Performance of SARC-F and SARC-CalF scores to predict risk of mortality in hemodialysis patients: a cohort study Sheila Borges, Renata Costa Fortes, Taís Ferreira Martins, Marcela Lopes Alves, Gerson Cipriano Jr, Graziella França Bernardelli Cipriano Scientific Reports, 2024 This study aimed to evaluate the association between the questionnaires SARC-F and SARC-CalF with risk of mortality in patients undergoing hemodialysis (HD). A cohort study, with patients on HD age ≥ 18 years, both sex, between June 2019 and April 2023. Body composition (anthropometry and bioelectrical impedance), muscle functional (handgrip strength and gait speed), screening of sarcopenia using the SARC-F and SARC-CalF, nutritional status and laboratory data were assessed. Follow-up for mortality up to 47 months. The sample consisted of 243 participants and the prevalence of risk of sarcopenia using SARC-F and SARC-CalF were 30% and 45%, respectively; 65 died for all reasons and three patients were censored due to transplantation. Multivariate analysis identified SARC-CalF as predictor of mortality in HD patients [hazard ratio 1.96; 95% CI (1.01-3.79); p = 0.04]. The survival analysis showed that there was a significant difference in the survival curves among the groups stratified by SARC-F and SARC-CalF for log-rank test. A higher specificity was found for SARC-CalF than SARC-F (80% vs. 77%) in receiver operating characteristic (ROC) curve. Both questionnaires were associated with anthropometric, parameters of body composition, physical measurements, and SARC-CalF was predictor of risk for mortality in HD patients.
Randomized, double-blind clinical trial comparing the healing of stasis ulcers in lower limbs with standard hydrocolloid gel dressings and with dressings with Syzygium cumini extract Gilson Roberto de Araújo, Bruna Stéfany Bento de Sousa Teles, Giovana Cardoso Xoteslem, Taís Gratieri, Renata Costa Fortes, Maria Rita Novaes Phlebology, 2022 Objective To evaluate the healing of venous ulcers in the lower limbs (VLU) using dressings with hydrocolloid gel associated with Syzygium cumini extract (SHG) compared to standard hydrocolloid gel dressings (HG). Method This prospective, double-blind, randomized trial recruited 90 patients with VLU divided into: dressings with SHG ( n = 44) and dressings with HG ( n = 46). Primary endpoint was healing in the 14th visit. Secondary endpoints were healing rate, complete healing during follow-up, and improvement in the pain scale and in quality of life. Results There were no significant between-group differences in healing ( p = 0.15). The wound area made healing difficult ( p = 0.008). Age, body mass index, and wound time demonstrated a tendency to worsen the healing. Positive culture for Pseudomonas aeruginosa resulted in 88% reduction in the healing risk ( p < 0.0001). Conclusion There was no difference between the dressings evaluated. The presence of Pseudomonas aeruginosa significantly impaired wound healing.
Nutritional status and clinical evolution of the elderly in home enteral nutritional therapy: A retrospective cohort study Caroline Soares Menezes, Renata Costa Fortes Revista Latino Americana De Enfermagem, 2019 Objetivo avaliar a evolução clínica e nutricional de idosos que recebem terapia nutricional enteral domiciliar. Método estudo observacional do tipo coorte retrospectiva. A coleta de dados foi realizada por meio de análise de prontuários dos registros clínicos e nutricionais. Foram analisadas as variáveis demográficas, nutricionais e clínicas. A amostra foi constituída por pacientes idosos em uso de terapia nutricional enteral domiciliar por via sonda ou estomia. Para a análise estatística, utilizou-se o programa Statistical Package for the Social Sciences, adotando-se o nível de significância de 5%. Resultados a amostra foi de 218 participantes, com uma média de idade de 76±10,12 anos, sendo 54,1% do sexo feminino. A principal morbidade foi a sequela por acidente vascular encefálico. A desnutrição foi o diagnóstico nutricional e a avaliação subjetiva global, o principal instrumento de avaliação nutricional. A via de administração da dieta mais prevalente foi a sonda nasoentérica/nasogástrica, contudo, após um ano de acompanhamento, a gastrostomia passou a ser a principal via. Constatou-se o predomínio de manutenção do estado geral e o desfecho clínico mais prevalente foi o óbito. Conclusão a maioria dos pacientes em terapia de nutrição enteral domiciliar apresentou manutenção e/ou melhora do quadro clínico e nutricional. Logo, essa terapia pode contribuir com uma melhor evolução clínica e nutricional.
