Spontaneous abdominal wall endometriosis: A case report Mohd Pazudin Ismail, Erinna Mohamad Zon, W Fadhlina W Adnan, Nasibah Mohamad, Nur Asma Sapiai, et al. Malaysian Family Physician, 2025 Abdominal wall endometriosis (AWE) is a rare type of endometriosis, with an incidence ranging from 0.1% to 0.4%. It requires a high index of suspicion to avoid delays in diagnosis and treatment. This case is rather special because AWE occurred without a history of abdominal surgery or pelvic endometriosis. Herein, we report the case of a 48-year-old para-3 woman with localised cyclical abdominal pain associated with abdominal distension. On assessment during menstruation, there were localised tenderness at the right suprapubic area and a non-tender suprapubic mass corresponding to a 14-week-sized gravid uterus. An ultrasound revealed the presence of a heterogeneous hypoechoic lesion at the subcutaneous layer of the right suprapubic region sized 1.8×3.2×4.4 cm with poor demarcation. There were also multiple uterine fibroids varying in size and location. She underwent exploratory laparotomy, total abdominal hysterectomy with bilateral salpingo-oophorectomy and abdominal wall mass resection. Intraoperatively, the right abdominal wall mass measuring 6x5 cm and involving the subcutaneous layer was found to adhere to the rectus sheath with some chocolate-stained areas without connection to the peritoneal cavity. Additionally, multiple uterine fibroids were noted. There was no pelvic endometriosis, and the other pelvic organs were normal. The histopathological diagnosis of the abdominal wall mass was endometriosis. AWE should be one of the differential diagnoses even in the absence of previous surgery when encountering a patient with an abdominal wall mass especially when it is related to the menstrual cycle.
A dark umbilical nodule in a postmenopausal woman: What is your diagnosis? Nur Hidayah Mohd Fauzi, Nik Ahmad Zuky Nik Lah, S. Hamzah, W Fadhlina W Adnan, Adibah Ibrahim, et al. Malaysian Family Physician, 2025 Sister Mary Joseph's nodule is a rare but significant clinical sign that often indicates advanced intraabdominal malignancy. It presents as a palpable umbilical nodule and may be the first indication of an occult primary cancer. In this article, we discuss the case of a 58-year-old postmenopausal woman who presented to a primary care clinic with a darkened umbilical lesion and progressive abdominal symptoms. She wps ultimately diagnosed with metastatic endometrial carcinoma. This uncommon presentation highlights the importance of a comprehensive approach to umbilical lesions in the primary care setting. Recognising this subtle sign in primary care can lead to expedited investigations and timely intervention.

A 30-year-old dermoid cyst Erinna Mohamad Zon, W Fadhlina W Adnan, Kaamini Planisamy, Nik Mohamad Zaki Nik Mahmood Bangladesh Journal of Medical Science, 2022 Mature cystic teratoma (MCT) is the most common tumour of the ovary and includes at least two well-differentiated germ cell layers. The incidence is higher in reproductive age group and rarely among postmenopausal age women. It is benign condition however associated with multiple complication such as torsion, ruptured and chemical peritonitis. As it is rare among postmenopausal age group, it can be presented as chronic ovarian mass with expected complication and possibility of malignant transformation. Thorough assessment with adjunct of radiological imaging may assist in diagnosis and further management. Radiological imaging modalities were sensitive and sensitive in diagnosis of MCT however there were low specificity in detection of complication. Management of MCT in postmenopausal women mandate thorough evaluation considering the risk of malignant transformation and associated complications. Probiotics Reduce Vaginal Candidiasis in Pregnant Women via Modulating Abundance of Candida and Lactobacillus in Vaginal and Cervicovaginal Regions Xin Ang, Uma Mageswaran, Yi Chung, Boon Lee, Siti Azhar, Nurhanis Roslan, Ili Saufian, Nor Mustaffa, Ermadina Kalam, Aini Ibrahim, Normala Abdul Wahid, Zakuan Deris, Chern-Ein Oon, Wan Adnan, Salina Sany, Min-Tze Liong Microorganisms, 2022 We previously reported on the effects of a lactobacilli probiotic (SynForU-HerCare; two capsules/day of 9.5 log CFU/capsule) in improving symptoms of vaginal irritation, discharge and burning in pregnant women with vaginal candidiasis upon administration for 8 weeks, accompanied by improved emotional and social quality of life parameters. Thus, the present study aimed to analyse vaginal microbiota and inflammatory changes in hope to better understand the improved clinical symptoms as observed previously. Patients in the probiotic group showed a decreased abundance of Candida glabrata after 8 weeks (p = 0.009) in the lower vaginal region, while patients in the placebo group did not show any changes over time. In the higher vaginal and cervicovaginal regions, patients in the placebo group showed a decreased abundance of Candida albicans only within 4 weeks (p < 0.05) but no changes in abundance of C. glabrata over time, while patients in the probiotic group showed a continuous decreased abundance of C. albicans and C. glabrata over 8 weeks (p < 0.05). Patients in the placebo group also had a decreased abundance of Lactobacillus crispatus over 4 weeks (p = 0.023) in the lower vaginal region and a decreased abundance of L. jensenii over 8 weeks in the cervicovaginal region (p = 0.001). Meanwhile, patients in the probiotic group had an increased abundance of L. crispatus in the lower vaginal region after 8 weeks (p = 0.012) and Lactobacillus jensenii over 4 weeks in the cervicovaginal region (p < 0.001). Inflammation may have occurred in both low and high vaginal regions, predominantly observed by the increased concentration of pro-inflammatory cytokine TNF-alpha in patients from the placebo group (p < 0.05), while the administration of probiotics has shortened the period of inflammation as observed from the reduced need for anti-inflammatory cytokine IL-4 and IL-10 over time (p < 0.05). Taken together, our present new data further support previous findings that probiotic SynForU-HerCare had a beneficial effect against vaginal candidiasis in pregnant women via modulation of the vaginal microbiota and microenvironment.
A rare cause of acute urinary retention in adolescents: A case of imperforate hymen Erinna Mohamad Zon, Nik Rafiza Afendi, Nurul Bazilah Mansor, W Fadhlina W Adnan Malaysian Family Physician, 2021 Imperforate hymen is a genital outflow abnormality that can occur in females. It can present with various symptoms and is associated with short- and long-term complications that may affect patients’ quality of life. Acute urinary retention in adolescents is a rare occurrence. We report a case of delayed diagnosis of imperforate hymen involving multiple visits to the clinic for urinary symptoms and the subsequent development of acute urinary retention. Awareness of this rare presentation is essential since delayed diagnosis is associated with hydronephrosis, endometriosis and infertility in later life.
