Taha Nazir was awarded the Ph.D Microbiology in 2010; M.Phil Pharmacology 2000 and graduation in pharmacy in 1997. Dr. Nazir has ≤25 year’s research, academic and professional experience along with exceptional knowledge epidemiology, cell pathology, molecular biology and biotechnology. He has sufficient teaching experience to the graduate and post-graduate students in different universities. That assumed to lead the responsibility of developing curriculum, designing programs and deploying the projects in defined budgetary order. The institutional needs of those that are a part of the organization often dictate him regarding the scope and magnitude the projects. Thus, Dr. Nazir has played multitudinous roles in a variety of academic, scientific and professional capacities/ positions. Additionally, he has strong experience of writing, reviewing, and publishing the research manuscripts. The recently explained cytokine, which is produced after the stimulation of interferon (IFN)-c, interleukin (IL)-2, and IL-18 is IL-32, has pro-inflammatory IFN-c, IL-2 and IL-18 are IL-32 mediator's properties that are generally entailed in many diseases, including infections, cancer, and chronic inflammation. After the initial statement in 2005, it promoted the osteoclast precursor's differentiation into TRAcP plus VNR plus multinucleated cells that express explicit osteoclast indicators. Furthermore, the loss of bone resorption might be accredited because of the collapse of the multinucleated cells, which are produced of the reaction to IL-32 to direct factoring that is ultimately essential for attaching the cells for bone resorption. Thus, in conclusion, IL-32, the pro-inflammatory mediator, has an important and indirect role in regulating osteoclast differentiation. In bone disorder's pathophysiology, critical role of IL-32 needs more scientific evidence to develop a rational treatment protocol. IL-32 can become a potent mediator of active osteoclast generation in the presence of receptor activator of NF-κB ligand (RANKL). This novel cytokine can introduce more favorable conditions for osteoclastogenesis in the rheumatic arthritis by increasing the RANKL and osteoprotegerin ratio in fibroblast-like synoviocytes.

The Mouth Disintegrating Extended Release Telmisartan (MDERT) tablets are designed for adequate pharmacokinetic profile to avoid the unusual peaks and troughs. Although, there are extensive advance techniques used to deliver drugs. But oral route is still remains effective in most of the therapeutical situations. Thus we aimed this study to formulate a dosage form with dual character of orodispersion as well as extended effectiveness. MDERTS dosage form was prepared by direct compression method. The major components of this preparation were Telmisartan (TLM), Carboxy methyl cellulose polyvinyl Pyrrolidone, Chitosan, Talc, Mg-stearate, Lactose. The MDERTS dosage form was characterized with different determinants. While, the drug release curves of all 6 formulations upto 12h, DSC spectra of TLM, Kyron T 134 , Primogel, TLM+Kyron T 134 +Primogel, Chitosan, CMC and different excepients are presented as comprehensive scientific figures. DSC and FTIR spectroscopic studies indicate the compatibility of drugs with each other and with excipients. Moreover, the formulation F2 containing more than 10% of Kyron T had shown best results. Whereas, the overall results had shown that Kyron T containing tablets had best, in vitro dispersion time, wetting time and wetting volume, water absorption ratio. The order in which superdisintegrants and polymers had produced desirable effect is Kyron T134 > Kyron T134134 + primogel > primogel and for polymers chitosan> chitosan+CMC> chitosan +PVP> CMC> CMC+PVP> PVP. Thus, Kyron T is the best superdisintegrant of others which were used in present study and direct compression method is the best used to prepare extended release mouth disintegrating tablets.

The current health practice of Punjab Province of Pakistan needs a serious attention to control the unnecessary drug usage, improve pharmaceutical care and establish excellent public health system. Hence, the health officials of Govt. of Pakistan have established “Drug Regulatory Authority”, and Punjab Health Department has chalked out Punjab Drug Rule, 2007 to deployed a revised health structure in Punjab. But, unluckily, the medical physicians, drug store owners and bureaucracy have not put it into actual practice. Punjab Health Department has made amendments in the Drug Rules, 2007; substituted the word “three” by the “ten” and suspended it until 2017 via notification printed in the official weekly gazette dated February 10, 2010. This potential confliction of interest can also be noticed in hospital settings, community clinical practice and other allied health institutions. The most awful part the current scenario is the insufficient aspirations to share the clinical burden to provide the health facilities for patients presented in hospital/ clinical setting. The consultant physicians and medical practitioners are very much reluctant to involve the pharmacy professionals to perform their primary professional role. They want them to stay away from their actual scientific job and keep doing the administrative and clerical work. Therefore, the current health system and drug jurisprudence needs a serious attention to address the health problem. That will help to minimize avoidable mortalities, wrong medication, irrational prescription and development of drug resistance. Moreover, a conclusive prescribing guidelines and appropriate drug jurisprudence will expedite the achievement of MDG of WHO.

