  • The morbidity of adult population in the Russian Federation and its Federal Okrugs and Subjects in 2016-2022

    The population morbidity is one of leading indicators of public health. On the basis of morbidity, not only population needs in various types of medical care are determined, but also development of medical organizations network is prognosticated and effectiveness of health care system is assessed. This approach permits to adjust functioning of population medical care services in each Subject of the Russian Federation. Hence, studying dynamics of adult population morbidity by disease classes and considering regional characteristics continues to be actual. The purpose of the study is to analyze dynamics of general and newly registered (primary) morbidity of adult population in the Russian Federation, Federal Okrugs and Subjects of the Russian Federation in 2016-2022. The research materials and methods included calculation of dynamic series indicators (visibility indicators, growth rates, growth/loss rates), direct ranking method. The indicators of total and newly registered (primary) morbidity per 100,000 of adult population (aged 18 years and older) in dynamics in 2016-2022 were calculated. The research materials are the statistics collections of the Ministry of Health of the Russian Federation and the Federal State Budget Institution "The Central Research Institute for Health Organization and Informatics" of Minzdrav of Russia of adult population morbidity of the Russian Federation. The software Microsoft Excel was applied to process statistical data. The results. The analysis of total morbidity of adult population in the Russian Federation in 2016-2022 demonstrated that in 2022 value of this indicator made up to 159,545.4 per 100 thousand of population that is 9.0% more than in 2016. The increase of total morbidity is observed in 66 Subjects of the Russian Federation that consists 77.65% of total number of the Subjects. The level of primary morbidity in the Russian Federation increased by 21.9% (from 55227.3 per 100 thousand adults in 2016 to 67326.6 in 2022). The increase of primary morbidity is observed in 81 Subjects of the Russian Federation that consist 95.3% of total number of the Subjects. The analysis of morbidity demonstrated that in 2016-2019 its indicators had no significant changes. In 2020-2022, sanitary and epidemiological well-being of population in the Russian Federation was determined by epidemic situation and global challenge associated with the new corona-virus infection. As a result, in 2020, occurred decreasing of morbidity registration for all classes of diseases, except respiratory diseases. In 2021, began to show increasing of registration of morbidity that proceeded in 2022 in most classes of the ICD-10.

  • National Results of Revascularization for Acute Coronary Syndrome in 2023

    Aim To analyze the results of myocardial revascularization in the Russian Federation (RF) for ACS in 2023 compared to previous years.Material and methods The analysis included the number of cases of ST-segment elevation myocardial infarction (STEMI), non-ST-segment elevation acute coronary syndrome (NSTE-ACS), myocardial revascularization in the above-listed ACS forms, the number of fatal outcomes depending on the ACS form and the revascularization method used. The data for this analysis were obtained from the 2023 Ministry of Health of Russia monitoring in the section of revascularization in ACS and were compared with the data for the past 8 years.Results, conclusion In 2023, 438,315 patients were hospitalized for ACS in the Russian Federation: 309,158 with NSTE-ACS and 148,729 with STEMI. The total number of hospitalizations for ACS per 1 million of the Russian population was 2,982: 1,011 with STEMI and 2,103 with NSTE-ACS. The availability of primary PCI in 2023 reached its maximum of 55.3% compared to previous years; the total number of PCI for STEMI was 75.7%, and the mortality rate in the whole STEMI group was a minimum of 10.7% for the past 8 years. In 2023, the maximum number of PCIs for NSTE-ACS for the past 8 years was recorded, both in absolute values (120,990) and in relative values (39.1%). In the whole NSTE-ACS group, mortality was 2.5%, which was also the lowest for the past 8 years.
  • Climatogeographic background for organizing medical care for the Far North Russian Federation population

    Seventy percent of the Russian Federation territory has the status of the Far North regions and equivalent areas. On the territory with an average population density of 1 person per 1 square kilometer, 11.5 million people live. Severe climatogeographic, complex ecological and socio-economic conditions, low population density and uneven settlement, transport problems, reduced availability of medical organizations, shortage of medical personnel have a negative impact on the health of the population and its life expectancy. This causes the main problems that arise in the organization of medical care in the Far North, for which it is necessary to develop a network of paramedic-obstetric stations and district hospitals, air ambulance and telemedicine technologies, the organization of mobile medical diagnostic complexes.
  • Impact of the COVID-19 Pandemic on Myocardial Revascularization in Patients with Acute Coronary Syndrome in the Russian Federation

