Serum Procalcitonin (PCT)-Is there a Role as an Early Biomarker in Infected Diabetic Foot Ulcer (IDFU) Patients? Omar J, Ahmad NS, Che-Soh NAA, Wan-Azman WN, Yaacob NM, Abdul-Ghani NS, Abdullah MR Malaysian Orthopaedic Journal, 2023 Introduction Infected diabetic foot ulcers may lead to serious complications if not recognised in the early stage. Diagnosis of infection is particularly challenging at that stage; thus, a sensitive inflammatory biomarker may be helpful. We aimed to evaluate the role of procalcitonin (PCT) as an early biomarker for infected diabetic foot ulcers (IDFU). Materials and method This cross-sectional study was conducted at Klinik Rawatan Keluarga (KRK), Orthopedic clinic and wards in Hospital Universiti Sains Malaysia (USM) from May 2020 to December 2020. A total of 264 participants were recruited and divided into three groups: 50 diabetic patients with no ulcers (control), 107 patients with non-infected diabetic foot ulcers (NIDFU), and 107 patients with infected diabetic foot ulcers (IDFU). The level of PCT was taken for all patients. Total white count (TWC) and C-reactive protein (CRP) were taken only for IDFU patients. Diagnosis of infection was based on the Infectious Disease Society of America-International Working Group of Diabetic Foot (IDSA-IMWGDF), and the severity of infection was graded according to the Wagner Classification. Results The level of PCT was higher in IDFU than in NIDFU and diabetic patient, with a median (IQR) of 0.355 (0.63) ng/mL, 0.077 (0.15) ng/mL and 0.028 (0.02) ng/mL, respectively. PCT and CRP showed moderate positive correlations in IDFU patients (p<0.001). The sensitivity and specificity were 63.6% and 83.2%, respectively, at the best cut-off at 0.25 ng/mL. Conclusion PCT is a valuable biomarker for the diagnosis of infection; however, it adds little value in the early diagnosis of IDFU in view of its low sensitivity.
Nexus between Periodontal Disease and Chronic Kidney Disease: A NarrativeReview Nurul Aliya Abd Rahman, Muhammad Hafiz Hanafi, Hanim Afzan Ibrahim, Julia Omar, Ahmed Chaudhry, KNS Sirajudeen, Nur Karyatee Kassim Bangladesh Journal of Medical Science, 2023 Background:Several decades of research have established the relationships between systemic diseases and periodontal diseases. Chronic kidney disease (CKD) is a chronic medical condition in which the homeostatic and emunctory activity of the kidneys is progressively declines. Periodontitis is a complex, polymicrobial disease that involves both the host and the environment. Tissue destruction is primarily associated with the host’s hyperresponsiveness, resulting in the release of inflammatory markers. Aim: This paper reviewed the evidence linking CKD, inflammatory markers, and periodontal disease and the effect of periodontal therapy on inflammatory markers and kidney function as well as dental parameters. Setting and Design: The sources of data were compiledand reviewed from MEDLINE, SCOPUS and Web of Sciences from 2010 to 2021. Result and Discussion: This review identifies biologically plausible bidirectional nexus between periodontitis and CKD. Periodontitis has emerged as non-traditional risk factor of CKD and vice versa. In addition, inflammatory markers are considered to play a role in the linkages between periodontitis and CKD. Recent study, has linked an increase in the production of inflammatory markers to a poorer renal outcome in patients with CKD. Periodontal therapy is effective in lowering the inflammatory markers levels and periodontal parameters as well as halting the progression of CKD. Conclusion : Understanding these links may help in identifying high-risk individuals and providing essential care at an early stage. Identification of Potential Biomarkers and Metabolic Changes in the Serum of Breast Lump Patients Among Kelantanese Based on 1H NMR Metabolomics Ahmad Zulkefly, Wan Norlina Wan Azman, Julia Omar, Nurul Syakima Ab Mutalib, Desy Fitrya Syamsumir, M Maulidiani, Faridah Abas, Maya Mazuwin Yahya, Muhamad Syahmi Yamin, Wan Faiziah Wan Abdul Rahman, Tengku Ahmad Damitri Al-Astani Tengku Din Progress in Microbes and Molecular Biology, 2023 This case-control study, conducted at the Hospital USM BestARi unit, aimed to identify the serum metabolic fingerprint among individuals with breast lumps and healthy controls, and to discover potential biomarkers. Serum samples from healthy controls, benign breast lump patients, and malignant breast lump patients were analyzed using proton nuclear magnetic resonance spectroscopy (1H NMR). A multivariate data analysis approach was employed, with the OPLS-DA and clustered heat map techniques effectively differentiating between the three groups. The study revealed significant metabolite variations across the groups and proposed D-glucose, glycerol, and glycine as potential biomarkers for breast cancer diagnosis. Metabolic pathways such as alanine, aspartate, glutamate metabolism, and glycine, serine, and threonine metabolism were implicated. The metabolomics approach coupled with multivariate analysis successfully identified key metabolites leading to group separation and suggested altered metabolic pathways. However, further research and integration with other ‘omics’ technologies are necessary for clinical translation.
