Methods: We analyzed data from an Italian case–control study, including 498 incident, histologically confirmed STS cases and 969 hospital controls. Odds ratios (OR) of STS for self-reported personal medical history and family history of cancer were estimated using Firth penalized logistic regression models. Results: STS risk was significantly increased with a history of burns [OR, 2.46; 95% confidence interval (CI), 1.10–5.47] and showed a nonsignificant excess with herpes zoster infection (OR, 2.31; 95% CI, 0.65–8.23). Hypertension (OR, 0.71; 95% CI, 0.53–0.94), hypercholesterolemia (OR, 0.66; 95% CI, 0.45–0.96), and tonsillectomy (OR, 0.69; 95% CI, 0.51–0.93) were more frequent among controls. No associations were observed for hepatitis, diabetes, immune-mediated diseases, fractures, surgeries other than tonsillectomy, tooth extraction, or blood transfusion. Family history of cancer showed no major associations, except for STS (0.6% of cases, 0.1% of controls, OR >7) and kidney cancer (OR, 4.69; 95% CI, 1.74–12.7). Conclusions: Our findings suggest a positive association with family history of STS although based on small numbers. Personal medical history and family history of other cancers had no major role; of interest are the suggestive associations of STS with a history of burns and herpes zoster. Impact: Further research is needed to replicate our findings and to explore potential underlying mechanisms.

Decisions regarding subsequent pregnancies after such a diagnosis are complex and influenced by medical, psychological, and social factors. This study aimed to investigate the probability of having a subsequent pregnancy following a diagnosis of a pregnancy‐associated cancer. Material and Methods We conducted a record‐linkage cohort study using the regional health databases of Lombardy, including the hospital discharge records and the certificate of delivery assistance. Pregnancies were deliveries and abortions occurring between 1 January 2007 and 31 December 2017. Pregnancy‐associated cancer was defined as any malignant neoplasm first diagnosed during pregnancy or within 1 year after the end of pregnancy, identified through hospital admissions carrying a new cancer diagnosis. To ascertain subsequent pregnancies after a diagnosis of pregnancy‐associated cancer, we tracked deliveries and abortions through 31 December 2023. We estimated the incidence of pregnancy‐associated cancer per 1000 pregnancies with the corresponding 95% confidence intervals (CIs) and calculated the cumulative probability of a post‐diagnosis pregnancy using the cumulative incidence function that accounts for the competing risk of death. Results A total of 832 incident pregnancy‐associated cancers were recorded among deliveries and 325 among abortions, corresponding to incidence rates of 1.12 per 1000 deliveries (95% CI, 1.04–1.19) and 1.27 per 1000 abortions (95% CI, 1.13–1.41). Breast cancer was the most frequent diagnosis in both groups (31% among deliveries, 40% among abortions), followed by thyroid cancer; the third most common site was lymphoma among deliveries and cervical cancer among abortions. During follow‐up, 77 women had a subsequent pregnancy (59 deliveries, 18 abortions), yielding a cumulative probability of 7.3%. Stratified by age at diagnosis, women under 35 had a substantially higher probability of subsequent pregnancy compared to women aged 35 or older (14.0% vs. 3.5%, p < 0.01). Furthermore, the cumulative probability of subsequent pregnancy was lower among women treated with antineoplastic therapy than among those who were not treated (4.3% vs. 10.1%, p < 0.01). Conclusions Subsequent pregnancies following a pregnancy‐associated cancer diagnosis were relatively uncommon, highlighting the need for integrated reproductive counseling within a multidisciplinary approach.