    Aim. To compare and analyze the results of myocardial revascularization in the Russian Federation (RF) with acute coronary syndrome (ACS) before the onset (2018-2019) and during the novel coronavirus infection (COVID-19) pandemic (2020-2021).Material and methods. The analysis included the number of cases of ST-segment elevation myocardial infarction (STEMI), non-ST-segment elevation acute coronary syndrome (NSTE-ACS), the number of cases of myocardial revascularization in the above forms of ACS, the number of deaths depending on the form of ACS and the method performed revascularization. The period of time before the start of the coronavirus pandemic corresponded to the annual data received in the Russian Federation for 2018-2019. The period of the coronavirus disease pandemic corresponded to the annual data received in the country for 2020-2021. Absolute, relative, estimated values of patient hospitalization, myocardial revascularization procedures, and mortality in ACS were compared between time periods before and during the COVID-19 pandemic. The data for analysis were obtained from the monitoring of the Ministry of Health of Russia.Results. In 2018 and 2019 in the RF, 531,019 and 501,238 patients were hospitalized with a diagnosis of ACS, and during the pandemic (2020-2021) - 403,931 and 397,930 patients, respectively. Reduction in the number of patients diagnosed with ACS admitted to hospitals in Russia by 22.32% in 2020-2021 years was mainly due to a significant decrease in hospitalizations of patients with a diagnosis of NSTE-ACS (by 29.03%). At the same time, admission to clinics of patients with STEMI decreased only by 6.02%. During the COVID-19 pandemic, mortality increased significantly in PPCI (by 9.6%) and in the general STEMI group (by 12.3%); significantly increased mortality both in the general group (by 48%) and during PCI in patients with NSTE-ACS (by 28.6%); there was an increase in the average annual number of PPCI (by 12.6%), which was accompanied by an increased average annual number of PPCI per 1 million of population (up to 451 per 1 million of population); a slight increase in the average time "symptom-balloon" (by 2 minutes) was recorded; there was an absolute slight decrease and a relative increase in the number of PCIs in NSTE-ACS (by 2.7% and 37.1%, respectively). In 2021, in the Russian Federation, primary PCI was performed in 50.2%, thrombolytic therapy - in 23.1%, and 26.7% of patients remained without reperfusion. Pharmacoinphasive strategy was applied in 60%, and isolated thrombolysis - in 40% of patients.Conclusion. During the COVID-19 pandemic, revascularization in patients with ACS in Russia corresponded to the following trends recorded in the literature: increased hospital mortality in PPCI and in the general STEMI group; hospital mortality both in the general group and during PCI in patients with NSTE-ACS. The indicators of myocardial revascularization in ACS in the RF during the pandemic were fundamentally different from the data of Western countries: there was an increase in the average annual number of PPCI and the average annual number of PPCI per 1 million population; a slight increase in the average symptom-balloon time was recorded; revealed an absolute slight decrease and a relative increase in the number of PCIs in NSTE-ACS.
  • Healthcare in Russia during the COVID-19 pandemic: challenges, systemic issues, and addressing priorities

    The new SARS-CoV-2 (COVID-19) coronavirus pandemic has placed unprecedented strains on healthcare systems worldwide. How-ever, the Russian Federation has demonstrated a high level of preparedness to combat the COVID-19 pandemic. First, it was re-flected in the quick reallocation of resources toward the efficient provision of medical care for patients with COVID-19. To that end, temporary hospitals were set up specifically for such patients, and the existing bed capacity was reassigned. In addition, routing schemes for patients with acute respiratory viral infections (ARVI) and community-acquired pneumonia were developed and approved. Also, provisional rules were approved for recording information and maintaining an information resource to pre-vent the spread of COVID-19 using the unified state health information system. Furthermore, from December 2020, our country began a large-scale vaccination of the adult population against COVID-19, and from January 2021, mass vaccination against CO-VID-19 was started. The mobilization of the country's human resources to combat the pandemic and the timely communication of relevant clinical guidelines to physicians enabled an effective response to the challenges of the COVID-19 pandemic.   • Trends in morbidity and in-hospital mortality from cardiovascular diseases during COVID-19 pandemic

    <h3></h3> С учетом особой значимости болезней системы кровообращения (БСК) в формировании основных показателей состояния здоровья населения представляет научный и практический интерес изучение тенденций заболеваемости БСК взрослого населения и динамики, связанной с ними больничной летальности до и во время пандемии COVID-19. <h3>ЦЕЛЬ ИССЛЕДОВАНИЯ</h3> Изучить тенденции показателей заболеваемости БСК и связанной с ними больничной летальности взрослого населения в Российской Федерации на фоне новой коронавирусной инфекции (COVID-19). <h3>МАТЕРИАЛ И МЕТОДЫ</h3> Показатели первичной и общей заболеваемости БСК, а также связанной с ними больничной летальности взрослого населения, рассчитаны на основании годовых форм федерального статистического наблюдения №12 и №14 за 2015—2020 гг. <h3>РЕЗУЛЬТАТЫ</h3> В течение периода с 2015 по 2019 г. в Российской Федерации отмечался рост как общей заболеваемости БСК (на 12,9%), так и первичной заболеваемости (на 14,6%). В 2020 г. по сравнению с 2019 г. была отмечена убыль показателя общей заболеваемости БСК (на 6,5%) и первичной заболеваемости (на 15,9%). Динамика показателя больничной летальности вследствие БСК за период с 2015 по 2019 г. характеризовалась незначительным ростом (на 7,3%); на фоне пандемии COVID-19 в 2020 г. по сравнению с 2019 г. рост этого показателя составил 58,9%. <h3>ЗАКЛЮЧЕНИЕ</h3> Убыль показателей заболеваемости БСК с одновременным значительным ростом и связанной с ними больничной летальности в 2020 г. отражает напряженную ситуацию, сложившуюся в системе здравоохранения Российской Федерации во время пандемии COVID-19, а также свидетельствует об утяжелении течения острых и хронических форм БСК.
  • Trends in Morbidity and Hospital Lethality from Neoplasms before and during the New Coronavirus Infection COVID-19

