Methods: A systematic review of the literature was conducted, which included observational cohort studies that followed patients with diabetes to see whether they developed a diabetic foot ulcer (DFU). A random-effect meta-analysis was performed, and odds ratios were used for nominal data or mean differences for continuous data. This study was registered at PROSPERO (CRD42020182527). Results: The analysis included eight studies. From 4914 patients, 607 developed a DFU (12%) within a median of 30 months follow-up, indicating a 4.8% annual rate. The following parameters were found to be related to DFU development: a history of DFUs; cerebrovascular accident; peripheral arterial disease and lower extremity amputation; duration of diabetes; glycated haemoglobin; use of insulin; visual impairment; claudication; altered monofilament sensitivity; foot deformity; onychomycosis; foot oedema; and local hyperkeratosis. The quality of evidence for most of the factors was either low or very low. Conclusion: The development of DFUs may be associated with various factors, including: a history of previous DFUs; amputation or clinical atherosclerosis syndrome, such as peripheral arterial and cerebrovascular disease; the severity of diabetes; and various clinical signs regarding the patient's foot. The low certainty of evidence for most of these factors indicates that there is still a gap in knowledge regarding the early detection of foot ulcers in people with diabetes. A large prospective study could identify more precisely which factors are the most significant. Contemporary technology could use such data to create devices that would be able to predict DFUs so they can be prevented or treated earlier, which would reduce the possibility of lower limb amputation.

Although the genetic basis of blood cholesterol concentration is well studied, there is currently a lack of studies investigating the genetics of its precursors from de novo biosynthesis. METHODS: We conducted a genome-wide association meta-analysis, combining data from KORA, LIFE-Heart, LIFE-Adult, LURIC, the Sorbs study, and YFS, resulting in up to 10,519 individuals. We investigated 14 traits related to serum concentrations of lanosterol, desmosterol, and cholesterol. Direct and indirect effects of lanosterol on CAD were investigated with a Mendelian randomisation mediation analysis. FINDINGS: ) associations not previously reported in the GWAS catalogue. These include two loci without prior connection to cholesterol, associated with lanosterol (7q21.2, CYP51A1) and free cholesterol (11q14.1), and two associations with lanosterol at loci previously reported for cholesterol (5q13.3, HMGCR; 11q23.3, APO cluster). We also replicated eight loci previously reported for associations with cholesterol-related traits. Lanosterol exhibited significant total and indirect effects on CAD, but its direct effect was not significant. INTERPRETATION: We demonstrate that the investigation of intermediate phenotypes can help to functionally fine map previously reported associations for cholesterol, improving our understanding of genetic regulation of cholesterol concentrations. Further, the effect of lanosterol on CAD is probably fully mediated by total cholesterol. Arterial thromboembolic events in testicular cancer patients: short- and long-term incidence, risk factors, and impact on mortality Florian Moik, Angelika Terbuch, Ariane Sprakel, Georg Pichler, Dominik A. Hutterer, Martin Pichler Journal of Thrombosis and Haemostasis, 2025 BACKGROUND: Patients with testicular germ cell tumors (TGCT) have a high cancer-specific survival rate. OBJECTIVES: We aimed to determine the short- and long-term risk of arterial thromboembolic events (ATE), their impact on mortality, and risk factors for ATE in TGCT patients. METHODS: Patients with TGCT treated between 1994 and 2020 were included in a single-center retrospective cohort study. The primary outcome was ATE (ie, acute coronary syndrome, ischemic stroke, and acute peripheral arterial occlusion). Cumulative incidences were obtained in competing risk analysis. The impact of ATE on mortality was analyzed in a multistate model. Cox regression was used to explore short- and long-term ATE risk factors. RESULTS: Overall, 1277 patients were included (median age, 35 years; seminoma: 56%; 44% cisplatin-based chemotherapy). Cumulative ATE incidences at 1, 10, and 25 years were 0.6% (95% CI, 0.3%-1.1%), 2.6% (95% CI, 1.8%-3.7%), and 12.0% (95% CI, 8.7%-15.9%), respectively. ATE diagnosis was independently associated with increased all-cause mortality (age-adjusted transition hazard ratio, 4.61; [95% CI, 2.40-8.85]; P < .001). Cisplatin-based chemotherapy was associated with ATE risk within 1 year after TGCT diagnosis (1.4% vs 0%, P < .001), whereas no differences were observed thereafter. Regarding long-term ATE risk, a point-based risk score was derived (age ≥ 35, smoking, and lactate dehydrogenase ≥ 250 IU/L), which efficiently stratified ATE risk (Harrel's C, 0.71 [95% CI, 0.63-0.78]), with cumulative ATE incidences in low-, intermediate-, and high-risk patients of 3.9%, 11.4%, and 22.7%, respectively. CONCLUSION: ATE represents a common complication in TGCT survivors and is associated with increased mortality. A simple point-based score efficiently stratifies long-term ATE risk, whereas cisplatin-based chemotherapy increases short-term ATE risk.

