  • Osteoporosis in the Elderly: A Cross-Sectional Study in Kazakhstan

    The aim of this study was to assess bone health in individuals over 60 years of age in Kazakhstan, focusing on the relationship between osteoporosis, body mass index (BMI), body composition, and nutritional factors. This study included 1961 participants, consisting of 1620 women and 341 men, aged 60 to 89. Bone strength was assessed using quantitative ultrasound of the calcaneus, while fracture risk was assessed with the FRAX tool. Osteoporosis was detected in 20.2% of women and 15.2% of men, and osteopenia affected 59.8% of women and 58.4% of men. A total of 73.7% of the participants were overweight, 38.2% were pre-obese, and 35.5% were obese. The results of the study emphasise that, in addition to classic nutrients (calcium, vitamin D, protein), a number of trace elements and vitamins (selenium, iodine, zinc, vitamin B6, phytosterols) also play a significant, possibly indirect, role in bone metabolism. An inverse correlation was observed between BMI and osteoporosis prevalence; with a decrease in BMI, the incidence of osteoporosis increased (women: χ2 = 26.0, df = 2, p < 0.001; men: χ2 = 4.29, df = 2, p < 0.014; total sample: χ2 = 32.3, df = 2, p < 0.001), thus confirming that excess body fat exerts a protective effect on bone health. Significant risk factors for osteoporosis included age, height, and weight. A link was found between the age of first osteoporosis onset and BMI (from 65 to 72.14 years). This confirms the value of FRAX for accurately assessing fracture risk and developing personalised recommendations based on anthropometric and dietary characteristics. Future longitudinal research is warranted to validate these results and further elucidate the underlying mechanisms, including the predictive power of novel anthropometric parameters such as the Body Roundness Index and Body Shape Index.
  • The role of nutrition in maintaining health and physical activity among adolescents assigned to special medical groups

    Objectives This study aimed to assess the dietary patterns of adolescents to develop targeted medical and organizational interventions for optimizing nutrition and physical activity. Methods Research was conducted in Astana in the fall of 2024 and involved 162 adolescents aged 15–19 years (mean age: 17.8 ± 0.41 years) who were classified into special medical groups based on preventive health examinations. Dietary data were collected using the World Health Organization-recommended 24-h recall method, and nutrient intake was assessed against national dietary recommendations and Food and Agriculture Organization/World Health Organization standards. Results The analysis demonstrated that adolescents in special medical groups had significantly reduced daily caloric intake (p<0.001) and an imbalanced macronutrient profile, characterized by excessive consumption of animal fats and simple carbohydrates alongside insufficient intake of polysaccharides and dietary fiber. Widespread deficiencies in essential micronutrients were observed, particularly in vitamins A, D, B1, B6, B9, C, and E, as well as calcium, iodine, phosphorus, magnesium, and iron. These nutritional inadequacies were closely associated with reduced physical endurance, increased fatigue, and lower academic and physical performance, underscoring the urgent need for targeted nutritional interventions. Conclusions These findings emphasize the necessity of dietary interventions and tailored physical activity programs to enhance the overall health and functional capacity of adolescents requiring specialized medical supervision.
  • Restoration of the function of the sciatic nerve and its branches after trauma

    <p><strong>Aim</strong> To investigate the efficacy of various methods for restoring the sciatic nerve and its branches after traumatic injuries to develop optimal treatment strategies, improve functional outcomes, and enhance patients' quality of life.<br /><strong>Methods</strong> A retrospective cohort study was conducted at the Neurosurgical Centre of Almaty, Kazakhstan, based on City Clinical Hospital No. 7. From 2013 to 2022, 227 patients with sciatic nerve lesions and their branches were operated. The proportion of patients of working age was 93.8%. Over half of the patients were hospitalized more than six months after the injury.<br /><strong>Results</strong> A high and satisfactory level of functional recovery after the surgical treatment of the sciatic nerve and its branches was achieved in 173 (77.5%) patients, with partial improvement in 21 (9.4%) and no significant improvement in 30 (13.1%). Two-stage restoration of the sciatic nerve function in cases with diastasis of more than 5 cm improved treatment results in 202 (89.2%) patients contributing to the restoration of motor function and gait within two to three years.<br /><strong>Conclusion</strong> When repairing the sciatic nerve with extensive defects, the tibial nerve is prioritized over the peroneal nerve due to better regeneration. Nerve autoplasty is preferred because of the rigidity of the sciatic nerve trunk and significant muscle load. For diastasis over 7 cm, the peroneal nerve trunk can be used for tibial nerve plasty. Two-stage reconstruction involves tendon-muscle plasty after signs of tibial nerve conduction appear, restoring motor function and gait and improving the patient's quality of life.</p>
