Safety and efficacy of LibidUp-PE supplement on premature ejaculation: A randomized, placebo-controlled double-blind, crossover study Jagadeesan M, Adhisaya A, T. Srinivasan, Alphienes Stanley Xavier, Pavithra Anandan, Rajappan Chandra Satish Kumar, Thangavel Mahalingam Vijayakumar Pharmaceutical Science Advances, 2024 Premature ejaculation (PE) is the most common male sexual dysfunction. Selective serotonin reuptake inhibitors (SSRIs) are currently recommended for PE. However, the side effects of SSRIs, such as nausea, vomiting, and xerostomia, pose challenges to clinicians. Simultaneously, evidence indicates that psychological and behavioral treatments are often insufficient. LibidUp-PE is a nutraceutical designed to enhance sexual wellness and contains a combination of essential amino acids to boost stamina and vigor, addressing concerns like erectile dysfunction (ED). This study aimed to assess the safety and efficacy of LibidUp-PE in the treatment of PE. In this crossover, double-blind, placebo-controlled trial, 76 patients with PE were randomly divided into two groups. Group A received LibidUp-PE, whereas Group B received a placebo for 12 weeks. Following a 2-week wash-out period, the groups switched treatments for another 12 weeks. Intravaginal ejaculatory latency time (IELT) and post-ejaculatory refractory time (PERT) were measured as the primary outcomes to evaluate PE improvement. Plasma serotonin levels were measured as secondary parameters using enzyme-linked immunosorbent assay. The results showed that LibidUp-PE significantly improved the IELT score, increasing from 0.8 ± 0.2 min to 2.9 ± 1.1 min (p < 0.01). PERT scores also significantly decreased from 15.8 ± 1.7 min to 5.6 ± 0.6 min after 12 weeks of LibidUp-PE treatment. Plasma serotonin levels significant increased from 93.6 ± 7.8 ng/mL to 168.4 ± 12.8 ng/mL (p < 0.001) with LibidUp-PE treatment. In contrast, no significant improvements were observed in the placebo group. Notably, none of the participants withdrew their consent due to adverse reactions, which is indicative of the safety and tolerability of LibidUp-PE. In conclusion, 12-week treatment with LibidUp-PE demonstrated a beneficial effect in reducing PE symptoms, as reflected by improved IELT, reduced PERT, and increased plasma serotonin levels. Therefore, LibidUp-PE could be a promising solution for individuals with PE.
Evaluation of Malnutrition by Skinfold Thickness Measurement and SGA-DMS among Haemodialysis Patients: A Cross-Sectional Study M Jagadeesan, Subramanian Sreedhar, T M, Vijayakumar Journal of Medicinal and Chemical Sciences, 2023 Protein-energy malnutrition occurs in a large portion of patients with chronic renal failure. It leads to the consequences of various factors like hormonal imbalance, reduced food intake caused by uremic toxicity, superimposed illnesses, infections, and protein and energy metabolism disturbances. Malnutrition could be acute or chronic in hemodialysis patients. One hundred fifty-two patients were involved in this cross-sectional study; we utilized SGA- DMS (Subjective Global Assessment- Dialysis Malnutrition Score). Percentage body fat through Skinfold Thickness Measurement was calculated using Durnin and Womersley equation. Pearson’s correlation coefficient was calculated to determine the correlation between the assessed parameters and the scores obtained from SGA-DMS. Student t-test was used to analyze the mean percentage of body fat observed between men and women. The values were considered statistically significant if the p-value was <0.05. On assessing the anthropometric parameters, the bicep skinfold thickness was positively correlated (p value=0.012) with the SGA- DMS tool, while all others were negatively correlated with the SGA- DMS. Percentage body fat results imply that patients have higher body fat levels, with males recording 31.34 ± 2.9 and women recording 37.35 ± 2.5, being statistically significant. Our study has not found significance in correlating SGA- DMS and factors like age, weight, etc. However, subjects observed have healthy nutritional status and mainly were over-weight. By employing other methods along with SGA-DMS, a better assessment of malnutrition could be made possible. In addition, efficient nutritional advice from healthcare, adherence to diet patterns, and periodic evaluation may prevent or minimise malnutrition.
