The dual impact of GBA1 in disease: from germline mutations in neurological disorders to alterations in cancer Valentina Fantini, Giulia Di Rauso, Valentina Fioravanti, Alessia Ciarrocchi, Francesco Cavallieri, Valentina Sancisi Cell Death Discovery, 2026 The GBA1 gene encodes the enzyme glucocerebrosidase, which is responsible for lysosomal degradation of the glycosphingolipid glucosylceramide. Biallelic mutations in GBA1 are causative for Gaucher disease, whereas either monoallelic or biallelic mutations are a risk factor for Parkinson’s disease. GBA1 mutations, beside reducing enzymatic activity and leading to substrate accumulation, influence a number of molecular and cellular pathways, including lipid homeostasis, endosome-lysosome pathway, endoplasmic reticulum to Golgi protein trafficking, autophagy and mitophagy. Given the critical role of GBA1 in these key pathways for cellular homeostasis, it can be expected that alterations in this enzyme may influence also cancer development and/or pathology, keeping in mind that Gaucher disease is associated with an increased risk of cancer development. Notably, a large fraction of patients affected by different cancer types carry an amplification of the long arm of chromosome 1, that includes the GBA1 gene. Furthermore, GBA1 expression is elevated in different cancer tissues, compared with healthy counterparts and associated with outcome in some cases. In this perspective, we narratively review the main evidence supporting a role for GBA1 in influencing tumorigenesis and we present our analyses on GBA1 amplification and expression throughout different cancer types. Taken together, these data suggest that the presence of a GBA1 germline mutation or a somatic amplification may influence cancer pathogenesis and/or response to therapies through context-dependent mechanisms that are still to be characterized.

Bhatia, Marios Politis European Journal of Nuclear Medicine and Molecular Imaging, 2026 We investigated serotonergic and dopaminergic terminal integrity role in PRKN-associated Parkinsonism (PRKN-PD) using [¹¹C]DASB Positron Emission Tomography (PET) and [¹²³I]FP-CIT Single Photon Emission Computerised Tomography (SPECT). Fourteen PRKN-PD patients (mean age 49.70 ± 10.83, disease duration 10.95 ± 7.59 years, H&Y 2.0 ± 0.68), and twelve iPD patients (mean age 65.40 ± 7.48 years, disease duration 5.05 ± 4.50 years, H&Y 2.0 ± 0.93) underwent clinical assessments, 3-Tesla MRI, [11C]DASB PET-CT, and [123I]FP-CIT SPECT, and compared with previously acquired healthy control (HCs) data. [11C]DASB distribution volume ratio (DVR) parametric images were generated and DVR-1 values, equivalent to BPND, sampled from a priori selected regions-of-interest (ROIs) with the posterior cerebellum as reference. [123I]FP-CIT images underwent reconstruction and normalization to standard space, and striatal Specific Binding Ratio (SBR) calculated from the eight hottest consecutive slices. PRKN-PD patients showed 20.8% to 45.2% [11C]DASB BPND loss across several cortical and subcortical ROIs compared to HCs (p ≤ 0.01). After adjusting for age and disease duration, no differences in [11C]DASB BPND were observed between PRKN-PD and iPD. In PRKN-PD, higher [11C]DASB BPND in the raphe, brainstem, ventral striatum, and amygdala, correlated with higher scores on the Non-Motor Symptoms Scale (p < 0.05). Higher [11C]DASB BPND in the caudate and putamen correlated with higher scores on the Beck Depression Inventory II scale (p < 0.05). No correlation was detected between [11C]DASB BPND and [123I]FP-CIT SBR in the caudate and putamen. PRKN-PD is characterized by widespread serotonergic dysfunction, which is independent of dopaminergic degeneration and linked to key non-motor symptoms, particularly depression. These findings provide novel insights into the pathophysiology of PRKN-PD.
