  • Microvascular Angina Pectoris: The Pathophysiology of Decreased Coronary Reserve
    M. A. Kravchenko, L. D. Khidirova
Russian Archives of Internal Medicine, 2026
A review of modern Russian and foreign literature on the problem of impaired coronary blood flow reserve as a manifestation of microvascular dysfunction has been conducted. When searching for information on this issue, materials from the following databases were used: RSCI, Best Evidence, Scopus, Elsevier, PubMed, Clinical Evidence, Cochrane Library. Violation of the coronary blood flow reserve is an important manifestation of microvascular dysfunction and a key diagnostic criterion for microvascular angina. The evaluation of this indicator has high prognostic and therapeutic significance. Given the heterogeneity of pathogenesis, further research is needed for a personalized approach to the treatment of patients with microvascular angina.
  • Ischemic Heart Disease After SARS-CoV-2 Infection: A Cross-Sectional Study

    Kazan Medical Journal, 2025
BACKGROUND: The incidence of newly diagnosed stable angina after a SARS-CoV-2 infection remains high. However, studies on the clinical, functional, and laboratory characteristics of this disease are inadequate. The manifestations of ischemic heart disease (IHD) following SARS-CoV-2 infection require further investigation to elucidate the pathophysiological mechanisms and personalize follow-up care.AIM: This study aimed to compare the clinical, functional, laboratory, and angiographic characteristics of stable angina in patients diagnosed with IHD following SARS-CoV-2 infection.METHODS: This cross-sectional study assessed patients’ medical histories, along with clinical, laboratory, and instrumental findings. The study enrolled 428 patients with stable IHD and documented SARS-CoV-2 infection (within 3–18 months of the onset of the disease). Group 1 (n = 195) consisted of patients with newly diagnosed IHD, and group 2 (n = 233) included patients with previously diagnosed IHD, with the infection being documented to have occurred ≥12 weeks prior to study enrollment. IHD was diagnosed through a comprehensive evaluation that included clinical presentation, electrocardiographic analysis, echocardiographic imaging, stress tests, and coronary angiography. Blood chemistry was performed using standardized methods. Statistical analysis was carried out in RStudio. Continuous variables were compared using the Mann–Whitney U test, whereas the comparison of binary and categorical variables was conducted using Fisher’s exact test. The critical significance level was set at p = 0.05.RESULTS: Group 1 was significantly younger (p = 0.009), had a lower body mass index (p 0.001), and had a shorter history of hypertension (p 0.001). These patients exhibited higher prevalence of functional class I angina pectoris (p = 0.006), NYHA class II chronic heart failure (p = 0.005), and lower prevalence of ventricular extrasystole (p = 0.002). Coronary angiography demonstrated that group 1 showed higher prevalence of unchanged coronary arteries (p = 0.003) and hemodynamically insignificant stenoses (p = 0.018), but lower proportion of hemodynamically significant stenoses (p 0.001). There were no significant differences found in the occurrence of multifocal atherosclerosis (p = 0.132). The treadmill test revealed comparable positive results across both groups (p = 0.479). However, group 1 exhibited a higher frequency of inconclusive results (p = 0.011). The laboratory profile of patients from group 2 showed increased Lp(a) (p = 0.023), triglycerides (p = 0.003), apoB (p = 0.022), NT-proBNP (p = 0.010), D-dimer (p = 0.001), high sensitivity C-reactive protein (p = 0.012), and cystatin C (p = 0.001), as well as higher fasting glucose (p = 0.030), but comparable HbA1c levels (p = 0.750).CONCLUSION: Patients who develop stable angina following SARS-CoV-2 infection demonstrate a less severe clinical and functional phenotype, less significant coronary atherosclerosis, and more favorable biomarker profile than those with previously diagnosed IHD who had SARS-CoV-2 infection.
  • Modern strategies for early diagnosis of cardiotoxicity caused by chemotherapy
    A.A. Lykova, L.D. Khidirova
    Profilakticheskaya Meditsina, 2025
Cardiotoxicity due to antitumor chemotherapy and targeted therapy is a severe complication that reduces the effectiveness of treatment and worsens the prognosis in patients with malignancies. Therefore, early diagnosis of myocardial injury is particularly relevant, allowing for a timely adjustment of the therapeutic approach. Objective. To evaluate current cardiovascular imaging and monitoring techniques used for early detection of cardiotoxicity. Materials and methods. The review methodology involved searching and analyzing scientific data from Russian and international publications, selected by keywords and topics, in the PubMed, Scopus, Web of Science, and eLibrary databases for the period from 2015 to 2025, considering relevance and scientific significance criteria. Results. Two-dimensional and three-dimensional echocardiography capabilities are presented, including evaluation of global longitudinal myocardial strain as a sensitive marker of early systolic dysfunction. The diagnostic advantages of magnetic resonance imaging (MRI) of the heart, which provides an accurate quantitative assessment of myocardial morphology and tissue characteristics, are highlighted. Radioisotope methods, including scintigraphy and positron emission tomography, are considered to assess perfusion, metabolism, and cellular damage. The advantages of using myocardial damage biomarkers to detect cardiotoxicity in the early stages were analyzed. Conclusion. The integration of these methods into clinical practice contributes to personalized monitoring, increased safety of anticancer treatment, and a reduced risk of cardiovascular complications.

