The prognostic role and predictors of ABI in patients with established coronary artery disease still remain debated, and especially among patients with acute myocardial infarction (AMI) and represented therefore the aim of the present study.METHODS: We included patients undergoing coronary angiography and PCI for AMI in a single center from May 2022 to November 2024 and with no established history of peripheral arterial disease. ABI was measured before discharge in a phase of hemodynamic stability. Peripheral Arterial Disease (PAD) was defined for ABI ≤0.90.RESULTS: Overall, 130 patients with AMI were included, of whom 28 (21.5%) had impaired ABI values. No clinical or demographic difference was observed according to ABI, but for lower platelet count (216.7±52.9 vs. 264.8±86.9, P=0.006), that emerged as the only independent predictor of impaired ABI (OR=0.989 [95% CI: 0.982-0.997], P=0.007). Patients with higher platelet count (III tertile, >267.6×103/µL, N.=44) displayed significantly higher white blood cells count (P<0.001) and lower use of acetylsalicylic acid (P=0.06). At multivariable regression analysis, we confirmed the independent association between higher platelet tertiles values and impaired ABI (adjusted OR=0.147 [95% CI: 0.037-0.576], P=0.006).CONCLUSIONS: Among patients with acute myocardial infarction, abnormal values of ABI are common, although similarly distributed across major established cardiovascular risk factors. In fact, platelet count emerged as the only independent predictor of impaired ABI and the inverse association between higher platelet count and ABI values was confirmed in different higher-risk subsets of patients. Future dedicated large-scale studies could provide the prognostic implications and more insightful understanding of our findings.

Despite technological innovations and improvements in stents and devices, sex-related discrepancies are still reported in the outcomes after ST-segment elevation myocardial infarction (STEMI), depending on biological and sex-specific pathophysiological differences, which have not been completely understood. The aim of the present study was to provide real-world data on the prognostic role of sex among patients with STEMI, enclosed into a recent up-to-date international registry. Methods. The ISACS-STEMI COVID-19 is a large-scale retrospective registry, including STEMI patients treated with mechanical reperfusion between 1 March and 30 June, 2019 and 2020. Patients, treated in 109 centers across Europe, Latin America, Southeast Asia, and North Africa, were grouped according to sex. Primary endpoint: In-hospital mortality; secondary endpoints: Time delay, 30-day mortality, and postprocedural Thrombolysis In Myocardial Infarction (TIMI) 3 flow. Results. We included 16,083 patients, 24.3% females (54.3% hospitalized in 2019, 45.7% in 2020). Women with STEMI were older, more often diabetic and hypertensive (p < 0.001), with a higher prevalence of hypercholesterolemia (p = 0.02), longer ischemia time (p = 0.01), ambulance referral (p = 0.03) and cardiogenic shock at presentation (p = 0.05), but less frequently smokers, with a previous cardiovascular event (p < 0.001) or anterior STEMI (p = 0.03) as compared to males. Preprocedural TIMI 0 flow, multivessel disease, need for thrombectomy (p < 0.001 and p = 0.001, respectively), use of Glycoprotein IIbIIIa inhibitors or cangrelor, radial access and implantation of drug-eluting stents (p < 0.001, p < 0.001 and p = 0.001, respectively) were also more common in men. Impaired postprocedural epicardial reperfusion (TIMI flow 0–2) was observed more frequently in females as compared to males (10% vs. 7.2%; adjusted OR [95% CI] = 1.30 [1.13–1.49], p = 0.01). In-hospital mortality was 5.8%, significantly higher among women (8.3% vs. 5%, p < 0.001, adjusted HR [95% CI] = 1.26 [1.06–1.5], p = 0.01). Similar data were observed for 30-day mortality (10.3% vs. 6.2%, p < 0.001, adjusted HR [95% CI] = 1.22 [1.06–1.38], p = 0.007). Conclusions. Among STEMI patients being treated with the most updated standard of care for primary percutaneous coronary intervention, female sex is still associated with higher complexity and impaired prognosis, displaying suboptimal epicardial reperfusion and increased in-hospital and 30-day mortality.
Dealing With Calcium in STEMI: A Unique Case of Intracoronary Fracture and Rescue of a Guide Catheter Extension Monica Verdoia, Orazio Viola, Andrea Rognoni Catheterization and Cardiovascular Interventions, 2026 Guide catheter extensions (GCE) represent a useful tool in percutaneous coronary interventions (PCI), allowing to increase support and successfully deliver material through calcific lesions or tortuous anatomy. We present the first case of fracture of the metallic collar braid of a Guidezilla TM II (Boston Scientific, Natick, MA, USA) GCE, remaining trapped in a severely calcified left anterior descending artery of a patient presenting with ST‐segment elevation myocardial infarction. The fragment was retrieved from the coronary artery with snaring technique, and the PCI was competed with conventional stenting without further complications.

