  • Community pharmacists’ knowledge and experience regarding malaria management: a cross-sectional study in Hodeida, Yemen

    BACKGROUND: Malaria remains a significant public health concern in Yemen, ranking fourth in incidence within the Eastern Mediterranean Region. Community pharmacists play a pivotal role in malaria management and often dispense anti-malarial medications without prescriptions. However, little is known about their knowledge and adherence to the National Policy for Antimalarial Drugs. This study aimed to evaluate pharmacists' knowledge, experience, and perceived barriers regarding malaria management in Hodeidah, Yemen. METHODS: A cross-sectional study was conducted between May and August 2023 across 320 community pharmacies in 16 districts of Hodeidah governorate. A validated questionnaire was used to assess pharmacists' knowledge, experience, and perceived barriers to effective malaria management. Descriptive statistics and logistic regression analysed the associations between sociodemographic factors and pharmacists' knowledge and experience. RESULTS: Of the 304 respondents, 10 were excluded due to incomplete data, yielding a valid response rate of 91.9% (294). Most pharmacies were independent (74.8%), and 51.4% were located in Hodeidah city. The majority of the pharmacists were male (92.2%), aged 19-30 years (50.3%), and held a pharmacy diploma (69%). Poor knowledge of malaria management was observed in 76.5% of respondents (mean: 9.96, SD 3.47), with only 34.4% and 43.2% accurately identifying first-line treatments for uncomplicated and complicated malaria, respectively. Additionally, 60.9% were aware of the prevalent malaria species, and 53.1% knew the correct diagnostic methods. However, only 17.0% recognized high-risk groups for complicated malaria, 18.4% identified causes of treatment failure, and 7.8% understood the consequences of malaria during pregnancy. In terms of experience, 76.2% of the participants reported inadequate experience related to anti-malarial medications (mean: 56.71, SD 7.32), with the main barriers being lack of training (78.9%) and insufficient knowledge (73.5%). Multivariable logistic regression revealed that older age (AOR: 6.827, p = 0.020), holding a pharmacy diploma (AOR: 2.555, p = 0.036), and fewer perceived barriers (AOR = 3.830, p < 0.001) were predictors of poor knowledge, whereas practicing in Hodeidah city (AOR = 1.865, p = 0.043) predicted inadequate experience. CONCLUSION: Community pharmacists in Hodeidah demonstrate significant gaps in knowledge and experience related to malaria management. Urgent educational interventions are needed to enhance pharmacists' competency, ensure effective malaria treatment, and prevent the emergence of drug resistance in Yemen.
  • Prevalence and determinants of depression and its association with social support among cancer patients: implications for enhancing oncology care

    BACKGROUND: Early detection of depression in cancer patients is essential for improving health outcomes. This study assessed the prevalence and determinants of depression among cancer patients and examined its association with social support. METHOD: A cross-sectional study was conducted at Al Amal Cancer Center, Al-Thawra Hospital, Hodeida City, Yemen (January-March 2023). Data on sociodemographic characteristics, depression (Patient Health Questionnaire, PHQ-9), and social support (Oslo Social Support Scale, OSSS-3) were collected. Multivariate binary logistic regression identified significant depression factors, and Pearson's correlation was used to assess the relationship between social support and depression. A p value of < 0.05 indicated statistical significance. RESULTS: Depression incidence among cancer patients was 55% (n = 192). Multivariate analysis revealed that married (AOR = 9.472, p < 0.001) and divorced/widowed patients (AOR = 11.649, p < 0.001) were more likely to have depression than single individuals were. Higher education (diploma or above) was protective (AOR = 0.071, p < 0.001). Elevated lipid levels were linked to a lower depression risk (AOR = 0.189, p = 0.003), and a family history of cancer (AOR = 4.239, p < 0.001), longer disease duration (> 4 years) (AOR = 3.197, p = 0.006), and difficulty in activities (AOR = 8.704, p < 0.001) increased depression risk. A significant negative correlation between depression and social support was found (r = - 0.237, p < 0.001). CONCLUSION: Over half of cancer patients experience depression, which is influenced by marital status, education, and disease-related factors. Social support significantly mitigates depression. IMPLICATIONS FOR PRACTICE: Enhancing social support and integrating mental health assessments by psychiatric professionals in oncology care is essential for improving the emotional well-being and overall quality of life of cancer patients.

