    Resumo Objetivo Nosso objetivo foi estudar a resistência à extração “in vitro” do parafuso SpineGuard/Zavation Dynamic Surgical Guidance Z-Direct (Parafuso DSG Guia Cirúrgico Dinâmico, SpineGuard Inc, Boulder, Colorado, USA), um parafuso projetado para ser inserido utilizando a técnica de inserção direta. Métodos Os parafusos DSG de 5,5 e 6,5 mm foram introduzidos em blocos de poliuretano com densidade de 10 PCF (0,16g/cm3). De acordo com o grupo experimental, os parafusos foram inseridos sem um orifício piloto, com um orifício piloto sem o macheamento, com macheamento e com macheamento linha a linha. Os testes de extração do parafuso foram realizados em uma máquina de teste universal, após a inserção do parafuso em blocos de poliuretano. Resultados Os parafusos inseridos diretamente nos blocos de poliuretano sem o orifício piloto e o macheamento mostraram uma resistência à extração estatisticamente maior. A inserção do parafuso sem o macheamento ou com o macho de menor diâmetro aumenta a resistência à extração do parafuso em comparação com o macheamento linha a linha. Conclusão O parafuso DSG apresentou a maior resistência à extração após a inserção sem o orifício piloto e o macheamento.
  • Predictors of hospital readmission within 30 days after surgery for thoracolumbar fractures: A mixed approach

    BACKGROUND Readmission followed by surgery to treat spinal fractures has a substantial impact on patient care costs and reflects a hospital's quality standards. This article analyzes the factors associated with hospital readmission followed by surgery to treat spinal fractures. METHODS This was a cross-sectional study with time-series analysis. For prediction analysis, we used Cox proportional hazards and machine-learning models, using data from the Healthcare Cost and Utilization Project, Inpatient Database from Florida (USA). RESULTS The sample comprised 215,999 patients, 8.8% of whom were readmitted within 30 days. The factors associated with a risk of readmission were male sex (1.1 [95% confidence interval 1.06-1.13]) and >60 years of age (1.74 [95% CI: 1.69-1.8]). Surgeons with a higher annual patient volume presented a lower risk of readmission (0.61 [95% CI: 0.59-0.63]) and hospitals with an annual volume >393 presented a lower risk (0.92 [95% CI: 0.89-0.95]). CONCLUSION Surgical procedures and other selected predictors and machine-learning models can be used to reduce 30-day readmissions after spinal surgery. Identification of patients at higher risk for readmission and complications is the first step to reducing unplanned readmissions.
  • EFFECTS OF HIP FLEXION CONTRACTURE ON SAGITTAL SPINOPELVIC PARAMETERS

    Objective: To assess the influence of hip flexion contracture on lumbar lordosis and spinopelvic parameters and the changes in these parameters after total hip arthroplasty (THA). Methods: Twenty adult patients with hip osteoarthritis were divided into two groups (ten patients with hip flexion contracture and ten without contracture). Patients were assessed preoperatively and six months after THA using the radiographic parameters sagittal vertical axis (SVA), lumbar lordosis (LL), pelvic incidence (PI), sacral slope (SS), and pelvic tilt (PT). Results: No statistical difference was found between pre- and postoperative LL values in the groups. After THA, both groups had increased PT and the group without hip flexion contracture had reduced SS. Conclusion: Patients with hip osteoarthritis and hip flexion contracture tend to have an increased LL in the orthostatic position compared to patients without contracture, but with no statistical significance. After THA, PT increased in both groups and SS decreased in patients without hip contracture. Studies should further investigate the role of hip flexion contracture on pelvic mobility and spinopelvic parameters to better understand these relations. Level of Evidence III, Case-Control Study.
  • COSTS ANALYSIS OF SPINAL COLUMN METASTASES SURGICAL TREATMENT

