    يُعد سرطان القولون والمستقيم من أكثر أنواع السرطان شيوعًا في المملكة العربية السعودية، حيث يحتل المرتبة الأولى بين الذكور والثالثة بين الإناث. وعلى الرغم من وجود برامج وطنية للفحص المبكر، إلا أن معدلات الوعي والمشاركة في الفحص لا تزال منخفضة، مما يعكس الحاجة لفهم مستوى المعرفة الحالي وتحديد العوامل المؤثرة على السلوك الصحي المتعلق بالفحص. هدفت هذه الدراسة إلى تقييم مستوى الوعي بسرطان القولون والمستقيم، بما في ذلك الأعراض وعوامل الخطورة، وكذلك مدى انتشار فحص الكشف المبكر، وتحليل العوامل الديموغرافية المرتبطة بهما بين البالغين في المنطقة الشرقية من المملكة. تم إجراء دراسة مقطعية شملت ٤١٢ مشاركًا من البالغين باستخدام استبيان إلكتروني موزع بين أبريل ويوليو ٢٠٢٤. تم تقييم الوعي بالأعراض وعوامل الخطورة والفحص، وتحليل العوامل المتنبئة بها. شملت البيانات الخصائص الديموغرافية، مستوى التعليم، وجود تاريخ عائلي للسرطان، والمكان الجغرافي. أظهرت النتائج أن ٥٧٪ من المشاركين لديهم وعي ضعيف بسرطان القولون والمستقيم. كانت الإناث أكثر وعيًا بالأعراض مقارنةً بالذكور، وسجلت الفئة العمرية من ١٨ إلى ٣٠ عامًا أعلى متوسط للوعي. وُجد ارتباط إيجابي بين التعليم العالي والوعي بعوامل الخطورة. بلغت نسبة من خضعوا للفحص المبكر للقولون ١١.٧٪ فقط، وارتفعت إلى ٢٤.٧٪ بين من تجاوزوا ٥٠ عامًا. من العوامل المتنبئة بالوعي الجيد: التعليم العالي، والتعرض المسبق لمعلومات حول السرطان، ووجود تاريخ عائلي. بينما كان العمر المتقدم، والإقامة في مناطق حضرية، ووجود تاريخ عائلي عوامل متنبئة بزيادة احتمالية الخضوع للفحص. تعكس الدراسة تدني مستويات الوعي والمشاركة في فحص سرطان القولون والمستقيم في المنطقة الشرقية، مما يستدعي تدخلات متعددة المستويات تشمل حملات توعوية موجهة، تعزيز دور الأطباء في التوصية بالفحص، وتفعيل أنظمة تذكير إلكترونية ضمن السجلات الطبية. تمثل هذه الخطوات ركائز لتعزيز الالتزام المجتمعي ببرامج الفحص وتقليل عبء المرض في المملكة. Colorectal cancer (CRC) is one of the most frequent cancers in KSA, ranking first among males and third among females. However, the uptake of the national screening programs is poor due to insufficient awareness and access barriers. Thus, this study evaluated the awareness of CRC and screening levels among adults in the Eastern Province of KSA, where recent data are limited. A cross-sectional study was conducted among 412 adults using a self-administered questionnaire between April 2024 and July 2024. Awareness of CRC signs, symptoms, risk factors, and screening was assessed. Logistic regression was used to determine the predictors of awareness and screening practice. In total, 57.0 % of the participants had low awareness of CRC. Females were more aware of the signs and symptoms of CRC than males ( p = 0.025). The lowest age group (18–30 years) had the highest mean awareness score ( p = 0.037). Higher level of education was positively related to knowledge of risk factors for CRC ( p < 0.001). Only 11.7 % had undergone CRC screening, which increased to 24.7 % among those aged 50 years and above. Positive predictors of awareness included higher education level ( p = 0.023) and prior knowledge of CRC ( p = 0.043). Age ( p < 0.001), living in main (central urban areas) areas ( p < 0.001), and family history ( p < 0.001) were predictors of undergoing screening. In the Eastern Province of KSA, awareness of CRC and screening rates are low. Thus, it is suggested that awareness campaigns, discussions with physicians, and electronic medical records-based reminder systems may help enhance compliance.