A Systematic Review and Meta-analysis on Alcohol Consumption and Uterine Fibroids: An Update from 2017 Noemi Salmeri, Sonia Cipriani, Elena Ricci, Giovanna Esposito, Martina Abodi, Alessandro Favilli, Fabio Parazzini, Francesca Chiaffarino Journal of Nutrition, 2026 BACKGROUND: Alcohol intake has been associated with an increased risk of several female-specific conditions. For uterine fibroids (UF), which are common benign uterine tumors, the literature is conflicting. OBJECTIVES: The aim of this systematic review is to provide an up-to-date summary of the available studies investigating the association between alcohol intake and UF. METHODS: A systematic search on PubMed, Embase, Scopus, and Cochrane databases for relevant articles published up to 20 March, 2025 was conducted. Eligible studies reported data on alcohol exposure in women diagnosed with UF and in controls. Pooled odds ratios (ORs) with 95% confidence intervals (CIs) were calculated using random-effects models. RESULTS: Thirteen studies including 135,370 women (79,060 ever drinkers and 56,310 never drinkers; 9944 with UF and 125,426 without UF) showed no significant association between ever compared with never alcohol consumption and UF risk (pooled OR = 1.04, 95% CI: 0.86, 1.26). Nine studies (3,375,571 women) on current compared with never alcohol exposure (1,710,978 current and 1,664,593 never drinkers; 160,708 with UF and 3,214,863 without UF) also showed no overall association (pooled OR = 1.09, 95% CI: 0.91, 1.32). However, subgroup analyses showed between-group differences by geographical region and diagnostic method of UF, with significantly higher risks in China (pooled OR = 3.48, 95% CI: 1.29, 9.43), and in studies using clinical/surgical confirmation (pooled OR = 1.18, 95% CI: 1.00, 1.39). CONCLUSIONS: Although studies are heterogeneous and evidence is limited, this meta-analysis suggests that alcohol consumption may not be associated with the overall risk of UF. In specific geographic regions, the risk appears to increase, suggesting context-dependent effects. The trial was previously registered at PROSPERO as CRD42024518769.

However, updated data on vitamin D status in Mediterranean countries, including Italy, remain limited, hindering effective public health strategies. Objective: To assess serum 25(OH)D levels and their seasonal variation in healthy blood donors aged 18–65 years living in Northern Italy and not taking vitamin D supplements. Given the latitude and the high levels of environmental pollution, cutaneous vitamin D synthesis may be impaired in this population. Recent Italian guidelines on supplementation emphasize the need for updated data on the prevalence of hypovitaminosis D and seasonal variation in endogenous vitamin D synthesis. Methods: In this exploratory retrospective cross-sectional study, 534 blood donors (268 men and 266 women) attending the Transfusion Medicine Unit of the Verona University Hospital were enrolled between April 2016 and May 2018. Serum 25(OH)D concentrations were analyzed by season. Clinical, lifestyle, pharmacological and dietary characteristics were also collected. Results: Among healthy, normal-weight individuals, the prevalence of vitamin D insufficiency (25(OH)D < 50 nmol/L) was low and limited to one-two months per year. Overweight and obesity significantly reduced the likelihood of achieving adequate 25(OH)D levels through cutaneous synthesis for several months. Mean 25(OH)D concentrations were higher than those previously reported in the same area, while seasonal variation remained preserved. Conclusions: In a relatively small non-supplemented population of blood donors living in a high polluted urban area of Northern Italy, seasonal vitamin D synthesis seems to be preserved. These updated data show higher 25(OH)D levels compared to past findings. Although these data certainly warrant further validation through a national survey involving other regions of Italy and in not selected population, they appear to be in line with the SIOMMMS recommendations against indiscriminate serum 25(OH)D testing and against routine supplementation for healthy normal-weight individuals under 70 years.
Family planning of infertile couples: a systematic review of intentions regarding parenthood and return to ART Letizia Li Piani, Giovanna Esposito, Marco Reschini, Jacques Donnez, Fabio Parazzini, Edgardo Somigliana Human Reproduction, 2026 STUDY QUESTION What motivations and barriers influence family planning decisions among infertile individuals? SUMMARY ANSWER In studying the family planning of infertile couples, this review found that a significant gap persists between desired and achieved family size. WHAT IS KNOWN ALREADY While ART has traditionally focused on live birth rate (LBR) as a primary success parameter, growing attention has been paid to whether treatments help couples achieve their desired family size. Evidence suggests that many infertile couples do not return to ART for subsequent children, despite having cryopreserved embryos available. STUDY DESIGN, SIZE, DURATION This review was conducted as a systematic review following PRISMA guidelines. A comprehensive search strategy was developed and implemented across PubMed and Embase databases, covering studies published in English up to May 2025. The search combined free text terms and MeSH/Emtree terms related to ‘infertility’ and ‘family planning’. PARTICIPANTS/MATERIALS, SETTING, METHODS We included observational studies reporting outcomes related to family size, return to ART, or intentions for subsequent children. Two reviewers independently performed screening, data extraction, and quality assessment using the Newcastle–Ottawa Scale. MAIN RESULTS AND THE ROLE OF CHANCE Of 2495 screened records, 9 studies were included. Across contexts, infertile couples consistently reported smaller family sizes compared to fertile ones. Return rates to ART for a second child ranged from 25 to 50%, even among those with cryopreserved embryos. Factors associated with return included younger age, availability of embryos, and previous treatment characteristics. However, emotional, financial, and social burdens often discouraged further ART use. Success rates for second ART pregnancies varied, with cumulative LBRs between 38 and 88%, depending on treatment strategy and prior history. LIMITATIONS, REASONS FOR CAUTION Scarcity of evidence and high heterogeneity across studies, including differences in design, populations, outcomes, and type of ART, may have limited comparability of the studies. WIDER IMPLICATIONS OF THE FINDINGS The low return rate to ART highlights unmet needs in post-treatment support and counselling. Future research should explore the psychosocial, economic, and systemic barriers that prevent couples from pursuing their reproductive goals, enabling more patient-centred care in reproductive medicine. Plant-Based Diets and Ovarian Cancer Risk Giovanna Esposito, Federica Turati, Silvia Mignozzi, Fabio Parazzini, Livia S. A. Augustin, Sara Vitale, Jerry Polesel, Luigino Dal Maso, Eva Negri, Carlo La Vecchia Nutrients, 2026 Objective: To assess the relationship between adherence to various plant-based diets, as measured by overall, healthy, and unhealthy plant-based diet indices (PDI, hPDI, uPDI), and ovarian cancer risk. Methods: We obtained data on 1031 cases of ovarian cancer and 2411 controls from a case-control study conducted in Italy. PDI, hPDI, and uPDI were calculated using data from a validated food frequency questionnaire. We used logistic regression to calculate the odds ratios (ORs) and their corresponding 95% confidence intervals (CIs) of ovarian cancer for PDI, hPDI, and uPDI, adjusting for several possible confounders. Results: PDI and hPDI were inversely related to ovarian cancer risk (OR = 0.70 for the fourth compared to the first quartile, 95% CI: 0.55–0.89, and OR = 0.67, 95% CI: 0.53–0.84, respectively). On the other hand, a higher uPDI was related to a higher risk of ovarian cancer (OR = 1.78, 95% CI: 1.40–2.28). The estimates for a 5-point increment in the indices were 0.88 (95% CI: 0.81–0.95) for PDI, 0.90 (95% CI: 0.83–0.96) for hPDI, and 1.15 (95% CI: 1.07–1.23) for uPDI. Consistent associations for the three indices were observed across strata of age, family history of breast/ovarian cancer, educational level, parity, oral contraceptives use, and menopausal status. Conclusions: Plant-based diets favorably influence ovarian cancer risk; plant-based diets characterized by a high intake of unhealthy plant foods are linked to an increased risk. Promoting diets rich in healthy plant foods could support the reduction of ovarian cancer risk.
Fertility awareness methods and waiting conduct in idiopathic infertility: a prospective observational study Michele Barbato, Serena Del Zoppo, Fabio Parazzini, Andrea Graziani, Maurizio Guida, Alberto Ferlin, Luigi Frigerio, Giuseppe Grande Frontiers in Reproductive Health, 2026 Introduction Couple infertility is a common clinical condition that is too often treated with assisted reproductive techniques (ARTs) without a proper evaluation of both male and female factors. To improve the likelihood of natural conception, fertility awareness methods (FAMs) are widely used. Methods We performed a multicenter prospective study enrolling couples with primary idiopathic infertility who were seeking natural conception. Participants were followed for 12 months using FAMs, and their outcomes were compared with those of couples who only used ARTs. The aim of our study was to evaluate the pregnancy rate after 12 months among couples with idiopathic infertility using FAMs compared with those who immediately pursued ARTs. We evaluated 41 couples in the FAM group and 56 couples in the ART group. Results In the FAM group, we reported a pregnancy rate (PR) of 51.22%. Among women aged &lt;34 years, we reported a PR of 90.9%, while it decreased to 36.7% among women aged 35–39 years. In the ART group, 10 couples achieved pregnancy (PR 17.8%). Within this group, we reported a PR of 30% among women aged &lt;34 years and 17.4% among women aged 35–39 years. Discussion After 12 months of unprotected intercourse without spontaneous conception in women younger than 35 years or after 6 months in women aged 35–39 years, couples should undergo a complete multidisciplinary diagnostic evaluation involving both the male and female partners. If a diagnosis of idiopathic infertility is established at the end of this process, couples (especially younger ones) may be advised to wait an additional 12 months while using FAMs, as no advantage has been observed with direct access to ARTs. They could then be referred to ART if a spontaneous pregnancy is not achieved during this period.
