Cost-effectiveness of microsurgical reconstructions in reconstructive plastic surgery within the Brazilian Public Health System: a 15-year overview (2009-2023) Dov Charles Goldenberg, Marcos Antonio Neves Noronha, Klaus Werner Wende, Ricardo Yugi Eri, Beatriz Peral Venet Ferreira, Lucas Andrew Jikal, Vitor Pelogi Arienzo Einstein Sao Paulo Brazil, 2026 OBJECTIVE: To evaluate the national distribution and cost-effectiveness of oncologic microsurgical reconstructions performed within the Brazilian Public Health System over a 14-year period. METHODS: A retrospective analysis of DATASUS data was conducted using three procedure codes related to oncologic microsurgical reconstruction. Surgical volume was assessed by state and geographic region. Hospital reimbursement values were compared with those of less complex reconstructive procedures, and inter-state patient referral patterns were analyzed to identify trends in service centralization. RESULTS: Between 2009 and 2023, a total of 1,902 microsurgical reconstructions were performed nationwide. The Southeast and South regions accounted for the majority of cases, with São Paulo (n=563) and Rio Grande do Sul (n=474) representing the highest-volume states. In contrast, the North and Center-West regions exhibited substantially lower availability of microsurgical services. Reimbursement analyses demonstrated that microsurgical procedures were compensated at levels comparable to those of less complex reconstructions, despite their greater technical and infrastructural requirements. Referral flow analyses revealed a pronounced concentration of patients toward a limited number of high-volume centers. CONCLUSION: Microsurgical reconstruction within the Brazilian Public Health System is cost-effective; however, its availability remains unevenly distributed, restricting equitable access to complex oncologic reconstructive care. Public policies focused on workforce training, capacity-building, and regional decentralization are necessary to expand access and reduce reliance on a small number of specialized centers.

As lesões variam conforme a idade: crianças geralmente apresentam ferimentos na cabeça e pescoço, enquanto adultos são mais atingidos nas extremidades. Durante a pandemia de COVID-19, alterações nas interações entre humanos e cães foram relatadas, com alguns estudos apontando aumento das lesões por mordedura de cães (LMC) em períodos de lockdown. Este estudo investigou as tendências de LMC em um hospital privado de São Paulo, Brasil, durante a pandemia, e avaliou a demanda por consultas em cirurgia plástica.Estudo retrospectivo observacional multicêntrico que analisou casos com o código W54 da CID-10 em cinco unidades de emergência do Hospital Israelita Albert Einstein entre janeiro de 2019 e junho de 2021. Os pacientes foram divididos em períodos pré- (Grupo 1) e pós-isolamento social (Grupo 2). Dados avaliados incluíram características demográficas, data e horário das visitas, dia da semana e demanda por cirurgia plástica. As análises estatísticas foram realizadas no software R.Entre 1129 casos, 558 ocorreram no Grupo 1 e 571 no Grupo 2. Apesar da redução nas visitas gerais ao PS, a frequência relativa de LMC aumentou durante a pandemia (p < 0,001). Consultas de cirurgia plástica cresceram discretamente, sem significância estatística (p = 0,0775). Não houve diferenças relevantes na idade, sexo ou horário das visitas.A pandemia influenciou a frequência relativa de LMC, destacando a importância de estratégias preventivas e investigações mais amplas para entender este problema.

METHODS: A meticulous selection process identified 22 articles relevant to this scoping review. RESULTS: The literature on the use of robotics in plastic surgery is sparse. Nonetheless, this review highlights emerging benefits in microsurgery, breast reconstruction, and transoral surgery. CONCLUSION: This scoping review identifies critical articles reporting the emerging use of robotics in plastic surgery. While the scientific medical community has yet to extensively document its use, the available evidence suggests a promising future for robotics in this field.

