Here, we report 2 patients with history of cocaine use living in the same geographic area who were referred for acute multifocal leukoencephalopathy with a Baló-like pattern.Patient 1, a 31-year-old woman originally from the Philippines with chronic cocaine use history, presented with 1 week of rapidly worsening nonfluent aphasia, neglect, severe right hemiparesis, and numbness on the same side. Two months later, patient 2, a 25-year-old man originally from Morocco with chronic cocaine use history, presented with acute left leg weakness, confusion, and neglect. In both patients, brain MRI revealed multiple T2-hyperintense rounded lesions in the white matter, characterized by a concentric target-like appearance and gadolinium-enhanced incomplete ring pattern. In patient 1, a stereotactic biopsy confirmed a demyelinating inflammatory process with microcystic degeneration, increased cellularity, and macrophage/CD4+ lymphocyte infiltration. Both patients were treated with high-dose IV corticosteroid, resulting in significant clinical and radiologic improvement sustained at 1-year follow-up. Although corticosteroids yield promising outcomes, long-term prognosis hinges on sustained treatment adherence and abstinence from cocaine.The temporal and geographic proximity of these cases, occurring within 2 months in the same Italian province (covering an area of approximately 70 miles), raises the possibility of a shared toxic exposure which might explain the unique pathology and provide clues to the immune mechanisms that drive Baló-like demyelination.

Peripherin represents a promising biomarker for axonal damage in the peripheral nervous system (PNS). This study aims to investigate serum peripherin levels in symptomatic and presymptomatic ATTRv subjects, alongside serum neurofilament light chain (sNfL) levels, and their correlations with disease progression and severity.Methods This multicenter, cross-sectional cohort study included 96 individuals with TTR gene variants (49 presymptomatic and 47 symptomatic ATTRv subjects) and 42 healthy controls (HCs). Serum peripherin levels were measured using a highly sensitive homebrew immunoassay, whereas sNfL levels were determined using a commercially available Simoa Neurology 2-Plex B assay. Statistical analyses included non-parametric Spearman correlations, Kruskal-Wallis ANOVA, ANCOVA, and binomial logistic regression.Results Serum peripherin levels were elevated in both presymptomatic (p < 0.001) and symptomatic ATTRv groups (p < 0.001) compared to HCs. Peripherin levels did not correlate with age, sNfL levels, or clinical severity, but showed a strong discriminative ability between ATTRv and HCs (AUC = 0.83, sensitivity = 76%, specificity = 76%, p < 0.001).Conclusions This study demonstrates that serum peripherin represents a promising biomarker for early detection of PNS damage in ATTRv, with elevated levels detectable even in presymptomatic stages.

Current research often independently explores neurophysiological, structural, or functional imaging and biological underpinnings of fatigue, thus lacking a multidimensional perspective. Here, we used a multidimensional approach to investigate the functional, structural and biological underpinnings of fatigue in MS and to assess the relative contribution of each factor. A cross-sectional study was conducted with 41 patients with relapsing–remitting multiple sclerosis and 21 healthy controls (female 14) (HC). MS patients were recruited by including only those with an Expanded Disability Status Scale score &lt; 4, and were categorized as fatigued (MS-F: 19, Female 13, FSS ≥ 4) or non-fatigued (MS-NF: 22, Female 11, FSS &lt; 4). Over five phases, participants underwent Transcranial Magnetic Stimulation, resting-state Electroencephalography, structural and functional Magnetic Resonance, clinical assessments, and blood tests for neurofilament light chain, serum glial fibrillary acidic protein and cytokine levels. Data were analysed using both non-parametric and parametric tests, based on the data distribution. Finally, a decision-tree model was applied to predict patient group assignment. Neurophysiologically, the two patient groups differed in several domains. Those with fatigue had increased θ-band EEG power in frontocentral regions with eyes open. Transcranial Magnetic Stimulation findings indicated significantly lower intracortical facilitation in the MS-F group. Neuroimaging revealed stronger functional connectivity between nodes of the Default Mode Network, between the left temporal node and the right prefrontal node, in the MS-F group. Furthermore, fractional anisotropy via Diffusion Tensor Imaging showed reduced white matter integrity in the corticospinal tracts and corpus callosum in these patients. No significant differences were observed in lesion load, brain volumes, clinical/psychological measures, or blood sample findings linked with neurodegeneration or inflammation; the only psychological variable that differed between the two groups was the depression scale score, with MS-F patients reporting higher scores than MS-NF patients. The decision tree analysis identified both ICF and significantly lower fractional anisotropy values as the most accurate predictors of fatigue, with a classification accuracy of 84.2%. Results highlight the importance of a multidisciplinary approach in defining central fatigue in multiple sclerosis, which would emerge through subtle, subclinical, regional abnormalities of myelin integrity and clearly manifest neurophysiological evidence of impaired glutamatergic activity in motor areas. They also suggest possible biomarkers for the diagnosis of fatigue, possibly useful for eventual targeting novel neuromodulatory treatments.

