Donor Human Milk Fat Content Is Associated with Maternal Body Mass Index Lucíola Sant’Anna de Castro, Bernardo Lessa Horta, Rebeca de Freitas Paiva, Ana Carolina Lavio Rocha, Mina Desai, et al. Breastfeeding Medicine, 2025 Introduction: Donor human milk is increasingly being utilized for both preterm and term infants when mother's milk is unavailable. With the rising prevalence of maternal overweight and obesity, it is crucial to evaluate the relationship between maternal body mass index and the fat and energy content of donor human milk. Objectives: To assess the impact of maternal body mass index on human milk fat content. Methods: A cross-sectional study was carried out using retrospective data from women who made their first human milk donation at ≥15 days postpartum at a human milk bank in São Paulo, Brazil, from January 2018 to December 2020. Data of sociodemographic, obstetric, health, and anthropometric measures were collected by the human milk bank staff. Milk fat and energy content were determined using the crematocrit test. Analysis of variance and multiple linear regression were used to compare means of crematocrit and fat. We determined the p-values using a test of heterogeneity and linear trend and presented the one with the lower p-value. Results: Most donors were between 25 and 35 years old, had higher education, were employed, and lived with a partner. At the time of milk donation, 40.9% of women were overweight or obese. The fat (1.09 g/dL) and energy (9.83 kcal/dL) content of human milk were higher in obese donor compared with eutrophic donors. Conclusions: The fat and energy content of human milk were associated with maternal body mass index, suggesting the potential value for selective use of high fat and high calorie donor milk for very low birthweight or premature infants.
Composition of Breast Milk in Women with Obesity Michael G. Ross, Kelly P. Coca, Ana Carolina Lavio Rocha, Bárbara Tideman Sartório Camargo, Luciola Sant’Anna de Castro, et al. Journal of Clinical Medicine, 2024 Background/Objectives: Among US breastfeeding women, those with obesity have significantly increased breast milk fat and caloric content from foremilk to hindmilk, with a 4-fold increase in fat content from the first to last milk sample. In view of different dietary norms and nutritional standards, we sought to evaluate the relationship between maternal BMI with breast milk fat and calorie content in women from Brazil, a low–middle-income country. Methods: Women who delivered singleton-term neonates were recruited from the Ana Abrao Breastfeeding Center (AABC) and Human Milk Bank at the Federal University of Sao Paulo, Brazil. These women were then studied at 7–8 weeks postpartum. Women were grouped by BMI categories of nonobese (NonOB; BMI 18.5–29.9) and obese (OB; BMI ≥ 30). A breast pump was applied, and milk samples were obtained continuously in 10 mL aliquots from foremilk to hindmilk; samples were analyzed for macronutrients and lipids, and maternal blood was analyzed for serum lipids and glucose. Results: As compared to NonOB women, those with OB had significantly higher milk fat in the mid (4.9 ± 0.3 vs. 3.9 ± 0.2) and last hindmilk (6.6 ± 0.4 vs. 5.5 ± 0.3) samples, though not in the first foremilk sample, as compared to NonOB women. In both NonOB and OB subjects, milk caloric and fat content increased 1.5 to 2-fold from foremilk to hindmilk, with the average milk caloric value being 11% greater in OB women. Protein content was significantly increased in all three milk samples (first, middle, and last) in women with OB. Conclusions: Although the value of breastfeeding remains clear, these findings may have significant implications for infant nutrition and excessive infant weight gain in women with OB.