  • The Utility of Negative Pressure Wound Therapy in the Management of Complex Deep Cardiothoracic Surgical Site Infections

    Background. Complex deep surgical site infection in the cardiothoracic surgery patient that reaches the sternum and even the mediastinum, causing osteomyelitis and mediastinitis, is associated with high rates of morbidity and mortality. Negative pressure wound therapy (NPWT) can aid in achieving favorable outcomes in patients with complex surgical site infections by promoting wound healing and shortening the hospital stay. NPWT is widely recognized for its advantages and has recently been used in both cardiothoracic and non-cardiothoracic settings. Objective. To evaluate the efficacy of NPWT in the management of complex deep surgical site infection after cardiothoracic surgery. Materials and Methods. A retrospective chart review of all complex cardiothoracic cases admitted to the cardiac and thoracic surgery divisions for surgical intervention to treat postoperative surgical wound infections. Results. A total of 18 patients were included, with a male-to-female ratio of 5:4. The mean (SD) age was 48.7 (16.5) years. The cases reviewed were complex, and the duration of the NPWT application ranged from 4 days to 120 days, with an average hospital stay of 62.8 days. Seventy-eight percent of patients required antibiotics (or had positive wound cultures); in 55.6% of these patients, polymicrobial infection was detected. No major complications were related to NPWT. Conclusion. The study findings show that using NPWT in complex deep sternal and thoracic infections can enhance wound healing, shorten the hospital stay, and decrease morbidity and mortality secondary to wound infection in cardiothoracic patients.
  • The Patterns and Quality of Acute Trauma Care in A University Hospital-A 5-year Analysis

    Background: Traumatic injuries contribute to over 5 million fatalities each year and cause a significant burden on healthcare systems' resources and availability. Traumatic injury patterns and a healthcare system’s efficiency are critical to predicting patients’ outcomes and should be regularly evaluated to deliver optimum care. Objective: The study will present and discuss trauma qualty indicators (TQI's) to display the local practices' opportunities for improvement and analyze patterns of traumatic injuries and outcomes. Methods: The study retrospectively analyzed trauma cases from January 2016 to December 2020, in which patients hospitalized and treated under the trauma unit were included. The collected data included several variables (e.g., demographics, mechanism of injury), and the patients were divided into early/late phase and severe/non-severe cases. Results: In total, there were 2024 cases, 70% were polytraumas with a mean age of 29 years and a male-to-female ratio of 3.8:1. The most common mechanism of injury was motor vehicle collision (55%) while the most common injuries were pneumothorax (18%) and lung contusions (17%). Notably, there was a 3.5-hour reduction in both the time to the operating room and the time to normotension when comparing the early to late phase. The 30-day outcomes for the patients were 90.5% recovery, 2.2% disability, 2.2% transfer, and 5.1% mortality rates. Conclusion: This study emphasizes how crucial trauma quality indicators are to achieve the best outcomes for patients. The results point to improvements in terms of trauma quality indicators and offer insightful information on patterns of traumatic injuries, aiding in the enhancement of trauma care. Thus, the continuous assessment of trauma quality indicators and the implementation of preventive measures are mandatory for the future advancement of trauma care.
  • Spigelian Hernia in Cirrhotic Patients: When and How to Repair?