    Introduction End-of-life cancer treatment is associated with substantial healthcare costs. Objective This study aimed to analyze the surgical treatment cost of spinal metastasis and epidural compression patients undergoing surgical treatment. Methods A retrospective cost analysis of 81 patients with spinal metastasis and epidural compression undergoing surgical treatment. Cost evaluation was defined in the following categories: medications, laboratory and imaging tests, nursery, recovery room, intensive care unit, surgical procedure, and consigned material. The cost of pain improvement, functional activity, and survival was also evaluated. Results The total cost of surgical treatment for 81 patients was $3,604,334.26, and the average value for each patient was $44,497.95. The highest costs were related to implants (41.1%), followed by hospitalization (27.3%) and surgical procedure (19.7%). Conclusion The cost of surgical treatment for spinal metastases is one of the most expensive bone complications in cancer patients. The cost of treatment related to outcomes showed differences according to the outcome analyzed. Hospital stay, tests, drugs, and intensive care play an important role in some of the costs related to the specific outcome. Level of Evidence II, Retrospective Study.
  • Outcomes and survival of spinal metastasis with epidural compression

    Objective: The goal of the study was to retrospectively evaluate the demographics, clinical manifestation, outcomes, treatment result, and survival of patients with spinal metastasis with epidural metastasis who underwent surgical treatment. Materials and Methods: A retrospective evaluation of 103 patients with spinal metastasis and epidural compression who underwent surgical treatment between 2009 and 2015 was performed. The recorded parameters selected for the study were general demographic data (gender, age, and educational level) and clinical data (primary tumor, performance status according to Karnofsky score, neurological status according to Frankel scale, pain, surgical treatment outcomes, and patient survival). Results: The mean age of the patients was 55.28 ± 15.79 years, and spinal metastasis was more frequent in males (61.7%). The two most frequent tumors were malignant breast cancer (26.21%) and prostate cancer (22.33%). Preoperative pain was presented in 96 (94.12%) patients and improvement was observed in 44 (47.31%) patients. Symptoms of spinal cord compression were the initial clinical manifestation of the primary tumor in 35 (33.98%) patients. Neurological deficit was observed in 66 (64.07%) patients, and improvement was observed in 43 (41.74%) patients. Improvement of functional outcome and pain was observed in 34 (37.38%) patients. The mean survival was 12.26 months. Longer survival (mean 19.13 months) was observed in patients who showed improvement in their ability to walk or kept it preserved (Frankel D or E). Conclusions: Surgical treatment of spinal metastasis can improve pain and functional activities. Longer survival was observed in patients that keep or recovery the walking ability.
  • Avaliacao mecanica in vitro de conectores das hastes de fixacao vertebral

    Objective Evaluate and compare the mechanical resistance and the fatigue behavior associated with the use of three different modalities of vertebral fixation system rod connectors through in vitro pre-clinical mechanical tests developed specifically for this application (linear, lateral with square connector and lateral with oblique connector). Methods Cobalt chromium rods 5.5 mm in diameter were used and coupled with three types of connectors: a) side rod with oblique connector, b) side rod with square connector, and c) rod and linear connectors. Quasi-static mechanical four-point bending and fatigue tests were performed. The variables measured were (I) the bending moment at the yield limit, (II) the displacement at the yield limit, (III) the rigidity of the system in flexion and (IV) the number of cycles until system failure. Results The linear system presented the greatest force and the greatest moment at the yield limit, as well as the greatest stiffness equivalent to bending. All specimens with square and oblique connectors endured 2.5 million cycles in the minimum and maximum conditions of applied moment. The specimens with linear connector endured 2.5 million cycles with fractions of 40.14% of the bending moment at the yield limit, but failed with levels of 60.17% and 80.27%. Conclusions Systems with linear connectors showed greater mechanical resistance when compared to systems with square and oblique connectors. All systems supported cyclic loads that mimic in vivo demands. Level of evidence V; In vitro research.
  • Unknown primary tumor sites in spinal metastasis