Levels of PIVKA-II and alpha-fetoprotein in unresectable hepatocellular carcinoma compared to healthy controls and predictive values of both markers with radiological responses after loco-regional interventions Norhanan Hamzah, Nur Karyatee Kassim, Julia Omar, Mohd Shafie Abdullah, Yeong Yeh Lee Peerj, 2023 Background The significance of the current study was to determine normative levels of PIVKA-II and AFP in patients with unresectable HCC and healthy participants. The second goal was to assess the roles of PIVKA-II and AFP in predicting radiological response after loco-regional therapy. Methods This prospective cohort study enrolled consecutive samples of HCC patients and healthy controls. Venous blood samples were obtained at baseline and after interventions to determine serum levels of PIVKA-II and AFP using the chemiluminescent microparticle immunoassay method. Radiologic responses were determined based on the WHO criteria. Results Fifty-four HCC patients (mean age 58.9 years, 49 males) and 40 healthy controls (mean age 33.5 years, 26 males) were recruited. The median serum levels of PIVKA-II and AFP in HCC vs. healthy controls were 988.4 vs. 24.2 mAU/ml and 13.6 vs. 1.7 ng/ml, respectively (both p < 0.001). With ROC curve analysis, the area under the curve (AUC) for PIVKA-II was 0.95 95% CI [0.90–0.99], and for AFP it was 0.98, 95% CI [0.95–1.0]). The cut-off value for PIVKA-II was 41.4 mAU/ml, and AFP was 4.8 ng/ml. PIVKA-II levels correlated significantly with radiological responses (r = 0.64, p = 0.02) but not AFP (r = 0.09, p = 0.2). Conclusion PIVKA-II and AFP levels are distinctive between unresectable HCC and healthy controls. However, PIVKA-II, not AFP, can predict the radiological response after loco-regional therapy.
The Association Between the Serum Level of IGF-1 and IGFBP-3 and the Risk of Breast Cancer among Women in the Gaza Strip Heba Arafat, Julia Omar, Noorazliyana Shafii, Ihab Naser, Nahed Al Laham, Rosediani Muhamad, Tengku Ahmad Damitri Al-Astani, Ohood Shamallakh, Kholoud Shamallakh Asian Pacific Journal of Cancer Prevention, 2023 OBJECTIVE The purpose of this research was to look at the relationship between insulin-like growth factor-1 (IGF-1) and insulin-like growth factor binding protein-3 (IGFBP-3) levels and the risk for breast cancer (BC) among women in the Gaza Strip. METHODS This case-control study, which included 334 participants (112 women with BC as cases, and 222 women without BC as control), was conducted between January 2021 and August 2022. Research data for the cases were gathered at the Oncology Departments of the Gaza Strip's two hospitals, "Al- Shifa Hospital and Turkish Palestinian Friendship Hospital," as well as from the electronic records of the Screening Mammogram Unit at Al- Remal Clinic for controls. The information about the participants was gathered using a standardized questionnaire. The key variables related to BC were identified using multivariate logistic regression. RESULTS According to multivariate logistic regression, participants' age was associated with an increase in the risk of BC (OR= 1.03; 95%CI, 1.007-1.060). There was an association between serum concentrations of fasting blood glucose (FBG) (OR= 1.027; 95% CI, 1.013-1.042), IGF-1 (OR= 1.010; 95% CI, 1.006-1.015), and the risk of BC, while there was no link between IGFBP-3 and the risk of BC. In an analysis of the risk according to menopausal status, premenopausal women were associated with an approximate 0.5 time decrease in risk of BC compared to women in post-menopause (OR= 0.428, 95% CI, 0.258, 0.710). Dairy product was also related to a decreased risk of BC. CONCLUSION The results suggest that age, lower physical activity, increased levels of FBG, and IGF- 1 increase the BC risk among females in the Gaza Strip. Meanwhile, premenopausal women and dairy products are linked to a reduction in the risk of BC. Furthermore, no link was found between IGFBP-3 and BC risk. Improving early BC detection rates in the Gaza Strip necessitates preventative interventions and screening for BC in the public and healthcare sectors.
