  • Secondary analysis of a combined harm-reduction treatment and extended-release naltrexone randomized clinical trial for alcohol use disorder: Differences across race, ethnicity and sex assigned at birth

    Background and Aims In a prior randomized clinical trial (RCT), combined pharmacobehavioral harm‐reduction treatment improved alcohol outcomes and physical health‐related quality of life (PH‐QoL). In this secondary analysis, we tested race, ethnicity and sex assigned at birth as predictors and moderators of these effects. Design Secondary study of a four‐arm RCT. Setting Community settings serving people experiencing homelessness in the US Pacific Northwest. Participants Adults aged 21–65 (N = 308) with alcohol use disorder (AUD) who experienced past‐year homelessness. Intervention In the parent RCT, participants were randomized to brief behavioral harm‐reduction treatment for AUD + extended‐release naltrexone (HaRT‐A + XR‐NTX); HaRT‐A + placebo; HaRT‐A alone, or services‐as‐usual control. Measurement We tested whether baseline outcomes, trial inclusion, data missingness, treatment adherence, side effects, and treatment effects on alcohol frequency, quantity, alcohol‐related harm, urinary ethyl glucuronide, and PH‐QoL differed by race, ethnicity, and sex assigned at birth. Findings Race, ethnicity and sex assigned at birth were not associated with differential trial inclusion, missingness, treatment adherence or side effects with one exception: the Multiracial/other people of color (POC) group attended fewer HaRT‐A sessions than the white group (odds ratio [ OR ] = 0.49, p = 0.03, 95% confidence interval [CI] [0.25, 0.95]). There were no statistically significant moderators of the behavioral HaRT‐A effect; however, Multiracial/other POC race ( B = ‐3.34, p = 0.03, 95% CI [−6.35, −0.34]) and sex assigned at birth ( B = 2.43, p = 0.04, 95% CI [0.14, 4.73]) moderated the XR‐NTX versus placebo effect on one variable: PH‐QoL. Simple slope analysis indicated the Multiracial/other POC group who received placebo showed improvement on PH‐QoL ( dy/dx = 2.61, p = 0.003, 95% CI [0.91, 4.31]); the XR‐NTX group did not ( dy/dx = −0.22, p = 0.79, 95% CI [−1.90, 1.45]). Among XR‐NTX recipients, the female group ( dy/dx = 2.89, p < 0.001, 95% CI [1.53, 4.25]) showed faster improvement rates than the male group ( dy/dx = 1.19, p = 0.02, 95% CI [0.23, 2.16]) on PH‐QoL. Conclusion Race, ethnicity and sex assigned at birth were largely not associated with trial inclusion, data missingness, treatment adherence, side effects, or outcomes in a combined harm‐reduction treatment and extended‐release naltrexone randomised clinical trial for alcohol use disorder (AUD). Although Multiracial/other people of color (POC) participants attended fewer brief behavioral harm‐reduction treatment for AUD sessions, they did not differ from white participants on alcohol and physical health‐related quality of life outcomes. Among participants receiving extended‐release naltrexone (XR‐NTX), women showed greater improvement than men on physical health‐related quality of life. Compared with white participants, Multiracial/other POC participants who received XR‐NTX showed less improvement than those who received placebo. Given their post hoc nature and limited power, findings are hypothesis‐generating. They suggest the equitable utility of behavioral harm‐reduction treatment and the importance of assessing sociodemographic differences in AUD treatment response.

