Mandibular Atrophy and Its Impact on Overdenture Performance: Insights From a 5-Year Longitudinal Study Fernanda Faot, Luciana De Rezende Pinto, Lucas Jardim Da Silva, Laura Lourenço Morel, Otacílio Luiz Chagas‐Júnior, et al. Journal of Oral Rehabilitation, 2026 Background The long‐term influence of mandibular atrophy on masticatory function, patient‐reported outcomes, and prosthetic maintenance in users of implant‐retained overdentures remains unclear. Objective To assess the impact of mandibular bone atrophy on masticatory function, quality of life, patient satisfaction, and prosthetic maintenance over a five‐year period. Methods Twenty‐four fully edentulous patients were rehabilitated with mandibular overdentures retained by two narrow‐diameter implants and divided into two groups according to mandibular bone height: atrophic mandible ( AM ) and non‐atrophic mandible ( NAM ). Masticatory performance ( MP ) and swallowing threshold ( ST ) were assessed, while quality of life and satisfaction were evaluated using the Dental Impact on Daily Living ( DIDL ) questionnaire. Prosthetic maintenance events were recorded over 5 years. Multilevel mixed‐effects regression evaluated temporal trends and group differences, and chi‐square tests were used for prosthetic events. Results Four participants were lost to follow‐up, resulting in 10 individuals per group at 5 years. The AM group showed significant reductions in MP_X50 (–9.66%; p = 0.00) and ST_X50 (–1.9%; p = 0.01); and increase in ST_ME5.6 (+43.32%; p = 0.01). The eating/chewing domain of the DIDL was significantly lower in the AM group (0.35 ± 0.72) compared to the NAM group (0.73 ± 0.47). Although overall prosthetic maintenance did not differ significantly, the AM group exhibited a higher frequency of Equator dislodgement (9.24%; p = 0.00) and attachment replacement (6.02%; p = 0.00). Conclusion Mandibular atrophy adversely affected masticatory function, patient‐reported chewing satisfaction, and the frequency of specific prosthetic complications over 5 years.
ACADEMIC PERFORMANCE IN THE ORAL SURGERY SUBJECTS OF UNDERGRADUATE DENTAL STUDENTS: A CROSS-SECTIONAL STUDY José Wittor De Macêdo Santos, Otacílio Luiz Chagas Júnior, Roberto Pereira Pimentel, Maísa Casarin, Francisco Wilker Mustafa Gomes Muniz Revista Cientifica Odontologica, 2026 Objective: This study assessed the factors influencing the academic performance of undergraduate dental students in the field of oral surgery at a public university in southern Brazil. Materials and Methods: It included 146 students who were regularly enrolled in an undergraduate dental program, who completed at least one oral surgery subject. The survey questionnaire encompassed various variables, including sex, age, sexual orientation, family income, ethnicity, part-time employment, use of substance (licit or illicit), and whether dentistry was their initial choice for an undergraduate course. Academic performance was gauged using a grading system ranging from 0.0 (lowest possible grade) to 10.0 (highest possible grade). Bi- and multivariable linear regression analyses were performed to examine the associations between the outcome and independent variables. Results: An inverse relationship between age and academic performance remained significant (β: -0.318; 95% confidence interval [95%CI]: (-0.101–-0.034). Female students outperformed male students (β:0.173; 95%CI:0.022–0.400), while students who reported an increase in drug use exhibited lower academic performance (β: -0.158; 95% CI: -0.354–-0.001). Conclusions: It was concluded that academic performance in oral surgery subjects was notably poorer in older students and those who reported increased drug use. Female students, on the other hand, demonstrated better academic performance.
Novel Titanium Tip Support in Cleft Rhinoplasty Kleber Alberto de Souza Seabra, José Wittor de Macêdo Santos, Wilson José Dewes, Otacílio Luiz Chagas Júnior, Francisco Wilker Mustafa Gomes Muniz Journal of Craniofacial Surgery, 2025 Patients with cleft lip and palate often present with severe nasal deformities that require complex surgical correction. Titanium miniplates can offer stable support to the nasal tip and septum, potentially improving both aesthetics and function. This study aimed to evaluate functional and aesthetic outcomes after rhinoplasty in patients with cleft who underwent nasal reconstruction with titanium miniplates. A retrospective cohort study was conducted at the Foundation for Rehabilitation of Craniofacial Deformities (FUNDEF), Lajeado-RS, Brazil, including patients with cleft lip and palate who underwent definitive rhinoplasty with titanium miniplates between 2000 and 2024. Functional outcomes were assessed using the NOSE-p questionnaire, while aesthetic satisfaction was evaluated using the Portuguese version of the Utrecht Questionnaire, which included a visual analog scale (VAS). Data were analyzed statistically using paired t tests and Wilcoxon tests. A total of 41 patients (53.7% female, mean age 30.3±10.1) were included; 58.5% received costal cartilage grafts and 41.5% septal cartilage grafts together with titanium miniplates for anterior nasal spine reconstruction. Postoperatively, mean NOSE-p scores improved significantly from 62.44±27.04 to 21.22±16.49 ( P <0.001). Visual analog scale scores measuring the aesthetic self-perception improved from a median of 3.0 ( Q 25 –Q 75 : 2.0–4.0 ) to 8.0 ( Q 25 –Q 75 : 7.0–10.0 ). The total Utrecht score decreased from 17.51±4.44 to 7.88±2.78 ( P <0.001), indicating enhanced patient-reported nasal appearance. Three patients experienced complications (7.3%). Titanium miniplate-assisted rhinoplasty appears to be an effective secondary cleft nasal reconstruction approach for nasal tip support, providing functional and aesthetic improvements with a rigid and stable framework.