Previous studies using electroencephalography alone have revealed altered movement-related beta Event-Related Desynchronization (ERD) and Synchronization (ERS) dynamics in fatigued patients, but without providing mechanistic insight. In this cross-sectional study, we combined electroencephalography with transcranial magnetic stimulation, structural magnetic resonance imaging with diffusion tensor imaging (DTI), and clinical measures to probe the mechanistic basis of movement-related beta modulation depth (ERS-ERD) and its link with fatigue. Based on the Fatigue Severity Scale score (FSS), we enrolled 41 relapsing-remitting MS patients, 19 with clinically significant fatigue, 22 without (aged 25–55 years, 25 females), alongside 18 age- and sex-matched healthy volunteers. Participants underwent neuropsychological assessment, blood sampling for inflammatory and neurodegeneration-related biomarkers, structural magnetic resonance imaging with DTI to assess grey and white matter integrity, transcranial magnetic stimulation to quantify cortical excitatory and inhibitory balance, and continuous electroencephalography during 300 cued pinch movements to characterize movement-related beta dynamics. Compared with non-fatigued patients and healthy volunteers, fatigued patients exhibited reduced beta peak ERD to ERS modulation (P &lt; 0.001), especially in frontal regions. The modulation depth correlated with fatigue severity (ρ = −0.54, P = 0.0006), intracortical facilitation (ρ = 0.49, P = 0.0009), and caudate nucleus volume (ρ = 0.35, P = 0.010). A nested elastic-net logistic regression integrating demographic, clinical, structural, and functional markers showed robust held-out performance (mean Receiver Operating Characteristic-Area Under the Curve = 0.92, Precision Recall-Area Under the Curve = 0.83, accuracy = 0.89, Brier score = 0.10). Variables with the strongest protective association with fatigue were higher intracortical facilitation, better mental health, larger caudate volume, greater beta modulation over the frontal regions, and higher corticospinal tract and superior longitudinal fasciculus integrity. These findings support frontal beta modulation as a mechanistically grounded, non-invasive biomarker of central fatigue in multiple sclerosis and highlight its potential utility for clinical diagnosis and targeted therapeutic intervention.

