    BACKGROUND: Emphysematous pyelitis represents the initial stage of emphysematous pyelonephritis, a rare and potentially life-threatening necrotizing renal infection caused by gas-forming bacteria. Diabetes mellitus and/or lithiasis constitute significant risk factors for such conditions. Renal point-of-care ultrasound is an invaluable diagnostic tool for the early detection of gas within the kidney and urinary tract. The present case provides an illustrative example of the crucial importance of routinely using renal point-of-care ultrasound in every patient with a suspected urinary tract infection in an outpatient setting. CASE PRESENTATION: A 55 year-old hypertensive, non-diabetic female patient of Wayuu indigenous ethnicity was evaluated for mild-to-moderate right flank pain accompanied by slight fever at night. She was referred to the renal outpatient clinic with a presumptive urinary tract infection. In the context of a systemic review for symptoms, the patient reports expulsion of gas bubbles during urination. She was evaluated by ultrasound, which detected gas in the pyelocaliceal system. A computed tomography scan confirmed this. The patient was admitted to the hospital and started on intravenous therapy with ceftriaxone. Urine culture revealed the presence of E. coli > 100 CFU sensitive to ceftriaxone. The response to antibiotherapy was favorable, with remission of clinical symptoms and moderate leukocytosis observed at the time of admission. CONCLUSION: Routine point-of-care ultrasound evaluation should be part of the physical evaluation of every patient presenting with a suspected upper urinary tract infection in the renal outpatient clinic.
  • Adult obesity phenotypes and their clinical relevance: Narrative review

    Obesidad, enfermedad multifactorial provocada por la alteración de la función del tejido adiposo que provoca inflamación crónica y sistemática. Considerada como uno de los principales problemas que enfrenta la sociedad en este siglo, con un incremento de su incidencia a nivel mundial, por lo que se ha convertido en un problema de salud; debido a su crecimiento se ha considerado como una epidemia global, siendo considerada como globesidad en 2011 por la Organización Mundial de la Salud (OMS). La obesidad, junto con el síndrome metabólico, nos ayuda a describir 4 grupos de fenotipos; sin existir un consenso claro. Los grupos revisados incluyen obesos metabólicamente sanos, obesos metabólicamente anormales, metabólicamente anormales, de peso normal y obesos sarcopénicos. El objetivo de esta revisión fue describir cada uno de los fenotipos metabólicos e identificar las patologías más comunes asociadas a cada uno de estos fenotipos; para ello se realizó una revisión narrativa de la bibliografía. Conclusión: Este trabajo deja ver que no existen protocolos unificados para el diagnóstico de cada fenotipo de obesidad; asimismo, cada fenotipo tiene características particulares en cuanto a detección y presentación en la composición corporal, no obstante, se asocian al desarrollo inminente de síndrome metabólico, enfermedades cardiacas y diabetes tipo 2.
  • Spontaneous Pneumomediastinum in a 15-Year-Old Adolescent After Unintended Overexposure to Paint Diluent (Thinner) Fumes: First Reported Case

    Spontaneous pneumomediastinum (SPM) is the presence of air in the mediastinal interstices in the absence of any surgical or medical procedure, chest trauma, or mechanical ventilation. SPM can occur during vigorous Valsalva maneuvers, such as weight lifting, coughing fits, hyperemesis gravidarum, and so on, or during inhalation of illicit substances or toxic agents, as a result of an abrupt increase in pressure in the tracheal tree. Preexisting underlying lung disease may be a contributing factor. In the present case, we report for the first time an SPM due to accidental overexposure to paint thinner in a 15-year-old male from a low-income rural family. He was offered a job painting the inside of a house, which he accepted to earn some money for the family household. However, due to his inexperience, he overdosed on a can of paint with thinner. About 2 h after starting work, he began to feel increasingly severe chest pain and had to be rushed to the local level one basic hospital by his parents. Physical examination revealed subcutaneous emphysema over the supraclavicular area and crackles in the precordial area. Chest radiographs showed a pneumomediastinum. In retrospect, the patient denied coughing or sneezing attacks after exposure. He was transferred to a regional tertiary hospital for further diagnostic evaluation to rule out airway/esophageal perforation. Chest computed tomography confirmed underlying SPM and subcutaneous emphysema. The oesophagogram and bronchoscopy were unremarkable. SPM, possibly secondary to overexposure to thinner vapors, a hydrocarbon-based compound, was the final diagnosis. The patient was discharged asymptomatic on day 5.