Functional, Patient-Centered and Bone Remodeling Outcomes in Two Narrow-Diameter Implant Retained Mandibular Overdenture Wearers After 5 Years of Follow-Up: A Prospective Case Series Anna Paula Da Rosa Possebon, Fernando Antônio Vargas Júnior, Fernanda Isabel Román Ramos, Otacílio Luiz Chagas‐Júnior, Luciana de Rezende Pinto, et al. Clinical Oral Implants Research, 2025 ObjectivesThis prospective longitudinal case series investigated changes in functional, radiographic, and patient‐centered outcomes in users of two narrow‐diameter implant‐retained mandibular overdentures (IMO) over a 5‐year follow‐up.Materials and MethodsTwenty‐four patients from a previous 3‐year study (8 man, 16 women, 71.58 ± 7.03 years, mean mandibular edentulism duration 23.9 ± 14.4 years) were invited for a 5‐year follow‐up evaluation of radiographic and functional parameters. Masticatory function (MF) was assessed using the Swallowing Threshold Test (ST), with X50 and B cut‐off values indicating satisfactory performance. Oral health‐related quality of life (OHRQoL) was evaluated using the OHIP‐Edent questionnaire. The Posterior Mandibular Area Index (PAI) was analyzed using digital panoramic radiographs. Data was analyzed using multilevel mixed‐effects linear regression.ResultsTwenty participants completed the 5‐year follow‐up evaluation. ST_X50 (4.00–3.70; p ≤ 0.01; 95% CI: 0.47–1.82) reached a significant reduction along with an increased number of masticatory cycles (59.33–65.60; p ≤ 0.01; 95% CI: 0.32–1.11) between years. The number of IMO wearers presenting satisfactory mastication increased but persisted as unsatisfactory in nine participants by year 5. Declines in the Functional Limitation and Physical Disability domains of OHRQoL were noted, with unsatisfactory mastication correlating with higher pain (coef: 2.5; p: 0.03; 95% CI: 0.25–4.74) and psychological disability scores (coef: 0.94; p ≤ 0.01; 95% CI: 0.19–1.68). PAI increased significantly (1.13–1.68; p ≤ 0.01; 95% CI: 0.24–0.56), indicating ongoing bone remodeling.ConclusionsAlthough MF and bone parameters improved over 5 years, persistent unsatisfactory mastication continued to negatively affect quality of life. Nasal Tip Reshaping in Orthognathic Surgery: A Novel Intraoral Minimally Invasive Approach Mario Santagata, Ciro Emiliano Boschetti, Otacilio Luiz Chagas-Junior, Alessandro Gualdi, Luigi Rugge, et al. Facial Plastic Surgery, 2025 Current concepts in orthognathic surgery focus on not just rectifying dentoskeletal irregularities but also improving facial aesthetic and function. In this respect, nasal reshaping holds extraordinary significance in determining surgery effectiveness and patient satisfaction. Facial preoperative evaluations emphasize the paramount functional and aesthetic impacts of caudal septum in tip reshaping. Deviations in this anatomical subunit could lead to significant nasal obstructions and aesthetic changes as twisted nasal tip, affecting projection, rotation, and columellar–alar relationship. These considerations are essential in orthofacial surgery, as maxillary movements might exacerbate or worsen preexisting nasal tip deformity. In this paper, we aim to describe a novel and innovative minimally invasive approach presenting a detailed account of a rhinoseptoplasty in which the entire nasal tip reshaping procedure was performed intraorally during orthognathic surgery. The technique focused on achieving optimal nasal tip projection and rotation, centering, and stabilization through strategic maneuvers such as incremental dissection of maxillary spine chondroperiosteal junction, superoposterior strut preservation, vertical and caudal septum resection, and anterior nasal spine tip support. Techniques including subnasal drilling and septal stabilization suture are detailed. Specific attention is given to addressing deficiencies in posterior septal angle and labiocolumellar relationship. Postoperative care is described and subjective evaluations were performed to assess patient and surgeon satisfaction with facial symmetry and overall aesthetic improvements. In our experience, the described approach offers valuable surgical finesse to orthognathic surgery, effectively targeting caudal septum deviations and nasal tip droopy. Its unique perspective enhances both functional and aesthetic results, particularly in the context of profiloplasty. No external nasal incision was performed and the totality of nasal tip reshaping was scarless and performed through an intraoral approach. Surgical precision in execution of this novel and tailored surgical approach has been shown to have a profound impact for enhancing outcomes of orthofacial surgery procedure.
