Therapeutic massage with nanostructured lipid carrier gel containing quercetin for muscle injury treatment in rats J.M. Bellé, J.P. de Moraes, R.P. Martins, G.O. Puntel, C. Franco, et al. Brazilian Journal of Medical and Biological Research, 2025 Quercetin has therapeutic potential in the treatment of musculoskeletal lesions, but presents poor oral absorption because of its low water solubility and structural instability. Its penetration through the skin can be enhanced by quercetin-loaded nanostructured lipid carriers (NLC-Q), which is increased when applied with massage, but this has not yet been tested. This study aimed to evaluate the effect of massage with NLC-Q gel on biochemical parameters after a traumatic lesion of the gastrocnemius muscle. Forty-five male Wistar rats were divided into five groups (control, lesion, lesion treated with NLC-Q gel, lesion treated by massage with placebo gel, and lesion treated by massage with NLC-Q gel). The gastrocnemius muscle was lesioned by mechanical crushing, and treatments began 24 h after injury. Massage was performed at 12 h intervals for a total of five 5-min sessions. Serum and muscle creatine kinase (CK) concentrations and muscle oxidative stress (concentration of reactive oxygen species [ROS], lipid peroxidation, protein carbonyls, and activities of superoxide dismutase and catalase enzymes) were evaluated 96 h after lesion formation. Blood CK levels increased in all injured groups (P<0.001); however, the interventions reduced plasma CK compared to the lesion group (P<0.05). Interventions reduced lipid peroxidation (P<0.05), but only the NLC-Q gel and massage with NLC-Q gel reduced the concentration of ROS and protein oxidation in the lesion group (P<0.05). These findings indicated that NLC-Q gel and massage with NLC-Q gel can help repair muscle damage and reduce oxidative stress parameters.

Its skin penetration can be enhanced by nanostructured lipid carriers loaded with quercetin (NLC-Q) applied in combination with pulsed therapeutic ultrasound (PTU). However, different intensities of 1 MHz ultrasound may compromise the physicochemical properties of NLC-Q, and this interaction has not yet been evaluated. The aim of this study was to assess the effects of different 1 MHz PTU intensities on the physicochemical properties of a gel containing nanostructured lipid carriers loaded with quercetin (NLC-Q). NLC-Q was developed using the high shear rate method. PTU (1 MHz, for 5 min, 20% duty cycle) was applied to the gel at intensities of 0.1, 0.2, 0.4, and 0.6 W/cm2 SATA (spatial average temporal average intensity). Physicochemical properties (pH, temperature, mean particle size, polydispersity index, and concentrations of quercetin within the nanostructured lipid carriers) were evaluated before and after the application of different PTU intensities. Intensities of 0.1 and 0.2 W/cm2 SATA did not alter the physicochemical properties of NLC-Q, while intensities of 0.4 and 0.6 W/cm2 SATA increased particle size by 14% and 44%, respectively (P < 0.001). These intensities also increased the polydispersity index by 28% and 88% (P < 0.001). PTU intensities above 0.4 W/cm2 SATA lead to instability of NLC-Q within the gel, which does not favor the topical delivery of the active ingredient.

Sua penetração na pele pode ser potencializada por carreadores lipídicos nanoestruturados carregados com quercetina (NLC-Q) aplicados em associação com o ultrassom terapêutico pulsado (PTU). No entanto, diferentes intensidades do ultrassom de 1 MHz podem comprometer as propriedades físico-químicas das NLC-Q, porém esta interação ainda não foi avaliada. O objetivo desta pesquisa foi avaliar os efeitos de diferentes intensidades do PTU de 1 MHz nas propriedades físico-químicas de um gel contendo carreadores lipídicos nanoestruturados carregados com quercetina (NLC-Q). As NLC-Q foram desenvolvidas pelo método de alta taxa de cisalhamento. O PTU (1 MHz, por 5 min, ciclo de trabalho de 20%) foi aplicado ao gel nas intensidades de 0,1, 0,2, 0,4 e 0,6 W/ cm2 SATA (intensidade espacial média-temporal). As propriedades físico-químicas (pH, temperatura, tamanho médio das partículas, índice de polidispersão e as concentrações de quercetina contidas nos carreadores lipídicos nanoestruturados) foram avaliados antes e após as aplicações das diferentes intensidades do PTU. As intensidades de 0,1 e 0,2 W/cm2 SATA não alteraram as propriedades físico-químicas das NLC-Q, enquanto as intensidades de 0,4 e 0,6 W/cm2 SATA aumentaram o tamanho das partículas respectivamente em 14% e 44% (P < 0,001). Essas intensidades também aumentaram o índice de polidispersão em 28% e 88% (P < 0,001). Intensidades maiores que 0,4 W/cm2 SATA do PTU promovem a instabilidade das NLC-Q contido no gel, o que não favorece a administração tópica do ingrediente ativo.

Low-intensity therapeutic ultrasound (LITUS) may influence endothelial function, but the effects remain uncertain.Objective This systematic review with meta-analysis of clinical trials aims to evaluate the effects of LITUS on endothelial function in humans.Methods Placebo-controlled randomized clinical trials evaluating the effects of LITUS on endothelial function in humans were included. The studies were registered on PROSPERO (CRD42024534383). Data were extracted from MEDLINE (PubMed), Cochrane CENTRAL, SCIELO, EBSCO, EMBASE, and Web of Science databases, with no restrictions on year of publication or language, up to February 6, 2025. Methodological risk of bias was assessed using the Cochrane Risk of Bias 2.0 tool. Meta-analyses were conducted using Review Manager version 5.3, and the certainty of evidence was evaluated by GRADE.Results Five randomized crossover clinical trials met the inclusion criteria, including data from 103 subjects (intervention and placebo). The methods used to evaluate endothelial function included flow-mediated dilation (in three studies), venous occlusion plethysmography, and laser Doppler flowmetry. The application of LITUS increased the endothelial function in pulsed (standard mean difference [SMD]: 0.88; CI 95%: 0.59 to 1.17; I2: 0%) and continuous (SMD: 1.09; CI 95%: 0.70 to 1.48; I2: 38%) waveforms. The certainty of the evidence was considered “moderate” for pulsed and continuous waveforms.Conclusion The pulsed and continuous waveforms of the LITUS improved endothelial function. These results indicates that the application of therapeutic ultrasound has benefits on vascular homeostasis.
