Peri-implantitis is a multifactorial inflammatory disease that can compromise the longevity of dental implants. Several studies have investigated the association between genetic variants. This systematic review aimed to assess the contribution of genetic polymorphisms to the risk of developing peri-implantitis. A systematic search was conducted in the PubMed database up to 2025 following the PICO strategy and PRISMA guidelines. Eligible studies met the following inclusion criteria: articles written in English, addressing genetic associations with peri-implantitis in human subjects, designed as clinical trials or observational (prospective or retrospective) studies, with full-text availability, and published within the last ten years. Exclusion criteria included studies in languages other than English, those not addressing the main research question, publications older than ten years, studies without full text, and secondary research such as meta-analyses or review articles. Selection and data extraction were performed independently by two reviewers. Risk of bias was evaluated qualitatively for control studies. Twenty-three studies met the inclusion criteria. Polymorphisms in genes related to inflammatory cytokines (e.g., IL-1β +3954 C/T, IL-10, TNF-α), bone metabolism (OPG, BMP4, FGF10), and immune regulation (CD14 rs2569190, miR-27a-3p) were frequently reported. IL-1β +3954 C/T, ET-1 and IL-1β, MMP-8 rs11225395 (T allele), MMP8 (−799 C/T), and CD14 rs2569190 showed consistent associations with increased peri-implantitis risk, while the results for TNF-α −308 G/A were inconsistent across populations. These findings suggest a potential role of genetic predisposition in peri-implantitis development. Identifying genetic biomarkers may help predict susceptibility and personalize management strategies. - Tissue Biomarkers for Predicting the Risk of Oral Cancer in Patients Diagnosed With Oral Leukoplakia: A Systematic Review of the Past 4 Years

BackgroundOral potentially malignant disorders may precede the development of oral cancer, and biomarkers are being investigated for their risk assessment. We aim to provide updated information on tissue biomarkers related to the risk of malignant transformation (MT) in patients with oral leukoplakia (OL) published during the last four years.MethodsA search strategy was developed using the electronic databases PubMed, EBSCO, and Cochrane Library to search for articles related to this topic (published from 2020 to 2024) (CRD42024527395).ResultsOf the 1385 articles identified, 14 were included, with a combined sample of 1479 patients of whom 216 suffered MT. There were 30 different biomarkers evaluated of which Bmi‐1, CD3/CD8, Ki‐67, p16, and DNA ploidy were evaluated in more than one study. Of the 9 studies with multivariate analysis, Bmi‐1, Tipe‐2, copy number alteration, DcR2, and Ki‐67, PTHrP, podoplanin (PDPN), and BubR1/Mad2 biomarkers presented independent significant value on MT. In one study, adding DNA ploidy status to the grade of dysplasia increased the model's predictive power.ConclusionAlthough heterogeneity continues to exist, this systematic review reconfirms the role of biomarkers previously described such as PDPN and DNA ploidy and some new additional biomarkers. - The Relationship of HbA1c Serum Levels with the Severity of Periodontal Disease in Patients with Type 1 Diabetes Mellitus: A Cross-Sectional Study

Objective According to the evidence, the level of glycemic control is of key importance in determining the increased risk of periodontal disease (PD). The aim of the study was to evaluate the role of metabolic control as a key factor leading to the development and severity of periodontitis and compare the periodontal and oral hygiene status with the glycated hemoglobin levels. Materials and Methods The evaluation was undertaken with diabetic patients (59 uncontrolled diabetics and 36 controlled diabetics) from a patient cohort of the Hospitalar Center of Tâmega e Sousa and subjects without diabetes (n = 95). Statistical Analysis The data were analyzed using IBM SPSS Statistics software (Statistical Program for Social Sciences), version 29.0 for Windows. In the logistic regression analysis, odds ratios (ORs) and 95% confidence intervals (CIs) were calculated. The significance level was set at 0.05. Results Periodontal parameters were increased in systemically compromised individuals especially those who were poorly controlled as compared with their healthy counterparts, which are important indicators of PD progression. Furthermore, uncontrolled type 1 diabetic mellitus (T1DM) patients showed increased plaque index (PI), which predisposes these individuals to a greater degree of periodontal destruction and tooth loss. Using a binary logistic regression, we observe a significant relation of the risk of severe periodontitis in T1DM subjects with family history of T1DM (p = 0.019; OR: 3.36; 95% CI: 1.22–9.21), alcohol consumption (p = 0.02; OR: 3.78; 95% CI: 1.23–11.63), periodontal probing depth (PPD) (p < 0.001; OR: 3.64; 95% CI: 14.74–90.34), and clinical attachment loss (p = 0.033; OR: 4.71; 95% CI: 1.13–19.59). Conclusion Increased glycated hemoglobin levels were positively related with periodontal status. Uncontrolled systemically compromised individuals showed an increased Plaque index (PI), which predisposes to greater periodontal inflammation and tooth loss. Increased clinical attachment level, Periodontal probing depth (PPD) and family history of T1DM, and alcohol consumption were identified as potential risk factors for severe periodontitis in subjects with T1DM. - Osteonecrosis of the Jaw Associated with Bisphosphonates Infusion for Treatment of Plasma Cell Myeloma—A Retrospective Observational Study of Northern Portuguese Population