Investigating FC longitudinal evolution and its relationship with disability progression can help clarify this issue. This study examined 5-year FC changes in pwMS and their clinical relevance. Methods From the Italian Neuroimaging Network Initiative database, we included 156 pwMS with two clinical visits and 3T-MRI scans acquired on the same scanner 4–6 years apart. Clinical/neuropsychological visits included the Expanded Disability Status Scale (EDSS), Nine-Hole Peg Test (9HPT), Timed 25-Foot Walk Test (T25FWT), Paced Auditory Serial Addition Test (PASAT3), and Symbol Digit Modalities Test (SDMT). One hundred fifty-six age- and sex-matched healthy subjects (HS) with baseline MRI and the same tests were also included. Based on the EDSS, pwMS were divided into three groups: low disability (0–1.5; N = 78), mild disability (2–3.5; N = 50), and high disability (≥ 4; N = 28). Resting-state networks (RSNs) were identified using independent component analysis. Baseline and longitudinal FC changes were correlated with baseline and follow-up clinical/neuropsychological measures. Results At baseline, the low-disability group showed significantly higher FC in all RSNs (FDR-corrected p < 0.05) compared to HS, which correlated with better baseline scores (SDMT, T25FWT) and less worsening at follow-up (PASAT3, 9HPT). The mild- and high-disability groups exhibited mixed FC abnormalities, with both higher and lower FC than HS in several RSNs. In the mild-disability group, higher FC was associated with worse baseline scores (SDMT, T25FWT) and greater clinical worsening (PASAT3, 9HPT, T25FWT). In the high-disability group, higher sensorimotor baseline FC correlated only with worse baseline 9HPT. Longitudinally, all RSNs showed FC increase in the low-disability group, but a FC decrease in the other groups. FC increases in the low-disability group generally correlated with better clinical outcome (T25FWT), while FC decreases in the mild-disability group correlated with clinical worsening (9HPT, T25FWT). Conclusions FC increases appear to reflect compensatory mechanisms in low-disability pwMS, while in more disabled patients, FC alterations likely represent maladaptive responses. These findings support resting-state FC as a biomarker for monitoring disease progression and treatment response in MS.

This study identifies co‐fluctuating patterns of structural and functional changes in MS using an MRI‐based multimodal fusion approach and assesses the added value to clinical outcomes. Methods Linked independent component analysis (ICA) was applied to spatial maps of white matter (WM) lesions, fractional anisotropy (FA), gray matter (GM) volume, and functional network connectivity to detect regions with differential co‐fluctuations. Graph theory (GT) was then used to reveal clusters of interconnected brain regions. Linear mixed‐effect models targeted regions with significantly different co‐fluctuating patterns between MS and HC. Multivariate stepwise regressions analyzed the associations between co‐fluctuating patterns and disability and cognitive dysfunction. Results The study included 147 patients with MS and 57 HC. Significant co‐fluctuating patterns of decreased FA, increased lesion probability in the thalamic radiation and corpus callosum, GM atrophy in sensorimotor and thalamic areas, and enhanced functional connectivity in the temporal parietal network distinguished MS from HC ( p < 0.001). GT revealed eight brain sub‐networks of spatially connected clusters. Regional‐ and modality‐specific loadings and GT changes explained physical (adjusted R 2 = 0.51, p < 0.001) and cognitive (adjusted R 2 = 0.44, p < 0.001) disability better than traditional MRI measures (adjusted R 2 = 0.12–0.33, p < 0.001). Conclusions Our multimodal MRI approach revealed co‐fluctuating regional patterns of lesions, structural disconnection, and functional hyperconnectivity in MS, offering a more comprehensive explanation of clinical outcomes than traditional MRI metrics.

Due to their low serum levels, precise detection methods are critical. This study aimed to scrutinize the comparability of two techniques: Single Molecule Array (SiMoA) and Ella automated immunoassay, analyzing serum NfL levels in ATTRv presymptomatic subjects and polyneuropathy patients.MethodsBlood samples were processed and analyzed using commercial Ella and SiMoA kits. Statistical analysis included the Wilcoxon signed‐rank test, Spearman correlation, Bland–Altman, and Passing‐Bablok regression. ANCOVA models were used to compare NfL levels between cohorts.ResultsThe study measured serum NfL levels in 55 symptomatic and 55 presymptomatic ATTRv subjects. Median NfL concentrations were significantly higher with Ella (median 27.5 pg/mL) than SiMoA (median 15.9 pg/mL). Both methods showed a strong positive correlation (r = 0.8, p < 0.001), but Ella overestimated NfL by 42%. Bland–Altman analysis revealed a mean bias of 15.4 pg/mL, with limits of agreement between −41.1 and 72.0 pg/mL. The slope of the Passing–Bablok regression line was 0.58, and the intercept was 3.48 pg/mL, suggesting that the Ella platform tends to overestimate NfL concentrations compared to the SiMoA platform, especially at higher concentrations. Both methods effectively distinguished presymptomatic from symptomatic patients (p < 0.001 for both).ConclusionsOur findings underscore that both platforms are effective in measuring serum NfL, with Ella consistently overestimating, especially at higher concentrations. The difference between the two platforms must be taken into account when deeming the concentrations as pathological or normal, as well as when conducting longitudinal studies.