Childbearing in women diagnosed with cancer during reproductive age Giovanna Esposito, Anna Cantarutti, Matteo Franchi, Fedro Alessandro Peccatori, Giovanna Scarfone, Eva Negri, Giovanni Corrao, Fabio Parazzini, Carlo La Vecchia Acta Obstetricia Et Gynecologica Scandinavica, 2025 Introduction Fertility after cancer represents a growing clinical concern. This study assessed childbearing outcomes among women diagnosed with cancer during reproductive age between 2012 and 2017 in Lombardy, the largest region in Italy. Material and Methods Women aged 15–45 years with a primary diagnosis of cancer recorded in hospital discharge records from regional healthcare databases were selected. Each woman diagnosed with cancer was matched with up to five cancer‐free women of the same age at diagnosis. The cumulative probability of childbirth up to December 31, 2022 was estimated using the Kalbfleisch–Prentice cumulative incidence function estimator. Cox regression models were used to estimate the cause‐specific hazard ratios (HRs) and the 95% confidence intervals (CIs) of childbirth according to the cancer diagnosis. Furthermore, in the group of cancer survivors, exposure to antineoplastic treatment was considered and included in the model as a time‐dependent covariate. Finally, a log‐binomial regression model was used to assess the association between antineoplastic therapy and medically assisted reproduction. Results A total of 13,877 women were diagnosed with cancer at reproductive age during the study period (1.16 per 1000 person‐years). The cumulative probability of childbirth was lower among women diagnosed with cancer compared to cancer‐free women across all age groups: 31.4% vs 32.2% ( p = 0.02) among those diagnosed under 30, 13.3% vs 22.7% ( p < 0.01) among those aged 30–39, and 0.8% vs 1.6% ( p < 0.01) among those aged 40 and over. The corresponding HRs were 0.93 (95% CI: 0.83–1.05), 0.58 (95% CI: 0.53–0.64), and 0.52 (95% CI: 0.40–0.68). When analyses were stratified by time since diagnosis, the reduced probability among cancer survivors was confirmed to be significant only within the first 5 years after diagnosis, also for younger individuals. Antineoplastic treatment was associated with a reduced probability of subsequent birth (HR = 0.46, 95% CI: 0.39–0.52). Moreover, the therapy was positively associated with medically assisted reproduction (RR = 1.71, 95% CI: 1.14–2.56). Conclusions The probability of childbearing was reduced within the first 5 years of diagnosis, regardless of the patient's age. A more pronounced reduction was observed in women diagnosed after the age of 30. Age and antineoplastic therapy were key factors in determining childbearing in women diagnosed with cancer.
Particulate matter (PM2.5, PM10) exposure and assisted reproductive technology outcomes in fresh cycles: a systematic review and meta-analysis Letizia Li Piani, Giovanna Esposito, Eva Negri, Irene La Vecchia, Vittoria Sterpi, Claudia Santucci, Rossella Bonzi, Carlo La Vecchia, Edgardo Somigliana, Paola Viganò, Fabio Parazzini Atmospheric Environment, 2025 Several studies have assessed the relationship between exposure to particulate matter with aerodynamic diameters ≤2.5 μm (PM2.5) and ≤10 μm (PM10) and assisted reproduction technology (ART) success rates, with controversial findings. This review aimed to consolidate the current evidence from fresh ART cycles and explore sources of heterogeneity across studies. The outcomes of interest were biochemical and clinical pregnancy, and live birth. Four exposure timeframes were considered: T0 (within 90 days before starting the stimulation protocol), T1 (from the first day of stimulation protocol to oocyte retrieval), T2 (from oocyte retrieval to embryo transfer), and T3 (from embryo transfer to pregnancy). Database searches were conducted on PubMed/Medline and Embase up to April 2025, using a combination of MeSH/Emtree and generic terms relating to ART patients and air pollution exposure. Fourteen studies, involving 333,438 cycles, were included in the meta-analysis. The studies were conducted between 2010 and 2024 primarily in China (10/14), while 3 were from the USA and one from Korea. We estimated pooled relative risks (RRs) and the corresponding 95% confidence intervals (CIs) for a 10 μg/m3 increase in PM2.5 and PM10 for biochemical pregnancy, clinical pregnancy, and live births, using random-effects meta-analytic models. The pooled RRs for 10 μg/m3 increment in PM2.5 and PM10 at T0 were respectively 0.97 (95%CI: 0.91–1.04) and 0.98 (95%CI: 0.95–1.02) for biochemical pregnancy; for clinical pregnancy they were respectively 0.98 (95%CI: 0.96–1.01) and 0.99 (95%CI: 0.98–1.00). All other findings were close to unity. Substantial heterogeneity was observed across studies for selected outcomes and exposure periods for both pollutants. This systematic review found no significant association between PM2.5 and PM10 exposure and ART outcomes after fresh embryo transfer for any of the four exposure windows.