Factors associated to the consumption of ultra-processed foods and sugar-sweetened beverages in infants under 24 months of age Vanizia Barboza da Silva Maciel, Kleynianne Medeiros de Mendonça Costa, Kelly Pereira Coca, Lucíola Sant’Anna de Castro, Juliana dos Santos Vaz, et al. Revista De Nutricao, 2024 Objective To review the consumption of ultra-processed foods and sugar-sweetened beverages among children under 24 months of age in an Amazonian municipality. Methods A cross-sectional study was carried out during the National Vaccination Campaigns in the years 2016 and 2017 using a two-stage cluster sampling approach. The survey instrument was based on the questionnaire from the II Breastfeeding Prevalence Survey issued by the Ministry of Health, Brazil. Data collection was carried out with the mothers or caregivers. Poisson regression with robust variance was used, with a significance level set at less than 5%. Results A total of 688 children were evaluated. The consumption of ultra-processed foods was 98.9% and that of sugar-sweetened beverages was 72.7%. Factors associated with such consumption were low paternal education, lower total family income, living in rural areas, and the child’s age range between 6 and 11 months. Conclusion The consumption of ultra-processed foods and sugar-sweetened beverages was high considering the recommendations of the Brazilian Ministry of Health. The factors identified as contributing to this practice emphasize the necessity of supporting the development of interventions to promote healthy eating habits.
Food diversity among indigenous children from two municipalities of the brazilian western amazon Vanizia Barboza da Silva Maciel, Kelly Pereira Coca, Lucíola Sant’Anna de Castro, Ana Cristina Freitas de Vilhena Abrão Ciencia E Saude Coletiva, 2021 Resumo Objetivou-se explorar a prática alimentar das crianças indígenas de zero a dois anos, residentes nos municípios de Cruzeiro do Sul e Mâncio Lima no Acre. Realizou-se estudo transversal com crianças indígenas das etnias Katukina (Cruzeiro do Sul), Nukini, Nawa e Poyanawa (Mâncio Lima) em outubro de 2013. Utilizou-se um questionário estruturado sobre o consumo alimentar do dia anterior à coleta, baseado nos indicadores alimentares propostos pelo Ministério da Saúde. Estudou-se 94 crianças (50% da etnia Katukina, 27% Poyanawa, 13% Nukini e 10% da Nawa). As crianças menores de 6 meses de idade, apesar da maioria receber leite materno, 42,1% delas consumiam água, 15,8% comida de sal e 11,1% preparados típicos da região. Nas faixas de idade de 6 a 12 meses e 13 a 23 meses identificou-se um alto consumo de alimentos ultraprocessados (52,6% e 28,6% respectivamente) enquanto que o consumo de alimentos complementares relevou-se insuficiente (33% de frutas e 25% de comida de sal na faixa de 6 a 13 meses e 41,2% de frutas e 19,6% de comida de sal na faixa de 13 a 23 meses). O perfil alimentar das crianças indígenas mostrou-se inadequado para a idade, refletindo uma situação preocupante na saúde infantil desta população.
Food safety and quality of distribution of raw human milk from a University Hospital Marina Aparecida da Silva MORENO, Lucíola Sant’Anna de CASTRO, Ana Cristina Freitas de Vilhena ABRÃO, Kelly Pereira COCA Revista De Nutricao, 2018 Objective To evaluate the quality of raw human milk distributed in the Neonatal Intensive Care Unit of a University Hospital of the city of São Paulo. Methods A cross-sectional study with raw human milk samples from mothers who attended the Human Milk Collection Station of a University Hospital, analyzed between May 2016 and January 2017, excluding mothers of twins. The quality of the raw human milk was assessed by verifying the presence of dirt, the coloration of the milk, the titratable acidity using the Dornic method, and through its energy content. Kruskal-Wallis and Mann-Whitney tests were used for the analysis of the energy profile and the degree of Dornic acidity, according to the stage of the raw human milk and the gestational age of the child. Results The study was composed of 40 samples of 40 different women, with a mean age of 27 years, an average of 11.8 years of education, most of them were multiparous and with a partner. Regarding milk analysis, 55.0% was classified as colostrum, 27.5% as mature milk and 17.5% as transitional milk. All samples presented negative results for dirt and normal coloration. The mean milk acidity was 3.24º Dornic and most of the samples were classified as hypercaloric energy content. There was no association between the lactation stage and gestational age with the acidity value and energy content. Conclusion The quality of raw human milk distributed in the Neonatal Intensive Care Unit of the institution evaluated was considered adequate and the samples analyzed had a high energy content and excellent Dornic acidity.
