  • Peri-hilar branching pattern and variations of the renal artery among Indian kidney donors using pre-operative computed tomography angiography: an anatomical study and review

    BACKGROUND The peri-hilar branching pattern of renal arteries and variations such as accessory renal artery and early branching of the renal artery are important factors to be evaluated preoperatively to minimize potential complications during renal transplantation and renal surgeries. The present was done to study the perihilar branching pattern of the renal artery and variations of the renal artery in kidney donors. MATERIALS AND METHODS 198 kidneys from 99 donors were analyzed using pre-operative CT renal angiograms prospectively, over 1 year 2 months. RESULTS Based on the branching pattern of primary and secondary branches of renal arteries, we identified 8 cardinal and 10 minor peri-hilar branching patterns. Type I (duplicated fork) peri- hilar branching pattern was the most found (70.2%) among 198 kidneys, followed by type II (triplicated fork) (19.2%) and type III (ladder) (10.6%). Duplicated fork pattern was common in both the right renal artery (75.7%) and left renal artery (64.6%) respectively. The prevalence of accessory renal artery was 39.4% with hilar artery at 21.2% and polar artery at 18.2%, and the prevalence of early division of renal artery was 20.2%. No statistical significance was associated between gender and laterality in the prevalence of both early division and accessory renal arteries (P > 0.05). Early division of renal artery was more frequent in females (28%) whilst accessory renal arteries were more common in males (40.7%). CONCLUSIONS The present study showed a consistent peri-hilar branching pattern with high individual variability among Indian renal donors. The knowledge about the peri-hilar branching pattern as well as renal artery variations conferred by this study would greatly compliment urological surgeons during renal transplant surgeries.
  • Relation of body mass index to heart rate variability in female medical students

    Heart rate variability (HRV) is a simple non-invasive, sensitive measure to evaluate autonomic system activity. Sympathovagal imbalance can explain increased incidence of sudden cardiac deaths associated with increased Body Mass Index (BMI). Understanding the mechanism that connects obesity and autonomic nervous system (ANS) functions is important because increasing obesity prevalence documented among men and women of all ages. The aim of the study is to assess the relationship of body mass index to heart rate variability in young females. In this study, 90 healthy female students, aged between 17-22 years were included. After calculating the BMI, subjects were divided into three groups. HRV was recorded for 5 min using INCO-NIVIQURE DIGITAL ACQUISITION SYSTEM VER.52.0. Low frequency (LF) component, high frequency (HF) component and LF:HF ratio were recorded and the relation between BMI, HF, LF and LF:HF ratio were analysed using ANOVA test. In this study, LF:HF ratio in overweight females was found to be high and parasympathetic was decrease when compared to normal weight and underweight females. Raised BMI is associated with reduced HRV, which correlates with decreased parasympathetic activity and sympathovagal imbalance, thus increases the chances of cardiac autonomic dysfunction and eventually leading to cardiovascular disease in overweight female.
  • Prevalence of menstrual symptoms and primary dysmenorrhea among medical undergraduates in south indian population.

    Dysmenorrhea is known as a painful period during menstruation. It is the widely prevalent and common complaint among young women, which affects their quality of life. The aim is to assess the prevalence of menstrual symptoms and primary dysmenorrhea during menstruation among medical undergraduates and its effects on the quality of their life. A cross-sectional study was conducted among 60 Medical undergraduates atSaveetha Medical college. The participants were asked to complete a self-reported questionnaire on menstrual symptoms and primary dysmenorrhea. Primary dysmenorrhea was reported in 43.9% (27) of participants. Abdominal pain was reported in 78% (46), and 52% (31) of girls felt weak and tired during menses. In 40% (24) of girls, there has been a family history of primary dysmenorrhea. Abdominal pain was found to be highly prevalent among adolescent girls. Family history, bleeding duration, tiredness are some of the risk factors associated with primary dysmenorrhea.
  • Analysis of morphometry of calyceal pattern in kidney donors using cangiogram images

    The advent of more conservative methods of treatment for renal pathologies has necessitated a precise knowledge of renal pelvicalyceal system. Knowledge on the patterns of pelvicalyceal system and its relation to the renal segmental arteries is of importance in procedures in kidney. The aim is to analyze the morphometry of calyceal system in kidney donors using 64- Slice Computed Tomography Angiography. The study was conducted in specialized scan center located in Chennai after getting appropriate permission from medical director of the center. The study group was drawn from kidney donors who approached the scan center who had no diseases related to kidney. In this study, a total of 99 kidney donors were included. This was a prospective cross-sectional study conducted from June 2014 to September 2017. The statistical analysis and plotting of graphs were carried out using Sigma stat 13.0 (Systat Software, USA). Various patterns of the pelvicalyceal system along with morphometric dimensions of the infundibulum, which leads from the major calyces to the renal pelvis were studied. The result was then correlated with clinically significant parameters.
  • Analysis of Intra Renal Arterial Pattern in Kidney Donors Using 64-slice Computed Tomography Angiography

    The renal artery when it reaches close to the hilum of kidney where it cleaves into anterior and posterior divisions were receives around 75% and 25% of blood respectively. Further these branches divide into many segmental branches. These segmental arteries does not have collateral system, so when there is any occlusion of these arteries which results in the ischemia of the particular segment of kidney. Anatomists believe that knowledge of presence of lower polar artery is essential for polar nephrectomy. Prior knowledge of variations in the renal vessels is required for urological surgery for preoperative investigations as there are reports of surgical complications which include enormus haemorrhage. In this study we analyzed the different variations in the intra renal branch of inferior segmental artery.

    Background: Stress and increase in free radicals levels are known to alter cognition, learning, memory, and emotional responses. There is a marked impairment of hippocampus-dependent memory and suppression of long-term potentiation in the CA1 region of the hippocampus during stress. Aim and Objective: The aim of this study is to evaluate the hypothesized neuroprotective effect of oral antioxidants and piracetam in chronic restraint stress-induced rats. Materials and Methods: Healthy Wistar rats were divided into 5 groups (n = 6) each. Control group received neither stress nor oral antioxidant. Stress group received chronic restraint stress for 6 h per day for 21 days. Three experimental groups were administered vitamin C (100 mg/kg), beta-carotene (7 mg/kg), and caffeine (8 mg/kg), respectively. Evaluation parameters were measurement of serum nerve growth factor (NGF) using ELISA method. The oxidative stress markers, glutathione peroxidase (GPx), glutathione reductase, and malondialdehyde were measured. Histological analysis of CA1 region of the hippocampus was done to evaluate the structural changes of pyramidal neurons. Results: Vitamin C caused statistically significant (P < 0.001) increase in serum NGF. The results revealed that vitamin C caused statistically significant (P < 0.05) increase in antioxidant enzymes GPx, glutathione reductase. Vitamin C caused increase in neuronal cell size and volume in CA1 pyramidal layer of hippocampus. Conclusion: The findings of study are suggestive of neuroprotection, offered by administration of vitamin C compared to other antioxidants against chronic restraint stress-induced rats. These naturally available dietary vitamins might serve as an adjuvant therapy to avoid progression of brain damage during stress.