It, however, remains elusive whether statins could also impact PE severity. Objectives: To investigate a potential association between statin use and the severity of PE in a retrospective cohort. Methods: We performed a retrospective data analysis of patients with PE confirmed by computed tomography pulmonary angiography between January 1, 2010, and December 31, 2019, at the University Hospital Graz, Austria. PE severity was assessed based on the 2019 European Society of Cardiology guidelines. Results: ), no significant differences in PE severity were found. Conclusion: Statin use was not associated with PE severity. The smaller proportion of low-risk PE in statin users is likely attributable to their older age, higher BMI, and comorbidities.

Raggam, Peter Rief, Leyla Schweiger, Nikolaus John, Gerald Seinost, Vasile Foris, Gabor Kovacs, Thomas Gary, Viktoria Nemecz, Andreas Meinitzer, Heimo Strohmaier, Balazs Odler, Florentine Moazedi-Fürst, Marianne Brodmann, Franz Hafner Rheumatology International, 2025 Evaluating the predictive role of endothelial dysfunction in the development of vasculopathy-mediated complications in patients with limited cutaneous systemic sclerosis (lcSSc). 38 patients with lcSSc who were naïve for vasculopathy-mediated complications, defined as absent pre-existing digital ulcers (DU), pulmonary hypertension (PH) and symptomatic atherosclerotic cardiovascular diseases, were prospectively observed during a 3-years follow-up period. At study enrolment, functional and laboratory parameters of endothelial dysfunction were assessed. Microvascular events, defined as the development of DU and PH, macrovascular events, defined as newly symptomatic atherosclerotic cardiovascular diseases, and clinical events, defined as additional interstitial lung disease (ILD), renal crisis and esophageal dysfunction, were recorded annually. 33 patients (86.8%) completed all follow-up study visits. During the follow-up period, DU, PH, carotid and vertebral artery disease and ILD occurred in eight (24.2%), one (3.0%), one (3.0%) and two patients (6.1%), respectively, without recorded renal crisis and esophageal dysfunction. Number of pathologic FMD, NMD and/or PWV values was significantly associated with the development of clinical events (p = 0.035), but was not a significant predictor for microvascular events. Number of pathologic FMD, NMD and/or PWV values was a significant predictor for disease activity assessed by EUSTAR index ≥ 2.5 at last study visit (OR 5.47 [95% CI 1.01–29.03], p = 0.049). Distinct endothelial dysfunction may promote the development of disease complications and disease activity in patients with lcSSc.
Two-dimensional vessel wall diffusion anisotropy of human carotids – A novel measure of vascular ageing? Stefan L. High resolution diffusion weighted imaging (DWI) in combination with a two-dimensional (2D) diffusion gradient sampling scheme has recently been demonstrated as feasible for evaluating diffusivity in the vessel wall of human carotids. We aimed to identify associations of altered diffusivity in human carotids with presence or absence of cardiovascular disease, body mass index (BMI), sex and age in a clinical setting. METHODS: In this single center case-control study we used DWI in combination with a 2D gradient in a 3 T MRI scanner to evaluate diffusivity parameters in carotid vessel walls of clinical patients with known cardiovascular disease (n = 17) and healthy controls (n = 21). We used a read-out segmented EPI (rs-EPI) sequence for DWI. RESULTS: A group comparison showed significantly decreased fractional anisotropy (FA) in patients older than 60 years (p < 0.001) and in patients with a BMI higher than 25 (p = 0.019). Patients with cardiovascular disease had higher values for mean diffusivity (p = 0.005) than patients without cardiovascular disease. A multiple linear regression analysis showed age and sex to be associated with FA. CONCLUSION: Two-dimensional vessel wall DTI of human carotids is feasible for clinical research. Besides patient age, we identified Sex, BMI, or the presence of cardiovascular disease as relevant factors for carotid vessel wall diffusivity. Decreased FA values might indicate early-stage atherosclerosis and vascular ageing.

