    Background The clinical, epidemiological and immunopathological profiles of atypical forms of pemphigus remain only partially known. Objectives To define the clinical, epidemiological and immunological characteristics, therapies and outcomes in patients with atypical pemphigus variants. Methods This was a 14-year multicentre retrospective observational study (VARIANT_P) on atypical variants of pemphigus across Italy. We collected demographic, immunopathological and clinical data, as well as information on comorbidities and prescribed treatments. Results We enrolled 61 patients [female/male sex ratio 1.77; 13 paraneoplastic pemphigus (PNP), 26 IgA pemphigus (PIgA), 22 pemphigus herpetiformis (PH)]. The median ages at onset and diagnosis were 70.6 (range 43.1–86.8) and 71.1 (range 46.9–86.9) for PNP; 62.2 (range 3.8–81.0) and 63.6 (range 4.0–82.4) for PIgA; and 49.4 (range 5.4–84.4) and 52.3 (range 5.9–85.9) for PH, respectively. The median diagnostic delay was 3.0 (range 0.0–45.6) months for PNP, 9.5 (range 1.0–140.0) months for PIgA and 2.0 (range 0–30.4) months for PH. The mortality rate was 55% (6/11) for PNP, 4% (1/26) for PIgA and 6% (1/17) for PH. Cutaneous involvement was present in all patients with PIgA and PH, and in 83% (10/12) of the patients with PNP. In contrast, oral mucosal involvement was observed in all patients with PNP with data (n = 12), but only in 8% (2/26) of those with PIgA and 21% (4/19) of those with PH. Histology, direct immunofluorescence, indirect immunofluorescence and enzyme-linked immunosorbent assay data demonstrated variable concordance with previously known data. Comorbidities included mainly solid malignancies for people with PNP, whereas cardiovascular and metabolic diseases were the most prevalent for those with PIgA and PH. Treatment mostly relied on systemic steroids and rituximab. Conclusions The VARIANT_P study contributes to data collection relating to atypical pemphigus variants in order to promote the development of specific therapeutical guidelines in the future.

    Dermatitis herpetiformis (DH) is the specific cutaneous manifestation of celiac disease (CD), both of which require lifelong management. A comprehensive literature review was conducted to gather evidence on gender-related differences aiming at reducing healthcare inequalities. While women tend to be more symptomatic, the diagnosis is often delayed, leading to a reduced quality of life and an increased risk of long-term complications. This may be partly a consequence of the fact that they are more likely to present with seronegative forms. Moreover, in women, reproductive issues, known to be associated with DH and CD, along with osteopenia/osteoporosis, and anemia are particularly prevalent. It should be also noted that beyond personal diagnosis, DH and CD also affect women indirectly, as they often take on caregiving roles for affected family members. The lifelong adherence to a gluten-free diet, the only effective treatment for DH, imposes a substantial lifestyle and financial burden. Gluten-free diet is frequently perceived as both gendered and class-based, placing a disproportionate strain on women and lower-income households. In this regard, recent recommendations to prioritize naturally gluten-free foods rather than "gluten-free" products, may offer some relief. However, the responsibility for acquiring the skills necessary for such dietary choices often falls predominantly on women.
  • An Overview of the Pathogenesis of Cutaneous Lupus Erythematosus

    Background/Objectives: Cutaneous lupus erythematosus (CLE) is a complex autoimmune skin disease driven by genetic predisposition, environmental triggers, and immune dysregulation. Environmental factors such as ultraviolet radiation, smoking, and certain drugs can initiate disease onset by inducing keratinocyte apoptosis. The subsequent release of nucleic acids and danger-associated molecular patterns activates pattern recognition receptors (PRRs) on keratinocytes and immune cells, leading to the production of type I and type III interferons (IFNs) and pro-inflammatory cytokines. The objective of this review is to summarize recent advances in understanding the immunopathogenesis of CLE, with particular attention to emerging cellular players and their therapeutic implications. Methods: A narrative review of the recent literature was performed, including experimental, translational, and clinical studies investigating the cellular and molecular mechanisms underlying CLE and novel targeted treatments derived from these findings. Results: Although plasmacytoid dendritic cells (pDCs) have traditionally been considered the major producers of IFN-I, recent data indicate that pDCs in CLE are functionally impaired and are not the primary source. Other cells, such as keratinocytes have emerged as key producers of IFN-I, contributing to a prelesional, IFN-rich microenvironment. This promotes the recruitment and activation of dendritic cells and other inflammatory myeloid subsets, which are now recognized as central players in amplifying local inflammation. Concurrently, T cells infiltrate the skin, where cytotoxic CD8+ T cells attack keratinocytes and CD4+ T cells further propagate inflammation via cytokine production. B cells and plasma cells produce autoantibodies, forming immune complexes that perpetuate inflammation. Neutrophils release neutrophil extracellular traps (NETs), exposing autoantigens and further stimulating IFN pathways. Macrophages contribute by presenting autoantigens, producing pro-inflammatory mediators, and failing to effectively clear apoptotic cells and immune complexes. Conclusions: The dynamic interplay between the innate and adaptive immune systems sustains the chronic inflammatory state characteristic of CLE. Based on the pathogenetic novelties, new therapeutic agents targeting specific molecules have been developed, which may improve the treatment of this complex disease in the future.
  • The impact of antibiotic therapy in psoriasis patients with active streptococcal infection: A prospective study

