Combined Melorheostosis and Osteopoikilosis: Uncommon Presentation with Sciatic Nerve Neuropathy: A Case Report Ismail Tawfeek Badr, Mohamed Kamal Mesregah, Sanjiv Rampal, Mahmoud Mohamed Moawad, Bahaa Zakarya Hasan Jbjs Case Connector, 2026 Case: A 27-year-old man presented with an 8-year history of chronic right gluteal and posterior thigh pain, worsened by sitting and unresponsive to conservative treatment. Imaging revealed widespread melorheostosis and osteopoikilosis of the right lower extremity, with 2 gluteal soft tissue masses compressing the sciatic nerve. The masses were excised using a dual surgical approach, achieving successful sciatic nerve decompression. Postoperatively, the patient regained normal sitting and sleeping tolerance, with complete pain relief maintained over a 15-month follow-up. Conclusion: Melorheostosis, a rare sclerosing bone disorder sometimes associated with osteopoikilosis, may require surgery when large lesions cause nerve compression.
Autogenous structural bone graft reconstruction of ≥ 10-mm-deep uncontained medial proximal tibial defects in primary total knee arthroplasty Ahmed Abdel-Monem Dewidar, Mohamed Kamal Mesregah, Mustafa Mohamed Mesriga, Ahmed Mohamed El-Behiry Journal of Orthopaedics and Traumatology, 2024 Background Management of uncontained medial proximal tibial defects during primary total knee arthroplasty (TKA) can be challenging, especially for defects ≥ 10 mm in depth. This study sought to assess the outcomes of autogenous structural bone grafts to address these defects. Materials and methods In this prospective study, patients with uncontained medial proximal tibial defects ≥ 10 mm in depth undergoing TKA were managed by autogenous structural bone grafts fixed by screws and were followed up for at least 36 months. Patients were followed-up clinically with Knee Society Score (KSS) and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Additionally, radiological follow-up was done to assess bone graft union and implant stability. Results The study included 48 patients with a mean age of 69.2 ± 4.5 years. The mean body mass index (BMI) was 31.4 ± 3.7 kg/m2. The mean defect depth was 17 ± 3.6 mm. With a mean follow-up period of 52.2 ± 12.3 months, the median KSS improved significantly from 30 preoperatively to 89, P < 0.001. The median WOMAC score reduced significantly from 85 preoperatively to 30.5, P < 0.001. The mean ROM increased significantly from 73 ± 12.4 preoperatively to 124 ± 8.4 degrees, P < 0.001. The mean graft union time was 4.9 ± 1 months. No significant complications were reported. Conclusions Autogenous bone graft reconstruction is a safe and effective method of addressing uncontained medial proximal tibial defects in primary TKA. Level of evidence Level IV.
Radiographic Risk Factors for Adjacent Segment Disease Following Anterior Cervical Discectomy and Fusion (ACDF): A Systematic Review and Meta-Analysis Mohamed Kamal Mesregah, Melissa Baker, Camilla Yoon, Hans-Joerg Meisel, Patrick Hsieh, et al. Objectives To assess the radiographic risk factors for adjacent segment disease (ASD) following anterior cervical discectomy and fusion (ACDF) for degenerative cervical spine pathologies. Methods PubMed, Embase and the Cochrane Library databases were searched up to December 2023. The primary inclusion criteria were degenerative spinal conditions treated with ACDF, comparing radiological parameters in patients with and without postoperative ASD. The radiographic parameters included intervertebral disc height, cervical sagittal alignment, sagittal segmental alignment, range of motion, segmental height, T1 slope, sagittal vertical axis (SVA), thoracic inlet angle (TIA), and plate to disc distance (PPD). Risk of bias was assessed for all studies. The Cochrane Review Manager was utilized to perform the meta-analysis. Results From 7044 articles, 13 retrospective studies were included in the final analysis. Three studies had “not serious” bias and the other 10 studies had serious or very serious bias. The total number of patients in the included studies was 1799 patients. Five studies included single-level ACDF, 2 studies included multi-level ACDF, and 6 studies included single or multi-level ACDF. On meta-analysis, the significant risk factors associated with ASD development were reduced postoperative cervical lordosis (mean difference [MD] = 3.35°, P = .002), reduced last-follow-up cervical lordosis (MD = −3.02°, P = .0003), increased preoperative to postoperative cervical sagittal alignment change (MD = −3.68°, P = .03), and the presence of developmental cervical canal stenosis (Odds ratio [OR] = 4.17, P < .001). Conclusions Decreased postoperative cervical lordosis, greater change in cervical sagittal alignment and developmental cervical canal stenosis were associated with an increased risk of ASD following ACDF.