Main complications and clinical outcomes of elderly trauma victims admitted to intensive therapy unit Ana Cristina Carvalho Da Costa, Renata Costa Fortes Cogitare Enfermagem, 2018 Objetivo: Identificar as principais intercorrências e o desfecho clínico de idosos internados por causas traumáticas naUnidade de Terapia Intensiva de um hospital de referência do Distrito Federal admitidos entre julho de 2012 e julhode 2014. Método: Estudo retrospectivo analítico, com coleta de dados em prontuário. Os dados foram analisados comos testes Qui-Quadrado e Mann-Whitney, considerando como significativo um valor de p<0,05. Resultados: Dentre 91idosos, houve prevalência do sexo masculino e da queda da própria estatura, como mecanismo de lesão. As principaisintercorrências foram: infecção pulmonar, traqueostomia decorrente de intubação prolongada e choque séptico.Observou-se alta mortalidade, 38 (41,7%), associada ao sexo masculino (p=0,029), às outras causas externas (p=0,03),ao choque séptico (p=0,039) e à diálise (p=0,015). Conclusão: A maior gravidade apresentada favoreceu a ocorrênciade múltiplas intercorrências durante a internação na terapia intensiva; entretanto, apesar do pior prognóstico, os idosospodem sobreviver ao trauma e suas complicações associadas.
Oncology ostomized patients’ perception regarding sexual relationship as an important dimension in quality of life Cristilene Akiko Kimura, Dirce Bellezi Guilhem, Ivone Kamada, Breno Silva de Abreu, Renata Costa Fortes Journal of Coloproctology, 2017 Introduction For ostomized oncological patients, the physical body alterations affects the quality of life, as the changes in the self-concept are factors that directly affect the sexual life.Objective To analyze the perceptions of ostomized men due to intestinal cancer regarding sexual relations as an important dimension of quality of life, treated at the Ambulatory Care Program for Ostomized Patients of the Health Secretariat of the Federal District, Brazil.Methods Epidemiological-based study, of the analytical type, with a cross-sectional descriptive design, with quantitative and qualitative approach considering the content analysis. The convenience sample included 56 participants. Sociodemographic, clinical, and the WHOQOL-BREF questionnaires were used, as well as an individual interview. Data were analyzed by Microsoft Office Excel 2010 and SPSS 20.0 software. Statistical significance was set at 5%.Results The Physical, Social Relations and Environment Domains are correlated with the mean score, statistical significance (p < 0.0001), and the content analysis resulted in five categories: Ostomy, Self-Care, Acceptance, Self-concept, and Companionship.Conclusion Sexuality should be considered as a process of daily living of ostomized individuals due to intestinal cancer.
Total lymphocyte count and serum albumin as predictors of nutritional risk in surgical patients Naruna Pereira ROCHA, Renata Costa FORTES Arquivos Brasileiros De Cirurgia Digestiva Abcd Brazilian Archives of Digestive Surgery, 2015 Background: Early detection of changes in nutritional status is important for a better approach to the surgical patient. There are several nutritional measures in clinical practice, but there is not a complete method for determining the nutritional status, so, health professionals should only choose the best method to use. Aim: To evaluate the total lymphocyte count and albumin as predictors of identification of nutritional risk in surgical patients. Methods: Prospective longitudinal study was conducted with 69 patients undergoing surgery of the gastrointestinal tract. The assessment of nutritional status was evaluated by objective methods (anthropometry and biochemical tests) and subjective methods (subjective global assessment). Results: All parameters used in the nutritional assessment detected a high prevalence of malnutrition, with the exception of BMI which detected only 7.2% (n=5). The albumin (p=0.01), the total lymphocytes count (p=0.02), the percentage of adequacy of skinfolds (p<0.002) and the subjective global assessment (p<0.001) proved to be useful as predictors of risk of postoperative complications, since the smaller the values of albumin and lymphocyte count and higher the score the subjective global assessment were higher risks of surgical complications. Conclusions: A high prevalence of malnutrition was found, except for BMI. The use of albumin and total lymphocyte count were good predictor for the risk of postoperative complications and when used with other methods of assessing the nutritional status, such as the subjective global assessment and the percentage of adequacy of skinfolds, can be useful for identification of nutritional risk and postoperative complications.
