  • Interdental Brushing and Obstetrical Outcomes in Nulliparous Pregnant Women: Insights From a Cluster Randomised Controlled Trial

    Aim Pregnancy gingivitis is potentially modifiable, making it a promising target for preventive strategies to reduce adverse obstetrical outcomes. The primary objective of this study was to evaluate whether an interdental hygiene intervention in early pregnancy could reduce obstetrical complications. Exploratory analyses focused on potential biological pathways linking oral inflammation and obstetrical outcomes. Materials and Methods A two‐arm randomised controlled trial (RCT) with cluster allocation (1:1) was conducted in six antenatal clinics in Senegal among nulliparous women at 12 weeks. Participants received either a daily calibrated interdental brushing intervention (Intervention) or usual oral hygiene (Control). Follow‐up visits were scheduled at 5 and 8 months. The primary endpoint was a composite of preterm birth, low birth weight, small for gestational age or pre‐eclampsia, analysed exploratorily under the intention‐to‐treat principle. Results A total of 323 women were randomised (162 intervention, 161 control). The composite outcome occurred in 28.4% of the intervention group and 24.2% of the control group ( p = 0.449). After adjustment, the intervention was not significantly associated (aOR = 0.65; 95% CI: 0.39–1.10; p = 0.107). Exploratory analyses suggested that interdental bleeding and C‐reactive protein (CRP) could act as mediators, accounting for ~60% of the effect, with the remaining ~40% representing a direct effect. No intervention‐related adverse events were observed. Conclusion This RCT did not demonstrate a significant reduction in obstetrical complications with early interdental prophylaxis. However, the intervention was safe, acceptable and feasible within antenatal care, highlighting the need for adequately powered trials to clarify its potential clinical impact.

    Objective This systematic review aimed to assess the available literature on the subgingival bacterial microbiota profiles of PLHIV undergoing HAART. Materials and Methods This research was conducted in accordance with the PRISMA guidelines. Electronic searches were performed in the Cochrane Library, LILACS, PubMed, and Science Direct databases to identify primary studies evaluating the subgingival bacterial microbiota of individuals living with HIV‐1 undergoing HAART. The search was performed by two independent researchers and covered the period from January 2001 to July 2022. Studies published in English, Portuguese, and Spanish were included. Results A total of 457 articles were initially retrieved. Of these, 9 met the eligibility criteria. The included studies revealed a high frequency of periodontal pathogens among individuals living with HIV‐1 on HAART, including Treponema denticola (44%, n = 4 studies) and Porphyromonas gingivalis (33%, n = 3 studies), as well as species not commonly found in patients with periodontal disease, such as Enterococcus faecalis (33%, n = 2 studies). Conclusions According to the present findings, the most frequently identified subgingival bacterial species in patients living with HIV‐1 and receiving antiretroviral therapy were T. denticola and P. gingivalis , without establishing a consistent or unique microbiological profile.
  • Periodontal Health in Individuals Living with HIV: An Exploratory and Descriptive Molecular Approach of Microbial Interspecific and Intraspecific Diversity in Brazilian Patients

    Oral manifestations of HIV infection can be an early sign of the disease and may indicate progression to AIDS. Although antiretroviral therapies, especially highly active antiretroviral therapy (HAART), have reduced the prevalence of HIV-related oral lesions, ongoing updates in diagnosis and treatment are essential due to the extended life expectancy of individuals living with HIV. Periodontal disease is a significant concern in these patients, influenced by altered immune responses and microbial dynamics, though the mechanisms are not fully understood. This exploratory study aimed to investigate the oral microbiota and periodontal disease prevalence in HIV-positive individuals by analyzing subgingival plaque samples from 24 patients. We identified 12 bacterial species using Polymerase Chain Reaction (PCR) and amplicon sequencing. Seven species were detected, with Filifactor alocis, Tannerella forsythia, and Porphyromonas endodontalis being the most common. Porphyromonas gingivalis was present in only 13.6% of samples, while T. forsythia was found in 58.3%. Genetic diversity was also observed in P. endodontalis and Selenomonas sputigena amplicons, with specific single nucleotide polymorphisms (SNPs) identified in both species. These results highlight the complex microbial interactions in the oral environments of people living with HIV, emphasizing the need for personalized diagnostic and therapeutic strategies for managing oral health in this population.
  • Association of anxiety, age and oral health-related quality of life with periodontitis: A case-control study