Rissanen, Flavio Ribichini, Antonio Colombo, Bernardo Cortese Asiaintervention, 2026 Background: Paclitaxel-coated balloons (PCB) have strong supporting evidence for use in small coronary vessels, while sirolimus-coated balloons (SCB) have shown variable angiographic results, raising questions about their overall effectiveness. Aims: coronary artery disease. Methods: This international, multicentre study included patients who underwent percutaneous coronary intervention and had elective angiographic follow-up 5-9 months later. Angiographic and physiological assessments were performed by a core laboratory, including Murray law-based quantitative flow ratio (μFR). Results: Based on a cohort of 293 patients, 227 lesions treated either with a PCB (n=148) or an SCB (n=79) were included. No differences in terms of baseline clinical or angiographic characteristics were reported between the two cohorts. PCB showed lower late lumen loss (-0.05±0.56 mm vs +0.10±0.59 mm; p=0.05) and a higher prevalence of late lumen enlargement (58.1% vs 40.5%; p=0.01). The primary endpoint of late functional loss was not statistically different, with a trend in favour of PCB (-0.01±0.15 vs +0.03±0.13; p=0.09). There was no difference in terms of target lesion failure, with a higher rate of ischaemia-inducing vessels at follow-up in the SCB group (14.9% vs 26.6%; p=0.03). A μFR <=0.86 following drug-coated balloon (DCB) treatment emerged as a reliable cutoff for predicting follow-up ischaemia, with an accuracy of 80%, whereas no significant interaction was observed for a post-DCB angiographic degree of stenosis >=30%. Conclusions: In this direct comparison between two classes of DCB, late functional loss was comparable between PCB and SCB, with PCB confirming superior angiographic performance.

Comparison of Different Glomerular Filtration Rate Formulas in the Prediction of Coronary Artery Disease Severity and Extent Monica Verdoia, Alyssa Clemente, Aurelio Malabaila, Chantal Montini, Andrea Rognoni Arquivos Brasileiros De Cardiologia, 2026 Background Chronic kidney disease (CKD) is an established risk factor for coronary artery disease (CAD), displaying a linear relationship with glomerular filtration rate (GFR). The prognostic role of the different formulas for the estimation of GFR is still debated. Objective To evaluate the impact of GFR calculation in the prediction of angiographically assessed CAD. Methods In consecutive patients undergoing coronary angiography, GFR was assessed at admission with the formulas Cockroft-Gault, Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI), and Modification of Diet in Renal Disease Study (MDRD). A 2-sided p-value < 0.05 was considered statistically significant. Results Among 1742 patients, 79.8% displayed CAD. No difference in the mean values of GFR was observed in patients with and without CAD with the three different GFR formulas. We observed a significant association between severe CAD and GFR, by Cockcroft Gault (26.6% vs 35% vs 36.9%, p<0.001), MDRD (27.1% vs 36.9% vs 39%, p<0.001), and CKD-EPI (28.9% vs 39.2% vs 22.2%, p=0.03). In multivariate analysis, only MDRD remained statistically significant (adjusted OR[95%CI]= 1.27[1.01-1.59], p=0.04). In ROC curve analysis, we identified the value of 12.4 ml/min/1.73m^2 as the best predicting value for severe CAD with the MDRD formula. Higher rates of calcifications, restenosis, and chronic occlusion were observed for the Cockroft-Gault and MDRD formulas. Conclusion The present study showed a significant association between the extent of angiographically-defined CAD and GFR, estimated by the MDRD, but not for the Cockroft-Gault and CKD-EPI formulas. MDRD values below 12.4 ml/min/1.73m^2 best predicted more severe CAD.