    Background. Taking into account the special medical and social significance of oncological diseases, as well as taking into account that patients suffering from neoplasms are a special risk group for developing complications of coronavirus infection, it seems relevant to study the impact of the COVID-19 pandemic on the level of morbidity and hospital lethality of patients with neoplasms.Aims to study the trends of morbidity and hospital lethality due to neoplasms in the Russian Federation before and during the pandemic of the new coronavirus infection COVID-19.Methods. The indicators of primary and general morbidity, hospital lethality from neoplasms of the adult population were calculated on the basis of federal statistical observation data contained in reporting forms No. 12 and No. 14 of all subjects of the Russian Federation for 20152020.Results. During the period from 2015 to 2019, an increase in the general (by 14.8%) and primary (by 4.8%) incidence of neoplasms was noted in the Russian Federation. In 2020, compared with 2019, on the contrary, the indicators of general (by 5.0%) and primary (by 17.4%) incidence of neoplasms decreased. Hospital lethality due to neoplasms also increased during 20152019, in total it increased by 19.2%, in one year (2020) hospital lethality increased by 6.6%. A similar situation has developed in all federal districts and most regions of the country.Conclusions. The decline in the incidence of neoplasms in 2020, with their previous continuous growth, reflects the difficult situation that has developed during the pandemic of a new coronavirus infection in the healthcare of the Russian Federation. The increase in hospital lethality from neoplasms indicates a more severe course of coronavirus infection in adult patients with malignant neoplasms and a more frequent development of fatal complications in them.
  • Dynamics of sickness rate in 15-17- year-old girls in the Russian Federation

    The children’s health condition in the Russian Federation, including adolescents, has been characterized by negative trends in the past thirty years.Objective. To identify the main trends in sickness rate (both total and registered for the first time) in 15–17-year-old girls in the Russian Federation, as well as to track the quantity and dynamics of chronicity of diseases by classes and types.Material and methods. The authors analyzed the data of form №12 of the federal statistics in 2000–2017 using the descriptive statistics method.Results. The researchers revealed a significant increase both in the total sickness rate from 1613,8 to 2532,5 per 1,000 girls (by 56,9%) and in the sickness rate registered for the first time from 967,5 to 1577,3 per 1,000 girls (by 63,0%). The increase in total incidence is observed in the most classes of diseases: neoplasms (2,4 times), injuries, poisoning and some other consequences of external causes (2,4 times), congenital anomalies (malformations; 2,3 times), cardiovascular diseases (2 times), diseases of the musculoskeletal system and connective tissue (2 times). Also there was revealed the increase in the incidence registered for the first time in the overwhelming majority of classes of diseases, for example: injuries, poisoning and some other consequences of external causes (2,5 times), diseases of the nervous system (2,2 times), cardiovascular diseases (2 times), neoplasms (2 times), diseases of the ear and mastoid process (2 times), diseases of the musculoskeletal system and connective tissue (1,9 times). High index of the chronicity for all classes of diseases reflect the initially high level of chronic pathology in 15–17-year-old girls, which is an unfavorable characteristic of their health condition. Having analyzed the chronization index the authors revealed its maximum growth for congenital anomalies (malformations; by 31,1%) and neoplasms (by 23,0%), and for certain nosologies: menstrual disorders (5,8 times) and renal failure (4,2 times).Conclusion. The health of children, and especially adolescents, determines the national health in the nearest future; therefore, its preservation and improvement require thorough attention.
  • Dynamics of the incidence in the 15-17-year-old men in the Russian federation

    In recent decades, the Russian Federation has seen an increase in the incidence of the population, including children and adolescents.Objective: to identify trends in the incidence and “to chronize” the pathology of young men in the Russian Federation. Using the method of descriptive statistics, the authors analyzed the overall and for the first time recorded incidence in 15–17-year-old men in 2000–2017. The authors noted a 35,8% increase in the overall incidence rate and a 35,4% increase in the for the first time recorded incidence rate. The maximum increase in the overall incidence is observed in the following classes: neoplasms (2,6 times), blood and bloodforming organs diseases (2 times), congenital anomalies (malformations; 1,8 times), circulatory system diseases (1,7 times), the musculoskeletal system and connective tissue diseases (1,6 times), endocrine and metabolic disorders (1,5 times). Based on theoverall and for the first time registered incidence the authors calculated the “chronicity” of diseases. With a general growth rate of this indicator of 0,3% there is a significant growth of this indicator for certain diseases: mental disorders and behavioral disorders (2,1 times), congenital anomalies (malformations; 1,9 times), endocrine system diseases, metabolic disorders (1,5 times).
