- Validation of the Italian version of the Short Mood and Feelings Questionnaire (SMFQ) in a sample of adolescents

Background Depression is a prevalent and often persistent mental disorder, with onset frequently occurring during adolescence and associated with serious long-term consequences. Consequently, the availability of valid, age-appropriate, and easily administered screening instruments is essential. The Short Mood and Feelings Questionnaire (SMFQ) is a widely used tool, available in multiple languages, for the screening of depressive symptoms in young people. Although an Italian version of the SMFQ has already been used in previous studies, it has not yet undergone formal validation. The present study aimed to evaluate the psychometric properties of the Italian SMFQ, assess measurement invariance (MI) across sex and age, and provide preliminary normative data. Methods A sample of 580 adolescents (317 males, 263 females), aged 14–20 years, completed the Italian SMFQ along with additional instruments to assess validity: the Personality Assessment Questionnaire and the Strengths and Difficulties Questionnaire for convergent validity, and the Weinstein’s Noise Sensitivity Scale for divergent validity. Analyses focused on dimensionality, measurement invariance, reliability, and validity. Results As regards dimensionality, both one- and two-factor models showed adequate fit, with the two-factor model showing better fit. MI analyses supported full invariance across age for the 13-item version and partial invariance across sex. The 12-item version (excluding item 6) achieved full invariance across both sex and age. The scale showed good internal consistency and good convergent and divergent validity. Finally, given the significant effects of sex and age, normative data were computed as a function of both factors. Conclusions The Italian SMFQ demonstrated excellent psychometric properties for assessing depressive symptoms in adolescents. Literature on the Inventory of Callous-Unemotional (ICU) traits has suggested different versions of the instrument for assessing these traits during development. However, consensus on the instrument version and the best factorial solution remains a matter of debate, with only a few studies having validated ICU versions from a longitudinal perspective. The current study aims to contribute to the literature by comparing ICU models in a longitudinal sample of early adolescents (N = 739; 70.6% of eligible subjects, 371 females and 368 males, in the 6th grade at baseline assessment and in the 8th grade at the second assessment). We tested the validity of various versions of the ICU scales and their respective dimensions by conducting a series of confirmatory factor analyses to verify the factor structure, alongside assessments of internal consistency. For the best-fitting structure, we then analyzed gender and longitudinal invariance in addition to construct and predictive validity, using internalizing and externalizing criteria as well as prosocial behavior. From the comparative analysis, it emerged that the abbreviated 11-item ICU scale version displayed overall better data fit than the full 24-item version. Moreover, its confirmed gender invariance underscores its applicability across genders within the studied age group. With regard to longitudinal invariance, our findings advise caution when comparing ICU scores across early adolescence. While mucosal-based melanomas of the head and neck region are uncommon lesions, when they do arise, they usually exhibit a highly aggressive clinical course. Experience with these tumors is, limited; as such, well worked out treatment protocols for the treat- ment of such lesions are in short supply. It appears as though mucosal melanomas (MuMs) develop more frequently in the nasal cavity and paranasal sinus region, and less often in the oral cavity. The incidence of nodal metastasis seems to be significantly lower in si- nonasal MuMs than it is in MuMs of the oral cavity; this observation may be useful in evalu- ating whether a neck dissection is necessary to determine the location of the primary MuM. - Normative scores and clinical cut-offs of the Cyclothymic–Hypersensitive Temperament Questionnaire in adolescence

Background The Cyclothymic–Hypersensitive Temperament (CHT) is a multidimensional, transdiagnostic affective disposition characterized by mood instability, interpersonal sensitivity, heightened emotional reactivity and impulsive behaviors. The CHT Questionnaire (CHTQ) currently lacks of normative references and empirically derived thresholds. Methods Study 1 established age‐ and sex‐adjusted normative scores and derived percentiles, tolerance limits, and Equivalent Score (ES)–based risk categories. Study 2 tested the clinical relevance of these norms in 196 adolescents with bipolar spectrum disorders and propensity score–matched controls, examining group differences, ES‐based risk distributions, and disorder‐specific thresholds. Results In Study 1, regression analyses showed a significant age‐by‐sex interaction for Total and subscale scores. Age was positively associated with Total in females ( R 2 = 0.029) and negatively in males ( R 2 = 0.009). The same pattern emerged for Impulsiveness/Emotional Dysregulation (IED; females: R 2 = 0.029; males: ns) and Moodiness/Hypersensitivity (MHS; R 2 = 0.017 both sexes), supporting age‐ and sex‐specific norms. Normative values and ES‐based classifications enhanced score interpretability. Moderate‐ and high‐risk thresholds were 15.32 and 17.24 for Total, 6.67 and 7.70 for IED, 9.80 and 10.87 for MHS. In Study 2, bipolar adolescents showed higher adjusted Total and IED than controls (Total: t = −3.33, p < 0.001, d = 0.34; IED: t = −4.42, p < 0.001, d = 0.45), with no MHS differences. Logistic regression showed Total (odds ratio [OR] = 1.08, p = 0.001) and IED (OR = 1.20, p < 0.001), but not MHS, predicted bipolar status. Receiver Operating Characteristic (ROC) analyses showed modest discrimination for Total (Area Under the Curve [AUC] = 0.61; cut‐off = 13.50) and IED (AUC = 0.63; cut‐off = 4.78), and chance‐level performance for MHS. Conclusion Integrating age‐ and sex‐adjusted norms with risk categories and clinical thresholds, the CHTQ may support developmentally informed early risk stratification and longitudinal monitoring. - Efficacy, effectiveness, and safety/tolerability of lithium in children and adolescents up to 18 years of age with conditions other than mood disorders: A scoping review