  • The way to effective prevention of cardiovascular complications: review
    L. D. Khidirova, S. G. Zakoryukina
Cardiovascular Therapy and Prevention Russian Federation, 2024
A descriptive review of modern Russian and foreign literature devoted to the polypill concept in internal medicine and cardiology practice is presented.Methodology. The following databases were used in the search for information: PubMed, Scopus, Web of Science and Cochrane Library, as well as Russian databases, including eLIBRARY and CyberLeninka. The search used keywords and phrases such as "polypill", "combination therapy", "cardiovascular diseases", "adherence", "polycomponent drugs" and their analogues in English and Russian. Particular attention was paid to studies published over the past 15 years, with an emphasis on studies conducted over the past 5 years to reflect current trends and innovations in the use of polypills.Results. Clinical evidence of the benefits of fixed-dose combinations allows the implementation of the polypill strategy in practice and to achieve maximum reduction of cardiovascular risk.Conclusion. Novel drug combinations have already been developed and are available for use. They need to be introduced so that the therapy is most adapted for many patients and effective in achieving different therapeutic goals in internal medicine and cardiology practice.

  • Cardiotoxicity mechanisms of antitumor therapy with immune checkpoint inhibitors: new achievements

The immune checkpoint inhibitors (ICTs) emergence has opened up new perspectives in cancer immunotherapy. Nevertheless, serious, including life-threatening conditions caused by ICT cardiotoxic effects pose a number of obstacles to clinical specialists. The lack of knowledge about pathophysiology of cardiovascular adverse events in the treatment of ICT tumors is one of the reasons why oncological specialists seek help from cardiologists. Some works consider the mechanisms of individual complications development, but the number of works that would systematize and summarize descriptions of all the most significant ICT inhibitor therapy complications is small. In this regard, a literature review on the use of ICT inhibitors was conducted with a search in PubMed, Embase, Web of Science, e-L ibrary, Google Scholar. The purpose was to analyze the accumulated data on the mechanisms of ICT therapy complications development; Preference was given to systematic reviews, randomized clinical trials, which would be supplemented by separate cohort studies and descriptions of some experiments. Thus, it was determined that the ICT inhibitors cardiotoxicity can affect any part of the cardiovascular system, causing changes in both inflammatory and non-inflammatory etiology. Understanding their mechanisms increases the ability of specialists to form an effective treatment strategy while minimizing the risk of complications. Although a lot of theoretical, experimental and clinical empirical data on the side effects of this class of anticancer drugs have been accumulated in oncological practice, the ICT inhibitors cardiotoxicity is a problem requiring further research.
