Cognitive function and its determinants in elderly Indonesians residing in long-term care: Insights from a cross-sectional study Etty Rekawati, Winda Eriska, Utami Rachmawati, Dwi Nurviyandari Kusuma Wati, Junaiti Sahar, Arief Andriyanto, Jing-Jy Wang, Sri Susanty, Faizul Hasan F1000research, 2025 Background Multiple medical conditions arising from reduced physical and physiological functioning, including cognitive decline, manifest in older persons. This study aims to examine the relationship between cognitive function and associated risk factors in older persons living in long-term care facilities in Indonesia. Methods This study involved 350 elderly individuals residing in long-term care institutions. A cross-sectional design utilizing an analytical survey methodology was implemented. Data were gathered via interviews employing a demographic questionnaire and the Montreal Cognitive Assessment (MoCA). Statistical analysis was conducted using SPSS (version 23). Results Univariate analysis demonstrated significant correlations between cognitive performance and gender, ethnicity, level of education, medical history, subjective memory issues, smoking habits, alcohol consumption, dietary intake of fruits and vegetables, and employment history (p < 0.05). Higher education (OR = 0.69, 95% CI: 0.56–0.84) and reduced subjective memory complaints (OR = 0.29, 95% CI: 0.20–0.44) correlated positively with enhanced cognitive function, but alcohol intake (OR = 6.79, 95% CI: 2.42–19.1) correlated with impaired cognitive function. Conclusions the level of education, subjective memory complaints, and alcohol intake are substantially correlated with cognitive performance in older persons residing in long-term care facilities. Evaluating demographic characteristics in elderly individuals can assist healthcare professionals in the early detection of cognitive impairment, facilitating prompt interventions in long-term care environments.
Lived experiences constraints of family caregivers in caring for older adults with chronic diseases during the Covid-19 pandemic: a qualitative study of Indonesian perspectives Bahtiar Bahtiar, Ida Ayu Kade Sri Widiastuti, Dwi Nopriyanto, Nurlaila Fitriani, Khumaidi Khumaidi, Arief Andriyanto, Iskandar Muda Working with Older People, 2024 Purpose The purpose of this study is to explore the lived experiences of the constraints of older adult family caregivers with chronic diseases in caring for and accessing health services during the COVID-19 pandemic. Design/methodology/approach This study used the descriptive phenomenology qualitative method. The sampling method was purposive sampling involving 16 older adult family caregivers. Findings The results of this study showed three themes such as difficulties in health services in hospitals during the COVID-19 pandemic (complaints of services provided by doctors, older adult treatment control problems and difficulty getting to hospital health facilities); difficulties accompanying taking older adult medicine (older adult non-compliance response to taking medication and older adult medicine assistance); and psychosocial complaints caring for and accompanying the older adult (negative emotions for the older adult, difficulty interacting with the older adult and the economic burden of caring for the older adult). Practical implications Barriers to family caregivers in caring for older adults with chronic diseases can help health-care service providers understand and support families caring for and assisting older adults, which may contribute to the quality of life and care for both family caregivers and older adults. Originality/value This study showed that Indonesian family caregivers faced difficulties caring for and living with older adults with chronic diseases during the pandemic. Family caregivers’ experiences are essential when developing an intervention to support and manage health care for older adults with chronic illnesses.
Differences in Glucose Control of Diabetic Patients with Family Care Support Interventions Ikbal Fradianto, Arief Andriyanto, Enny Virda Yuniarti, Veryudha Eka Prameswari Malaysian Journal of Nursing, 2023 Introduction: Patients with diabetes mellitus need support from their families to control their glucose needs because family plays a very important role in reducing risk factors. This study aims to analyze differences in glucose control before and after implementing family care support interventions. Methods: This study employed a quasi-experiment and involved 123 samples of diabetes mellitus patients and their families. These samples were selected using a random sampling technique. The measuring instrument employed an HbA1c blood sugar examination carried out twice. Meanwhile, bivariate statistical analysis employed the significance level of an alpha value <0.05 with a paired t-test. Results: Statistical tests have revealed some changes before and after the intervention in both groups. The mean glucose of the intervention group is 9.463 + 7.000, and that of the control group is 9.469 + 8.262. The results are p = 0.001 in the intervention group and p = 0.062 in the control group. Conclusion: The statistical tests have proven significantly different glucose control in DM patients before and after the intervention. Based on the findings, this study recommends that a nurse provide nursing interventions for diabetes mellitus patients by involving family participation. As a result, the client and family could provide care on their own.
