  • Evaluation of the Association Between Body Mass Index and Semen Analysis Parameters

    Introduction Male factor infertility contributes substantially to infertility burden worldwide. Excess adiposity has been proposed as a modifiable risk factor that may impair spermatogenesis through endocrine disruption, oxidative stress, and inflammatory pathways. However, evidence regarding the association between body mass index (BMI) and conventional semen parameters remains inconsistent, and data from Southeast Asian infertility populations are limited. Materials and Methods This cross‐sectional study was conducted at the Fertility Unit of Hospital Universiti Sains Malaysia. Medical records of men aged ≥18 years undergoing infertility evaluation between January 2020 and January 2025 were reviewed. BMI was calculated as weight (kg)/height 2 (m 2 ) and categorized according to World Health Organization criteria (underweight, normal, overweight, obese, and morbidly obese). Semen fluid analysis (SFA) parameters included total sperm count, progressive motility, and normal morphology, assessed according to World Health Organization 2010 reference standards. Comparisons across BMI categories were performed using analysis of variance. Pearson correlation assessed associations between BMI and semen parameters. Multivariable logistic regression was used to evaluate the association between BMI categories and abnormal SFA. Statistical significance was set at p < 0.05. Results A total of 374 men were included. Mean (SD) total sperm count, progressive motility, and normal morphology were 77.7 (70.9) million, 49.7 (22.6)%, and 62.3 (28.3)%, respectively. There were no statistically significant differences in sperm count ( F = 2.112, p = 0.079), progressive motility ( F = 1.496, p = 0.203), or normal morphology ( F = 1.221, p = 0.302) across BMI categories. A significant deviation from linearity was observed for sperm count ( p = 0.043), suggesting a nonlinear relationship across BMI groups. Conclusion BMI was not strongly associated with conventional semen parameters among men undergoing infertility evaluation. These findings suggest that BMI alone has limited clinical utility as a predictor of routine semen quality and should be interpreted within a broader assessment of male reproductive and metabolic health.
  • Association of DPF3 polymorphisms (rs10129954) with male infertility in the Malaysian population-A preliminary study

    Male infertility is a multifactorial disorder with genetic abnormalities contributing to 15-30% of cases. Numerous genetic polymorphisms have been investigated to understand their role in male infertility, but few studies show significant associations. The Double PHD Fingers 3 (DPF3) gene is essential for chromatin remodelling and histone-to-protamine transition during spermatogenesis. This study aims to investigate the association between DPF3 (rs10129954) polymorphism and male infertility in Malaysians. This case-control study involved 30 infertile and 30 fertile control males. Genomic DNA was extracted, and genotyping was performed using the polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) technique. Associations between genotypes and infertility risk were assessed using Fisher’s Exact Test and odds ratios (ORs) with 95% confidence intervals (CI). The frequencies of CC, CT, and TT genotypes of DPF3 (rs10129954) polymorphism were 3.3%, 36.7%, and 60.0% in infertile males, and 0%, 23.3%, and 76.7% in fertile controls, respectively. The allele frequencies for C and T were 21.7% and 78.3% in infertile males, compared to 11.7% and 88.3% in fertile control males. No significant differences were observed between the frequencies of DPF3 (rs10129954) genotype and allele in infertile and fertile groups (p>0.05). The T allele was associated with a reduced risk of male infertility (OR: 0.478; 95% CI: 0.176-1.297; p=0.074), differing from studies in Chicago, Japanese and Han Chinese populations, which linked the T allele to an increased risk. Our findings suggest that the T allele of DPF3 (rs10129954) is not significantly associated with male infertility. Larger studies are needed to confirm this observation.
  • Hypothalamic amenorrhoea as an extraintestinal manifestation of Crohn’s disease: A case report

