The rapid integration of Large Language Models (LLMs) into healthcare necessitates a rigorous evaluation of their performance in specialized medical fields. In metabolic bariatric surgery (MBS), LLMs have the potential to revolutionize education and clinical support, yet their accuracy and reliability are not well-established. This study provides a critical assessment of the capabilities of current LLMs in the context of MBS. This cross-sectional validation study assessed the performance of six LLMs (ChatGPT-3.5, ChatGPT-4o, Gemini, Copilot, GROK, and DeepSeek) in answering 100 evidence-based binary and multiple-choice questions related to MBS. Questions were constructed from international guidelines and categorized into six thematic domains. Expert consensus answers served as the reference standard, with inter-rater reliability measured using Fleiss’ κ. Model outputs were scored for accuracy. Comparisons across LLMs were first assessed using an overall test for differences between multiple related groups. Pairwise comparisons were then conducted between LLMs to identify specific differences in performance. Across the dataset, the mean number of correct LLM responses per question was 3.9 (SD = 1.8). ChatGPT-4o achieved the highest accuracy (66.0%), while DeepSeek recorded the lowest (60.0%). Accuracy varied across domains, highest for indications/contraindications (78.7%) and complications/management (68.0%), and lowest for preoperative preparation (52.0%) and postoperative care (58.4%). Binary questions yielded higher accuracy (69.1%) than multiple-choice questions (62.0%). Inter-expert reliability was substantial (κ = 0.742, 95% CI: 0.71–0.77). Agreement between LLMs and experts ranged from fair (DeepSeek κ = 0.349) to moderate (ChatGPT-4o κ = 0.446). No significant accuracy differences were detected across models (Friedman test, p = 0.662). LLMs represent a promising, yet imperfect, adjunct in MBS education. Their utility is currently limited by inconsistencies in accuracy, particularly in areas requiring nuanced clinical judgment. While these models can supplement traditional learning resources, they are not yet a substitute for expert clinical guidance. This study underscores the need for continued refinement and validation of LLMs to ensure their safe and effective integration into clinical practice. LLMs show moderate accuracy in bariatric surgery education, strongest in guideline-based domains. Newer models (ChatGPT-4o, Gemini, Copilot) performed slightly better, but gains were modest. Food allergies are common, affecting a sizable portion of children and adults in the Western world. In recent years, a promising treatment known as allergen immunotherapy (AIT) has emerged. This technique aims to address the fundamental immune system dysfunction that underlies food allergies. The study's objective was to evaluate the safety of combining probiotics with AIT, compared to placebo in treating patients with food allergies. A comprehensive literature search in several databases was conducted. All included studies were randomized controlled trials(RCTs). Meta-analyses were performed using R software. Statistical tests to assess safety outcomes and risk of bias were used. After reviewing 519 studies, three studies were included in the systematic review and meta-analysis, involving a total of 258 patients (141 in the probiotics group, 144 in the placebo group). The evaluation of the safety profile of the integrated probiotic therapy revealed an overall favorable outcome, with no significant differences in adverse events compared to placebo for most organ systems examined. However, a potential increase in oral adverse effects was observed in the probiotics group (RR = 1.7573, 95% CI: 0.3941 to 7.8359), although this result did not reach statistical significance. Background Roux-en-Y gastric bypass (RYGB) is a widely performed metabolic and bariatric surgery (MBS) procedure with proven efficacy. However, significant variability exists in its technical execution. This study aimed to evaluate global practice patterns among surgeons affiliated with the International Federation for the Surgery and Other Therapies for Obesity (IFSO), identifying key variations and factors influencing intraoperative decision-making. Methods A cross-sectional survey was distributed between January and November 2025 to active IFSO members performing RYGB. The survey explored techniques related to the gastric pouch configuration, intestinal limb lengths, gastrojejunostomy, ring augmentation, and adjunct use. Responses from 245 surgeons across 47 countries were analyzed using descriptive statistics, subgroup comparisons, and multivariable logistic regression. Results Technical variability was widespread. Only 53.5% routinely measured gastric pouch length; 67.3% described their pouches as “short” or “small,” with variable anatomical endpoints. Bougie sizes ranged from 28 to 46 Fr, with 36 Fr being most common (41.2%). Only 21.6% of respondents reported routinely measuring total bowel