Intentional Insulin Omission (Diabulimia) in Patients with Insulin-Dependent Diabetes: An Eating Disorder? A Systematic Review Maria Benedetta Anesini, Mario Pinto, Michela Bellezza, Georgios D. Kotzalidis, Tommaso Callovini, Silvia Montanari, Camilla Scialpi, Gabriele Sani, Lorenzo Moccia, Delfina Janiri Journal of Clinical Medicine, 2026 Background/Objectives: Intentional insulin omission (IIO), commonly referred to as diabulimia, is a high-risk behavioural phenomenon observed mainly in adolescents and young adults with type 1 diabetes mellitus (T1D). Defined as the deliberate reduction or omission of insulin to influence body weight, IIO lies at the intersection of metabolic management and eating disorder psychopathology. Despite serious health risks, including diabetic ketoacidosis, microvascular complications, and increased mortality, it remains under-recognised due to stigma, diagnostic ambiguity, and overlap with routine diabetes self-management. This review aimed to examine the prevalence, psychological mechanisms, and clinical consequences of IIO. Methods: On 1 April 2026, we conducted a systematic search of PubMed, Scopus, APA PsycInfo/PsycArticles and Cinahl for studies investigating intentional insulin omission and related metabolic and psychological outcomes in T1D. Clinical and epidemiological studies assessing prevalence, risk factors, and interventions were included. Results: Twenty-nine studies met the inclusion criteria. Prevalence estimates ranged from 20% to 45%, with higher risk among females, adolescents, and individuals experiencing diabetes-related distress or body dissatisfaction. Psychological factors—including fear of weight gain, emotion dysregulation, depressive symptoms, and identity conflicts—were associated with IIO onset and maintenance. IIO was consistently linked to poor glycaemic control, elevated HbA1c levels, and adverse metabolic and psychological outcomes. Screening tools such as the Diabetes Eating Problem Survey–Revised (DEPS-R) may support early identification, while effective management requires integrated multidisciplinary care. Conclusions: Although IIO is not formally classified as an eating disorder in current diagnostic systems, it shares important psychopathological features with eating disorders and may represent a diabetes-specific disordered eating behaviour with life-threatening consequences.
Lifetime Mixed Depression and Childhood Trauma in Individuals With Bipolar Disorders Francesca Bardi, Mario Pinto, Alessio Simonetti, Marco Di Nicola, Marianna Mazza, Giovanni Manfredi, Georgios D. Kotzalidis, Gabriele Sani, Delfina Janiri Acta Psychiatrica Scandinavica, 2026 Background and Aims Mixed Depression (MxD), characterized by the co‐occurrence of depressive and excitatory symptoms, is a prevalent yet often underdiagnosed presentation in bipolar disorders (BD), with significant implications for prognosis and treatment. Childhood trauma is a key environmental risk factor associated with a more severe course of BD, as it influences its onset, progression, and clinical presentation. To date, no studies have specifically investigated the relationship between lifetime childhood trauma and lifetime MxD in individuals with BD. The aim of our study was to address this gap in a large and well‐characterized sample of patients with BD type I and type II. Methods A total of 376 individuals, 220 euthymic outpatients with BD (MxD = 100, non‐MxD = 120) and 156 healthy controls (HC), were enrolled. Childhood trauma was assessed using the Childhood Trauma Questionnaire (CTQ). Lifetime MxD was identified according to the Koukopoulos criteria. The relationship between childhood trauma and MxD was evaluated using two different multivariate models. Results The first model (Wilks' Lambda = 0.834, p < 0.001) revealed that BD patients with lifetime MxD reported significantly higher CTQ total scores compared to both non‐MxD ( p = 0.029) and HC ( p < 0.001). When examining childhood trauma subtypes, BD patients with MxD reported significantly higher emotional abuse scores than those without MxD ( p < 0.001). Furthermore, compared to HC, the MxD group showed significantly elevated scores in emotional abuse, emotional neglect, and physical abuse (all p < 0.001). The non‐MxD group scored higher than HC on physical abuse ( p = 0.008) and physical neglect ( p < 0.001). Regression analyses confirmed specifically emotional abuse as a significant predictor of lifetime MxD. Conclusions The findings demonstrate a strong association between childhood trauma, particularly emotional abuse, and lifetime MxD. These results underscore the significance of childhood trauma as a key predictor of clinical outcomes in BD.