A double-blind, randomized and placebo-controlled clinical trial with agaricus sylvaticus fungus in anthropometric profile of women with colon cancer Renata Costa Fortes, Jhuly Amado Souza, Maria Rita Carvalho Garbi Novaes Journal of Coloproctology, 2015 Introduction Colorectal cancer is a disease influenced by genetic and environmental factors. Medicinal fungi and/or its extracts have been used in the adjuvant therapy of cancer because of their pharmacological, nutritional and immunomodulatory properties.Objective To evaluate the anthropometric profile of colorectal cancer women after dietary supplementation with Agaricus sylvaticus fungus.Methods Randomized, double-blind, placebo-controlled clinical trial was conducted in a public hospital in the Federal District – Brazil for six months. Sample of 32 patients with colorectal cancer, female, was separated into two groups: supplemented with Agaricus sylvaticus (30 mg/kg/day) and placebo. We conducted anthropometry (weight, height, body mass index, arm circumference, triceps skinfold, arm muscle circumference and fat percentage) during the treatment. The results were analyzed at three different times (before the start of treatment, three months and after six months supplementation) using the Microsoft Excel 2007 and SPSS 19.0, using Student's t-test and F, with significance for p ≤ 0.05.Results The Agaricus sylvaticus group showed a significant increase in body mass index, arm circumference, percent body fat and triceps skinfold, and non-significant increase in arm muscle circumference after six months of supplementation. These results were not observed in the placebo group.Conclusion The results suggest that dietary supplementation with Agaricus sylvaticus is capable to have benefits in anthropometric parameters of women with colorectal cancer.
Perception of sexual activities and the care process in ostomized women Cristilene Akiko Kimura, Ivone Kamada, Dirce Guilhem, Renata Costa Fortes Journal of Coloproctology, 2013 Introduction the making of a stoma may result in adverse effects on the social and psychological dimensions, affecting relationships with family and friends, at work and in sexual activity.Objective to analyze the perceptions of ostomized women regarding sexual activity as an important dimension of quality of life and in the care process.Methods cross-sectional, descriptive epidemiological study. A sample of 40 patients enrolled in the Ostomized Patient Program of Health Secretariat of the Federal District – Brazil was assessed through a demographic and clinical questionnaire and personal interviews. Data were analyzed using the software programs Microsoft® Office Excel 2010 and SPSS (Statistical Package of the Social Sciences, SPSS Inc, Chicago, USA) for Windows 20.0. Statistical significance was set at 5%.Results ostomized women face several adaptation and rehabilitation problems that interfere with social relationships and, consequently, with body image and self-esteem, which reflects on the sexual activity.Conclusion the results show the need for health professionals involved with these patients to have a wider view on the making of the stoma, their sequelae and rehabilitation, to ensure the process of care that will improve the quality of life of ostomized women.
Poisonous mushrooms; a review of the most common intoxications Alessandro de Lima, Renata Costa Fortes, María Rita Carvalho Garbi Novaes, Sandro Percário Nutricion Hospitalaria, 2012 Mushrooms have been used as components of human diet and many ancient documents written in oriental countries have already described the medicinal properties of fungal species. Some mushrooms are known because of their nutritional and therapeutical properties and all over the world some species are known because of their toxicity that causes fatal accidents every year mainly due to misidentification. Many different substances belonging to poisonous mushrooms were already identified and are related with different symptoms and signs. Carcinogenicity, alterations in respiratory and cardiac rates, renal failure, rhabidomyolisis and other effects were observed in toxicity studies with various species including edible and therapeutic ones. Proper identification is important to avoid accidents and toxicity studies are necessary to assure the safe use of mushrooms as food and for medicinal purposes.
Quality of life from oncological patients with definitive and temporary colostomy Renata Costa Fortes, Themes Máira Ribeiro Cavalcante Monteiro, Cristilene Akiko Kimura Journal of Coloproctology, 2012 INTRODUCTION: Colorectal cancer is one of the most prevalent neoplasms in the western world. Colostomy, temporary or permanent, can be a serious limiting factor affecting the quality of life of oncological patients. OBJECTIVE: To compare the quality of life of oncological patients with definitive and temporary colostomy. METHODS: Descriptive cross-sectional study carried out at a public hospital in Brasilia, Federal District of Brazil. The study analyzed 39 patients with colorectal cancer, 76.9% males, sorted into two groups: definitive ostomy (n=26) and temporary ostomy (n=13). The World Health Organization Quality of Life (WHOQOL)-bref questionnaire was used to assess the quality of life. Data were analyzed using Excel 2007 and Statistical Package for Social Sciences 19.0. RESULTS: 50% of definitive and 76.9% of temporary ostomy patients reported that the physical pain did not interfere in the affairs. 38.5% of definitive and 46.2% of temporary ostomy accepted their physical appearance. 46.2% of definitive and 53.8% of temporary ostomy reported good quality of life. 84.6% of definitive and 46.2% of temporary ostomy did not enjoy their life. 50% of definitive and 53.8% of temporary ostomy were satisfied with their sleep. 57.7% of definitive and 53.8% of temporary ostomy considered the environments as healthy. Over 70% of definitive and over 50% of temporary ostomy had financial constraints and were dissatisfied with leisure opportunities, respectively. 69.2% of definitive and 46.2% of temporary ostomy presented lower libido. 88.5% of definitive and 61.5% of temporary ostomy attended the church. CONCLUSION: The results showed that temporary ostomy patients suffer the same afflictions as the patients with definitive ostomy, equally affecting the quality of life of these patients.