    Background: Spigelian hernia, also known as spontaneous lateral ventral hernia, is defined as a protrusion of abdominal contents through an abdominal wall defect within the transversus aponeurosis. The entity was first reported in 1742 and named after Adrian van der Spieghel. It is recognized as a rare condition, accounting for 1 to 2% of all abdominal wall hernias. Cirrhotic patients, in particular, are more predisposed to hernias of all types. When Spigelian hernia is accompanied by hepatic cirrhosis, the decision to repair gets into a controversial aspect. Herein, we present the emergency management of an incarcerated spigelian hernia in a cirrhotic patient, highlighting the challenges in managing similar cases. Case Presentation: A 65-year-old lady, presented to our emergency department complaining of a left lower quadrant (LLQ) abdominal pain associated with a painful swelling for 9 hours duration. Upon assessment, the patient was jaundiced and haemodynamically stable. Abdominal examination revealed a soft and lax but distended abdomen, with irreducible tender swelling over the LLQ, measuring around 3x2 cm. Laboratory investigations showed anemia, hypoalbuminemia, hyperbilirubinemia, lactic acidosis and prolonged coagulation profile. A contrast-enhanced computed tomography (CT) scan of the abdomen showed evidence of incarcerated left spigelian hernia. Moreover, advanced cirrhosis of the liver was detected with hypertrophy of the caudate lobe, extensive ascites and splenomegaly. After establishing the diagnosis of incarcerated spigelian hernia, with a picture of advanced liver cirrhosis, Child-Pugh-Turcotte (CPT) score of C and a Model for End-Stage Liver Disease (MELD) score of 19 and Mayo score for post-operative mortality of 16% in 7 days and 53% in 30 days. Given the patient’s condition which necessitates urgent operative intervention beside the risk of decompensation of pre-existing liver disease and high mortality. Decision was made to proceed with laparoscopic hernia repair. Intraoperatively, ischemic small bowel segment was resected with creation of end ileostomy. Conventional anatomical repair of the hernia defect was performed. Postoperatively, the patient was managed and resuscitated in critical care unit and then discharged home in a satisfactory condition on post-operative day 15. She passed away prior to liver transplantation, thirty-two days post-operatively in a different institution. Conclusion: The surgical management of complicated hernias in an emergency setting comprise a notable number of cases encountered by surgeons. It always needs special attention. Moreover, cirrhotic patients with their predicted high morbidity and mortality require even further vigilance. Therefore, a patient-tailored approach is always recommended when managing similar cases to provide optimal outcomes.
  • Angio-Jet Thrombolysis for Traumatic Inferior Vena Cava Thrombosis; Case Report and Review of Literature

    Background: Inferior vena cava thrombosis (IVC-Th) is a rare clinical entity after blunt abdominal trauma. It has both diagnostic and therapeutic dilemmas. Pulmonary embolism is the most dreadful complication and the leading cause of mortality after IVC-Th. Therefore, accurate prompt diagnosis is crucial. Objective: The aim of this article was to present a case of IVC-Th in a young male patient who had a blunt traumatic abdominal injury after a motor vehicle accident. Case presentation: The patient was brought to emergency department and was successfully managed by angio-jet thrombolysis. He developed a transient contrast nephropathy that was recovered after continuous renal replacement therapy. Several management options have been proposed in the literature, including conservative, endovascular and operative management. Conclusion: Angio-jet is a recent promising technique for managing of venous thrombosis. However, its use in cases of IVC-Th is not extensively discussed in the literature.
  • Esophageal Injury After Anterior Cervical Spine Fixation: What Can Thoracic Surgeons Offer?

    Background: The anterior approach for cervical spine surgery was introduced in the late 1950's and was then extensively used with a known considered risk for an esophageal injury. Objective: Therefore, we designed this study to look into our institution’s experience in the treatment of esophageal injury post cervical spine fixation looking at possible causes, presentation and risk factors. Methods: A retrospective analysis of esophageal perforation cases secondary to cervical spine fixation, encountered in King Fahad hospital of the University (Eastern province, Dammam, Saudi Arabia) in between (2008-2019) were studied. Results: The patients included were 11, in which the most common presenting symptoms were dysphagia, neck pain, persistent discharge from the cervical wound with or without food particles and odynophagia, while the mean period for presentation was 358.9 days (ranging from 4 to 2555 days). The pharyngo-esophageal perforation repair was done in 9 patients, while 2 patients were treated conservatively. In addition, supportive measures (i.e., frequent wound care, broad spectrum intravenous antibiotics, non per oral and adequate feeding via alternative routes) were done for all patients. Conclusion: Esophageal injury, although uncommon in anterior spinal approach, still considered a life-threatening complication and need a low threshold of suspicion.