  • Myiasis in a Neglected Elderly: Urgent Action Needed in South America

    Background Myiasis is an infestation by fly larvae. Some species exclusively affect humans, contaminating wounds, mucous membranes, and other tissues. It is a disease with marked socioeconomic connotations. Case Presentation The current case involves a 95-year-old woman, an inhabitant of the Andean region of Ecuador with a history of resection of basal cell carcinoma in the left zygomatic region and a diagnosis of chronic leukemia. The surgical wound was secondarily infested with Cochliomyia hominivorax fly larvae and the patient was readmitted to the hospital to treat this complication. A marked clinical improvement was observed after surgical debridement, removal of larvae and administration of ivermectin and antibiotics. Conclusion The determinants of this infestation were advanced age, neglect, and destitution in a patient with an open wound on the face after resection of a basal cell carcinoma. This case illustrates the appalling reality of the marginalized and excluded population of South America. Also of concern is the expansion of myiasis-producing fly populations to areas outside their natural humid and warm habitat. South American governments are called upon to act jointly and effectively against this ominous disease.
  • Efficacy of pharmacological therapy in the treatment of overweight and obesity in adults

    Calcium participates in many physiological processes. Calcinosis cutis is a disease caused by calcium deposition in the dermis. Idiopathic scrotal calcinosis (ICS) is a common form of idiopathic calcinosis cutis. A 28-year-old male patient was evaluated, with symptoms of multiple white masses in the scrotum, of various sizes, not painful. Calcium and phosphorus levels were with-in normal ranges, characteristic of idiopathic calcinosis, while vitamin D and PTH were outside the normal limits. Minor surgery was performed on circum-scribed nodular lesions of various sizes with regular borders. Post-biopsy histo-logical examination confirmed the diagnosis of calcinosis cutis. A low-calcium diet was prescribed as a long-term preventive method. Despite the infrequency of this condition, it should be taken into account when making a differential diagnosis. Its etiology is still unknown and it can present asymptomatically. The important thing is to provide the patient with a better quality of life, avoid discomfort and low self-esteem due to the aesthetic aspect.

    Las infecciones de transmisión sexual (ITS) y conductas sexualesde riesgo, son prevalentes en mujeres encarceladas. Este estudio mostróque las ITS fueron más frecuentes en mujeres reclusas (66,6 %) que en mujeresen libertad (p ≤ 0,001). Solo el grupo de encarceladas presentó combinacionesde ITS (39%) (p≤0,019), siendo significativas las infecciones por VPH (p≤0,007) y Chlamydia trachomatis (p≤ 0,003) con otros agentes causales. VPHy Chlamydia trachomatis fueron más frecuentes en reclusas (35,7%), mientrasque las mujeres libres presentaron 9,5% (p≤0,004) y 7,1% respectivamente(p≤0,001). Las reclusas presentaron combinaciones de diferentes genotiposde VPH (p≤0,011). Las infecciones por Treponema pallidum ocurrieron en 7,1% en reclusas y estuvieron ausentes en las libres. El virus de la hepatitis B, estuvopresente en un 9,5% de reclusas y ausente en mujeres libres (p≤0,040). Losmarcadores para hepatitis C y el virus de la inmunodeficiencia humana (VIH)fueron negativos en ambos grupos; los virus herpes simplex tipo 1 y 2 (VHS) seencontraron en 9,5 % de las reclusas y en 7,1 % de mujeres libres. El consumo de tabaco, alcohol y el uso de otras drogas, mostró una frecuencia de 100%,89,3% y 78,6% respectivamente en mujeres reclusas y un 40%, 30% y 10% en laslibres (p≤0,001). El número de parejas sexuales en relación a la presencia deITS entre ambos grupos, mostró significancia para un número de dos o más parejas(p<0,006). Se observó una elevada prevalencia de ITS únicas o mixtas enmujeres reclusas, así como factores asociados a conductas sexuales de riesgo.
