Psoriasis is a chronic inflammatory skin disease that affects patients' quality of life. This study aimed to enhance the efficacy of topical application of fluticasone propionate (FP) using a eucalyptus oil‐based nanoemulsion, an oil possessing anti‐inflammatory activity and extracted from the leaves, fruits, and buds of Eucalyptus globulus or Eucalyptus maidenii, to improve the skin deposition of FP and aid its anti‐inflammatory effect. Box‐Behnken design was employed to optimize NE formulations, which were characterized for globule size, zeta potential, polydispersity index, rheological behavior, microscopic morphology, ex vivo skin permeation/deposition, and in vivo efficacy using imiquimod‐induced psoriatic lesions. The optimized formulation depicted a droplet size of 188 ± 22.4 nm, a zeta potential of −17.63 ± 1.66 mV, and a viscosity of 204.9 mPa s. In addition to the increased FP retention in different skin layers caused by the NE and the reduced PASI score compared to the marketed cream, the levels of inflammatory cytokines IL‐1α, IL‐6, IL17a were markedly lowered, indicating the improved anti‐psoriatic curable efficacy of the optimized formulation in comparison to the FP‐marketed product.
From Oral to Sublingual: A Redefined Avanafil Tablet with a Breakthrough in Bioavailability and First-Pass Metabolism Avoidance

Introduction Avanafil (AVA) is a very efficient phosphodiesterase type 5 inhibitor for the treatment of erectile dysfunction. However, it has limited bioavailability when taken orally and considerable first-pass metabolism. Enhancing its solubility and choosing an alternative delivery route may enhance its effectiveness and duration of action. Methods Eight complex formulations were elaborated and analyzed at various ratios using different polyethylene glycols and hydroxypropyl-beta-cyclodextrin (HP-β-CD). Sublingual tablets containing AVA were designed and optimized using the Quality-by-design approach. The tablets’ pre-compression and post-compression properties were evaluated. The in-vivo pharmacokinetic behavior of the optimized tablet was assessed and compared with that of the commercial oral tablets in human volunteers. Results The HP-β-CD–AVA inclusion complex (1:1 molar ratio) showed an optimum solubilization capacity with an amount suitable for incorporation into sublingual tablets. The total amounts of superdisintegrants and Plasdone XL and the percentage of starch significantly influenced the length of time it took for 80% of the AVA to be released from the sublingual tablets, the tablet hardness, and the length of time for tablet disintegration. The optimized AVA sublingual tablet exhibited a 5.98-fold increase in the AVA mean residence time over the commercial tablet, with greater plasma exposure over 72 hours and 1356.42% relative bioavailability. Conclusion The sublingual tablets of the solubility-enhanced HP-β-CD–AVA inclusion complex represent a promising strategy to improve AVA bioavailability and bypass the first-pass effect. Furthermore, their extended activity offers potential clinical benefits, particularly for ED patients, such as ease of administration and reduced side effects.

New developments in nanoparticle-based oral medicine have resulted in a profusion of studies to improve the solubility, permeability, and chemical stability of various medications. Nanosponges (NSs) are one type of carriers utilized in this many carrier systems. NSs are nanosized carriers with a sponge-like shape. They have hydrophilic cavities and hydrophobic branches, which aid in the loading of both hydrophilic and hydrophobic medicines. Nano-sponges have a 3-dimensional network and a nanometric cavity size. NSs are very porous, with the capacity to entrap active moieties and the advantage of controlled release. These tiny sponges circulate in the body to reach a specific place and release the medicine in a controlled and predictable manner, assisting in the resolution of numerous issues such as drug toxicity and low bioavailability. One of their significant impacts is the ability to enhance oral absorption and bioavailability. The primary goal of this review is to provide brief updates on NSs for increasing medicine oral absorption as well as their evolutions in loading drugs for enhancing their oral deliverability and treatment of a variety of diseases.
