- Development and Validation of Menstrual Disorder of Adolescent (MenDA) Questionnaire in the Malay Language

Introduction: Menstrual disorders negatively affect adolescents’ quality of life, with physical discomfort and emotional distress often contributing to anxiety. To address this, the present study aimed to develop and validate the Menstrual Disorder of Adolescent (MenDA) questionnaire for assessing adolescents’ knowledge, attitudes, and practices (KAP) regarding menstrual disorders. Materials and Methods: The questionnaire was systematically developed through literature review, focus group discussions, and expert consultation. Six experts assessed content validity, while face validity was tested among 46 adolescents. A cross sectional validation study was conducted with 485 adolescents. The MenDA tool comprised three domains: knowledge (23 items), attitude (18 items), and practice (21 items). Construct validity was evaluated using exploratory factor analysis with principal component extraction and varimax rotation. Internal consistency was determined using Cronbach’s alpha coefficient. Results: The finalized MenDA questionnaire contained 62 items, structured into three domains. The knowledge domain included 23 items and demonstrated good reliability with Cronbach’s alpha of 0.739. The attitude domain, comprising 18 items, showed a Cronbach’s alpha of 0.711, while the practice domain, with 21 items, achieved a Cronbach’s alpha of 0.793. These results confirmed satisfactory internal consistency across all domains. Factor analysis further supported construct validity, reinforcing the appropriateness of the three-domain structure in evaluating adolescents’ menstrual health KAP. Conclusion: The MenDA questionnaire has demonstrated strong validity and reliability as a tool to assess knowledge, attitudes, and practices regarding menstrual disorders among Malaysian adolescents. Foul-smelling vaginal discharge in adolescents often stems from infectious origin, foreign body insertion, neoplasm, congenital anomaly, and sexual abuse. Diagnosing the causes of the discharge remains challenging, especially in cases where sexual and reproductive health awareness is relatively low in the population from marginalized ethnographies and socio-cultural backgrounds. Suspicion of sexual abuse must be explored carefully and sensitively. A history of sexual exposure might not be forthcoming in adolescents where premarital sex is frowned upon. The process of eliciting the causation may be a challenge to healthcare professionals, delaying the diagnosis and subsequent management. In rare circumstances, imaging studies may help confirm the origin of the infection, giving insight into the presentation’s aetiology. We describe a case of Obstructed Hemivagina and Ipsilateral Renal Anomaly (OHVIRA) Syndrome presented with foul-smelling vaginal discharge in a newly attained menarche girl in the North-East region of Peninsular Malaysia. Introduction: A retroperitoneal hematoma in obstetrics is very rare following spontaneous vaginal delivery. It requires a high index of suspicion, especially in a hemodynamically unstable patient with no active bleeding or external trauma postpartum, as early intervention is needed to prevent maternal morbidity and mortality. Conservative management may be an appropriate strategy in managing large hematomas even though surgical evacuation is classically recommended. Case: We report the case of a 39-year-old woman, para three, with a hematoma of the Retzius space after spontaneous uncomplicated vaginal delivery. The patient had sudden abdominal pain associated with urinary retention, two hours after delivery. She had a palpable mass at the anterior upper part of the vaginal wall, near the fornices, but no abnormality was detected at the labia majora, labia minora and lower part of the vagina. A transabdominal ultrasound scan revealed a mixed-echogenicity mass between the bladder and lower part of the uterus measuring 110 × 90 × 60 mm. The patient's hemoglobin level dropped from 12 g/dL to 10 g/dL. Given the patient's clinically stable state, she was managed conservatively with antibiotics, analgesics and one unit of packed red blood cells, and was discharged home day 5 postpartum. Conclusion: This case supports the conservative management of even large Retzius space hematomas for hemodynamically stable patients. A high index of suspicion and thorough evaluation are required when encountering a complaint of urinary retention and abdominal pain postpartum.