Konrad Circulation, 2025 BACKGROUND: ANGPTL3/4/8 (angiopoietin-like proteins 3, 4, and 8) are important regulators of LPL (lipoprotein lipase). ANGPTL8 forms complexes with ANGPTL3 and ANGPTL4. ANGPTL4/8 complex formation converts ANGPTL4 from a furin substrate to a plasmin substrate, and both cleavages generate similar C-terminal domain–containing (CD)—ANGPTL4 fragments. Whereas several studies have investigated associations of free ANGPTL proteins with cardiovascular risk, there are no data describing associations of the complexes and CD-ANGPTL4 with outcomes or describing the effects of the complexes on LPL bound to GPIHBP1 (glycosylphosphatidylinositol HDL-binding protein 1). METHODS: Recombinant protein assays were used to study ANGPTL protein and complex effects on GPIHBP1-LPL activity. ANGPTL3/8, ANGPTL3, ANGPTL4/8, and CD-ANGPTL4 were measured with dedicated immunoassays in 2394 LURIC (Ludwigshafen Risk and Cardiovascular Health) study participants undergoing coronary angiography and 6188 getABI study (German Epidemiological Trial on Ankle Brachial Index) participants undergoing ankle brachial index measurement. There was a follow-up for cardiovascular death with a median (interquartile range) duration of 9.80 (8.75–10.40) years in the LURIC study and 7.06 (7.00–7.14) years in the getABI study. RESULTS: ANGPTL3/8 potently inhibited GPIHBP1-LPL activity and showed positive associations with LDL-C (low-density lipoprotein cholesterol) and triglycerides (both P <0.001). However, in neither study did ANGPTL3/8 correlate with cardiovascular death. Free ANGPTL3 was positively associated with cardiovascular death in the getABI study but not the LURIC study. ANGPTL4/8 and especially CD-ANGPTL4 were positively associated with the prevalence of diabetes, CRP (C-reactive protein; all P <0.001), and cardiovascular death in both studies. In the LURIC and getABI studies, respective hazard ratios for cardiovascular mortality comparing the third with the first ANGPTL4/8 tertile were 1.47 (1.15–1.88) and 1.68 (1.25–2.27) when adjusted for sex, age, body mass index, and diabetes. For CD-ANGPTL4, these hazard ratios were 2.44 (1.86–3.20) and 2.76 (2.00–3.82). CONCLUSIONS: ANGPTL3/8 potently inhibited GPIHBP1-LPL enzymatic activity, consistent with its positive association with serum lipids. However, ANGPTL3/8, LDL-C, and triglyceride levels were not associated with cardiovascular death in the LURIC and getABI cohorts. In contrast, concentrations of ANGPTL4/8 and particularly CD-ANGPTL4 were positively associated with inflammation, the prevalence of diabetes, and cardiovascular mortality.
Support of Home-Based Structured Walking Training and Prediction of the 6-Minute Walk Test Distance in Patients With Peripheral Arterial Disease Based on Telehealth Data: Prospective Cohort Study Fabian Wiesmüller, Andreas Prenner, Andreas Ziegl, Gihan El-Moazen, Robert Modre-Osprian, Martin Baumgartner, Marianne Brodmann, Gerald Seinost, Günther Silbernagel, Günter Schreier, Dieter Hayn Jmir Formative Research, 2025 Background Telehealth has been effective in managing cardiovascular diseases like stroke and heart failure and has shown promising results in managing patients with peripheral arterial disease. However, more work is needed to fully understand the effect of telehealth-based predictive modeling on the physical fitness of patients with peripheral arterial disease. Objective For this work, data from the Keep Pace study were analyzed in depth to gain insights on temporal developments of patients’ conditions and to develop models to predict the patients’ total walking distance at the study end. This could help to determine patients who are likely to benefit from the telehealth program and to continuously provide estimations to the patients as a motivating factor. Methods This work analyzes continuous patient-reported telehealth data, in combination with in-clinic data from 19 Fontaine stage II patients with peripheral arterial disease who underwent a 12-week telehealth-based walking program. This analysis granted insights into the increase of the total walking distance of the 6-minute walk tests (6MWT) as a measure for physical fitness, the steady decrease in the patients’ pain, and the positive correlation between well-being and the total walking distance measured by the 6MWT. Results This work analyzed trends of and correlations between continuous patient-generated data. Findings of this study include a significant decrease of the patients’ pain sensation over time (P=.006), a low but highly significant correlation between pain sensation and steps taken on the same day (r=−0.11; P<.001) and the walking distance of the independently performed 6MWTs (r=−0.39; P<.001). Despite the reported pain, adherence to the 6MWT measurement protocol was high (85.53%). Additionally, patients significantly improved their timed-up-and-go test times during the study (P=.002). Predicting the total walking distance at the study end measured by the 6MWT worked well at study baseline (root mean squared error of 30 meters; 7.04% of the mean total walking distance at the study end of 425 meters) and continuously improved by adding further telehealth data. Future work should validate these findings in a larger cohort and in a prospective setting based on a clinical outcome. Conclusions We conclude that the prototypical trend estimation has great potential for an integration in the telehealth system to be used in future work to provide tailored patient-specific advice based on these predictions. Continuous data from the telehealth system grant a deeper insight and a better understanding of the patients’ status concerning well-being and level of pain as well as their current physical fitness level and the progress toward reaching set goals.
