  • Interrelationship between mental health and changes in stress-regulating systems in children

    Aim - to investigate the relationships between indicators of stress-regulating systems depending on the mental state of children and adolescents with type 1 diabetes mellitus (T1DM) during the period of military actions in Ukraine. Materials and methods. A total of 93 patients with T1DM (44 boys, 49 girls) aged 9-18 years were examined. Patients were stratified into three groups: 1 - normal mental health, 2 - non-pathological stress reactions, and 3 - mental disorders. Serum levels of cortisol (C), malondialdehyde (MDA), carbonylated proteins (CP), and superoxide dismutase activity (SODs) were measured. In whole blood, serotonin (S), glutathione peroxidase activity (GPx), and cellular superoxide dismutase activity (SODc) were assessed. Results. Mental health status in adolescents with T1DM was associated with changes in stress-regulating systems, with sex-specific features. In patients with normal mental health, increased levels of C, S, MDA, SODs, and SODc were observed. Non-pathological stress reactions were characterized by elevated CP and reduced SODc and GPx activity. In adolescents with mental disorders, increased CP, decreased S, and reduced SODs and SODc activity were found. In girls, correlations in the first two groups reflected compensatory-adaptive mechanisms, whereas in those with mental disorders an imbalance between stress-regulating indicators was evident. In boys, even with normal mental health, findings suggested tension within the stress-limiting systems, progressing to exhaustion in groups with impaired mental state, alongside the accumulation of stress-realizing markers. Conclusions. Stress-regulating systems are closely associated with mental health status in children and adolescents with T1DM during wartime. The imbalance between stress-limiting and stress-realizing systems is most pronounced in those with mental disorders, particularly in boys. The study was conducted in accordance with the Declaration of Helsinki and approved by the local ethics committee. Informed consent was obtained. No conflict of interest was declared by the authors.
  • Preventive examinations of schoolchildren as an effective tool for preserving the health of adolescents — future defenders of Ukraine

    Background. The issue of organizing and providing medical care to children and adolescents during wartime is of particular importance for Ukraine’s future and is an acute problem for primary healthcare professionals. The purpose of the study is to summarize data from global, national, and personal experience in organizing preventive care for school-age children and adolescents in order to improve the competence of primary healthcare professionals in the preventive component of medical care for school-age children and adolescents — future defenders of Ukraine. Materials and methods. The experience of implementing European models of primary care for schoolchildren was analyzed, as were regulatory documents on the organization and implementation of preventive medical examinations of school-age children during military aggression. A survey was conducted among 145 doctors who provide medical care to children and adolescents. Results. Preventive medical examinations of school-age children and adolescents are conducted in almost all European countries, but the teams of specialists, the timing, and the scope of the examinations vary. According to domestic data, annual medical examinations of schoolchildren are effective as part of the health care system. Based on the results of our own research on the organization of medical care for adolescents and schoolchildren in Ukraine during military aggression, we have identified insufficient routing of medical examinations involving specialist physicians, a shortage of pediatric specialists in various fields, and insufficient interaction between parents, teachers, and medical professionals. The implementation of the current order of the Ministry of Health of Ukraine No. 2003 dated December 3, 2024, which places all responsibility for organization, conduction, and conclusions of preventive medical examinations on primary care physicians, without ensuring cooperation with educational institutions, will not solve the issue of preserving the health of adolescents — future defenders of Ukraine. Conclusions. The organization and conduction of preventive medical examinations of school-age children and adolescents need to be improved in terms of ensuring access to specialist physicians, laboratory and instrumental examinations, improving the competence of doctors in the physiology and pathology of children of different ages, and overcoming the inconsistency between the relevant order on preventive examinations and the scope of work of primary health care doctors.
  • The state and prospects of development of medical care for schoolchildren and adolescents in Europe and Ukraine

    In the unfavorable conditions of martial law in Ukraine, preserving the health of the younger generation is becoming a crucial task of state policy. Aim - to determine the optimal model of medical care for school-age children and adolescents of Ukraine based on world and national experience in peacetime, wartime and postwar times. The object of the study is the global and national trends in medical care for schoolchildren in peacetime time and period of a humanitarian crisis: publications of the European Bureau of the WHO, articles in the scientific communication media (2000-2024). The WHO Global School Health Initiative is aimed at improving the educational level of children and promoting healthy lifestyles at the regional, national and global levels. The WHO Office for Europe’s reports determine that investing in the health of children and adolescents brings greater economic and social benefits compared to secondary and other types of disease prevention among the adult population. The concept proposes quality standards for school health services and competencies for school health professionals. According to the results of the project “Child Health Models Appraised” (2018-2020), it was found that there are different models of medical care in educational institutions, differing in terms of subordination, funding, and staff training. The SI "Institute for Children and Adolescents Health Care at the NAMS of Ukraine" (Kharkiv) proposed a program of medical and preventive measures to preserve the health of school-age children and adolescents in the context of a persistent humanitarian crisis. Conclusions. The most optimal model of medical care for school-age children and adolescents is a multidisciplinary approach to the formation of a healthy lifestyle and the creation of appropriate conditions for its implementation. The solution to the problem of maintaining the health of school-age children and adolescents is of particular relevance in times of humanitarian shocks and needs to be improved in Ukraine today.   • Dynamics of the psychological state of internally displaced school-age children and adolescents