    OBJECTIVE This case-control study evaluated the association of sociodemographic profile, mental health disorders and oral health-related quality of life (OHRQoL) with periodontitis. METHODS Patients with periodontitis (PP, n = 50) and control patients (CP, n = 50) were allocated into 2 groups after a complete periodontal examination. Data collection included age, sex, marital status, education and application of 2 questionnaires: Hospital Anxiety and Depression Scale for the diagnosis of anxiety/depression and Impact Profile on Oral Health (OHIP-14) for classification of well-being. An adjusted multiple binary logistic regression analysis was performed to assess the effect of all studied covariates on periodontitis. RESULTS The results show that mean clinical attachment loss and periodontal probing depth were 5.92 (SD = 0.42) and 5.46 (SD = 0.78) in PP and 0.00 (SD = 0.00) and 2.85 (SD = 0.23) in CP, respectively (p < 0.001). The regression analysis demonstrated a significant effect on periodontitis for age (OR = 1.13; p < 0.0001; 95% CI: 1.07-1.20), with the PP having more people aged 50 years or older than CP, anxiety (OR = 1.25; p = 0.020; 95% CI: 1.04-1.50) and OHIP-14 (OR = 1.17; p < 0.0001; 95% CI: 1.08-1.226). CONCLUSIONS The findings showed a positive association between anxiety, OHRQoL and age with periodontitis.
  • Factors influencing the outcome of nonsurgical root canal treatment and retreatment: A retrospective study

    This study assessed the influence of diverse variables on the outcome of nonsurgical root canal treatment/retreatment. In general, 304 teeth from 218 patients were treated/retreated and the outcome evaluated by the periapical index (PAI). Teeth with apical periodontitis lesions that have not completely healed were classified as success or failure based on lenient and rigid criteria, respectively. Findings were evaluated using a logistic regression analysis. The overall success rates were 74% and 82% using the PAI‐rigid and lenient success criteria, respectively. Specifically for treatment, the success rates were 73% (rigid) and 82% (lenient), while for retreatment they were 78% (rigid) and 83% (lenient). The treatment outcome was negatively affected by overextension, presence of preoperative lesion, lesion size &gt;10 mm, and higher number of treatment visits (with no intracanal medication). Regarding retreatment, the chance of success was greater for teeth with adequate coronal restorations.

    OBJECTIVE To evaluate the prevalence and clinical aspects of oral candidiasis in patients hospitalised in the intensive care unit. METHODS This is a longitudinal and prospective study that included 48 participants hospitalised in the intensive care unit. Sociodemographic data, presence of systemic disorders, use of medications, laboratory tests, cause of hospital admission, type of breathing, and length of hospital stay were obtained from medical records. Oral clinical inspection and cytopathological examinations were performed on all participants. The diagnosis of clinical candidiasis was based on the presence of clinical alterations together with positive cytopathological examination results. The diagnosis of subclinical candidiasis was based on the absence of clinical lesions and a positive cytopathological examination. The absence of oral candidiasis was considered when the participant did not present oral lesions and had a negative cytopathological examination. RESULTS Clinical candidiasis was present in 18.8% of the 48 participants, and 45.8% of them had the subclinical form. Levels of urea (P = 0.005), creatinine (P = 0.009), haemoglobin (P = 0.009), haematocrit (P = 0.011), bands (P = 0.024), international normalised ratio (INR; P = 0.034), types of breathing (P = 0.017), length of hospital stay (P = 0.037), and outcome (P = 0.014) demonstrated statistically significant differences between the groups with and without oral candidiasis. CONCLUSIONS Clinical and subclinical forms of oral candidiasis are frequent in intensive care unit patients. Levels of urea, creatinine, haemoglobin, haematocrit, bands, INR, type of breathing, length of hospital stay, and outcome can be associated with the presence of candidiasis.