    Importance Millions of children worldwide are experiencing prolonged symptoms after SARS-CoV-2 infection, yet social risk factors for developing long COVID are largely unknown. As child health is influenced by the environment in which they live and interact, adverse social determinants of health (SDOH) may contribute to the development of pediatric long COVID. Objective To identify whether adverse SDOH are associated with increased odds of long COVID in school-aged children and adolescents in the US. Design, Setting, and Participants This cross-sectional analysis of a multicenter, longitudinal, meta-cohort study encompassed 52 sites (health care and community settings) across the US. School-aged children (6-11 years; n = 903) and adolescents (12-17 years; n = 3681) with SARS-CoV-2 infection history were included. Those with an unknown date of first infection, history of multisystem inflammatory syndrome in children, or symptom surveys with less than 50% of questions completed were excluded. Participants were recruited via health care systems, long COVID clinics, fliers, websites, social media campaigns, radio, health fairs, community-based organizations, community health workers, and existing research cohorts from March 2022 to August 2024, and surveys were completed by caregivers between March 2022 and August 2024. Exposure Twenty-four individual social determinant of health factors were grouped into 5 Healthy People 2030 domains: economic stability, social and community context, caregiver education access and quality, neighborhood and built environment, and health care access and quality. Latent classes were created within each domain and used in regression models. Main Outcomes and Measures Presence of long COVID using caregiver-reported, symptom-based, age-specific research indices. Results The mean (SD) age among 4584 individuals included in this study was 14 (3) years, and 2330 (51%) of participants were male. The number of latent classes varied by domain; the reference group was the class with the least adversity. In unadjusted analyses, most classes in each domain were associated with higher odds of long COVID. After adjusting for many factors, including age group, sex, timing of infection, referral source, and other social determinant of health domains, economic instability characterized by difficulty covering expenses, poverty, receipt of government assistance, and food insecurity were associated with an increased risk of having long COVID (class 2 adjusted odds ratio [aOR], 1.57; 95% CI, 1.18-2.09; class 4 aOR, 2.39; 95% CI, 1.73-3.30); economic instability without food insecurity (class 3) was not (aOR, 0.93; 95% CI, 0.70-1.23). Poorer social and community context (eg, high levels of discrimination and low social support) was also associated with long COVID (aOR, 2.17; 95% CI, 1.77-2.66). Sensitivity analyses stratified by age group and adjusted for race and ethnicity did not alter or attenuate these results. Conclusions and Relevance In this study, economic instability that included food insecurity and poor social and community context were associated with greater odds of pediatric long COVID. Those with food security, despite experiencing other economic challenges, did not have greater odds of long COVID. Further study is needed to determine if addressing SDOH factors can decrease the rate of pediatric long COVID.
  • Common factors and unique pathways for linkages between HIV/STI prevention and syndemic behaviours in high-risk youth: protocol for a secondary analysis of harmonised data from six clinical trials

    Introduction HIV/sexually transmitted infection (STI) prevention interventions are only modestly successful among youth, particularly for young people of colour and sexual and gender minority (SGM) youth. Even among disparate intervention modalities delivered with high fidelity, differences between intervention types have been minimal. One consistent theme has emerged: the role of the youth:provider relationship in predicting intervention response. In line with calls for examination of relational factors, the next essential step is a harmonised analysis to evaluate connections between the youth:provider relationship and co-occurrence of alcohol and cannabis use, in youth HIV/STI prevention intervention response. Methods and analysis Our team has completed six sizeable HIV/STI behavioural prevention studies, generating n=1136 independent youth (baseline M age =17, range=13–24; 43% female; 21% SGM; 54% Hispanic; 9% African American; 7% Native American/Alaska Native) who received prevention programming and were followed at 1-, 3-, 6-, and/or 12 months. We will harmonise these studies and build a longitudinal mixed-effects machine learning model, with youth:provider relationship as a predictor of intervention response. Participant factors, provider factors and their interaction will be included in the model. Given high rates of alcohol and cannabis comorbidity, we will also examine syndemic outcomes (co-occurring HIV/STI risk behaviours, alcohol use and cannabis use). These data are crucial to informing next step HIV/STI and syndemic intervention programming with this age group. Ethics and dissemination This secondary analysis study is exempt from human subjects regulations under category 4(iii) as determined by the Institutional Review Board at UConn Health. Results will be disseminated via presentations at annual scientific conferences, submissions to peer-reviewed journals, to mental health and substance use providers, as well as community programmes for youth at high risk for HIV/STI and substance use.
  • Distinct substance use patterns and risk of unintentional injury, violence, and mortality in adolescence: a latent class analysis and 8-year prospective cohort study of 68301 students aged 15–19 years

