Assessment of STOP-Bang Test Ability to Predict Postoperative Respiratory and Airway Complications of Supratentorial Craniotomy Parisa Sezari, Masoud Nashibi, Farhad Safari, Kamran Mottaghi, Elaheh Younesi, et al. Journal of Cellular and Molecular Anesthesia, 2025 Background: Sleep apnea is a commonly undiagnosed condition that can have serious consequences. The STOP-Bang Questionnaire is a well-recognized screening tool for obstructive sleep apnea (OSA) and is utilized to diagnose OSA in both medical and surgical patients. Objectives: The recent study aimed to assess the effectiveness of the STOP-Bang Questionnaire in predicting postoperative airway and respiratory complications and difficulty in intubation in supratentorial craniotomy. Methods: This cohort study involved 200 patients being considered for supratentorial craniotomy surgery and were referred between July 2019 and September 2020. The STOP-Bang Questionnaire was completed before the operation or during the visit to the anesthesia clinic. The Mallampati score was assessed by examining the patient's demographic characteristics, Body Mass Index (BMI), neck and waist size, underlying disease, and airway. Based on the STOP-Bang scoring system, the patients were divided into low-risk (score less than 3) and high-risk (score greater than or equal to 3). The data were analyzed using SPSS software version 20. Results: Out of all the patients, 125 (62.5%) were deemed low risk, while 75 (37.5%) were classified as high risk. There was no significant difference between the two groups regarding gender (P = 0.108). However, the high-risk group had a higher prevalence of BMI over 35 kg/m2, age over 50 years, and Mallampati III or IV (29.3% vs. 6.4%). Smoking was also more common in the high-risk group (29.3% vs. 16.8%). Patients in the high-risk group experienced a significantly higher frequency of airway obstruction, re-intubation, hypoxia, and longer hospital stays in both the recovery and the intensive care unit (ICU). The ROC curve for airway obstruction intervention indicated a 70.2% level (95% confidence interval: 79.6 - 6.7). The STOP-Bang score was a good predictor for the need for nasal or oral airway implantation, re-intubation, hypoxia, hypotension of arterial blood, and difficulty breathing or speaking. A score higher than two showed the predictive power for all evaluated outcomes. Conclusions: According to our research, patients scoring three or higher on the STOP-Bang test are more likely to experience premature respiratory and airway complications after supratentorial craniotomy surgery. This test can effectively detect adverse complications post-surgery.
Evaluation of Different Doses of Intravenous Magnesium Sulfate on Intraoperative Anesthetic Drugs Usage and Postoperative Pain in Posterior Spinal Fusion Surgery Masoud Nashibi, Parisa Sezari, Kamran Mottaghi, Farhad Safari, Faranak Behnaz, et al. Archives of Anesthesiology and Critical Care, 2025 Background: In this study, the effect of different doses of magnesium sulfate on the consumption of anesthetic drugs during surgery and pain after spine surgery has been investigated. In complex spine surgeries, a large amount of sedative and analgesic drugs is prescribed, which will have various systemic effects during and after the operation.Methods: This study was a double-blind clinical trial, and 80 patients who were candidates for posterior spinal fusion surgery at Luqman Hakim Hospital were included in the study. Patients were randomly divided into four groups (20 people in each group): Group M received magnesium sulfate in three doses (M1=10mg/kg/hr), (M2=15mg/kg/hr), and (M3=20mg/kg/hr). and group S received normal saline. Due to a drop in blood pressure and severe bradycardia in the M3 group, we had to stop the infusion, so the M3 group was excluded from the study, and 60 patients from the M1, M2, and S groups were studied.Results: The M2 group experienced a decrease in bleeding volume, intraoperative fentanyl consumption, and postoperative pain score compared to the M1 and S groups.Conclusion: In this study, we showed that the administration of magnesium sulfate during surgery can have positive and protective effects on the quality of anesthesia and the postoperative period. The recommended dose of magnesium was 15 mg/kg/hr because it has the lowest amount of narcotic consumption during the operation and the lowest amount of narcotic and analgesic consumption after the operation, and on the other hand, it has good hemodynamic stability. At the same time, doses higher than 15 mg/kg/hr are not recommended due to the drop in HR and MAP.
Analyzing Patterns in Anesthesiology Residents' Exam Performance Using Data Mining Techniques DACCPM, Maedeh Karimian, Shahabedin Rahmatizadeh, Zeinab Kohzadi, Zahra Kohzadi, et al. Anesthesiology and Pain Medicine, 2024 Background: Residency is a critical period in the development of medical professionals. It provides hands-on training and exposure to various medical specialties, enabling residents to improve their skills and achieve expertise in their chosen field. Objectives: This study aimed to extract frequent patterns in annual and board examination performance among anesthesiology residents by analyzing results from the department's weekly exams. Methods: This cross-sectional study was conducted in the Department of Anesthesiology, Critical Care, and Pain Medicine (DACCPM) from September 2022 to June 2023. Weekly intra-group exams were administered at the university's electronic exam center for residents in their first to fourth years (CA-1 to CA-4), with a total of 61 participants. Learner grades were categorized as excellent (A), good (B), average (C), poor (D), and inferior (E). The Apriori algorithm was employed to extract frequently repeated patterns in these exams and compare them with results from the final national examination. Results: A total of 24 exams were conducted, with all 61 residents participating. The most frequent patterns, identified with a minimum support of 0.41, revealed that residents generally achieved average scores in exam 7 and very poor scores in exams 1 and 5. The study found a statistically significant relationship between residents’ scores in in-training examinations (ITEs) and their national examination performance. Conclusions: Analyzing residents’ exam performance using frequent pattern recognition can help identify their strengths and weaknesses. Faculty members can utilize these insights to better plan curricula and enhance the quality of education.

The effect of erector spinae plane block on the use of anesthetic medications in lumbar spine surgery Kamran Mottaghi Agri, 2023 OBJECTIVES To reduce the drug side effects and facilitate the emergence from anesthesia after complex spine surgery, various methods have been proposed. One of these methods is ESPB, which has been less studied. Hence, we conducted this study to evaluate the effectiveness of ESPB on the use of anesthetic drugs in lumbar spine surgery. METHODS In this study, 70 patients undergoing lumbar spine fusion surgery were studied. Patients were randomly divided into two groups: the case group (n=35), in which bilateral ESPB was done, and the control group (n=35). After standard anesthesia protocols, anesthesia was maintained with isoflurane in both groups. Intraoperative isoflurane and perioperative opioid consumption were recorded. Statistical analysis was performed using SPSS software version 21. RESULTS Intraoperative use of fentanyl in the case group was significantly lower than the control group (14.29±21.5 vs. 65.96±73.33 µg, p<0.001). Furthermore, isoflurane consumption in the intervention group compared to the controls was significantly lower (20.71±5.02 versus 28.83±8.68 mL, p<0.001). Moreover, the emergence time was significantly shorter in the case group than in the control group (8.49±4.30 minutes versus 15.00±4.94, p<0.001). In the post-anesthesia care unit 1 h after surgery, the pain scores in the case group were significantly lower than the controls (p<0.001). CONCLUSION ESPB under ultrasound guidance is an effective method of regional anesthesia/analgesia for lumbar spine surgery (fusion) by decreasing the consumption of anesthetics during and following the surgery.
