  • Beyond prevention: Optimizing reproductive health choices for people living with HIV in India - A look at contraceptive use and unmet need from National Family Health Survey −4 data

    Problem considered: There is limited analysis on family planning needs among People Living with HIV (PLHIV) in India.Although the National Family Health Survey (NFHS) provides national estimates of unmet need for family planning, these data are rarely analyzed separately for PLHIV.This study aimed to describe contraceptive use in PLHIV and estimate the unmet need for family planning among WLHIV by analysing data from NFHS-4. Methods:This study analysed secondary data from India's National Family Health Survey (NFHS-4).Data for 546 individuals identified as HIV-positive were disaggregated by sex.Contraceptive knowledge and practices were assessed using variables related to awareness of contraceptive methods and current contraceptive use among PLHIV.Results: Among 546 individuals identified as HIV-positive in NFHS-4, 51% (n=278) were women, with a mean age of 35.089.3years.Although awareness regarding modern contraceptives was high (98.7%,n=539), about 61.4% (n=335) were not currently using any contraceptive method.Among the 278 Women living with HIV(WLHIV), 6.8%(n=19) had an unmet need for contraception, of which 1.1%(n=3) were for spacing, and 5.9% (n=16) were for limiting contraceptive methods.A higher proportion of unmet needs was observed among WLHIV aged more than 30 years, urban residence, lower income quartiles, which may be interpreted cautiously given limited number of WLHIV with unmet needs. Conclusion:Although PLHIV were aware of contraceptive methods, their utilisation remained low.Unmet need for family planning was present among women living with HIV, particularly for limiting births.Strengthening integration of family planning services within the National AIDS and STI Control J o u r n a l P r e -p r o o f Programme may help address contraceptive needs and promote dual protection among women living with HIV.
  • Putting TB notification into practice among private pharmacies in Mangalore city, India – Did we succeed?

    <h2>Abstract</h2><h3>Problem considered</h3> Globally, tuberculosis (TB) is the second leading infectious killer after Covid-19 with 1.18–1.43 million deaths in 2022. As per the Global TB Report 2023, an estimated 10.6 million people developed TB in 2022. An estimated 3.1 million cases were missed globally, posing a challenge in breaking the chain of transmission. India is one of the major contributors to the missed cases. Almost 50 % of patients are being treated in the Private sector in India, but fewer are notified to the public health system. This study explored the role of sensitization of pharmacies in TB Case Notification in Mangalore city. <h3>Methods</h3> In this study using a before and after intervention design, the programmatic data was utilized to compare the TB case notifications by the private pharmacies using TB reporting formats for mandatory notification. The intervention was sensitization of the pharmacy owners and pharmacists by a trained field assistant and monthly reminder messages using a social media tool. The data was collected for 2 consecutive quarters October 2019–March 2020 in the prescribed formats. <h3>Results</h3> Post Intervention, there was an increase in the number of pharmacies reporting TB cases from 6 to 14, and prescriptions were being maintained and shared with the District TB Office. An increased number of case reporting was also observed post-intervention. <h3>Conclusion</h3> Intensified sensitization led to improvement in the reporting mechanism of TB case notification by private pharmacies, though it did not translate into a concurrent rise in notification of TB cases in the district.
  • Determinants of HIV status disclosure to children living with HIV in coastal Karnataka, India

    The caregivers of HIV-positive children either delay or avoid disclosure of HIV status to the child due to several reasons. Keeping the child’s HIV status a secret puts them at considerable risk of stopping therapy and transmitting HIV to others. Hence this study was conducted to assess the determinants of disclosure of HIV status to infected children in coastal Karnataka. A cross-sectional study was conducted in a District Hospital in Coastal Karnataka from October 2014 to July 2015. One hundred eighty-five caregivers were recruited for the study purposively and interviewed using a pre-tested, semi-structured questionnaire after written informed consent. Of the 185 caregivers interviewed, 78 (42%) caregivers had not disclosed the HIV status to the child. The child being too young was the most common reason for non-disclosure of HIV status. The majority of the caregivers intended to disclose the HIV status fully to the child after the child attained ten years of age. Nearly two-thirds of the caregivers wanted to disclose it themselves in their own homes. The significant determinants of disclosure of HIV status were the age of the child, and person involved in childcare, and the venue of the disclosure.
  • Pattern of paediatric HIV status disclosure in coastal Karnataka

    Background & objectives: Karnataka is a high HIV prevalent State in India. Although benefits have been shown by disclosing the HIV status to the child, information regarding HIV status disclosure in Karnataka is limited. Hence, this study was conducted to estimate the proportion of children who had been disclosed of their HIV status and its pattern among those who visited the district antiretroviral therapy (ART) centre in coastal Karnataka. Methods: A cross-sectional study was conducted in an ART centre in a district hospital in coastal Karnataka between October 2014 and July 2015. Caregivers of paediatric HIV patients were approached and willing participants were administered a pre-formed, pre-tested semi-structured questionnaire developed for the study. Results: A total of 185 caregivers of the HIV-positive children were interviewed. Mean age of the children was 11±4 years. Only 107 (57.8%) children were aware of their HIV status. Of these, 95 were disclosed fully. Counsellors in rehabilitation centres were the most probable person to disclose and planned events. Children were told of their HIV status for their knowledge and were more likely to be disclosed if they were around 11-15 yr of age and staying in rehabilitation centres. Interpretation & conclusions: Older children were more likely to be disclosed of their HIV status by counsellors. Steps need to be taken to counsel and encourage caregivers to fully disclose the HIV status to the children at least when they attain the age more than 11 years.
