Cross sectional study on fallacies in wound certification in a tertiary care teaching hospital hyderabad telangana K. Srinivasulu, Om Shanti Jupaka, Y. Nikitha, Chittireddy Sainithya, Gowdaperi Sirimanjari, Aditi V. Sajjanar International Journal of Medical Toxicology and Legal Medicine, 2022 Medical record documentation is an integral part of the duty of medical practitioner, it is necessary to ensure that rightful information to be provided in all medico legal cases. Good medical records always provide a good defense in the court of law. A cross sectional study on Fallacies in wound certificate documentation was conducted in a tertiary care teaching hospital in Hyderabad, Telangana, 300 cases of wound certificate documents were collected from emergency department, casualty and analyzed for errors in documentation. The components of the certificate were divided into minor and major group. Majority of the errors were observed in major group. In major group, dimension of the wound and causative weapon was not incorporated precisely. In minor group address and identification marks are not mentioned accurately. Major group consist of 51.9% and minor group consist 23.3% of errors. Periodical workshops and CME's on medical documentation can minimize the errors.

Study on profile of acute poisoning in a rural tertiary care hospital in Telangana K. Srinivasulu, K Vishal Pratik, T Rahul Reddy, Harsha Kumari, Sangam Anurag Reddy Medico Legal Update, 2019 Poisoning has become an increasing cause of concern over the past decade, not only a major medical issue in India but also a significant global health problem. Nearly 3 million people consume poison each year resulting in 220000 deaths annually, majority with the intention of suicide. A Retrospective, hospital record based study was conducted in a rural medical college hospital including 100 cases of acute poisoning. Snake bite cases were excluded. Agricultural poison specifically organ phosphorus poisoning is the most predominant one observed in our study, with a total of 61 percent cases reported to the hospital. Majority of the poisonings were in the age group of 21–40 years. Gender based in-equivalence observed among males and females with a ratio of 3: 1. A significant percentage of suicides were found with an estimate of 95%. Around 39% cases treated with a specific antidote and the remaining 61% had other modalities of treatment. The early initiation of gastric lavage, specific antidote and appropriate supportive management achieved a better outcome. The mortality of poisoning is 1% in the current study. The lower mortality rate observed due to large number of cases about 25% are referred to higher centers. Highest number of suicides was observed among farmers due to losses in agriculture, financial hardship and domestic issues. Psychologically depressed farmers committed suicide by consuming nearest available poison, organo-phosphorous compounds. Psychiatry counseling, agriculture support and financial help should be given to them at community level to bring down the mortality. Hospitals should develop adequate diagnostic facilities, equipment support and skilled supportive staff to combat poisoning deaths.
Rights and responsibilities of the patient in health care system K. Srinivasulu, B. Dedipya, B. Mrudula Medico Legal Update, 2018 Patient rights have come into the limelight ever since the Nuremberg trials post-World War II. Afterwards, great strides were taken to ensure there is an international consensus regarding human rights. Due to lack of education and awareness, many patients are still being wronged and cheated of their basic rights. Patients can demand better services and facilities, only when they have an understanding of their rights. The purpose of the study was to assess patients’ awareness of their rights and responsibilities in association with health care. This study was a cross-sectional analysis. The research sample consisted of 400 individuals who have attended hospitals earlier for treatment purpose, including males and females of varied educational backgrounds. The data was gathered through questionnaires and analyzed by descriptive and analytical statistics. In this study, results indicated the awareness was not universal. Amongst the rights that were evaluated, individuals were most aware of their right to receive treatment in a safe and hygienic hospital environment. On the other hand there was minimal awareness of the right to access their medical records as well as the right to be informed about their disease and its progression. The study revealed a trend showing increased awareness with higher levels of education. Measures should be taken by the government as well as organizations to improve awareness levels in the lesser educated population. This will consequently improve the quality of health services. In addition, hospital development committees should take extra care during the implementation of these rights.
Study on informed consent in medical practice K Srinivasulu, A B Sai Samhitha Indian Journal of Forensic Medicine and Toxicology, 2017 Consent is a voluntary agreement between a doctor and a patient prior to any examination or treatment. Proceeding without consent is tantamount to assault and battery, even though it is beneficial to the patient and done in good faith. In this case the patient can file a complaint against the doctor. Informed consent is a must in medical practice. A study was conducted regarding documentation procedure of informed consent in Hyderabad and Ranga Reddy Dist. the consent part of the case sheets were thoroughly examined. and several inconsistencies were found in the consent forms. All errors were analyzed to explain medico-legal issues.

Whenever a case of medical negligence filed against a hospital, the only defence is medical record. Good records always provide a good defence. A cross sectional study was done on Medical record documentation at a rural medical college hospital, with an objective to appraise the importance of Medical record documentation with regard to various medico legal aspects. 100 case sheets were collected from the medical record department of the hospital and information from the case sheet was collected in to a data sheet and analysed for errors. None of the case sheet was free from documentation errors. We observed too many lacunae in the case sheets and explained how it leads to medico legal issues.

Unusual presentation of snake bite- a case report P Jayalakshmi, K Srinivasulu, N V Tejaswini, V Aravind Reddy Indian Journal of Forensic Medicine and Toxicology, 2015 Snake bite is a commonest condition in tropical countries like India, Pakistan and Bangladesh, we have reported 4 cases of Neurotoxic snake Krait bite with an unusual presentations at our Rural Medical college Hospital, all the four cases are admitted in the intensive care unit under department of paediatrics, the most common and early presentation in all the 4 cases are Pain Abdomen, vomiting and autonomic disturbance followed by altered sensorium and developed neurotoxic Manifestations, In all the four cases the time of snake bite was mid night and early hours of the night, all the cases were treated with Polyvalent Anti snake venom with Artificial ventilation support.
Analytical study of medical certification of cause of death K Srinivasulu Indian Journal of Forensic Medicine and Toxicology, 2014 The study on assessment of Medical certification of cause of death was conducted at a rural medical college hospital in Andhra Pradesh to find out the errors in cause of death certificates for a period of one year from 1st January 2011 to 31st December 2011. The certificates are collected from Medical record department of the hospital along with case sheets and analysed for errors. The errors are grouped in to two Major and Minor, Not even a single certificate is free from error, majority of the errors an average about 47% are found in Major group and minor group consist 21% of errors, Most of the errors are avoidable with proper guidance and periodical training to the medical practitioners.