Coping strategies of healthcare providers on social stigma due to COVID-19 using the Roy adaptation model approach Ana Zakiyah and Gaceta Medica De Caracas, 2022 Introduction: The negative stigmatization of COVID-19 in healthcare has caused psychosocial problems, including stress. An effective coping strategy is needed to overcome existing problems. This study aimed to determine healthcare providers’ coping strategies and the social stigma about COVID-19 using Roy’s adaptation model approach and its influencing factors. Methods: The design used descriptive analysis with incidental sides, so the total sample size was 530 with accidental sampling.
Determinant Factors of Quality and Life Satisfaction of the Older People Etty Rekawati, Junaiti Sahar, Dwi Nurviyandari Kusumawati, Arief Andriyanto Jurnal Keperawatan Indonesia, 2022 Older people’s health care is carried out by involving the family as a support group. If done optimally, the support provided will help maintain the health of older people. The purpose of the study was to analyze the determinant factors (family support and burden) on the quality and satisfaction of life the older people. This descriptive correlational research was the second stage of previous research related to the identification of caregivers' burden when caring for older people. A sample of 104 respondents was taken through simple random sampling. The results showed that the family support and family burden affected the older people's quality of life (< 0.05), but did not affect the older people's life satisfaction (> 0.05). Family support and burdens affect the QoL for older people, which is due to the family's ability in caring for older people and the need for long-term services at home. Recommendations for health workers, especially nurses, are to focus not only on the elderly but also to the family's physical, psychological, and financial burdens to increase family support.AbstrakFaktor Determinan dari Kualitas dan Tingkat Kepuasan Hidup Lansia. Lansia yang sehat dapat dicapai dengan melibatkan seluruh anggota keluarga sebagai support group dalam merawat lansia. Karenanya, perawatan yang optimal oleh keluarga dapat menjaga kesehatan lansia itu sendiri. Studi ini bertujuan untuk menganalisis faktor determinan (dukungan keluarga dan beban keluarga) pada kualitas hidup dan kepuasan hidup pada lansia. Penelitian deskriptif korelasional ini merupakan studi tahap kedua dari penelitian sebelumnya, yaitu studi identifikasi masalah pada beban yang dihadapi oleh caregiver. Sebanyak 104 responden sebagai sampel untuk studi ini dipilih melalui teknik simple random sampling. Penelitian ini menunjukan bahwa faktor dukungan dan beban keluarga mempengaruhi kualitas hidup lansia (< 0,05), namun tidak memengaruhi kepuasan hidup pada lansia (> 0,05). Dukungan keluarga yang memengaruhi kualitas hidup lansia terjadi karena faktor kemampuan keluarga itu sendiri dalam merawat lansia dan faktor kebutuhan jangka panjang dalam merawat lansia itu sendiri. Rekomendasi untuk para tenaga kesehatan, khususnya perawat, adalah untuk fokus tidak hanya pada lansia namun juga pada keluarga dari aspek beban fisik, mental dan finansial, untuk meningkatkan dukungan keluarga.Kata Kunci: kepuasan hidup, kualitas hidup, lansia
The Effectiveness of Social Support Management of Diabetes on Glucose Control of Type 2 Diabetes Mellitus Patients: Quasi-Experiments Arief Andriyanto, Ika Rofi’ah, Bahtiar Bahtiar, Arif Wicaksono, Ana Zakiyah, Indra Yulianti, Raras Merbawani Open Access Macedonian Journal of Medical Sciences, 2022 BACKGROUND: Family plays a role in managing risk factors for diabetes mellitus (DM) in family members. The family can use diet and activity management as a form of prevention of risk factors and care for family members diagnosed with type 2 DM to keep it from getting to the level of severity. AIM: The study aims to analyze Social Support Management of Diabetes (SoSMeD) for glucose control of type 2 DM patients. METHODS: The study used a quasi-experiment pre-post-test with a control group design. The SoSMeD intervention was conducted for 3 months. The samples were taken by a random sampling technique of 50 respondents in each group. The measurement tool checks for glucose during the GDA-test 3 times, analyzed using a paired t-test. RESULTS: The results show significant differences between the intervention and control groups after being given SoSMeD in carrying out glucose control for patients with type 2 DM (p = 0.001). CONCLUSION: SoSMeD interventions provide type 2 DM sufferers with glucose control. Based on the findings, it is recommended that nurses should provide proper nursing interventions by involving families in diabetes management.