Cancer chemotherapy exerts deleterious effects in patients, causing structural and physiological changes to their vital organs. These drugs are capable of destroying bone marrow cells and may reduce lymphocyte count. This study was aimed to investigate the frequency of lymphocytopenia in cancer patients taking vinorelbine and its combination as part of cancer chemotherapy. A total 60 adult cancer patients were selected and divided into two groups; Group-1 patients were either on Vinorelbine alone treatment protocol, while group 2 patients were on either Vinorelbine/Cisplatin or Vinorelbine/Doxorubicin treatment protocol. The mean ± SEM lymphocyte counts (× 10^{3}) per uL, pre and post chemotherapy were noted. The outcomes demonstrated no statistically important difference in the patients who were either on vinorelbine alone, vinorelbine plus cisplatin or vinorelbine plus doxorubicin combinations. On comparison of the lymphocytopenia over time for Group-I & II (P-values 0.064, 0.23), and at every week (P-value -0.063, 0.427), we observed the non significant statistical differences. However, comparison of mean values before with that of at week-1,2 and 3 showed significant (P-value^{3} 0.003, 0.003 and 0.055), and at week-4 no significant difference (P-value^{3} 0.727). Thus, the overall lymphocytopenic syndrome in both of the chemotherapy protocols allows the clinical oncologists and consultant physicians to select either of the chemotherapy protocol. Vinorelbine may be a choice of cancer chemotherapy, as they do not compromise immunity in cancer patients. Hence; therapeutic efficacy should constitute the intervening consideration in treating a particular neoplasm.

Rational drug usage, clinical regulations and pharmaceutical care are important features of health practice. Thus; we have aimed case report to elaborate the clinical pharmaceutical services. A 3 year old girl, was presented in a hospital with minor burn that happened by spilt hot water on her wrist. She was in severe pain. The emergency ward staff applied an ointment. The doctor instructed injection Dormicum two times with short gape and then 3rd injection of Pavulon (Pancuronium). Patient went silent and started losing consciousness and eventually passed away. In view of the above facts and circumstances the inquiry board and judiciary took action against the negligence, mishandling. While; going through the investigation the inquiry board did not bothered the channel through which this injection came without proper check that is therapeutical drug monitoring, prescription reviewing, bio-safety and pharmaceutical evaluation. Furthermore, it is not a single death but representation of so many casualties because of the wrong medications. The medial practice needs some more legal, professional and principled restrictions to make sure the safe and correct therapy plans. The clinical legislation, drug rules and medical practice seriously need the attention of judiciary, society and leadership to assure the safety of precious live.

The present study was conducted to find out the ethambutol resistance pattern of indigenous isolates of Mycobacterium tuberculosis from Tuberculosis diagnosed human patients. A total of 172 specimens were collected from six different sources and comprised of 84.9% sputum, 10.5% pus and 4.7% bronchial washings. There were 70.9% males and 29.1% females with 84.30% pulmonary and 15.69% extra-pulmonary tuberculosis. The Mycobacterium tuberculosis isolates collected from primary culture were further studied to determine their pattern and level of resistance. The inoculums were prepared using 0.5 Mac Farland turbidity standards. Five different concentration of ethambutol were used in Lowenstein Jensen (LJ) medium i.e. 2μg/ml, 4μg/ml, 6μg/ml, 8μg/ml and 10μg/ml for sensitivity testing. Data showed 10 (5.8%) resistant and 162 (94.2%) sensitive Mycobacterium tuberculosis out of total 172 clinical isolates. The growth was not inhibited at 1st (2μg/ml) and 2nd (4μg/ml) drug levels, while growth of 50% isolates inhibited at 3rd level (6μg/ml), 30% inhibited at 4th level (8μg/ml) and 20% at 5th level (10μg/ml). The last three levels are above the therapeutic index and not recommended in actual clinical practice. It is thus conceivable to explore some other more effective chemotherapeutic agents, modify combinations or find more effective procedures to stop morbidity and mortality due to ethambutol resistant Mycobacterium tuberculosis.