    Complementary and Alternative Medicine (CAM) offers various non-conventional therapeutic approaches that are increasingly adopted by patients with lung cancer to alleviate symptoms, improve quality of life, and complement standard treatments. CAM interventions include herbal therapies, nutraceuticals, mind–body practices, acupuncture, massage, and energy-based therapies. These therapies provide symptom relief, such as reducing pain, nausea, fatigue, and anxiety, while also promoting immune modulation and psychological well-being. Despite promising anecdotal and clinical evidence, the integration of CAM into lung cancer care presents challenges, including inconsistent efficacy data, safety concerns, and potential interactions with conventional therapies. Healthcare professionals play a vital role in guiding patients by offering evidence-based advice on the safe use of CAM and supporting informed decision-making. Further research is essential to explore the mechanisms, efficacy, and long-term outcomes of CAM in lung cancer prevention and treatment. This chapter emphasizes the need for collaboration between CAM practitioners and oncologists to develop integrative oncology programs that provide holistic patient care.

    Prostate cancer is one of the most prevalent malignancies among men, prompting the exploration of complementary and alternative medicine (CAM) to enhance prevention, treatment, and quality of life. CAM therapies, including herbal remedies, nutraceuticals, mind–body interventions, and lifestyle modifications, offer potential benefits by reducing inflammation, improving immune function, alleviating treatment-related side effects, and enhancing emotional well-being. Common herbal remedies such as saw palmetto, turmeric, and pomegranate have shown promise through anti-inflammatory and antioxidant effects, while nutraceuticals like omega-3 fatty acids, selenium, and probiotics play crucial roles in immune modulation and oxidative stress reduction. Other CAM practices such as acupuncture, yoga, meditation, and aromatherapy provide effective symptom management, reducing anxiety, fatigue, and pain among prostate cancer patients. Integration of CAM into healthcare systems is expanding, though challenges remain due to varying levels of scientific evidence and regulatory concerns. The role of healthcare professionals is pivotal in guiding informed decisions, as knowledge, attitudes, and practices (KAP) among both providers and patients affect the use and outcomes of CAM. While CAM cannot replace conventional treatments, its use in prostate cancer management highlights the importance of a holistic, patient-centered approach. Further clinical studies are required to establish standardized protocols, ensure safety, and optimize outcomes through the safe integration of CAM with conventional care.

    This exploratory qualitative study examines the awareness, perceptions, and roles of Jordanian community pharmacists in providing emergency contraception. Twenty semi-structured interviews were conducted with pharmacists from two major governorates, exploring their counseling practices, knowledge of emergency contraceptives, product differences, concerns, and views on patient requests. Findings show that pharmacists' decisions are influenced by societal stigma, legal constraints, and personal beliefs, with market trends also affecting dispensing practices. The gap was evident in how pharmacists were short on knowing the variety of different types and approaches to emergency contraception. Patient initiative in seeking information significantly mattered and influenced how pharmacists responded to such situations. Also, they reported ethical dilemmas when they handled these medications. Pharmacies in Jordan are so intertwined with community, where people sought to seek pharmacists’ help in almost every occasion, proving ever strongly how it makes a difference whether the space is safe and supportive or not, especially for reproductive health issues. Our study highlighted where do we fall short and the need to design programs that fill the gaps in knowledge, perception, and practice.
  • The Potential Effect of Dapagliflozin and Liraglutide in Attenuating Cardio-Renal Injuries in Diabetic Rats

    Objective: To evaluate the therapeutic benefits of dapagliflozin and liraglutide as treatments for type 2 diabetes mellitus and their combined effects on T2DM-related complications, specifically cardio-renal injury. Methods: Thirty rats were randomly allocated into two groups, with the first group serving as the control group, which had 6 rats. The second group was the experimental group, which had 24 rats administered a high-fat diet for four weeks, followed by a single dose of streptozotocin (STZ) to induce diabetes mellitus (DM). This, combined with a high-fat diet, a low dose of STZ was used to cause sub-lethal damage to beta cells. HFD/STZ is an easy method to successfully create a rat model resembling human T2DM, causing insulin resistance, but it does not fully capture the complexity of human T2DM. The experimental group was randomly divided into a positive control group, a liraglutide (0.4 mg/kg, s.c) group, a dapagliflozin (1 mg/kg, orally) group, and a combination of Dapa and lira group, which were administered daily for four weeks. Blood samples were analyzed for glucose, insulin, and cardiac and kidney function markers. Cardiac and kidney tissue were examined to assess redox balance, glutathione (GSH), catalase (CAT), and malondialdehyde (MDA). Results: Dapa and/or lira administration improved the body weight, lipid profile, cardiac and kidney function markers. Furthermore, all treating groups exhibited restoration of the balance between oxidants and antioxidants. Histological studies also revealed a reduction in cardiorenal tissue injury caused by diabetes. Interestingly, the combined management of Dapa and Lira showed a more beneficial protective effect than individual treatments. This study uniquely explores the simultaneous impact on cardiac and renal systems in a diabetic model, offering novel insights into cardiorenal interaction and the combined therapeutic potential of Dapa and Lira. Conclusion: These findings suggest that the combination of dapagliflozin and liraglutide provides superior protection against diabetes-induced cardiorenal injury compared to either treatment alone, highlighting their potential as adjunctive therapies in reducing type 2 diabetes mellitus complications.