    Adolescence is a life stage characterized by physical, social, and emotional changes including a shift towards peer orientation and an explorative approach to substance use. Previously, we identified distinct adolescent substance use patterns. Here we characterized adolescents with distinct patterns of substance use and tested whether patterns were associated with acute outcomes. Data from the Danish National Youth Cohort 2014, comprising 68 301 participants aged 15–19 years attending upper secondary education, were used. Previously identified substance use patterns were Alcohol Only (48.8%), Frequent Binge Drinking (23.3%), Experimental Use (16.3%), and Early Multiple Use (11.6%). Adolescents with distinct substance use patterns were characterized by adverse childhood experiences, social networks, parental support, and mental health. Associations between patterns and the risk of unintentional injury, violence, and mortality were assessed over an 8.2-year follow-up period. An accumulation of adverse childhood experiences, lack of parental support and poor mental health were observed in adolescents with most substance use. Substance use patterns were associated with the risk of unintentional injury, violence, and mortality in a dose-dependent manner. For instance, compared to Alcohol Only, hazard ratios (95% CI) for severe unintentional injury were 1.25 (1.08–1.44), 1.40 (1.17–1.66), 1.50 (1.25–1.79) for adolescents with Frequent Binge Drinking, Experimental Use, and Early Multiple Use. Adolescent substance use patterns were associated with short- and long-term risks of acute health outcomes. Additionally, disadvantages cluster in adolescents with more substance use, highlighting the aggregation and potential interaction of challenges faced by vulnerable subgroups that extend into adulthood.

    Alcohol use during adolescence has both immediate as well as long-term negative effects; in turn, delaying onset and preventing hazardous use in early adolescence is one essential avenue to protect and bolster adolescents’ socioemotional growth and neurodevelopment. This trial examines the effectiveness, feasibility and acceptability of a novel neurodevelopmentally informed intervention – Adolescent Developmentally-Appropriate health Promotion Therapy (ADAPT) among students in middle school. We hypothesize that, compared to the delayed treatment condition (DT), ADAPT students will show statistically significant behavior change across measures, including: intentions to drink (primary outcome), alcohol use and alcohol-related consequences, well-being and life satisfaction (secondary outcomes) from baseline to 3 months. The study employs a 2-condition cluster randomized controlled trial comparing: (1) ADAPT with a (2) DT condition. Approximately N = 1000 8th grade students (ages 13–15) will be recruited from public middle schools in a global region with elevated rates of adolescent drinking (Denmark; during the school year 2025/2026). Data evaluating primary and secondary outcome measures (e.g., intentions to drink, alcohol use, alcohol-related consequences, well-being, life satisfaction) will be collected via online questionnaires at baseline, and 1- and 3-months post intervention. Feasibility and acceptability will be assessed via attendance in ADAPT group sessions and parent meetings, student surveys, and interviews with middle school staff. This is the first trial to examine effectiveness of the novel neurodevelopmentally informed intervention ADAPT on adolescent drinking and well-being. Novel interventions are needed to address the high prevalence of early drinking and the past decade decline in adolescent well-being, particularly in regions of the globe with elevated adolescent drinking rates. The findings of the study hold substantial significance for the health and well-being of adolescents and can inform evidence-based decisions on alcohol prevention policy, and professional health care practices in Scandinavia and throughout the globe.     Purpose: Little is known about transgender and gender expansive (TGE) adolescents' and young adults' (AYAs') pain and psychosocial experiences in the acute postsurgical period following gender-affirming surgery (GAS). This study describes pain symptomatology and psychosocial functioning within 1 month after GAS among TGE AYAs, examines immediate postsurgical associations of cannabis use with pain symptomatology, pain catastrophizing, and psychosocial functioning, and explores pain persistence, cannabis use, and psychosocial functioning in a subgroup of individuals 6 months after surgery. Methods: -tests examined differences by age, past 30-day cannabis use, and from the postsurgical period to 6-month follow-up. Results: Participants reported acute and persistent pain following GAS. Younger age was associated with improved pain interference in the postoperative stage. Reports of past 30-day cannabis use were high in this sample, and cannabis use was associated with higher pain interference, anxiety, and depression. Conclusion: This study is the first to assess AYA pain functioning, mental health, and cannabis use following GAS within 1 month of surgery and at 6 months. Findings suggest that 6 months after GAS is a postsurgical adjustment phase, necessitating more supportive perioperative and psychosocial resources, including attention to substance use, during this crucial window.
  • ADHD (Attention-Deficit Hyperactivity Disorder) Symptoms Are Associated With Chronic Pain Interference: Results From a Prospective Cohort Study