Since then, considerable clinical evidence was generated in patients with de novo lesions. Whether drug-eluting stents (DES) or DCB angioplasty are associated with better clinical outcomes for de novo lesion treatment is still a matter of debate. Objective The objective of this post-hoc analysis was to assess the impact of vessel diameter in a real-world, unselected patient population treated with either paclitaxel-coated balloon (PCB) angioplasty or polymer-free sirolimus-eluting stent (PF-SES) implantation based on patient-level data. Methods Four all-comers observational studies based on similar protocols were pooled and vessel-diameter matched to study the clinical outcomes following revascularization with either PCB or PF-SES. The primary endpoint in all studies was the accumulated clinically driven target lesion revascularization (TLR) rate at 9–12 months complemented with secondary endpoints such as the rates for major adverse cardiac events (MACE), myocardial infarction (MI) and cardiac death. Results In the unmatched patient population, 3035 patients received either PCB (1614 patients) or PF-SES (1421 patients). The ‘unmatched’ TLR and MACE rates were low with 2.5% (PCB) vs. 2.0% (PF-SES, p = 0.319) and 4.5% (PCB) vs. 5.0% (PF-SES, p = 0.493), respectively. In the large vessel group (≥ 3 mm), 253 patient pairs were matched with reference diameters of 3.19 ± 0.27 mm (PCB) and 3.18 ± 0.27 mm (PF-SES, p = 0.787), and lesion lengths of 17.5 ± 9.8 mm (PCB) and 17.3 ± 7.1 mm (PF-SES, p = 0.839). The accumulated TLR rates were not significantly different (PCB 2.0% vs. PF-SES 2.8%, p = 0.786). Likewise, MACE rates in this large vessel subgroup were similar (5.6% vs. 6.4%, p = 0.723). In the small vessel group (< 3 mm), 420 patient pairs were matched with reference diameters of 2.49 ± 0.18 mm (PCB) and 2.49 ± 0.18 mm (PF-SES, p = .995), and lesion lengths of 16.0 ± 9.0 mm (PCB) and 16.7 ± 7.1 mm (PF-SES, p = 0.665). The accumulated TLR rates were not significantly different (PCB 1.9% vs. PF-SES 2.2%, p = 0.810). Likewise, MACE rates in the small vessel subgroup were similar (4.4% vs. 5.8%, p = 0.346). Individual clinical event rates were low and not significantly different between patients with small and large vessels. Conclusions In this real-world experience PCB and PF-SES angioplasty were associated with low clinical event rates. There was no difference in clinical efficacy between PCB and PF-SES in large and small vessel PCI in terms of clinically driven TLR and MACE. Previously reported higher rates for clinical events after DCB angioplasty in large coronary vessels ≥ 3 mm could not be observed. Graphical abstract