Kotzalidis, Tommaso Callovini, Mario Pinto, Camilla Scialpi, Matteo Bucci, Maria Benedetta Anesini, Gabriele Sani, Lorenzo Moccia, Delfina Janiri Brain Sciences, 2026 Background/Objectives: Schizophrenia is a severe psychiatric disorder frequently characterised by sleep and circadian disturbances, which are closely linked to cognitive dysfunction, symptom exacerbation, and poor functional outcomes. A growing body of evidence implicates the orexin (hypocretin) system—an essential regulator of arousal, sleep–wake stability, metabolic processes, and motivated behaviour—in the pathophysiology and treatment response of psychotic disorders. We aimed to investigate the relationships between the orexinergic system and psychoses. Methods: On 3 March 2026, we searched the PubMed, Scopus, PsycInfo/Articles and Cinahl databases for studies dealing with the orexin system and psychotic disorders and treatment response. Results: We found 20 eligible studies reporting variable and inconsistent alterations in orexin signalling in patients with schizophrenia. Studies were mostly cross-sectional and heterogeneous in design. Antipsychotic medications interfere with orexin-dependent pathways, potentially contributing to both therapeutic effects and adverse outcomes such as sleep disruption and metabolic dysregulation. Conclusions: While evidence from preclinical studies could point to an influence of dopaminergic activity through orexinergic mechanisms, with possible attenuation of antipsychotic-induced motor side effects and improvement of attentional deficits associated with NMDA receptor hypofunction, the utility of dual orexin receptor antagonists (DORAs) in psychoses is unclear. Despite the high prevalence of insomnia in schizophrenia, its pharmacological management remains suboptimal, with current treatments often limited by reduced efficacy or tolerability concerns. DORAs, which are currently approved medications for the treatment of insomnia, represent a novel and mechanistically distinct therapeutic option that may improve sleep while modulating arousal- and cognition-related circuits relevant to psychosis.

This study aimed to evaluate seasonal variations in the striatum using ¹²³I-FP-CIT SPECT in patients with MDD, and examine associations with specific psychopathological symptoms. : In this retrospective study, DAT SPECT scans from 85 patients with MDD were analyzed according to the season of imaging—fall-winter (FW) or spring-summer (SS). Psychometric assessments included the Hamilton Depression Rating Scale (HAMD), Hamilton Anxiety Rating Scale (HAMA), Snaith-Hamilton Pleasure Scale (SHAPS), and Depression Retardation Rating Scale (DRRS). : No overall differences in DAT availability were observed between FW and SS. However, anhedonia levels were higher in FW (p = 0.050). Patients with severe depression (HAMD ≥ 25) showed lower DAT availability in the left putamen, especially during SS (p = 0.014). Patients with marked psychomotor retardation (DRRS ≥ 18) exhibited reduced DAT availability in the left putamen (p = 0.002), with further reductions across all striatal regions during SS. Patients with suicidal ideation showed decreased DAT in the right (p = 0.029) and left putamen (p = 0.015). A negative correlation was found between DRRS scores and left putamen DAT availability (p = 0.034). : Reduced DAT availability is associated with key depressive symptoms, notably psychomotor retardation and suicidal ideation. Seasonal effects, especially in the SS period, may exacerbate dopaminergic dysregulation. These findings support integrating seasonal and neurobiological factors in the assessment and management of severe MDD.
Combination of Antidepressants and Chemotherapeutic Agents to Overcome P-Glycoprotein-Mediated Resistance in Cancer Patients: A Systematic Review Antonio Restaino, Mario Pinto, Giulio Carriero, Antonio Maria D’Onofrio, Silvia Montanari, Delfina Janiri, Giovanni Camardese, Lorenzo Moccia, Gabriele Sani, Alessio Simonetti Medical Sciences, 2026 Background/Objectives: P-glycoprotein (P-gp, ABCB1/MDR1) is a key ATP-binding cassette transporter involved in multidrug resistance in cancer, limiting intracellular accumulation of various chemotherapeutic (CT) agents. Several antidepressants (ADs) have been shown to modulate P-gp function. This dual pharmacological profile raises the possibility of repurposing ADs as chemosensitizers to enhance anticancer drug efficacy. The objective of this review was to summarize the available evidence on the combined use of ADs and chemotherapeutics to overcome P-gp-mediated resistance. Methods: A systematic search was performed in PubMed, Scopus, and PsycInfo/PsycArticles databases using a comprehensive search string combining terms for P-gp, ADs, chemotherapy, and drug resistance. Inclusion criteria were preclinical or clinical studies investigating the effect of ADs in combination with chemotherapeutics on P-gp-mediated resistance in cancer models. Eleven relevant studies were identified and qualitatively analyzed. Results: Across diverse cancer models, including colon, breast, and multidrug-resistant cell lines, several ADs significantly enhanced the cytotoxicity of many chemotherapeutic agents. The proposed mechanisms involved downregulation of P-gp expression, inhibition of efflux activity, and increased intracellular drug accumulation. Conclusions: The combination of ADs with CT agents shows promising potential in overcoming P-gp-mediated multidrug resistance, enhancing antitumor efficacy in preclinical models. Further translational and clinical research is needed to validate these findings, optimize dosing strategies, and assess the risk–benefit profile in cancer patients, particularly those with comorbid depressive disorders.