Duration of seroprotection of the live attenuated SA-14-14-2 Japanese encephalitis vaccine in children in India L Preethi, M S Alina, Lakshmi Chandran, S Asvin, M Jagadeesan, T M Vijayakumar, V Chitra, Ashok Kumar Pandey, Mahendra M Reddy, Brij Ranjan Misra, Rajni Kant, Prudhvi Lal Bhukya, Gururaj Rao Deshpande, Priya Abraham, Gajanan Sapkal, Kamran Zaman Journal of Travel Medicine, 2023 Background Acute encephalitis syndrome (AES) is a major public health concern in India, and the Japanese Encephalitis (JE) virus is the most common cause of viral encephalitis in Asia affecting children under the age of 15 years. In India, despite the introduction of the JE vaccine (SA-14-14-2) in the immunization programme, JE continues to account for 15–20% of AES cases to date. This study evaluates the immunogenicity of live attenuated SA-14-14-2 JE vaccine in terms of persistence of the humoral response after two doses. Methods A cross-sectional study was conducted among 266 children belonging to one of the JE endemic regions of Uttar Pradesh, India. Blood samples were taken from children (2–10 years) and grouped according to the duration (in years) after two doses of the vaccine (5 groups with a class interval of 2 years). Informed written consent was obtained from the parents/guardians. All the samples collected were tested for the presence of anti-JEV-specific IgG antibodies by enzyme-linked immunosorbent assay (ELISA) and further confirmed by micro neutralization test (MNT) and immunofluorescence assays. Results Of the 266 samples tested by ELISA for anti-JEV-specific IgG antibodies, 260 (97.74%) were negative and 6 (2.26%) were equivocal. The geometric mean immune status ratio across the five groups, 0–2 years (n = 59), 2–4 years (n = 73), 4–6 years (n = 65), 6–8 years (n = 48) and 8–10 years (n = 21) post-two doses of SA-14-14-2 JE vaccine was 1.143, 1.059, 1.138, 1.075 and 1.130, respectively, and the geometric mean titre obtained from MNT across the five groups was 10.77, 8.400, 8.453, 9.517 and 9.674, respectively. Conclusion The study showed a decreasing trend of anti-JEV specific IgG antibody titres across the five groups based on the duration following two doses of SA-14-14-2 vaccine. The results emphasize the significance of booster doses of vaccine for children living in endemic areas.
A study on prescribing patterns in hemodialysis and renal transplantation patients UdayVenkat Mateti, Abdul Nafih, Pradeep Shenoy, Sirimalla Shivaprasad, Jagadeesan Moorthy Egyptian Pharmaceutical Journal, 2022 Background Patients treated with hemodialysis and renal transplant require complex therapy regimens that manage comorbid conditions such as diabetes, hypertension, and so on; as a result, they may develop drug-related issues. Inappropriate medication usage raises the risk of drug related problems, which can manifest as excessively extended hospital stays, higher expenses, and overuse of medical services. Prescribing pattern among the patients treated with hemodialysis and renal transplantation are not well characterized previously. Objectives The objective of the study is to investigate drug prescription trends in hemodialysis patients and to study the prescribing patterns of medications in kidney transplantation patients. Materials and methods The prospective observational study was conducted over a period of 8 months, i.e. from October 2021 to June 2022 in end stage CKD patients treating with maintenance hemodialysis and renal transplant. Different classes of drugs prescribed and percentage of drugs per prescription was estimated in this study. Data were analyzed descriptively. Results 105 patients recruited have been analyzed of which 76 (72.38%) were male and 29 (27.6%) were female. Polypharmacy (use of ≥5 medications) was observed in 91.5% in hemodialysis patients and 100% in renal transplant patients.The most prescribed drugs in hemodialysis patients were Cardiovascular Drugs 72 (100%), and in renal transplant patients, immunosuppressant were highly prescribed 33 (100%). Conclusion This study concludes that the cardiovascular agents and immunosuppressant were the most common drugs prescribed among the hemodialysis and renal transplant patients respectively. polypharmacy among overall patients were observed and it may initiate drug related problems.
A case study on schizophrenia induced multiple comorbidities Jagadeesan M, Kiran Kumar R, Justin Jacob Abraham Asian Journal of Pharmaceutical and Clinical Research, 2018 Schizophrenia is a mental disorder characterized by abnormal social behavior which includes false beliefs, confusion, and auditory hallucination. Antipsychotic drugs therapy increases the risk of developing diabetes mellitus and coronary artery disease (CAD) in schizophrenic patients. Hence, we have planned for a systematic approach toward the management of comorbidities induced in schizophrenic patients. A case study was conducted in 42-year-old female patient diagnosed with schizophrenia along with Type-2 diabetes mellitus, hypothyroidism, diabetic retinopathy, diabetic nephropathy, systemic hypertension, CAD-acute coronary syndrome recent inferior wall myocardial infarction. The patient was treated with atypical antipsychotics, antiplatelets, antianginals, statins, hypoglycemic agents, and other supportive measures. The patient improved symptomatically. The antipsychotic treatment for schizophrenia induces abnormal metabolic syndrome which results in decreased glucose and lipid metabolism that leads to obesity, hyperglycemia, and dyslipidemia associated with cardiovascular risks. Often antipsychotics are combined with benzodiazepines and antiparkinson agents to reduce the risks caused from large doses of antipsychotic medication. However, people receiving first-generation antipsychotics have higher prevalence of developing diabetes mellitus and cardiac risks compared to second-generation antipsychotics. Hence, we conclude that atypical antipsychotic drugs such as amisulpride, aripiprazole, and ziprasidone should be given to schizophrenic patients because these drugs have little effects on abnormal metabolic syndrome when compared to other antipsychotics. There is a need for proper screening of blood glucose level and cardiovascular risks assessment before the administration of antipsychotic medications to schizophrenic patients and also during the course of treatment regularly.