Objectives: To verify medication-related osteonecrosis of the jaw (MRONJ) frequency among patients with plasma cell myeloma (PCM) that had been treated with bisphosphonates, to identify predisposing factors that could influence the development of osteonecrosis. Methods: This observational retrospective study was performed at the Department of Hematology of Hospital Center of Porto (CHUP), Portugal. Results: The study population (n = 112) had a 15.2% (n = 17) prevalence of osteonecrosis. Clinically, bone exposure was the most frequently observed sign, present in 100% (n = 17) of the patients, followed by inflammation in 82.4% (n = 14), orofacial pain in 70.6% (n = 12), suppuration in 47.1% (n = 8), and intra or extra-oral fistula in 17.6% (n = 3) of the cases. The most frequent triggering local factor was dental extraction (82.4%). There was a dependence between the presence of extractions and the development of MRONJ (p < 0.001) but not with the time elapsed from the initiation of infusions with BPs and dental extractions (p = 0.499). In the sample of patients with multiple myeloma (MM), 13.8% were found to be more likely to develop MRONJ after an extraction. Conclusions: The most common local predisposing factor was dental extraction. No dependence was observed between the development of osteonecrosis and the time elapsed from the beginning of treatment with bisphosphonates infusions to surgical procedures. - Osseodensification vs. Conventional Osteotomy: A Case Series with Cone Beam Computed Tomography

Introduction: Osseodensification is a non-extraction technique using specially designed drills to increase bone density while extending an osteotomy, allowing bone to be preserved and condensed by compacting autograft during osteotomy preparation, increasing bone density around implants, and improving mechanical stability. Aim: The objective of this study is to compare conventional osteotomy and osseodensification protocols in implant placement and analyze whether there are differences in bone density. Materials and Methods: Study variables were defined, namely, osseodensification technique, conventional osteotomy technique, bone density, sex, area of location, implant dimensions, implant dimensions, and implant stability. Eligibility and exclusion criteria were defined. A step-by-step surgical protocol was developed. The surgeon and radiologist underwent intra-examiner calibration. A total of 15 patients were selected according to the eligibility criteria, and a total of 41 implants were inserted, 20 implants by conventional osteotomy and 21 by osseodensification. A cone beam computed tomography was performed one year after prosthetic rehabilitation to estimate bone density. Data were collected and recorded, and in the analysis of the association of variables, non-parametric tests were applied. Results: Significant statistical results were found in bone density values, with higher values being obtained with the osseodensification technique, that is, median density values of 1020, and median density values of 732 for the bone drilling technique. The results of the correlation between bone density in both techniques and sex, primary implant stability, implant dimensions and location area were statistically non-significant. Conclusions: Within the limitations of this study, there are differences in bone density between conventional osteotomy and osseodensification protocols. Bone density is increased with osseodensification over a study period of one year. - Analysis of peri-implant bone defects by using cone beam computed tomography (CBCT): an integrative review

The objective of this work was to perform an integrative review of the inspection of peri-implant bone defects using cone beam computed tomography (CBCT). An electronic search was performed in the PubMed database using the following scientific terms: CBCT or Cone Beam computed tomography; dental implant; peri-implant; bone loss; defects. The survey identified 267 studies, of which 18 were considered relevant to this study. These studies provided important data taking into account the accuracy of cone beam computed tomography in the detection and measurement of peri-implant bone defects such as fenestrations, dehiscence and intraosseous circumferential defects. The effectiveness of CBCT in aiding in geometric bone calculations and in the diagnosis of peri-implant defects was influenced by factors such as artefacts, defect size, bone wall thickness, implant material, adjustment of acquisition parameters and observer experience. A not insignificant number of studies compared intraoral radiography to CBCT in the detection of peri-implant bone loss. CBCT was clearly superior to intraoral radiography in the detection of all peri-implant bone defects, except for defects located in the interproximal zone. In general, studies have shown that peri-implant bone measurements adjacent to the implant surface can be correctly determined, as well as the diagnosis of peri-implant bone defects with an average discrepancy of less than 1 mm from the actual measurement of the defect. - Assessment of Oral Health Status in a Prison Population in Northern Portugal

Periodontal disease is a common worldwide oral inflammation/infection affecting tissues that surround and support teeth. This study aims to evaluate the prevalence, extent and severity of periodontal diseases and its risk factors, according to the most recent periodontal classification, in an adult population of Northern Portugal. This observational study collected data from clinical records of patients who attended the University Clinic of Gandra between April 2021 and April 2022. Of a total of 941 patients included, 457 (48.6%) had periodontitis, 253 (26.9%) had gingivitis and the remaining 231 (24.5%) were healthy patients. The prevalence of stage III severe periodontitis was 51.2%, more prevalent in males, and in the age group of 61–70 years. Gingivitis was more prevalent in females, and in the age group of 31–40 years; in both diseases, the most prevalent extension was the generalized one. Using a binary logistic regression, we observe a significant relation of the risk of periodontitis with age (p = 0.019; OR 1.033; 95% CI 1.005–1.062), tooth brushing (p = 0.002; OR 0.25; 95% CI 0.105–0.599) and dental flossing (p = 0.015; OR 0.63; 95% CI 0.09–0.768). This study revealed a high prevalence of periodontitis. Increased age, lack of tooth brushing and flossing were identified as potential risk factors for periodontitis in the investigated Portuguese population. - Podoplanin Expression Independently and Jointly with Oral Epithelial Dysplasia Grade Acts as a Potential Biomarker of Malignant Transformation in Oral Leukoplakia