This study aims to synthesize current evidence on the epidemiology, risk factors, mechanistic pathways, and therapeutic approaches of LOD, integrating biological, psychological, and social dimensions. Methods: This narrative review synthesizes recent evidence across epidemiology, clinical symptomatology, neurobiology, and treatment. Where conceptually appropriate or empirically overlapping, we incorporate findings from the broader late-life depression (LLD) literature. Results: LOD emerges (as a distinct clinical and biological entity in later life) as a clinically and biologically meaningful presentation of depression in later life, representing a minority of depressive cases. It is defined by prominent apathy, psychomotor slowing, and cognitive impairment, and is closely linked to frailty, medical comorbidity, and heightened dementia risk. Pathophysiological mechanisms converge on vascular, inflammatory, oxidative, and neuroplasticity pathways, while psychosocial adversity further shapes onset and course. Treatment prioritizes efficacy and tolerability amid multiple morbidity; SSRIs and SNRIs are first-line, with pro-dopaminergic or dual-action agents addressing anhedonia and apathy, and neuromodulation or augmentation strategies reserved for resistance. Integrative approaches combining pharmacotherapy, psychotherapy, and lifestyle interventions are essential to optimize outcomes in aging populations. Conclusions: Late-onset depression (is a distinct, biologically and psychosocially driven disorder) represents a biologically and psychosocially enriched subtype in its own within the spectrum of late-life depression, requiring integrated, personalized care. Addressing neurovascular mechanisms, psychosocial adversity, and prevention through coordinated geriatric and psychiatric strategies may improve outcomes in aging populations.

Baldessarini Nordic Journal of Psychiatry, 2026 BACKGROUND: Many reports associate particular phases of the human reproductive cycle with psychiatric symptoms including depression, although details of associations of morbidity of depressive, bipolar, and anxiety disorders with pre-, peri-, and post-menopausal phases require further clarification. This naturalistic study evaluated the timing of onset and morbidity of mood and anxiety disorders in women during these menopausal phases. METHODS: = 928) with a primary diagnosis of BD type I or II (BD1 or BD2), major depressive disorder (MDD) or an anxiety disorder were clinically evaluated after menopause to determine the onset of affective illness during the pre, peri-, or post-menopausal phases. Psychometric scales were used to assess affective illness and multivariable regression modeling was used to compare menopausal phases. RESULTS: Many descriptive and clinical measures differed significantly with illnesses starting in the three menstrual phases. Women with onset of affective illness in the perimenopause phase spent 1.33 more time ill than those starting in the premenopausal phase and 1.46 more time ill than women with postmenopausal onset, with corresponding differences in illness episodes/year as well as suicidal risks. CONCLUSIONS: Study findings are consistent with the view that onset in the peri-menopausal phase may be associated with an excess of affective morbidity in both MDD and BD and that the post-menopausal phase may involve less morbidity.

Nevertheless, the relationship between pain and suicide in oncology populations remains insufficiently characterized. A clearer understanding of this interplay is essential to guide personalized approaches aimed at reducing cancer-related burden and improving quality of life. Methods: We searched PubMed and PsycInfo without imposing limits regarding publication date using pain* AND (suicid* OR “self-harm” OR “self-injurious behavior” OR “self-inflicted injury” or “self-killing”) AND (cancer* OR oncolog* OR tumor* OR neoplasm* OR metasta*). A total of 832 articles were identified, and 15 of them were included in our review. Results: Inadequately managed pain in cancer patients is associated with a significantly elevated risk of suicidal ideation. This association is further exacerbated in individuals presenting with depressive symptoms, advanced-stage disease, or limited access to timely psychological support. These factors may interact synergistically, intensifying the emotional and cognitive burden of pain, thereby increasing vulnerability in cancer patients. Conclusions: Cancer-related pain should be conceptualized as a highly variable indicator of psychological vulnerability. Factors influencing this variability include cancer type and severity, as well as the presence of past psychopathology. These findings support the need for a personalized medicine approach, whereby pain management and psychosocial interventions are tailored to patient-specific factors such as disease stage, psychological comorbidity, and access to supportive care.

Despite the evidence supporting the role of pharmacotherapy, real‐world data on the effectiveness and tolerability of different antidepressant classes in cancer populations remain limited. Objective To assess the effectiveness and tolerability of antidepressant classes and individual agents in oncology patients with major depressive disorder (MDD), and to investigate antidepressant prescription trends. Methods This 6‐month retrospective observational study was conducted in 170 oncology outpatients with MDD. Patients received monotherapy with tricyclic antidepressants (TCAs), selective serotonin reuptake inhibitors (SSRIs), serotonin and norepinephrine reuptake inhibitors (SNRIs), or a multimodal antidepressant class including only vortioxetine. Psychiatric symptoms were evaluated at baseline and after 1 and 6 months. Evaluation included rates of response, remission and changes in rating scales assessing depression, anxiety, suicidality, and agitation. Results All antidepressant classes showed comparable effectiveness and capability of reducing depression, anxiety, and suicidal ideation over time. Vortioxetine showed a favorable tolerability profile, with no reported adverse effects, whereas SSRIs and SNRIs had higher dropout rates due to side effects. SSRIs were the most prescribed class, followed by multimodal agents. Vortioxetine was the most frequently prescribed individual antidepressant, particularly among patients undergoing chemotherapy. Limitations The small sample size and the lack of stratification by cancer type limit the generalizability of the results. Conclusion Antidepressants were equally effective in treating depression among cancer patients. Vortioxetine emerged as a well‐tolerated and preferred option for chemotherapy patients. These findings emphasize the importance of tolerability and drug‐interaction profiles in oncology care.