The use of single-point magneto-inertial measurement for gait and dynamic balance assessment of patients with Parkinson’s disease and Parkinsonisms: a systematic review with critical appraisal of clinical applications and quality of reporting Lorenzo Cavazzuti, Benedetta Damiano, Andrea Merlo, Maria Chiara Bò, Sara Scaltriti, Francesco Cavallieri, Giulia Di Rauso, Giacomo Portaro, Giacomo Argenziano, Valentina Fioravanti, Isabella Campanini Frontiers in Neurology, 2026 Introduction Gait and balance impairments are among the most disabling motor symptoms of Parkinson’s disease (PD) and parkinsonisms. Traditional clinical assessments are practical but often lack sensitivity to detect subtle functional changes and to monitor disease progression. Wearable magneto-inertial measurement units (mIMUs) offer an objective, portable, and low-cost alternative. Single-point mIMUs, positioned over the lower trunk, are particularly promising for clinical integration due to their simplicity. This systematic review critically evaluates how single-point mIMUs are used to assess functional mobility in PD and atypical parkinsonisms, and examines the quality and clinical transferability of current evidence. Methods Following PRISMA 2020 guidelines, a systematic search was conducted in Medline, CINAHL, Embase, and Scopus (January 2025). Eligible studies included adults with PD or atypical parkinsonisms assessed with a single wearable sensor during straight-line walking and/or the Timed Up and Go test. Data extraction included clinical characteristics, assessment protocols, sensor specifications, and computed parameters. A customized critical appraisal tool evaluated five domains: methodology, clinical aspects, assessment protocol, technical aspects, and clinical transferability. Studies were classified as high-, medium-, or low-quality. Results From 3,812 records, 41 studies met inclusion criteria, involving 395 participants (mean age 66 ± 5.9 years). Fourteen studies used the instrumented TUG, and 27 evaluated straight-line walking. Most studies were observational (90%). Only 12% achieved a high-quality rating, while 80% were medium-quality. “Technical aspects” was the best-rated domain, whereas “clinical aspects” and “assessment protocol” scored lowest, mainly due to incomplete reporting of sensor placement, acquisition conditions, computational methods, and clinical characterization of participants. Frequently reported outcomes included gait speed, step length, cadence, and TUG duration; however, methodological heterogeneity hindered cross-study comparability. Discussion Single-point mIMUs can detect clinically relevant gait and mobility impairments and hold potential for monitoring disease progression, evaluating treatment effects, and distinguishing patient subgroups. Nevertheless, the overall quality of evidence is moderate, and insufficient methodological transparency currently limits clinical translation. Standardization of sensor placement, task instructions, data processing, and reporting—along with clearer clinical rationale and sample characterization—is needed to support reproducibility and facilitate adoption in routine practice. Objectives The aim of this study was to assess whether Artificial Intelligence (AI) could predict GBA1 ‐mutated genotype in PD ( GBA1 ‐PD). Particularly, the main objective was to identify a Machine Learning (ML) model capable of accurately providing a pre‐test estimate of GBA1 ‐mutated status, relying on the clinical and demographic variables with the highest predictive value. Methods A cohort of GBA1 ‐PD patients has been paired with non‐mutated PD (NM‐PD). The dataset comprised 58 GBA1 ‐PD and 58 NM‐PD, for each of whom 124 features were recorded. A Leave‐One‐Out cross‐validation method was employed for testing and SHapley Additive exPlanations (SHAP) for examine each feature's contribution. XGBoost resulted the most effective ML model for this supervised classification task. Results Through AI, we developed a model based on four specific clinical features with significant impact in predicting GBA1 ‐mutated genotype with an accuracy of 73%, reaching 94% in a subset of patients where the model has a SHAP confidence level greater than 80%. These variables included family history and scores for cognitive (MDS‐UPDRS 1.1) and motor impairment (MDS‐UPDRS 3.8a and 3.8b and rigidity subscore). Conclusions This study underlies the potential of AI in enhancing targeted genetic screening in PD, especially in clinical settings where resources are limited. Main limitations of this study are the modest sample size and lack of external validation. Further studies on larger, independent cohorts are needed to refine the predictive model.

It encodes glucocerebrosidase (GCase), whose dysfunction - seen in Gaucher disease - leads to the accumulation of glucosylceramide and its derivate glucosylsphingosine (GlcSph). However, it remains unclear whether GCase and GlcSph are relevant in PD patients carrying no or monoallelic GBA1 variants, and what their clinical impact might be. OBJECTIVE: Investigating the relationships between GBA1 mutations, GCase, GlcSph, and clinical features in a large PD cohort. METHODS: We performed a cross-sectional study of PD patients screened for GBA1 mutations, GCase activity, and GlcSph via dried blood spot tests. Patients were classified as heterozygous mutation carriers (GBA1-PD) or non-carriers (nonGBA1-PD). Collected data included motor and non-motor parameters. Molecular and clinical differences were compared between GBA1-PD and nonGBA1-PD. Distinctive clinical features were further investigated through multivariate models to test their correlations with biochemical data. RESULTS: The cohort included 611 subjects (225 GBA1-PD, 386 nonGBA1-PD). GBA1-PD presented earlier onset, lower cognitive scores, higher incidence of mood disturbances and more advanced stage. Motor assessment revealed a higher frequency and severity of dyskinesias, independently from disease duration and LEDD. GlcSph levels showed an independent correlation with dyskinesia severity and time at onset in GBA1-PD patients, which was independent of sex, LEDD, UPDRS-III, disease duration and GBA1 mutation class. CONCLUSIONS: This study reveals an association between GlcSph and dyskinesias in GBA1-PD, that should prompt further investigation to assess the GlcSph role as a possible biomarker and target to tackle dyskinesias in GBA1-PD.
