  • Six-month outcomes of a three-arm prospective study comparing Da Vinci vs. Hugo RAS vs. versius robotic radical prostatectomy: (the COMPAR-P trial)

    To compare postoperative, oncological, and patient-reported outcomes of robot-assisted radical prostatectomy (RARP) performed with three CE-marked robotic platforms: Da Vinci Xi, Hugo RAS, and Versius. The COMPAR-P trial is a prospective, monocentric, post-market study conducted at the Azienda Ospedaliera Universitaria Integrata of Verona, Italy (ClinicalTrials.gov NCT05766163). From March 2023, 150 patients with organ-confined prostate cancer were consecutively enrolled and allocated to undergo RARP with Da Vinci Xi (n = 50), Hugo RAS (n = 50), or Versius (n = 50). Two high-volume robotic surgeons performed all procedures, experienced with Da Vinci but naïve to Hugo RAS and Versius before trial initiation. Surgical technique, perioperative protocols, and follow-up were standardized across cohorts. Outcomes at 6 months included serum PSA, complications (Clavien–Dindo classification), late sequelae (> 90 days), and health-related quality of life (SF-36 and UCLA-PCI questionnaires). Longitudinal analyses used linear mixed-effects models. Baseline demographics and disease characteristics were comparable across groups. At 6 months, PSA was undetectable in most patients, with no significant intergroup differences. Complication rates and late sequelae were low and evenly distributed. Questionnaire completion exceeded 90% at all time points. No significant long-term differences emerged in most SF-36 and UCLA-PCI domains. However, at 1 month, Hugo RAS and Versius patients reported lower Sexual Function scores than Da Vinci (–20 and − 28 points, respectively), and Versius patients reported lower Sexual Bother scores (–25 points, all p < 0.05). These differences disappeared at 3 and 6 months. Hugo RAS patients showed a temporary advantage in Physical Functioning at 1 month (+ 17 points vs. Da Vinci). Within the present study, and notwithstanding its inherent limitations, Da Vinci, Hugo RAS, and Versius achieved comparable safety, oncological, and short-term patient-reported outcomes in RARP. Early functional differences were transient and resolved over time. Surgical expertise, rather than the platform type, appears to be the primary determinant of outcomes.
  • A Prospective, Comparative Evaluation of Single-Use Versus Reusable Flexible Cystoscopes for Diagnostic Cystoscopy: Operator and Patient Perspectives

    Cystoscopy is a cornerstone diagnostic procedure in urology, especially for diagnosing and monitoring various bladder pathologies, including urothelial tumours and haematuria of unknown origin. Recently, advancements in endoscopic technology have introduced single‐use flexible cystoscopies (SU‐FC) as an alternative to traditional reusable flexible cystoscopies (R‐FC), potentially simplifying workflow and reducing infection risk compared with reusable scopes. This prospective study aims to compare patient‐ and operator‐reported outcomes, including comfort, pain levels, image quality and overall satisfaction when employing single‐use versus reusable flexible cystoscopes. From October to December 2023, a single‐centre study was conducted, enrolling 200 patients undergoing diagnostic flexible cystoscopy, 10 urologists and 10 urologic scrub nurses. Standardized questionnaires, adapted from validated instruments, were used to collect data on perceived pain (Visual Analogue Scale), device performance, image quality and operator satisfaction. Statistical analyses (Wilcoxon rank‐sum, Fisher's exact) tested for significant differences between SU‐FC and R‐FC. Of the 200 patients, 100 underwent cystoscopy using SU‐FC and 100 with R‐FC. Patients in the single‐use group reported significantly lower or milder pain ( p = 0.0112) and higher satisfaction distributions ( p &lt; 0.0001). Operator assessments (physicians and nurses) indicated higher satisfaction with single‐use devices in terms of image clarity, navigation and instrument performance ( p &lt; 0.0001). SU‐FC confer notable benefits for both patients and healthcare professionals, including reduced pain, superior image quality and enhanced workflow. These findings support the broader investigation of disposable cystoscopy in routine practice while urging cost‐effectiveness and sustainability evaluations.

    Background: Predicting postoperative recurrence of prostate cancer (PCa) after robot-assisted radical prostatectomy (RARP) remains challenging. Endogenous testosterone density (ETD) has emerged as a potential biomarker, though its exact prognostic value remains underexplored in specific surgical cohorts. To evaluate preoperative ETD, the ratio of endogenous testosterone to prostate volume (PV; mL), as a predictor of both unfavorable tumor grade and disease progression in clinically localized low-grade (ISUP 1) and high-grade (ISUP 4/5) prostate cancer (PCa). Methods: Between November 2014 and December 2019, 186 patients were selected according to the study criteria. Statistical methods evaluated associations of ETD with study endpoints. Results: In the surgical specimen, 63 cases (33.9%) were low grade (ISUP 1) and 123 (66.1%) high grade (ISUP 4/5). Median (IQR) follow-up was 40 (25–50). PCa progression occurred in 48 subjects (25.8%). Patients presenting with increased ETD levels above 10 ng/(mL × dL) were more likely to associate with high-grade cancer in the surgical specimen (OR = 2.098; 95% CI: 1.028–4.124; p = 0.021) than to undergo disease progression (HR 2.278; 95%CI: 1.258–4.124; p = 0.007). Conclusions: Preoperative ETD was an independent parameter for stratifying clinically localized PCa. ETD levels increased according to the risk of unfavorable tumor grade and disease progression.

