leidy Bech Barcaz, David Ribas-Pérez, Paloma Villalva Hernandez-Franch, Luis El Khoury-Moreno, Julio Torrejón-Martínez, Antonio Castaño-Séiquer Dentistry Journal, 2025 Introduction: Dental caries and molar–incisor hypomineralisation (MIH) are prevalent conditions affecting children’s oral health, with functional, aesthetic, and psychosocial implications. In Spain, previous studies have highlighted geographic and sociodemographic disparities in their distribution, particularly among rural and migrant populations. Objective: To characterise oral health status, in terms of caries and MIH, among 6–7-year-old children from the towns of Palos de la Frontera, Mazagón, and San Bartolomé. Methods: A cross-sectional study was conducted involving 229 children recruited from public primary schools. Sociodemographic, anthropometric, and behavioural data were collected through clinical examination and interview. Statistical analysis included univariate and multivariate logistic regression. The study protocol was approved by the Ethics Committee of Huelva. Results: The prevalence of caries (DMFT ≥ 1) was 53.3%, with mean DMFT and dft indices of 1.78 and 0.31, respectively. MIH affected 32.8% of the cohort, with a predominance in the first permanent molars (teeth 36 and 26). Multivariate analysis identified independent predictors of caries: African (OR = 7.47; 95% CI: 2.84–23.8) and European (OR = 4.56; 95% CI: 1.26–22.3) parental origin, poor oral hygiene (OR = 3.07; 95% CI: 1.60–6.03), and the presence of MIH (OR = 3.20; 95% CI: 1.64–6.42). The municipality of San Bartolomé was associated with a higher risk of MIH (OR = 2.90; 95% CI: 1.21–7.45). Conclusions: The high prevalence of caries and MIH in the Condado-Campiña district, exceeding national averages, reflects oral health inequities linked to social determinants (migrant origin, locality) and clinical factors (MIH, oral hygiene). Targeted preventive interventions are urgently needed in high-risk populations, including culturally tailored education and policies ensuring equitable access to dental care services.
Survival Evaluation of Restorations in Pulpotomized Primary Teeth with MTA or Biodentine®: A Systematic Review María Biedma-Perea, María Moscoso-Sánchez, María José Barra-Soto, Marcela Arenas-González, David Ribas-Pérez, Carolina Caleza-Jiménez Journal of Clinical Medicine, 2025 Objective: Due to the increasing esthetic demand among pediatric patients and different restorative materials, we focused on analyzing which of the options of restorations may provide superior clinical outcomes. Methods: A systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) using PubMed and Cochrane databases. Results: Five articles met all inclusion criteria from an initial pool of 359 articles identified in the initial search. Greater bond strength was observed when pulpotomized teeth were restored with Biodentine® and resin composites compared to resin-modified glass ionomer cements (RMGICs). When comparing pulpotomies in primary teeth with MTA and Biodentine®, the fracture resistance values were higher in the pulpotomized teeth with Biodentine® than with MTA. Additionally, following a narrative synthesis in MTA-treated teeth, a higher risk of failure was observed using RMGICs or composite instead of stainless-steel crowns (SCCs) as the final restorative material. Conclusions: Variables such as the type of final restoration can affect the survival of primary teeth after pulpotomy reconstruction. Regardless of the pulp material, survival with SSCs is higher, but resin composites appear to be a viable restorative material after Biodentine® application.
Impact of Parental Knowledge on Prevention Risk of Caries in Seville Children Between 6 and 14 Years Old, Applying the CAMBRA Protocol Esther Pérez de Mora, José María Barrera-Mora, Marcela Arenas-González, Asunción Mendoza-Mendoza, David Ribas-Pérez Children, 2025 Background/Objectives: To explore the association between parental knowledge on dental caries prevention and the risk of caries in pediatric patients aged 6 to 14 years who reside in the province of Seville, using the CAMBRA preventive protocol as an assessment tool. Methods: After the approval granted by the Ethics Committee, a descriptive and analytical observational study was conducted. Caries risk was established using the CAMBRA Questionnaire, pH measurement, and salivary flow rate. To assess the socioeconomic background of the patients and their hygiene and dietary habits, parents completed two surveys: the first about the quality of the patient’s diet, and the second directly related to the CAMBRA questionnaire used and validated by the University of Seville. Results: The final study sample consisted of 300 pediatric patients, aged 6 to 14 years, of whom 54% were boys and 46% were girls. The caries risk distribution was as follows: 33% low, 7% moderate, 48.6% high, and 11.3%. A total of 61.7% of the participants live in urban areas, while 38.3% are from peri-urban regions. There is a statistically significant association between socioeconomic status and family circumstances in children with a risk of caries. Furthermore, an association was established between caries risk, dietary habits, and oral hygiene. Conclusions: Parental knowledge about dental caries prevention and caries risk in children was found to have a strong association with reduced caries risk in children.