  • Non-obstetric Acute Abdomen in Pregnancy: a Review of Literature

    Background: Acute abdomen in pregnancy (AAP) is defined as intensive abdominal pain lasting less than 24 hours that may require urgent surgery. It is a challenging situation to diagnose and manage, as it is associated with pain due to the normal anatomical and physiological changes that occur during pregnancy. Objective: Therefore, understanding these changes and their effect on almost every system, will help us appreciate the upcoming causes of AAP, mainly the non-obstetric surgical emergencies (e.g., appendicitis, cholecystitis). Methods: This article highlights the importance of the well-rounded care that should be offered to every pregnant patient presenting to any center with a Non-obstetric Acute Abdomen. The causes are discussed separately (for surgical pearls), laparoscopic approach and radiologic modality decision-making in pregnancy, which is an academic and a practice-based helpful summary. Results and Discussion: This article highlights the importance of the well-rounded care that should be offered to every pregnant patient presenting to any center with a Non-obstetric Acute Abdomen. The causes are discussed separately (for surgical pearls), laparoscopic approach and radiologic modality decision-making in pregnancy, which is an academic and a practice-based helpful summary. Results and Discussion: In addition, the sequence of ideas and language used in the article was based to help the reader understand the topic, rather than inform them about it. In addition, the utility of laparoscopy in pregnancy remains a concern due to the possible risk of injury to the fetus and fetal acidosis. Although, trials showed that laparoscopy was associated with less blood loss and a shorter hospital stay. Moreover, in some cases radiographic imaging is necessary, posing a diagnostic dilemma. Conclusion: AAP is a major concern that requires early interventions to pinpoint the cause and manage the patient, properly.
  • Efficacy of Mediastinoscopy in Patients With Isolated Mediastinal Lymphadenopathy

    Background: Since its introduction in 1959 by Carlens [1], Mediastinoscopy has been, for long, used for assessment of the mediastinum (superior and middle) for establishing a histological diagnosis of mediastinal masses of undefined cause, and for Lung carcinomas staging. The use of Mediastinoscopy has been decreasing lately due to the introduction of other less invasive techniques (e.g., endoscopic ultrasound-directed fine needle aspiration cytology), however, it is still a cheap and effective tool that can be utilized in underprivileged centers. Objective: To emphasize how does Mediastinoscopy plays an important role in confirming the clinical diagnosis of isolated mediastinal lymphadenopathy and reviewing its utility. Methods: These are a retrospective analysis of medical charts for patients who underwent diagnostic cervical mediastinoscopy during (2012 – 2018) at a University hospital in Saudi Arabia. The included patients are presented with an isolated mediastinal lymph node enlargement, in the absence of underlying cause and was found to be significant (>1cm in its short axis) by computed tomography. The patient who had a known cause (e.g., Sarcoidosis) or were diagnosed via other tools, was excluded. Results: Mediastinoscopy was performed on 56 patients, 38 of them were males (68%) and 18 females (32%), with a mean age of (37.5 ± 10 years). The patients' most common presenting symptoms were persistent cough (49%), fever of unknown origin (38%) and weight loss (36%) with an average of 2 symptoms per patient, while in 4 patients (7%) lymphadenopathy was discovered incidentally during the CT scan for other reasons. In addition, the histopathological examination of specimens obtained confirmed the most common diagnoses, Sarcoidosis in 17 patients (30%), lymphoma in 12 patients (21%) and TB in 10 patients (18%). The mean hospital stay (calculated from the day of the procedure) was (2.5 ± 4 days) including work up, with only one mortality (2%) and 3 patients (5%) had experienced post-operative complications. Conclusion: The diagnostic Mediastinoscopy is both safe and efficient in the diagnosis of patients with isolated mediastinal lymphadenopathy, requiring a minimal surgical setup and is considered cost-effective. Therefore, it is a valid choice of investigating such cases in other underprivileged centers, as it reaches a tissue-based diagnosis, while other techniques are used for staging purposes.