Otherwise, the combination of congenital midline cervical cleft and thyroglossal duct fibrosis in the same patient is as rare as important to be registered with the intention to inform and offer specific managements details for the literature. Case Presentation: Eight-year-old boy with simultaneous congenital midline cervical cleft and a thyroglossal duct fibrosis. The anatomic, clinical, radiologic, and pathologic characteristics of the congenital midline cervical cleft are described as well as surgical technique for removal and repair with Z-plasty. Conclusion: Congenital midline cervical cleft is a rare condition and when diagnosed must be surgically treated as early as possible. Its differential diagnosis is a clinical challenge.
Response to sirolimus in capillary lymphatic venous malformations and associated syndromes: Impact on symptomatology, quality of life, and radiographic response Elissa R. Tollefson, Ionela Iacobas, Deborah Schiff, Shoshana Greenberger, Michael Kelly, Ilona Frieden, Nibal Zaghloul, Beth Drolet, Amy Geddis, Dov Goldenberg, Kiersten Ricci Pediatric Blood and Cancer, 2023 BackgroundCapillary lymphatic venous malformations (CLVM) and associated syndromes, including Klippel–Trenaunay syndrome (KTS) and congenital lipomatous overgrowth, vascular malformation, epidermal nevi, skeletal, and spinal syndrome (CLOVES), are underrecognized disorders associated with high morbidity from chronic pain, recurrent infections, bleeding, and clotting complications. The rarity of these disorders and heterogeneity of clinical presentations make large‐scale randomized clinical drug trials challenging. Identification of PIK3CA (phosphatidylinositol‐4,5‐bisphosphate 3‐kinase catalytic subunit alpha [gene]) mutations in CLVM has made targeted medications, such as sirolimus, attractive treatment options. The aim of this study was to investigate the safety and efficacy of sirolimus therapy in CLVM.ProcedureA combined prospective and retrospective cohort of pediatric and young adult patients with CLVM treated with sirolimus was evaluated for disease response, including symptom improvement, quality of life (QOL), and radiologic response. Sirolimus dosing regimens and toxicities were also assessed.ResultsTwenty‐nine patients with CLVM, including KTS and CLOVES, were included. Ninety‐three percent of patients reported improved QOL, and 86% had improvement in at least one symptom. Most significantly, improvement was noted in 100% of patients with bleeding and 89% with thrombotic complications with corresponding decreases in mean D‐dimer (p = .008) and increases in mean fibrinogen (p = .016). No patients had progressive disease on sirolimus. Most common side effects included neutropenia, lymphopenia, infection, and aphthous ulcers/stomatitis. No toxicities were life‐threatening, and none required long‐term discontinuation of sirolimus.ConclusionSirolimus appears to be effective at reducing complications and improving QOL in patients with CLVM and associated syndromes. In this patient cohort, sirolimus was well tolerated and resulted in few treatment‐related toxicities.

Stroke Due to Orthognathic Surgery: Case Report of a Rare Complication Luana Lima Bulgarelli, Dov Charles Goldenberg, Rafael Maffei Loureiro, Andre Felix Gentil, Roberto Franco Morgulis, Eduardo Carvalhal Ribas Plastic and Reconstructive Surgery Global Open, 2022 Summary: Le Fort I osteotomy is a frequent surgical procedure used in orthognathic surgeries to treat severe malocclusions and is associated with relatively rare surgical complications. Here, the authors report a case of thrombotic ischemic stroke as a result of this procedure, a complication still not described in the literature. A 19-year-old man with class II malocclusion and retrognathia underwent orthognathic surgery for aesthetic purposes. The surgery included a Le Fort I maxillary osteotomy with vertical impaction, bilateral sagittal split ramus osteotomy for mandibular advancement, and genioplasty. Postoperatively, the patient developed left eye blindness, headache, somnolence, aphasia, and right hemiplegia. Medical imaging showed the Le Fort I line of fracture extending from the maxillary osteotomy to the left optic canal and to the left carotid canal, with osseous fragments impinging the petrous segment of the internal carotid artery, left carotid artery occlusion and associated to an ischemic stroke at the left middle cerebral artery territory. Treatment required decompressive craniectomy and later focused on clinical stabilization, infection management, orthognathic care, neurorehabilitation, and cranioplasty. The hemiplegia and aphasia partially recovered during 12 months, and final dental occlusion was appropriate. Our report demonstrates that an unfavorable Le Fort I fracture trajectory can lead to ischemic stroke and severe neurological deficits.