Glial fibrillary acidic protein (GFAP) and neurofilament light chain (NfL) are potential biomarkers for astrocyte activation and neuroaxonal damage, respectively. This study investigates serum GFAP (sGFAP) and NfL (sNfL) levels in ATTRv patients, pre-symptomatic subjects, and healthy controls (HCs) to evaluate their utility as biomarkers of disease progression and CNS involvement. Methods Our multicenter cross-sectional study included 111 ATTRv patients (56 symptomatic, 55 pre-symptomatic subjects) and 183 HCs. Serum levels of sGFAP and sNfL were measured using ultrasensitive immunoassays. The statistical comparisons were performed using ANCOVA models (age and sex adjusted), with correlations examined between serum biomarkers and disease severity (Neuropathy Impairment Score, NIS). Results sGFAP levels were elevated in symptomatic (median: 238.35 pg/ml) and pre-symptomatic subjects (median: 105.50 pg/ml) vs. HCs (median: 75.5 pg/ml, p < 0.001). sNfL was elevated only in symptomatic patients (median: 43.68 pg/ml) compared to pre-symptomatic subjects (median: 9.36 pg/ml) and HCs (median: 7.54 pg/ml, p < 0.001). Both biomarkers correlated significantly with NIS, reflecting disease severity. Female HCs had higher sGFAP levels than males (median 88.6 pg/ml vs. 59.8 pg/ml; p 0.011). Conclusion sGFAP and sNfL mark distinct ATTRv stages, with sGFAP indicating early preclinical changes and sNfL correlating with neurological progression. Sex differences in sGFAP levels among HCs suggest that sex should be considered as a covariate in biomarker analyses.

Objectives: To investigate disease activity and pregnancy outcomes in a retrospective cohort of women exposed to DMF in early pregnancy. Methods: Women discontinuing DMF after pregnancy confirmation were identified from 29 Italian MS Centers. Disease activity 12 months before conception, during pregnancy, and 12 months postpartum were recorded, exploring reactivation predictors. Pregnancy and fetal outcomes were assessed. Results: The study analyzed 137 pregnancies (12 pregnancy losses, 125 live births) from 137 women (mean age 32.9 ± 4.7 years), discontinuing DMF within a median (interquartile range (IQR)) interval of 4.9 (3.7–5.7) weeks from conception. In live birth pregnancies, annualized relapse rate (ARR) significantly decreased during pregnancy (ARR = 0.07, 95% confidence interval (CI): 0.03–0.14, p = 0.021) compared to pre-conception (ARR = 0.21 (95% CI: 0.14–0.30)) and increased postpartum ((ARR = 0.22 (95% CI: 0.15–0.32), p = 0.006). Median time to first relapse (TTFR) was 3.16 (IQR: 1:87–5.42) months. Higher pre-conception relapse number (hazard ratio (HR) = 2.33, 95% CI: 1.08–5.02) and Expanded Disability Status Scale (EDSS; HR = 1.81, 95% CI: 1.17-2.74) were associated with shorter TTFR, while treatment resumption with longer TTFR (HR = 0.29, 95% CI: 0.11–0.74). Fetal outcomes were unaffected by DMF exposure. Conclusion: DMF discontinuation does not increase relapse risk during pregnancy. Early therapy restart prevents postpartum relapses. Early DMF exposure shows no adverse fetal outcomes.