    BackgroundDespite a known relationship between attention‐deficit hyperactivity disorder (ADHD) and chronic pain, the association between ADHD symptoms and pain interference has not been prospectively investigated.MethodsYoung adults were recruited following receipt of a prescription opioid from ambulatory surgery clinics, outpatient clinics and emergency departments as part of a larger study. Participants completed measures of ADHD symptoms, depression, pain catastrophizing, adverse childhood experiences and pain interference at enrolment and 6 months later. Logistic regression was used to examine the relationship between ADHD symptoms and covariates on moderate‐to‐severe pain interference at 6 months.ResultsParticipants were 116 young adults who completed the baseline ADHD symptoms measure; 71 completed the 6‐month timepoint. Moderate‐to‐severe pain interference was present among 89.7% at baseline and 52.3% at 6 months. ADHD symptoms (OR [odds ratio] = 1.42, 95% CI [confidence interval] = 1.05–1.93), depression (OR = 1.08, 95% CI = 1.02–1.14) and pain catastrophizing (OR = 1.06, 95% CI = 1.01–1.12) were significantly associated with odds of moderate‐to‐severe pain interference at 6 months. In multivariable regression, ADHD symptoms (OR = 1.52, 95% CI = 1.09–2.12) and pain catastrophizing (OR = 1.07, 95% CI = 1.01–1.13) were significant predictors of moderate‐to‐severe pain interference at 6 months.ConclusionsParticipants with more ADHD symptoms were at greater risk for chronic pain interference. This is consistent with prior studies examining the role of attention in chronic pain. Individuals with ADHD symptoms may be at greater risk for chronic pain and future research should investigate tailored prevention and treatment approaches.

    ImportanceMost research to understand postacute sequelae of SARS-CoV-2 infection (PASC), or long COVID, has focused on adults, with less known about this complex condition in children. Research is needed to characterize pediatric PASC to enable studies of underlying mechanisms that will guide future treatment.ObjectiveTo identify the most common prolonged symptoms experienced by children (aged 6 to 17 years) after SARS-CoV-2 infection, how these symptoms differ by age (school-age [6-11 years] vs adolescents [12-17 years]), how they cluster into distinct phenotypes, and what symptoms in combination could be used as an empirically derived index to assist researchers to study the likely presence of PASC.Design, Setting, and ParticipantsMulticenter longitudinal observational cohort study with participants recruited from more than 60 US health care and community settings between March 2022 and December 2023, including school-age children and adolescents with and without SARS-CoV-2 infection history.ExposureSARS-CoV-2 infection.Main Outcomes and MeasuresPASC and 89 prolonged symptoms across 9 symptom domains.ResultsA total of 898 school-age children (751 with previous SARS-CoV-2 infection [referred to as infected] and 147 without [referred to as uninfected]; mean age, 8.6 years; 49% female; 11% were Black or African American, 34% were Hispanic, Latino, or Spanish, and 60% were White) and 4469 adolescents (3109 infected and 1360 uninfected; mean age, 14.8 years; 48% female; 13% were Black or African American, 21% were Hispanic, Latino, or Spanish, and 73% were White) were included. Median time between first infection and symptom survey was 506 days for school-age children and 556 days for adolescents. In models adjusted for sex and race and ethnicity, 14 symptoms in both school-age children and adolescents were more common in those with SARS-CoV-2 infection history compared with those without infection history, with 4 additional symptoms in school-age children only and 3 in adolescents only. These symptoms affected almost every organ system. Combinations of symptoms most associated with infection history were identified to form a PASC research index for each age group; these indices correlated with poorer overall health and quality of life. The index emphasizes neurocognitive, pain, and gastrointestinal symptoms in school-age children but change or loss in smell or taste, pain, and fatigue/malaise–related symptoms in adolescents. Clustering analyses identified 4 PASC symptom phenotypes in school-age children and 3 in adolescents.Conclusions and RelevanceThis study developed research indices for characterizing PASC in children and adolescents. Symptom patterns were similar but distinguishable between the 2 groups, highlighting the importance of characterizing PASC separately for these age ranges.
