Comparison between the levels of anti mullerian hormone after treatment with clomid versus letrozole in polycystic women: an experimental study Manal Madany Abdel Qader, Shatha Sami Hussein, Huda Fadhil Jadi Journal of the Pakistan Medical Association, 2024 Objective: To compare the effect of two ovulation induction drugs on anti-Mullerian hormone level in polycystic women.Method: The experimental cohort study was done at the infertility outpatient clinics of Al-Yarmouk Teaching Hospital, Baghdad, Iraq, from May 2020 to April 2021, and comprised infertile women aged 18-35 years with confirmed diagnosis of polycystic ovarian syndrome. Baseline blood level of anti-Mullerian hormone was tested in all patients. The patients were divided into two groups. Group A patients received clomid 50mg twice daily to a maximum of 200mg for 5 days starting from the third day of the menstrual cycle. Group B patients received letrozole 2.5mg twice daily for 5 days starting from the third day of the menstrual cycle. After ovulation induction on day 12-14, a second sample of serum anti-Mullerian hormone was tested, and intergroup comparisons were done. Data were analysed by using the available statistical package of SPSS-28 (Statistical Packages for Social Sciences- version 28). Students-t-test , Paired-t-test , Pearson correlation , t-test or ANOVA test Statistical significance was considered for P value equal or less than 0.05.Results: Of the 83 women, 41(49.4%) were in Group A with mean age 26.0±4.9 years, while Group B had 42(50.6%) subjects with mean age 27.0±5.1 years. Post-intervention, the level of anti-Mullerian hormone decreased significantly in both groups (p<0.05). The decrease in Group B was more than Group A, but the difference was not significant (p>0.05).Conclusions: Ovulation induction drugs clomid and leterzole decreased the level of anti-Mullerian hormone significantly.Key Words: Polycystic Ovary, Anti-Mullerian Hormone, Letrozole, Infertility, Ovulation Induction, Ambulatory Care, Menstrual Cycle, Clomiphene
Kisspeptin-10: A Predictor for Fetal Growth Restriction among Preeclamptic Women that Discriminated Early Onset Cases Manal Madany Abdalqader, Shatha Sami Hussein, Huda Fadhil Jadi, Wassan Nori Clinical and Experimental Obstetrics and Gynecology, 2024 Background : Preeclampsia (PE) is a major cause of maternal and neonatal morbidity. Fetal growth restriction (FGR) shares many pathophysiological roles with PE. Kisspeptin-10 is a peptide secreted by placental syncytium. It was linked to many adverse pregnancy events. The current study aimed to examine Kisspeptin’s-10 role in predicting FGR in PE pregnancies and to verify whether it can predict its onset as early or late FGR. Methods : An observational case-control study enrolled 120 eligible cases at matched gestational age (28–40 weeks) and body mass index (BMI); they were divided into 2-groups: (60) healthy controls and (60) PE cases. PE cases were subdivided into early onset FGR (28/60), who had a gestational age less than 34 weeks, and late-onset FGR (32/60) with a gestational age equal to 34 weeks. A collection was made of the following data: first: pregnant primary criteria [age, BMI, systolic and diastolic blood pressure (BP), and urine for albumin], second: serum Kisspetein-10 was evaluated via enzyme-linked immunosorbent assay (ELISA), and third: ultrasonic criteria [estimated fetal weight, resistance, and pulsatility index (RI, PI)] were recorded for all. Results : Serum Kisspeptin-10 was significantly higher among the controls (309.56 ± 67.72) followed by late-onset FGR and early onset FGR (235.46 ± 68.97) vs . (212.09 ± 58.44) ng/dL; p = 0.0001 respectively. It was negatively linked to systolic, diastolic BP, and urine for albumin; Pearson correlation coefficient ( r ) was (–0.29, –0.48, –0.28) respectively; p < 0.0001, 0.0018, 0.028 respectively. Kisspeptin-10 was positively linked to estimated fetal weight ( r = 0.27; p = 0.034); it had an odds ratio (OR) of 3.04; 95% confidence interval of (1.37– 4.765); p = 0.0001 in discriminating healthy pregnancies from FGR cases. Conclusions : The significant correlation of Kisspeptin-10 with PE parameters and estimated fetal weight with high sensitivity, specificity and reliable area under the curve in predicting early onset FGR cases make it recommended for practice in predicting FGR onset.
Metastatin as a Marker for Hyperandrogenemia in Iraqi Women with Polycystic Ovary Syndrome Manal Madany Abdalqader, Shatha Sami Hussein Obstetrics and Gynecology International, 2020 Objective. Kisspeptin 1 might reflect increased androgen level in polycystic ovarian syndrome instead of other markers. Study Design. A case control study was performed in Al-Yarmouk Teaching Hospital from 1st of July 2016 to 1st of July 2017; it involved 87 women divided into two groups: 44 women diagnosed as PCOS, 22 women with BMI ≥ 25 kg/m2 and 22 women with BMI < 25 kg/m2, and another 43 women without PCOS, 22 women with BMI ≥ 25 kg/m2 and 21 women with BMI < 25 kg/m2. Hormonal, metabolic profiles, and hirsutism scores, as well as serum kisspeptin level, were assessed by using Human Kisspeptin 1(KISS-1) ELISA Kit. The blood samples between days 2 and 5 of menstrual cycle were drawn by an disposable sterile syringe and collected in EDTA containing tubes (as anticoagulant), and the hormonal profile was measured using a biotech ELISA reader. Result. Serum level of kisspeptin was significantly higher in PCOS compared to control (322.4 vs. 235.3 ng/L, respectively). There was no significant difference in age, BMI, and parity between control and PCOS; the frequency of hirsutism, acne, elevated LH, and increased free testosterone (fTT) were significantly higher in PCOS compared to control. Kisspeptin shows a direct significant correlation with hirsutism and fTT (r = 0.648, 0.238, respectively). In ROC analysis, kisspeptin had AUC (95% CI) = 0.874 (0.785–0.935) for predicting PCOS. Conclusion. Kisspeptin levels might be used as a marker for hyperandrogenemia in polycystic ovarian syndrome.
