  • Enigma of Folliculitis Decalvans in Two Pediatric Sisters, Treated with Low-Dose Isotretinoin and Erythromycin

    Folliculitis decalvans (FD) is a rare form of primary cicatricial alopecia, characterized by chronic inflammation and destruction of hair follicles, occurring predominantly in young to middle-aged men and uncommon in women under 30 years of age. Treatment for FD is mainly centered around the use of antimicrobial agents for the clearance of S. aureus. Isotretinoin, an analog of vitamin A, has been shown to have direct immunomodulatory effects in addition to its primary action on sebaceous glands. In this report, we present a unique instance of FD in two young sisters, treated with low-dose isotretinoin and erythromycin.
  • Morel-Lavallèe Lesion; An Often Missed Diagnosis

    Morel-Lavallèe lesion is a closed degloving injury caused by shearing forces due to polytrauma. As this lesion is rare and can mimic many other lesions clinically, pathologically, and radiologically, its diagnosis is often missed. Delayed diagnosis can cause prolonged morbidity, worsen the outcome and complicate the treatment. Magnetic Resonance Imaging is the investigation of choice for this lesion. This case report throws light on when to suspect this lesion and also mentions the Fine Needle Aspiration Cytology findings of this lesion, literature on which is scarce.

    Genital ulcers may be located on the genital region and may be due to infectious or noninfectious etiology. Although sexually transmitted infections are a common cause of genital lesions, it is essential to consider nonvenereal disorders in the differential diagnosis. Squamous cell carcinoma appearing in lesions of chronic vulval lichen sclerosus et atrophicus is a rare occurrence of clinical significance. We present a case report of a 62-year-old postmenopausal woman who presented with a long-standing history of vulval pruritus, and mild pain.
  • Gingival Swelling; Surprisingly Actinomycosis!

    Actinomycosis is a rare disease. Its diagnosis is challenging clinically as it mimics many other infectious diseases and neoplasms. In this scenario, a simple timely biopsy can come to the clinician’s rescue. Actinomycosis can be reliably diagnosed on histopathological examination accompanied by special stains without losing much time. Culturing this organism is a tedious job in view of its fastidious nature and requires a long waiting time. Polymerase chain reaction (PCR) can be useful and is less time-consuming, but histopathology is more cost-effective in our country like India. This case report emphasizes the fact that a rare possibility of actinomycosis should also be kept in mind even in immunocompetent individuals in rare sites like gingiva even if there are no discharging sinuses noted clinically. Actinomycosis requires long-duration antibiotic treatment, so if diagnosis time is reduced, timely treatment of actinomycosis can be started and complications can be prevented.
  • Cytomorphological Study of Fluid Aspirates: Comparison between Conventional Cytology Smears and Cell Blocks

    Introduction: Conventional cytology smear method and cell block method are two important methods used for cytological diagnosis. Limitations of conventional smears have to be dealt with daily while reporting cytology. Despite the many advantages of the cell block method, it has been an underestimated diagnostic tool. The purpose of this study is to compare conventional smears and cell block preparations with different parameters and find out which one is more effective in evaluating body fluids. Materials and Methods: Sixty body fluids were studied by the conventional smear method and cell block method. Data were tabulated and analyzed. Results: The cell block method showed a significant increase in cellularity (83.3%) compared to the conventional method (50%) [P = 0.0001, significant]. The limitation of low cellularity was significantly reduced by the cell block method (8.3%) as compared to conventional smears (33.3%) [P = 0.0007, significant]. Morphology preservation was significantly more by cell block method (95%) versus conventional smears (60%) [P = 0.00001, significant]. The yield of malignancy increased by 3.33% in the present study. Suspicious cases on conventional smears could be confidently segregated as malignant or benign by the cell block method. Conclusion: The cell block technique should be routinely used along with conventional smears to improve diagnostic accuracy and resolve diagnostic dilemmas on conventional smears.
  • Testicular Regression Syndrome: Histopathological Evaluation of 20 cases from Rural India with Review of Literature

    Context: Testicular regression syndrome (TRS), clinically termed “vanishing testes,” is a condition with atrophy of testes in late fetal or early neonatal life. The spermatic cord ends blindly or into a small fibrovascular nubbin. Histopathology reveals a fibrovascular nodule with associated hemosiderin-laden macrophages and dystrophic calcification. Few cases show the presence of seminiferous tubules and germ cells. The presence of germ cells is associated with the risk of developing a testicular germ cell tumor. Aims: To study the histopathological features of testicular nubbins and to determine the presence of germ cells and associated neoplasm. Settings and Design: It is a retro-prospective observational study carried out on patients referred to a pediatric surgical pathology center in Miraj, who were clinically diagnosed with undescended testis, over a period of 1 year. Data were collected after the study was approved by the IEC-MGCH on December 19, 2022. Materials and Methods: Twenty patients diagnosed with cryptorchidism, undescended testis, or atrophic testis were included. Findings of hematoxylin and eosin-stained slides of each case were noted, and Prussian Blue (special staining) was performed in four cases. Statistical analysis used: As this is a descriptive observational study, simple arithmetic calculations were performed. No unique statistical analysis was performed. Results: Among twenty cases of vanishing testes syndrome, 20 cases showed fibrovascular tissue only, and 1 case showed the presence of seminiferous tubules with Sertoli cells. Conclusions: Vanishing testes syndrome is a rare condition that is difficult to diagnose on clinical examination, and a definitive diagnosis may be missed if the pathologist is unaware of the condition. Also, there is a risk of germ cell tumor if there are residual germ cells in the testicular nubbin.
  • Deciphering the enigma of glomus tumors: A case series

    Glomus tumors are rare contributing to less than two percent of all soft tissue tumors. Given the rarity of this tumor, there is a possibility of low suspicion and a missed clinical diagnosis. This case series suggests diagnostic and morphological clues for early diagnosis of this tumor.This retrospective study includes nine interesting cases of glomus tumors from January 2015 to January 2024. Data was tabulated and analysed by descriptive statistics using Microsoft Excel 2016. In this series the glomus tumors showed a mean age of (37.67± 17.5 years). Male to female ratio was 1.25:1. Most common sites were digital parts of extremities (5/9, 55.56%). Four of these digital locations were subungual (4/5, 80%). The left side (5/9, 55.56%) was the most common laterality. Duration of symptoms till diagnosis was established showed a median of three months [range was one month to nine years]. All nine cases presented with pain. The mean size of glomus lesions was 2.1±1.5 cm). Multiplicity was observed in (2/9, 22.22%) cases. Eight cases were benign (8/9, 88.89%) while one was malignant (1/9, 11.11%). Two out of these eight benign cases were glomangiomatosis. Excision biopsy was the surgical procedure performed in all of our benign cases while the malignant case was treated with amputation of the finger.Diagnosis of glomus tumors requires a high index of clinical suspicion, especially in extra-digital locations. Awareness is essential for early diagnosis and treatment of this lesion, as it is an extremely painful condition and pain is out of proportion to the size of the lesion. Magnetic Resonance Imaging (MRI) is the first investigation of choice for imaging. An early biopsy can confirm the diagnosis of a glomus tumor in time. Complete surgical excision is curative with a low risk of recurrence.

    Schwannomas are benign peripheral nerve sheath tumours that may occur throughout the body. They rarely occur in the nasal cavity. Paranasal schawannomas are uncommon lesions, representing less than 4% of all head and neck schawannomas. Here we report a case of sinonasal schwannoma in a 35-year-old man who presented with a history of nasal blockage since two years. The mass was removed successfully without any postoperative complication and there was no recurrence within a year of follow up.