    The association between psoriasis and streptococcal infection has been widely explored in both children and adults. However, the exact impact of Streptococcus pharyngeal infection on the course of psoriasis is not fully comprehended. This study explored the impact of Streptococcus pharyngeal infection on psoriasis and investigated the effectiveness of systemic antibiotic therapy in conjunction with standard topical treatment for psoriatic patients with concomitant streptococcal throat infection. The research involved 115 patients with mild‐to‐moderate psoriasis, clinically assessed using the Psoriasis Area and Severity Index (PASI). Patients with active streptococcal infection were administered adjunctive systemic antibiotic therapy along with standard local treatment for psoriasis, while psoriasis patients without evidence of infection received the local topical treatment only. Streptococcal infections were more common in psoriasis patients compared to healthy controls. A group of psoriasis patients with active streptococcal throat infections, treated with antibiotics in addition to standard topical psoriasis therapy, did not show any difference in PASI score reduction compared to those without evidence of active infection. While our study did not show a statistically significant reduction in PASI scores in psoriasis patients with streptococcal throat infections treated with antibiotics, it highlights the complex interaction between infection and psoriasis. Larger studies with longer follow‐up may better clarify this relationship, contributing to stronger evidence for or against the use of antibiotics in managing psoriasis triggered by streptococcal infections.
  • Stevens-Johnson syndrome/toxic epidermal necrolysis induced by lamotrigine in a patient with a cerebral cavernous malformation: a case report

    Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are rare but serious cutaneous reactions characterized by epidermal and mucocutaneous detachment, most often drug-induced. SJS and TEN are considered the opposite extremes of the same spectrum of disease, where the percentage of skin involvement is <10% in SJS and >30% in TEN; the in-between range is called an SJS/TEN overlap. We present the case of a 64-year-old patient who was treated with lamotrigine, an anti-epileptic drug, and developed SJS/TEN. After being hospitalized and recovering for three days due to the worsening of the clinical presentation, he was transferred to a burn center. Making an early diagnosis and identifying the appropriate drug is crucial for setting the correct treatment and reducing mortality. Advanced supportive care is required.

    Bullous Pemphigoid (BP) is a common autoimmune blistering disorder with unknown aetiology. During the last decades, its association with various common comorbidities, including cardiovascular, metabolic, neuropsychiatric, and neoplastic disorders, has been established. However, in recent years an increasing number of BP cases have also been reported in association with rarer diseases, including Acquired Reactive Perforating Collagenosis (ARPC). Patients with coexisting BP and ARPC have been reported to share common clinical features including metabolic comorbidities e.g., Diabetes Mellitus (DM). As the evolution from ARPC cutaneous involvement to classic BP lesions has been more frequently described, it has been suggested that it may represent a new clinical variant of BP with a specific pathogenetic background. Here is reported a challenging case in which a typical onset of BP was later followed by the eruption of atypical ARPC lesions in a patient with multiple non-compensated metabolic and cardiovascular comorbidities.

    We read with great interest ‘New onset of systemic lupus erythematosus following COVID-19 mRNA vaccination’ by Hansen et al.1 The authors present the case of a 26-year-old female with no family or personal history of systemic lupus erythematosus (SLE) who developed SLE 2 weeks after the second dose of the COVID-19 mRNA vaccination. Her clinical history was not characterized by photosensitivity or other subtle signs of a latent autoimmune condition. However, along with the onset of specific cutaneous manifestations, the laboratory findings, including mild leukopenia, positive ANA, ENA antibodies, ds-DNA antibodies, positive RNP/Sm antibodies and decreased C3 levels, were compatible with an autoimmune preclinical condition that may have been triggered by vaccination. In fact, it has been observed that autoantibodies become detectable many years before the diagnosis of SLE, and their appearance tends to follow a predictable course.2 In our perspective, it is crucial to differentiate between latent presentations and inductions. In fact, it is known that inductions have a more favourable course, while exacerbations often follow a more challenging and complex path.3, 4 In this regard, the laboratory findings, particularly the autoantibody profile, may serve as a biomarker to differentiate between the two conditions. Accordingly, we recently described a subacute cutaneous lupus erythematosus (SCLE) presenting after Pfizer COVID-19 vaccination but serologically negative,5 with no signs of recurrence nearly 2 years later. In a similar manner, drug-induced cases of dermatomyositis (DM) have been mostly reported to be characterized by pathognomic skin findings (85%) and negative serological markers.6 The potential mechanisms by which COVID-19 vaccines may trigger autoimmunity in genetically predisposed individuals have been reported to be molecular mimicry, autoantibody production and activation of the inflammasome by vaccine adjuvants.6 Notably, Sprent and King hypothesized that most reactions to COVID-19 vaccines are driven by the production of interferon type 1 (IFN-I).7 IFN-I, primarily produced by plasmacytoid dendritic cells, is a known a central player in the pathogenesis of SLE, SCLE and DM. In particular, COVID-19 infection8 and COVID-19 mRNA vaccination9 have been suggested to induce DM as a result of the production of IFN-I, promoted by the apoptosis and amplification of the innate immune response.7 However, inquiries into whether the mechanism activated in vaccine-induced DM may differ from the conventional one, potentially impacting the prognosis9 have been made. In conclusion, drug-induced interferonopathies may be characterized by specific pathogenetic pathways and usually have a more favourable outcome than latent forms.