    Crohn's disease (CD), a chronic inflammatory bowel condition, although rare in Malaysia, can present with atypical systemic features, including reproductive dysfUnction. We report the case of a 31-year-old woman referred for secondary amenorrhoea and presumed endometriosis, later diagnosed with CD following worsening gastrointestinal symptoms and significant weight loss. Hormonal evaluation revealed hypothalamic-pituitary axis suppression secondary to malnutrition and systemic inflammation. Treatment with corticosteroids and nutritional rehabilitation led to weight gain and resumption of menses within 3 months. This case highlights the importance of recognising functional hypothalamic amenorrhoea as a reversible consequence of chronic illness and nutritional deficiency. It also underscores how systemic diseases such as CD may mimic gynaecological conditions, potentially delaying accurate diagnosis. Early multidisciplinary collaboration is essential in evaluating menstrual disturbances with overlapping gastrointestinal symptoms, especially in regions where inflammatory bowel disease remains uncommon.

    Sister Mary Joseph's nodule is a rare but significant clinical sign that often indicates advanced intraabdominal malignancy. It presents as a palpable umbilical nodule and may be the first indication of an occult primary cancer. In this article, we discuss the case of a 58-year-old postmenopausal woman who presented to a primary care clinic with a darkened umbilical lesion and progressive abdominal symptoms. She wps ultimately diagnosed with metastatic endometrial carcinoma. This uncommon presentation highlights the importance of a comprehensive approach to umbilical lesions in the primary care setting. Recognising this subtle sign in primary care can lead to expedited investigations and timely intervention.
  • Comparison between hCG and GnRH Agonist for Ovulation Trigger in GnRH Antagonist In-Vitro Fertilization Cycles in a Tertiary Hospital in Malaysia: An observational study

    Objective hCG is commonly used as an ovulation trigger in IVF. Its usage is associated with OHSS. GnRH agonist is an alternative to hCG and is associated with reduced incidence of OHSS. This study compared the cycle outcomes of GnRH agonists with hCG as an ovulation trigger in IVF cycles. Methods The medical notes of 209 IVF cycles receiving GnRH agonist and hCG as ovulation trigger over 18 months were reviewed in this retrospective study. The number and quality of mature oocytes, the number and quality of embryos, pregnancy rates, and outcomes were compared using Independent T-test or One-way ANOVA for normal distribution. The Mann-Whitney test or Kruskal-Wallis test was used for not normally distributed. p<0.05 was considered statistically significant. Results The cycle outcomes of 107 GnRH agonist-trigger and 102 hCG-trigger were compared. The MII oocytes retrieved and 2PN count was significantly higher in the GnRH agonist trigger group (p<0.001). Clinical pregnancy rate and ongoing pregnancy were higher in the GnRH agonist trigger group but were not statistically significant. The GnRH agonist trigger group was associated with low OHSS than the hCG trigger group (n=2(1.9%) and n=12(11.8%) respectively, p=0.004). Conclusion GnRH agonist trigger is an option as a final maturation trigger in high-responder women undergoing IVF or ICSI cycles.

    Introduction: A retroperitoneal hematoma in obstetrics is very rare following spontaneous vaginal delivery. It requires a high index of suspicion, especially in a hemodynamically unstable patient with no active bleeding or external trauma postpartum, as early intervention is needed to prevent maternal morbidity and mortality. Conservative management may be an appropriate strategy in managing large hematomas even though surgical evacuation is classically recommended. Case: We report the case of a 39-year-old woman, para three, with a hematoma of the Retzius space after spontaneous uncomplicated vaginal delivery. The patient had sudden abdominal pain associated with urinary retention, two hours after delivery. She had a palpable mass at the anterior upper part of the vaginal wall, near the fornices, but no abnormality was detected at the labia majora, labia minora and lower part of the vagina. A transabdominal ultrasound scan revealed a mixed-echogenicity mass between the bladder and lower part of the uterus measuring 110 × 90 × 60 mm. The patient's hemoglobin level dropped from 12 g/dL to 10 g/dL. Given the patient's clinically stable state, she was managed conservatively with antibiotics, analgesics and one unit of packed red blood cells, and was discharged home day 5 postpartum. Conclusion: This case supports the conservative management of even large Retzius space hematomas for hemodynamically stable patients. A high index of suspicion and thorough evaluation are required when encountering a complaint of urinary retention and abdominal pain postpartum.