    Surgical complications remain a major source of preventable morbidity, mortality, and health care expenditure, but existing frameworks such as the Clavien-Dindo classification and Comprehensive Complication Index are clinician-centred and intervention-focused and fail to capture cumulative patient-centred outcomes. This protocol outlines the Complications After Major and Minor Urological Surgery (CAMUS) initiative, a global, multiphase effort to redefine complication reporting, risk stratification, and outcome measurement in urological surgery. CAMUS aims to address these limitations via an integrated, seven-arm programme combining retrospective and prospective data analysis, consensus development, and digital infrastructure design. Arm 1 has assembled a retrospective data set of 130 034 major urological procedures from 180 centres across 33 countries, the largest of its kind. Arms 2 and 3 have completed Delphi surveys with physicians ( n = 1113) and pilot nursing participants ( n = 20) and has generated consensus on novel grading domains and highlighted the importance of multidisciplinary perspectives. Arm 4 will incorporate patient-reported outcomes and behavioural economics methods to quantify subjective burdens, while arm 5 will develop the CAMUS Intraoperative and Postoperative Risk and Difficulty Estimation Index (IPRADES) for surgical risk and difficulty prediction. Arms 6 and 7 will build a secure e-database and dictionary and prospectively validate the system using >2000 new cases. Statistical methods include multivariable regression, meta-analysis of individual patient data, and machine-learning approaches to model predictors of morbidity and mortality. Outputs will be benchmarked internationally to facilitate both clinician- and patient-driven definitions of complication severity. Ultimately, CAMUS will deliver a reproducible, patient-inclusive classification system with broad applicability to clinical practice, audits, education, and policy. By integrating more than 130 000 procedures with global Delphi consensus, CAMUS represents the most comprehensive complication classification initiative undertaken in surgery. Its outputs are expected to improve transparency, standardise reporting, and inform patient-centred risk stratification worldwide.
  • Impact of Prior Holmium Laser Enucleation of the Prostate on Robot-Assisted Radical Prostatectomy Outcomes: A Systematic Review and Meta-Analysis of Comparative Studies

    Background: The prevalence of incidental prostate cancer (iPCa) after holmium laser enucleation of the prostate (HoLEP) ranges from 5% to 20%. The hypothesis is that prior HoLEP may complicate the treatment of such cancers with RARP and negatively impact post-RARP functional outcomes. Methods: In February 2025, PubMed®, Scopus®, and Web of Science™ were assessed to retrieve data of men with PCa undergoing RARP (P) after HoLEP (I), compared to those without prior HoLEP (C). The outcomes of interest included surgical results (O) from retrospective and prospective comparative studies (Studies). Surgical intra- (OT, EBL, number of nerve-sparing RARP and bladder neck reconstruction), and postoperative (LOS, time to catheter removal), pathological (PSM and tumor stage), oncological (BCR), and functional (postoperative continence and sexual function recovery) outcomes were meta-analyzed, using WMD for continuous variables and odd ratio (OR) for dichotomous variables. Results: After study selection, three comparative retrospective studies were included in the SR and MA, comprising 63 patients undergoing RARP after HoLEP and 322 patients who underwent RARP. Compared to patients who underwent RARP without prior HoLEP, patients undergoing RARP after HoLEP had a statistically significantly longer OT (WMD: 31.99 minutes, p &lt; 0.001), higher likelihood of bladder neck reconstruction (OR: 9.90, p &lt; 0.001), and longer time to catheter removal (WMD: 0.48 days, p = 0.003). However, no differences in terms of pathological, oncological, and functional outcomes were reported between the two groups. The main limitation relates to the inclusion of only three retrospective comparative studies and the small cohort of post-HoLEP patients, which limits the statistical power and generalizability of the findings. Conclusion: Prior HoLEP may introduce certain surgical challenges, reflected in longer OT, a greater need for bladder neck reconstruction, and a longer time to catheter removal. However, these procedural differences do not appear to affect oncological or functional outcomes after RARP.