In the general population, left anterior descending (LAD) artery involvement predicts adverse outcomes, but data in the very elderly are limited. We aimed to evaluate the impact of LAD culprit lesions on 30-day mortality in this population. Methods In this multicenter registry (six Italian Hub hospitals, 2010–2023), 586 consecutive STEMI patients aged ≥85 years undergoing coronary angiography and percutaneous coronary intervention (PCI) were retrospectively analyzed. Patients were stratified by culprit vessel: LAD ( n = 288) vs. non-LAD ( n = 298). Demographic, clinical, procedural characteristics, and 30-day outcomes, including mortality and major adverse cardiovascular events, were collected. Multivariable analysis identified independent predictors of mortality. Results Baseline differences included prior myocardial infarction (MI) (9.4% vs. 18.5%, P < 0.01), prior CABG (0.3% vs. 4.7%, P < 0.01), and dyslipidemia (35.4% vs. 45.6%, P = 0.02), less frequent in LAD STEMI. Single-vessel disease was more common in LAD STEMI (42.0% vs. 31.5%, P = 0.01). Overall, 30-day mortality was 19.3% (113/586), higher in the LAD group [hazard ratio (HR) 1.57; 95% confidence interval (CI) 1.06–2.33; P = 0.024]. In LAD STEMI, cardiogenic shock at presentation was the strongest independent predictor of death (HR 6.10; 95% CI 3.70–10.35; P = 0.01), while dyslipidemia was associated with lower mortality (HR 0.51; 95% CI 0.28–0.92; P = 0.03). Conclusions In very elderly STEMI patients, LAD culprit lesions are associated with higher short-term mortality. Cardiogenic shock at presentation and absence of dyslipidemia identify a high-risk subgroup, emphasizing the need for tailored risk stratification and careful procedural planning in this vulnerable population.

These agents have significantly improved survival and quality of life in molecularly selected subgroups, potentially converting NSCLC into a chronic disease requiring prolonged treatment exposure. However, extended survival has led to increasing recognition of cancer treatment–related cardiovascular (CV) disease as a clinically relevant and sometimes dose-limiting complication. Unlike conventional chemotherapy, CV toxicities associated with targeted therapies frequently arise from on-target or off-target interference with signaling pathways that are essential for myocardial survival, endothelial function, vascular regulation, and the cardiac conduction system. From common pathophysiological mechanisms, a broad spectrum of clinical manifestations arises, ranging from asymptomatic electrocardiographic changes to arterial hypertension, dyslipidemia, venous thromboembolism, arrhythmias, and heart failure. This review provides a comprehensive overview of CV toxicities associated with targeted therapies in NSCLC, integrating mechanistic insights with clinical evidence. We summarize class-specific CV risk profiles across EGFR, ALK/ROS1, RET, MET, NTRK, BRAF, and KRAS-G12C inhibitors, as well as ADCs and BsAbs, highlighting both shared and distinct patterns of cardiotoxicity. As targeted therapies continue to expand across disease stages and treatment lines, CV toxicity is expected to play an increasingly important role in therapeutic decision-making. Integrating CV considerations into oncologic care is therefore essential to preserve treatment continuity, optimize long-term outcomes, and maximize the benefits of modern targeted therapies in NSCLC.

However, validation in large-sized studies is still lacking. Therefore, the aim of the current study was to validate the use of the ZRS in a contemporary global population, including patients who were treated during the SARS-CoV-2 pandemic and enrolled in a large intercontinental observational study. Methods: The ISACS-STEMI COVID-19 is a large-scale retrospective multicenter registry involving primary PCI centers from Europe, Latin America, South-East Asia, and NorthAfrica, including patients treated from March 1st until June 30th, in 2019 and 2020]. ZRS was calculated for each patient. The patients were additionally categorized according to the following values of the ZRS [≤3; 4-6; 7-9; ≥10]. Our study outcomes were in-hospital and 30-day mortality. The discriminatory capacity of the ZRS was assessed by the area under the ROC curve [c statistic] as an index of model performance. Results: Our population is represented by 16084 STEMI patients undergoing mechanical reperfusion enrolled in 109 centers. The score showed a very good performance in the predicting mortality both in-hospital [AUC=0.83 [0.82-0.85], p<0.0001] and at 30- day follow-up [AUC=0.82 [0.81-0.84, p<0.0001]. The results were confirmed when the ZRS was separately applied to patients treated in 2019 and 2020, with good stability across time. ZRS was able to identify a large cohort [n=10672, 66.3%] of low-risk patients [score ≤3] with a very low mortality rate at 2 days [1%] and between 3 and 10 days [0.7%], with a very good negative predictive value for in-hospital [98.3%] and 30-day mortality [97.7%], with similar results in 2019 and 2020. Conclusion: This study is the first to demonstrate the good prognostic performance of the ZRS in a large-scale contemporary global multicenter validation set. Similar results were obtained both in the pre-pandemic and the COVID-19 era.