    Internally displaced persons (IDPs) are one of the most disadvantaged population groups in countries affected by hostilities, man-made disasters or other social conflicts and violence. Aim - to identify gender peculiarities of the emotional state of school-age children and adolescents in war conditions in order to develop effective strategies for psychological support in a timely manner. Materials and methods. In May 2022, 1017 IDPs adolescents and 4144 adolescents from Lviv and Lviv region (LR) were surveyed anonymously, and a year later, 1173 adolescents in Lviv region, including 139 IDPs. Results. An analysis of responses about feelings, impressions and emotional state of school-age children and adolescents shows that fear for the lives of their relatives and their fate was the most significant feeling for both IDPs and local residents. Fear for home, housing, and family property remained the second most important fear. A constant feeling of anxiety, helplessness and despair was reported by one-third of IDPs children and adolescents at the beginning of hostilities and evacuation, but during the year this figure decreased to the level of LR. These feelings were more often experienced by girls, regardless of their city of residence. Almost half of all respondents, regardless of their city of residence and gender, reported anger at the situation. Most adolescents expressed confidence that everything will be fine. By almost all indicators, girls from the IDPs group were more emotionally stressed. Conclusions. The problem of the health status of school-age children and adolescents of IDPs and the organization of medical and psychological assistance to them requires careful study and solution. The traumatic events of the war in Ukraine and the internal migration of school-age children and adolescents have had a significant impact on the mental state of not only them, but also local residents, especially girls. This group of children should be considered as a risk group. Determining their emotional state is of great importance for the prevention of reducing the prevalence of psychopathology, psychosocial dysfunction, timely diagnosis and treatment.   • Characteristics of risky forms of behavior that were formed against the background of prolonged social isolation during lockdown, and their influence on the state of students’ youth health

    Background. The issues of risky forms of behaviour have actual importance. During the long period of self-isolation, new conditions were created to study the risky forms of behaviour. Therefore, how different manifestations of addictive behaviour showing themselves in the case of long-term restriction on the state of health, it is a new direction of research, especially among student youth. Unresolved issues regarding the emergence and spread of risky forms of behaviour among student youth in completely new social conditions determine the relevance of this work. Purpose – to determine the RFB of student youth, that were formed during long-term social self-isolation, and to establish their impact on the state of health of student youth. Materials and Methods. The presence of the risky forms of behaviour was evaluated by an anonymous survey of 799 higher education applicants using Google Forms. The questionnaire used was developed by the staff of the institution where the research was conducted. 287 students were identified who noted the appearance of risky forms of behaviour (poor nutrition, smoking, alcohol consumption, indifference to studies, and appearance) during long-term social self-isolation. The obtained data were statistically processed by obtaining the relative value and its error (P ± m, %). Results. It is shown that the risky forms of behaviour as a result of long-term self-isolation are determined both individually and in a series of combinations. 50.53% of the respondents noted that during the lockdown they had one of the types of the risky forms of behaviour, and 49.47% – everal risky forms of behaviour. The most common risky forms of behaviour were indifference to appearance (44.03%) and apathy towards education (36.57%). Every tenth student stated that he started smoking or drinking alcoholic beverages during the lockdown. About 10% of respondents point out that junk food was added to their diet. It was detected that under the risky forms of behaviour such complaints as headache (p < 0.01), cardialgia (p < 0.01), anxiety (p < 0.01), impaired vision (p < 0.01), hypodynamia (p < 0.01), weight gain (p < 0.01) was reliably determined more often than in the control group without the risky forms of behaviour. It is also shown that with more combinations of different risky forms of behaviour – the frequency of identified complaints increased reliably. Conclusions. Long-term self-isolation led to the development of risky forms of behaviour, which occurred in a third of the respondents. During the lockdown, the frequency of bad habits (smoking, alcohol) increased and signs of social maladaptation (indifference to education and appearance) were identified. In half of the respondents with the risky forms of behaviour, a large number of their combinations were determined, among which every fifth case was the risky forms of behaviour from fast food, smoking, alcohol, indifference to studies, and appearance. The risky forms of behaviour affected the frequency of several complaints (hypodynamia, weight gain, nervousness, irritability, heart pain, headache, impaired vision, anxiety), the frequency of which increased with an increase in the number of the risky forms of behaviour.
  • HORMONE LEVELS IN ADOLESCENTS WITH PHYSIOLOGICAL AND PATHOLOGICAL COURSE OF PUBERTY AT DIFFERENT INTENSITY OF PHYSICAL ACTIVITY