    Objective: The goal of the study was to report the prevalence of spinal metastasis with unknown primary tumor, clinical features, treatment results and patient survival. Methods: A retrospective evaluation of 103 patients with spinal metastasis and epidural compression, who underwent surgical treatment between March 2009 and August 2015. The prevalence of metastatic spinal tumor with unknown primary tumor was evaluated, as well clinical features, survival and treatment results. Results: Of 103 patients with spinal metastasis and epidural compression, seven patients (6.8%) with unknown primary tumor site were identified; five (71.4%) male and two (28.6%) female, with ages ranging from 37 to 67 years (50.7 years). The metastasis was located in the thoracic spine in six of the patients (85.7%) and in the cervical spine in one (14.3%). The mean survival time was 44.8 days. Conclusion: Spinal metastasis with unknown primary tumor was found in 6.8% of patients. The prognosis and outcomes were poor, and patients had poor survival rates.   • DIRECT PEDICLE SCREW INSERTION PULLOUT STRENGTH RESISTÊNCIA AO ARRANCAMENTO DO PARAFUSO PEDICULAR DE INSERÇÃO DIRETA r

    Objective: Study the in vitro pullout strength of SpineGuard/Zavation Dynamic Surgical Guidance Z-Direct Screw (DSG Screw), a screw pedicle designed to be inserted using a direct insertion technique. Methods: DSG Screws of 5.5 mm and 6.5 mm were introduced into polyurethane blocks with a density of 10 PCF (0,16 g/cm3). According to the experimental group, screws were inserted without pilot hole, with pilot without tapping, undertapping and line-to-line tapping. Screw pullout tests were performed using a universal test machine after screw insertion into polyurethane blocks. Results: Screws inserted directly into the polyurethane blocks without pilot hole and tapping showed a statistically higher pullout strength. Insertion of the screw without tapping or with undertapping increases the pullout screw strength compared to line-to-line tapping. Conclusion: DSG Screw showed the highest pullout strength after its insertion without pilot hole and tapping. Level of Evidence V, Expert Opinion.
  • Rotational dislocation C1-C2 after otoplasty under local anesthesia

    Non-traumatic rotational atlantoaxial subluxation (NTARS) is rare and mostly reported after infection of the upper respiratory tract and named Grisel's syndrome. NTARS has also been reported after head-and-neck surgery, but it is extremely rare after otoplasty. A case of NTARS after bilateral otoplasty is reported under local anesthesia, a 15-year-old female being presented with painful torticollis. The diagnosis of atlantoaxial rotatory subluxation was performed using radiographs and computed tomography 2 weeks after the surgery. Closed reduction was performed by traction of the head and transoral direct pressure over an anterior dislocated C1 mass. The reposition of the joint was achieved, but it was very unstable, and it was not possible to keep the reduction. Open posterior reduction and posterior C1–C2 arthrodesis were performed followed by the use of a soft collar during 3 months.
  • Influence of diameter and geometry in the tapping of the pilot hole in pedicle screws

    Objective: To evaluate the insertion torque and the pulling force of each screw with different diameters and tap. Methods: Polyurethane blocks with a pilot hole of 2.7 mm were used in the study. An experimental group with 5 blocks was formed, the insertion torque was evaluated with a torque meter, and the pullout strength of each Globus screw of 5.5 mm and 6.5 mm was assessed. Results: The comparison of the insertion torque on the 5.5 mm screws with pilot hole without tapping and with a smaller diameter than that of the screw (4.5 mm) and a different thread, and with the tapping with the same diameter as that of the screw (5.5 mm) and equal or different thread presented a statistical difference with a higher value of the insertion torque in the group in which the tapping was not performed. As for the pulling force of the 5.5 mm screw, the non-tapping of the pilot hole resulted in statistical difference with the same diameter of the screw (5.5 mm) and with a different thread of the screw. The pullout force on the 6.5 mm screw was higher in the group where the pilot hole was not tapped according to the non-parametric Kruskal-Wallis test, with significance level of p <0.05 in the comparison of the groups. Conclusions: Pilot hole tapping reduced insertion torque and pullout resistance of the pedicle screw influencing the fixation with tapping with the same screw diameter and different thread design.