    Home delivery of medications is an emerging service that can enhance treatment continuity, reduce travel burdens, and improve public health outcomes in low-resource settings like Yemen. However, community pharmacists’ perceptions in Sana’a remain unclear. To assess community pharmacists’ perceptions of home medication delivery services in terms of their advantages, disadvantages, and the factors influencing their acceptance. A cross-sectional survey was conducted among 375 community pharmacies in Sana’a from April to July 2023. Logistic regression identified factors affecting service acceptance. Of 390 pharmacies approached, 375 (96.2%) participated. The majority were male (85.9%), and 49.3% held a Bachelor of Pharmacy degree. More than two-thirds (68.3%) were aware of the service, 47.2% were willing to adopt it, and 31.7% currently offered it. The main disadvantages identified were dispensing errors (69.1%), verification limitations (71.5%), and challenges in marketing over-the-counter items (70.7%). In contrast, the primary advantages included facilitating treatment during pandemics (83.2%), aiding vulnerable populations (84.5%), providing convenience for parents (80.8%), and potentially reducing congestion in health facilities (72.0%). In multivariable analysis, willingness to adopt home delivery was higher among pharmacists currently providing it (AOR = 9.10), partially involved (AOR = 4.05), and those aware of it (AOR = 4.95; all p < 0.001). Most community pharmacists in Sana’a are aware of home medication delivery, and nearly half express a willingness to adopt it for its continuity of care, reduced facility congestion, and convenience. However, Concerns about medication errors remain, and acceptance is strongly influenced by awareness and involvement.
  • Attitudes, motivators, and barriers toward influenza vaccination for children: a study from a conflict-ridden country

    Background Despite the increased recommendations for influenza vaccination, particularly among high-risk groups such as young children, Yemen lacks an influenza vaccination program, and the influenza vaccine is not included in the national immunization regime. This is exacerbated by the country’s fragile infrastructure, as well as the devastating consequences of the ongoing conflict, which include child undernutrition and strained healthcare resources. Thus, the objective of the current study is to assess the public attitudes and perceptions toward vaccinating children against influenza in Yemen. Methods A cross-sectional study was conducted by distributing a validated survey questionnaire to potential participants using convenience sampling. Descriptive statistics were used to summarize sociodemographic data, knowledge of influenza vaccines, and attitudes and perceptions regarding vaccinating children against influenza. Logistic regression analysis was employed to identify associations between independent variables and the acceptance of vaccines for children. Results A total of 853 eligible individuals, parents and non-parents, successfully completed the survey. The uptake of the influenza vaccine among the participants was notably low as the majority (69.2%) had not previously received the vaccine, although the majority expressed a willingness to get vaccinated in the future (59.4%). The majority (68.5%) were willing to vaccinate children. The largest percentage of the participants who expressed hesitancy toward children’s vaccination cited multiple reasons to reject the vaccine (39.7%), with concerns regarding the safety of the vaccine being the predominant barrier to its acceptance for children (29.6%). On the other hand, motivating factors for vaccinating children included the validation of the vaccine’s safety and efficacy, endorsement of the vaccine by the government and physicians, integration of the vaccine into the national immunization program, and the provision of the vaccine free of charge and through schools. Significant predictors for vaccine acceptance in children included male gender, knowledge of the protective effect of the influenza vaccine, previous receipt of the vaccine, and a willingness to receive the vaccine in the future. Conclusions The study highlights the need for educational health campaigns to raise awareness and remove misconceptions regarding influenza and the role, benefits, and availability of its vaccine. These findings can serve as a robust foundation for the future design and implementation of an influenza vaccination program for children in Yemen.