Increasing Knowledge, Attitudes, Skills, and Glucose Control in Type-2 Diabetic Patients through EMAS Interventions Arief Andriyanto, Etty Rekawati, Dwi Cahya Rahmadiyah Nurse Media Journal of Nursing, 2019 Background: Diabetes Mellitus (DM) is a chronic and non-communicable disease that has serious impacts. Previous studies have focused on a single intervention in the management of DM. Therefore, EMAS (education, nutrition management, physical activities, and stress management) interventions are proposed to convey the pillars of diabetes mellitus as endorsed by the Indonesian Ministry of Health.Purpose: This study aimed to analyze the effects of EMAS interventions on the knowledge, attitudes, skills, and glucose control in patients with type-2 DM.Methods: This study used a pretest-posttest quasi-experimental design without control groups. The samples were 86 diabetic patients recruited using a purposive sampling technique. The EMAS interventions were conducted for six months and eight sessions (October 2018 to March 2019). The EuroQoLfive-dimensional (EQ-5D) questionnaire was used to collect the data, and the paired t-test was used for data analysis. Results: The results showed that there were significant differences in the knowledge, attitudes, skills (p=0.001), and glucose control (p=0.04) of type 2 diabetes mellitus after the implementation of EMAS interventions. Conclusion: EMAS interventions significantly increased the knowledge, attitudes, and skills in patients with type 2 diabetes to behave healthier to control their blood sugar. Community nurses can use EMAS intervention for the management of DM among diabetic patients.
The Degree of Diabetic Wounds Affects Kidney Function Damage Emyk Windartik, Ima Rahmawati, Ita Ainun Jariyah, Raras Merbawani, Indah Lestari, Ifa Ro’ifah, Arief Andriyanto Nurse Media Journal of Nursing, 2019 Background: The peripheral neuropathy is the cause of hospitalization for patients with diabetes mellitus (DM) and damages kidney function due to the circulatory system with high sugar levels. High sugar levels in DM patients with diabetic wounds can trigger glomerular damage resulting in the decrease of kidney function.Purpose: This study aims to analyze the relationship between the degree of diabetic wounds and kidney functions in patients with DM.Methods: A cross-sectional study was conducted on 723 DM patients who experienced diabetic wounds in a hospital in Mojokerto, East Java, Indonesia. A purposive sampling technique was used to recruit the samples. The data of this study were medical records of diabetic patients. A simple linear regression test was employed to analyze the data.Results: The result showed that the degree of diabetic wound was significantly related to kidney function damage (p=0.000). The relationship between the degree of diabetic wounds and the decline in kidney function was shown by an R-squared value of 0.768, meaning that the degree of diabetic wound affected the decline of kidney function by 76.8%, while the 23.2% was affected by other factors.Conclusion:The degree of diabetic injury affects the decline of kidney function in DM patients by 76.8%. Nurses should do health promotion about controlling blood sugar levels in DM patients with the prevention of four pillars of diabetes, including education, nutrition, physical activity, and stress.