In youth, lithium is an effective medication for mood disorders, particularly for mixed and manic episodes of bipolar disorder, and is generally well-tolerated. In some clinical contexts, lithium is used off-label to manage other conditions. We conducted a scoping review of studies on the efficacy/effectiveness and safety/tolerability of lithium for treating youths with psychiatric conditions other than mood disorders or neurological disorders. We searched EMBASE, MEDLINE, PsycINFO, PubMed, and ClinicalTrials.gov up to March 31, 2025, with no restrictions on language or document type. We included studies of any design involving children and adolescents (mean age up to 18) treated with lithium, either as monotherapy or in combination with other psychotropic agents. We assessed study quality using the appropriate NHLBI tools and visually summarized the results with a heat map displaying sample size by study design and conditions, as well as the timeline of included studies' publication years. From 2687 records initially identified, after de-duplication removal and screening, 367 full-text reports were assessed, and 41 studies were included in the review, grouped by type of psychiatric or neurological disorder, most of which had a small sample. Among the assessed studies, 60 % of were considered of "fair" quality and 40 % of "poor" quality. Overall, although the clinical use of lithium beyond bipolar disorder in youth is increasing, the underlying evidence base remains limited. More rigorous research based on RCTs and observational studies with designs aimed at reducing confounding are needed to guide clinical practice. - Delphi consensus on the transition from pediatric to adult care in Italian ADHD youth

Paranoia is the erroneous idea that people are targeting you for harm, and the cognitive model suggests that symptoms increase with emotional and relational distress. A factor potentially associated with paranoia is mistrust, a milder form of suspiciousness. This study investigated the longitudinal course of non-clinical paranoia in a sample of 739 students (age range 10–12 at baseline assessment, 12–14 at second assessment) using data from the Social Mistrust Scale (SMS) and the paranoia subscale of the Specific Psychotic Experiences Questionnaire (SPEQ). Prevalence of mistrustful and high paranoia children was 14.6 and 15% respectively. Independently, baseline internalizing symptoms (b = 0.241, p < 0.001) and mistrust (b = 0.240, p < 0.001) longitudinally predict paranoia after controlling for confounders. The interaction of mistrust and internalizing symptoms at T1 increases the possibility of the onset of paranoia at T2. Therefore, the effect of mistrust on paranoia is more marked when internalizing symptoms are higher. Our results confirm the role of mistrust as a factor involved in the developmental trajectory of paranoia in adolescence, enhanced by the presence of internalizing symptoms. The implications of these results are both theoretical and clinical, as they add developmental information to the cognitive model of paranoia and suggests the assessment and clinical management of mistrust and internalizing symptoms in youth may be useful with the aim of reducing the risk of psychotic experiences. - Behavioural, cognitive, and neurophysiological effects of a synbiotic supplementation enriched with pigmented corn extract or cornstarch in drug-naïve children with attention-deficit hyperactivity disorder: A randomised, double-blind, comparison-controlled clinical trial

Background Cyclothymic‐Hypersensitive Temperament (CHT) is characterised by mood instability, interpersonal hypersensitivity, and emotional hyperreactivity, traits frequently observed in general population as well as clinical settings but often eluding strict diagnostic classifications. CHT offers valuable insights into adolescents' emotional and behavioural patterns, but further research is needed to explore its longitudinal associations with psychopathology and clarify its clinical significance. Methods Data were collected from 780 students (50.1% male, mean age at first assessment: 11.35 ± 0.49 years). The study used a longitudinal design with two assessments 2 years apart. Psychopathological symptoms were assessed using the Strength and Difficulties Questionnaire, whereas CHT Questionnaire (CHTQ) assessed CHT. Longitudinal Measurement Invariance of the CHTQ was tested to ensure construct consistency, and a Cross‐Lagged Panel Model (CLPM) was used to examine bidirectional relationships between CHT and psychopathology over time. Results The CHTQ demonstrated configural, metric, scalar, and strict invariance throughout time, confirming its robustness and stability. The CLPM revealed a substantial stability of CHT across time ( b = 0.430, SE = 0.078, 95% CI [0.277, 0.583], β = 0.475, p < 0.001) and a specific significant longitudinal association between CHT and psychopathology ( b = 0.290, SE = 0.103, 95% CI [0.088, 0.492], β = 0.256, p < 0.001). Conclusion The demonstrated invariance underscores the CHTQ's value as a reliable tool for future research on developmental trajectories and longitudinal patterns of CHT in early adolescence.