    Background/Objectives: The ratio of percentage of prostate cancer (PCa) biopsy-positive cores (BPC) to prostate volume as the index density factor (Id-BPC) was used to predict the risk of high tumour grades in the surgical specimens of European Association of Urology (EAU) intermediate-risk patients treated with robotic surgery. Methods: From January 2013 to December 2021, we evaluated 651 patients without any prior treatment for PCa. In the surgical specimen, tumour grades were classified as indolent (International Society of Urological Pathologists Classification (ISUP) 1), significant (ISUP 2/3), and lethal (ISUP 4/5). Associations with the risk of significant and lethal cancers were assessed by the multinomial logistic regression model. Results: Tumour grade was clinically significant (ISUP 2/3) in 522 (80.2%) cases and lethal (ISUP 4/5) in 99 (15.2%), while the results were not significant (ISUP 1) in 30 (4.6%) subjects. The association of Id-BPC was always stronger than BPC for ISUP 1 vs. 4/5 (OR = 0.284; 95% CI: 0.128–0.632; p = 0.002), ISUP 2/3 vs. 4/5 (OR = 0.744; 95% CI: 0.586–0.946; p = 0.016), and ISUP 1 vs. 2/3 (OR = 0.382; 95% CI: 0.176–0.828; p = 0.015), and this trend held after adjusting for clinical factors. Conclusions: Accordingly, Id-BPC was positively associated with lethal disease, as, when it increased or decreased, it was more or less likely, respectively, to find ISUP 4/5 in the surgical specimens of the operated subjects, who could have been stratified according to Id-BPC risk levels.

  • Ureterorenoscopy performed by residents under expert supervision: a training approach ensuring patient safety and effectiveness

    OBJECTIVES: to evaluate the outcomes and postoperative complications of ureteroscopy (URS) and retrograde intrarenal surgery (RIRS) for urolithiasis performed by residents under supervision compared to those performed by specialists. MATERIALS AND METHODS: The institutional endourology database was reviewed. Patients, stone characteristics – including Guy’s stone (GSS) and S.T.O.N.E. scores to describe the complexity of renal stones – perioperative and postoperative data, complications, and stone-free rate (SFR) were retrieved. Procedures were stratified based on whether a specialist or a supervised resident performed them. Multivariate logistic regression was used to analyze the association between stone characteristics and/or operator, and SFR. RESULTS: Out of 280 patients analyzed, 164 (58.6%) were operated on by specialists vs. 116 (41.4%) by supervised residents. Specialists treated larger stones (p < 0.04), but GSS and S.T.O.N.E. scores showed no significant differences. Supervised residents performed more URS than specialists (23.3% vs. 13.4%, p = 0.038). Operative time was shorter for supervised residents (70 vs. 75.5 min, p = 0.02). Patients operated by specialists were more likely discharged with indwelling ureteral stent (82.3% vs. 69.8%, p = 0.02), for a longer time (p = 0.004). Complication rates were similar, but patients operated by specialists had more stone-related events within 90 days (29.9% vs. 16.4%, p = 0.01). SFR was 58.5% for specialists and 63.8% for supervised residents (p = 0.4). Stone volume and stones’ multifocality were predictors of a poorer SFR, while an operation performed by a supervised resident was not a predictor (p = 1). CONCLUSIONS: Under the controlled environment and careful case selection typical of a supervised residency training program, resident-performed ureterorenoscopy for urolithiasis is safe and yields satisfactory postoperative outcomes.

Additionally, to identify patient factors that can aid in predicting lymph node metastasis.Patients and MethodsIn this retrospective, multicentre study, we analysed patients with cN+ M0 BCa staged by CT and treated with upfront radical cystectomy (RC) and pelvic lymph node dissection. We stratified patients by the interval between TURBT and CT into three groups: (1) before TURBT; (2) within 30 days after TURBT; and (3) more than 30 days post‐TURBT. Staging accuracy, defined as concordance between clinical and pathological lymph node status, was evaluated. We utilised logistic regression analyses to identify patient factors, including the optimal timing of staging, in predicting pathological lymph node status at RC.ResultsAmong 183 patients with cN+ disease, 90 (49%) had pN0 disease at RC. Of these, 40, 36 and 14 were staged before TURBT, within 30 days after TURBT, and more than 30 days post‐TURBT, respectively (P = 0.2). Pathological downstaging was most common in cN1 (22%) and cN2 (20%) disease. The overall concordance rate was 23%. The timing of staging did not correlate with pathological lymph node status on logistic regression (all P &gt; 0.05). Lymphovascular invasion (LVI) at TURBT was associated with pN status (odds ratio 4.25, confidence interval 2.02–9.34; P &lt; 0.001) at RC.ConclusionOverall, we found no association between the timing of CT‐based staging and pathological lymph node metastases in cN+ BCa. The data suggest that performing a TURBT prior to staging does not increase the finding of false‐positive nodes on imaging. LVI was the only factor at the time of TURBT associated with pathological lymph node metastasis at RC. Limitations include the multicentre retrospective design and the inclusion of only patients with clinically node‐positive disease.