    Scientific evidence suggests a global pandemic of motor inactivity in adolescents that contributes to a number of chronic diseases. At the same time, regular physical activity has a favorable effect on the growth and sexual development of children and adolescents. Purpose - to evaluate the influence of physical activity (PA) on the hormonal blood spectrum of adolescents with physiological and pathological course of puberty. Materials and methods. We examined 55 boys and 46 girls aged 11-17 years old. Concentration of total testosterone, estradiol, cortisol, prolactin was determined by enzyme immunoassay, serotonin content was determined by fluorometric method. PA was studied using the questionnaire of IPAQ (International Physical Activity Questionnaire). Depending on the course of pubertal period adolescents were divided into 2 groups: the Group 1 - with physiological course of puberty; the Group 2 - with pathological course of puberty (boys with delayed puberty and girls with menstrual disorders). Adolescents were divided into 2 groups according to their PA level: low (<7 points) and high (>9 points) levels of PA. Results. It was found that in boys with a physiological course of puberty high PA compared with low activity leads to an increase in testosterone concentration and has a positive effect on physical development. High PA, on the contrary, decreases the content of testosterone in the pathological course of puberty and possibly aggravates puberty in boys. In girls high PA against the physiological course of puberty leads to an increase in serotonin, an indicator of the stress-limiting system. In the pathological course of puberty high PA has a positive effect, reducing the levels of testosterone and prolactin compared with low activity, which may contribute to the normalization of menstrual function, as well as reducing stress levels. Conclusions. Physical activity in adolescents should continue to be encouraged, given its positive effect on the hormonal blood spectrum. However, gender differences, pubertal development status and different mechanisms of adaptation to PA of girls and boys should be taken into account. It is very important to determine the optimal level of PA to meet the needs of children and adolescents, which would have a beneficial effect on both physical and psycho-emotional development. The research was carried out in accordance with the principles of the Helsinki Declaration. The study protocol was approved by the Local Ethics Committee of the participating institution.   • Peculiarities of the psychological state of internally displaced school-age children and adolescents in war conditions

    Since the war escalated on February 24th, 2022, 1,148 children have been killed or injured, and nearly 5.9 million have become internally displaced persons (IDPs) according to UNICEF. The state of health of children who are IDPs requires the close attention of doctors and psychologists. Purpose - to investigate the psychological and psycho-emotional state of schoolchildren and adolescents who are IDPs in the conditions of war in order to substantiate the strategy and tactics of the use of health-preserving technology. Materials and methods. Psychological and psycho-emotional state of 1017 school-age children and adolescents who are IDPs aged 11-17 years and their parents (n=1231) who lived in Lviv and the Lviv region at the time of the study. In May 2022 anonymous questionnaire was conducted for them. The questionnaires were developed by the executors of the scientific research work «To study the age and gender characteristics of the predictors of the transformation of behavioral risk factors for the health of adolescents in the conditions of war». Results. It was found that 53.9% of children were forced to be separated from their families, 21.3% were under shelling; 5.8±0.7% experienced the death of their relatives; 11.4±1.0% suffered from hunger, lack of drinking water and cold. One in three children felt helpless and despair, and one in five had panic attacks. At the same time, 85% of children believed that the horrors of war would end and everything would be fine. 23% of children experienced dizziness, headaches, heart palpitations, and abdominal pain when there was no reason for it. 26.5% of children sought medical help. Conclusions. Almost a third of all interviewees had a constant feeling of anxiety, a fifth had panic attacks, a sense of danger, insecurity. Children had increased negative emotions compared to pre-war times. One third of the children’s mental health has deteriorated, but almost none of them have sought psychological support from specialists. Feelings of dizziness, headache, rapid heartbeat, abdominal pain, when there was no reason for this, were identified in every fifth child, which can be the basis for the formation of psychosomatic pathology. The factor that stabilized the psycho-emotional state of both children and their parents was the belief that the horrors of the war will end and everything will be fine, and almost 85% of the respondents answered this way. This indirectly indicates a sufficient potential for viability. The organization of a multidisciplinary team consisting of psychologists, doctors, and teachers is an actual form of medical and psychological assistance to school-age children and adolescents who are IDPs. The research was carried out in accordance with the principles of the Helsinki Declaration. The study protocol was approved by the Local Ethics Committee of all participating institutions. The informed consent of the patient was obtained for conducting the studies. No conflict of interests was declared by the authors.