length. Ring-augmentation was used by 20.4%, with significant regional and experiential variation. Linear stapling was preferred for gastrojejunostomy (81.2%), but stoma size and suture materials varied. Routine closure of both Petersen’s space and the jejunojejunal mesenteric defect was common (69%) yet non-uniform. High-volume surgeons had shorter hospital stays and longer follow-up, but technical patterns were similar. Conclusion Global RYGB practice exhibits heterogeneity in several key operative steps. Improved reporting of intraoperative technical variables may help clarify which differences are most relevant to perioperative, long-term, and registry-based outcomes, and may support future consensus-building in areas where standardization is clinically justified. - Comment to: A comparative study of magnetic sphincter augmentation and Nissen fundoplication in the management of GERD

Background Laparoscopic sleeve gastrectomy (LSG) is one of the most commonly performed metabolic and bariatric surgeries (MBS) worldwide due to its simplicity and efficacy. However, long-term outcomes have revealed limitations, including recurrent weight regain, inadequate weight loss, and nutritional deficiencies, leading to an increasing need for revisional bariatric surgery. While revisional procedures can restore weight loss efficacy, they may also carry additional risks of micronutrient and protein deficiencies due to anatomical and physiological alterations. Methods This review was conducted in accordance with PRISMA guidelines and included studies published up to April 2025. A systematic search of PubMed, Scopus, Web of Science, and Cochrane Library was performed. Eligible studies reported nutritional outcomes after revisional bariatric procedures following LSG. Data on vitamin D, vitamin B12, iron, calcium, albumin levels, and anemia were extracted. Meta-analyses were conducted for both continuous and categorical outcomes. Results Fourteen studies with a total of 1,049 patients were included. Deficiencies in key micronutrients such as vitamin D, vitamin B12, albumin, calcium, zinc and iron were reported across revision types. A co-author network analysis was performed to identify and address overlapping cohorts. The analysis revealed notable rates of nutritional deficiencies across all procedures. Vitamin D deficiency was more frequent in certain revision surgeries. Iron deficiency showed statistically significant differences between groups ( p = 0.025), with one revision type associated with a higher risk ( p = 0.0106). Albumin and anemia rates did not differ significantly between procedures, but anemia remained a common complication overall. Conclusion Micronutrient deficiencies are notably prevalent following revisional bariatric surgery after LSG. Despite the routine use of supplementation regimens, deficiencies in vitamin D, vitamin B12, iron, and protein continue to be frequently reported. Considerable variability among surgical techniques highlights the need for individualized nutritional monitoring and the implementation of standardized supplementation protocols. - Efficacy and safety of water-free topical cyclosporine for moderate to severe dry eye disease: a systematic review and meta-analysis

Background Dry eye disease (DED) is a widely prevalent ocular symptom, with an estimated global prevalence of around 12%. The increasing prevalence of this disease among adults poses a significant morbidity burden. The aim of this study is to analyze the efficacy and safety of water-free cyclosporine, a new intervention suggested to be more effective in treating DED. Methods A comprehensive literature search was conducted using databases as PubMed, Cochrane, Scopus and Web of Science. Keywords used include “dry eye”, “water-free cyclosporine”, and “DED” among others. Randomized controlled trials (RCTs) evaluating the use of water-free cyclosporine in patients with dry eye disease were included. Results A total of 4 RCTs were included in our analysis, consisting of 1,471 participants. Primarily, efficacy outcomes were evaluated such as total and central corneal fluorescence staining score (tCFS and cCFS) and dryness score. Our secondary outcomes evaluated the safety of these drugs by considering the treatment-emergent adverse events (TEAEs) reported. Water-Free cyclosporine showed no significant improvement in tCFS (MD: -1.08; 95% CI, -3.48 to 1.32) and cCFS (MD: -0.21, 95% CI, -0.50 to 0.08). However, water free cyclosporine showed statistically and clinically significant results in improving the dryness score (MD, -7.48; 95% CI, -14.5 to -.082). Pooled analysis of total participants reporting TEAEs, ocular and serious TEAEs was not significant. Conclusion Our meta-analysis analyzed the safety and efficacy of water-free topical 0.1% cyclosporine eye drops for the treatment of moderate to severe DED. A significant reduction in dryness scores provides hope for further development of this drug, potentiated with the little to no significant increase in risk of adverse events.