The association between breast cancer and consumption of dairy products: a systematic review Heba Mohammed Arafat, Julia Omar, Noorazliyana Shafii, Ihab Ali Naser, Nahed Ali Al Laham, Rosediani Muhamad, Tengku Ahmad Damitri Al-Astani, Ashraf Jaber Shaqaliah, Ohood Mohammed Shamallakh, Kholoud Mohammed Shamallakh, Mai Abdel Haleem Abusalah Annals of Medicine, 2023 Background Among the world, the most frequently discovered and fatal cancer in women is breast cancer (BC). From the perspective of public health, estimating the BC risk linked to dairy intake may aid in comprehensive management. In epidemiological research data on the association between eating dairy foods and the risk of BC are conflicting. Therefore, we sought to assess the link between dairy food consumption and the development of BC. Main text To summarize and quantify the most recent findings on consuming milk or other dairy foods and the development of BC, we performed a systematic literature review. We checked through several databases for relevant publications published in English up to January 2022. Of the 82 articles identified, only 18 met the inclusion criteria and were analyzed. Nine Prospective, seven Retrospective and two Cross-Sectional studies were finally identified. Conclusions Overall, dairy consumption was inversely associated with the risk of developing breast cancer. Future studies will help elucidate the role of dairy products in human health, and their use within a balanced diet should be considered. KEY MESSAGES The effect of different types of dairy products, and possible dose-response relationships on BC risk remains unknown. Estimating BC risk associated with dairy consumption may help to take the decision-making of physicians and public health policy in developing preventive strategies to reduce its occurrence. This systematic review was conducted to assess dairy consumption and BC risk. Overall, inverse associations were found when looking at dairy consumption and BC risk.

Elevated CA 125 in A Male Patient with Lung Carcinoma: A Laboratory Perspective Julia Omar, Tuan Salwani Tuan Ismail, Nur Karyatee Kassim, Aniza Mohammed Jelani Iium Medical Journal Malaysia, 2022 CA 125 is a tumour marker that is widely used for the management of patients with ovarian malignancy. However, elevated CA 125 has been reported in various non-ovarian cancer patients, including males. This report describes a case of an elevated CA 125 level in a 49-year-old male patient who presented with chronic cough associated with constitutional symptoms. The patient was diagnosed with lung carcinoma based on radiological investigations and tissue histopathological examination. According to recent studies, CA 125 has a potential role as a marker for lung malignancies. However, questions arise about the effectiveness of its measurement for such patients due to its poor specificity and improper utilization of the test. This report focuses on the clinical role of CA 125, the appropriateness of the test request for this patient, the significance of the result, and its implications for the patient and laboratory management.

Effects of Time Delay in Processing Common Clinical Biochemical Parameters in an Accredited Laboratory Julia Omar, Wan Norlina Wan Azman, Tan Say Koon, Nor Aziantie Abdul Wahab, Samantha Xin-Yuin, Law Xi Ling, Chew Hsiao Junn, Azlan Husin, Hans Van Rostenberghe, Mohamed Rusli Abdullah Iium Medical Journal Malaysia, 2022 INTRODUCTION: Analyte stability time is challenging to employ in clinical settings. The diversity of processing time may affect the accuracy of results. This study evaluates the maximum time delay permissible before sample centrifugation and analysis. MATERIALS AND METHODS: We evaluated 15 serum electrolytes in 40 samples centrifuged and analysed using an automated analyser. The time was divided into 1, 2, 6, and 24 hours. RESULTS: Most analytes studied remained stable for up to 24 hours before centrifugation. However, delayed processing affected the value of potassium, magnesium, LDH, and calcium after 2 hours up to a maximum of 6 hours at room temperature for total protein, phosphate, and sodium. CONCLUSIONS: Most analytes were unaffected by a delay in centrifugation at room temperatures, but several critical analytes were severely affected.
