  • Oral Immunotherapy-Induced Changes in IgE, IgG, and IgA: A Review of Antibody Isotype Shifts and Their Clinical Relevance in Food Allergy

    Background: Food allergy is a growing public health concern, and oral immunotherapy (OIT) has emerged as a promising approach to induce desensitization and potentially sustained unresponsiveness to allergenic foods. Changes in humoral immunity, particularly in allergen-specific immunoglobulin levels, play a central role in the immunological mechanisms underlying OIT. This review aims to summarize the current evidence on how OIT modulates allergen-specific immunoglobulin E (IgE), G (IgG) and A (IgA) responses in individuals with food allergy. Methods: We conducted a review of original research articles reporting longitudinal data on allergen-specific IgE, IgG, and/or IgA in patients undergoing OIT for common food allergens. Results: OIT was consistently associated with a transient increase in allergen-specific IgE levels during early phases, followed by a gradual decline. In contrast, Allergen-specific IgG4 levels showed a robust and sustained increase, correlating with desensitization and proposed to function as blocking antibodies. Several studies also reported an increase in allergen-specific IgA, particularly secretory IgA at mucosal sites, suggesting a potential role in enhancing mucosal tolerance and immune exclusion of allergens. Conclusions: Humoral immune responses during OIT are characterized by distinct and dynamic changes in immunoglobulin patterns. In particular, the rise in IgG4 and, in some cases, IgA suggests a role in promoting tolerance. Monitoring these biomarkers may offer insights into treatment efficacy and support individualized approaches to OIT.
  • Double-balloon enteroscopy evaluation and surveillance in patients with short bowel syndrome

    Background: Safety and usefulness of double-balloon enteroscopy (DBE) in patients with short bowel syndrome (SBS) are unknown. DBE could be effective in exploring the residual small bowel (SB) entirely, in monitoring patients receiving teduglutide (TED) treatment and, if necessary, could be operative. Objective: This study aimed to assess the safety, feasibility, and efficacy of enteroscopy in SBS patients. Design: Single center, retrospective. Methods: We collected data on patients with SBS patients undergoing DBE from December 2019 to October 2023. We analyzed the technical success total enteroscopy rates, adverse events, diagnostic yield, and residual SB length measurement. Results: A total of 12 SBS patients (8 females, median age at first enteroscopy, 66 years, interquartile range (IQR) 58–79) were included; among them, 10 received TED therapy (median interval from TED prescription to DBE surveillance 9.1 months (IQR 3.2–32.9)). Overall, 15 DBE procedures (3 retrograde) were performed, all of which were technically successful and completed without adverse events. The overall diagnostic yield was 60% for relevant findings, defined as conditions unknown before DBE. No SB malignancies were detected during the study period. In 80% of cases, a complete visualization of the SB was obtained with a reevaluation of the SB length. The median depth of maximal insertion was 85 cm (IQR 50–187). Conclusion: DBE is feasible, safe, and effective in SBS patients. Total enteroscopy can be frequently achieved with a single approach in these patients, allowing easy diagnostic and oncologic surveillance for SB and colonic malignancy.
  • The Near-Fatal Bath: A Case Report With Literature Review of Diagnosis and Management of Cannabinoid Hyperemesis Syndrome

    Cannabinoid Hyperemesis Syndrome (CHS) is a severe condition associated with chronic cannabis use, characterized by cyclic vomiting, abdominal pain, and compulsive hot baths for symptom relief. We present the case of a 35‐year‐old male with at least a decade‐long history of cannabis use who presented with recurrent episodes of vomiting and dehydration. He experienced a potentially fatal episode of hypothermia and cardiac arrest after losing consciousness during a prolonged hot bath. Despite extensive diagnostic evaluations after many years, CHS was not identified until it was confirmed based on the Rome IV clinical criteria and elevated urinary cannabinoid levels (449 ng/mL). Management included rehydration, parenteral nutrition, and capsaicin cream targeting the transient receptor potential vanilloid Type 1 (TRPV1), as traditional antiemetics were ineffective, in conjunction with cannabis cessation. This case highlights the need for early recognition of CHS to prevent severe complications and avoid unnecessary diagnostic investigations. Prolonged cannabis abstinence remains the only definitive treatment, requiring a multidisciplinary approach. Increased awareness among clinicians is essential for timely diagnosis and appropriate management.

    Background Irritable Bowel Syndrome (IBS) is a common functional gastrointestinal disorder affecting 5%–10% of the global population and is characterized by recurrent abdominal pain and dysregulation of bowel movements. Many patients do not benefit from full remission of symptoms by conventional treatments. Recently, yoga has demonstrated its potential benefits in various medical conditions including IBS, as a result of its effect on the gut‐brain axis. This systematic review evaluates the effectiveness of yoga for the management of IBS. Methods We conducted a comprehensive literature search in PubMed and Scopus until March 2025 to identify studies on yogic posture, pranayama, and meditation in IBS. After excluding reviews, case reports, and studies not meeting age or design criteria, 10 studies were included. These were evaluated for methodological quality and classified as randomized controlled trials (RCTs) or non‐controlled trials. We analyzed outcomes related to gastrointestinal symptoms, quality of life, and psychological symptoms and examined the type of yoga administered. Results Yoga interventions led to amelioration of gastrointestinal symptoms, decreased anxiety and depression, and enhanced quality of life compared to usual care or wait‐list controls. Of note, the studies that fixed the change in the IBS Symptom Severity Scale scores as the primary outcome showed a moderate‐large effect of yoga on the improvement of symptoms compared to controls (Cohen's d range: 0.37–3.60). Conclusion These preliminary findings suggest that yoga could favorably influence both the physical and psychological aspects of IBS. However, high‐quality larger sample studies are needed to confirm the findings reported in the review.
  • The Role of Age at Onset on the Clinical Course and Biochemical Parameters of Anorexia Nervosa

    Background: Anorexia nervosa (AN) has the highest mortality rate among psychiatric disorders, making early diagnosis and tailored management crucial. This study aimed to evaluate the impact of age at onset (AAO)—childhood/adolescence versus adulthood—on the clinical course and associated biochemical parameters. Methods: Seventy-six female patients with AN were divided into two groups based on AAO (<18 years vs. ≥18 years). Group comparisons were performed using t-tests for continuous variables and χ2 tests for categorical variables. Correlation analyses assessed associations between AAO and continuous variables. Significant findings were entered into regression models, including a binary logistic regression with AAO as the dependent variable and a linear regression with significant correlations as predictors. Results: The early-onset group showed significantly higher potassium levels and a lower sodium/potassium ratio (Na+/K+) compared with the late-onset group (potassium: t = 0.93, p < 0.01; Na+/K+: t = 3.39, p < 0.01). AAO was strongly inversely correlated with potassium levels (r = −0.75, p < 0.01) and positively correlated with cholesterol (r = 0.574, p < 0.01) and Na+/K+ (r = 0.78, p = 0.01). Binary logistic regression correctly classified 87% of cases, showing that lower Na+/K+ was associated with earlier onset (OR = 2.23, p = 0.03). Linear regression confirmed significant associations of AAO with cholesterol levels (B = 0.07, p = 0.02) and Na+/K+ (B = 1.68, p < 0.01). Conclusions: AAO in AN is strongly associated with specific biochemical parameters. Early-onset patients exhibit more severe electrolyte imbalances, while late-onset cases show higher cholesterol levels, suggesting increased cardiovascular risk. These findings emphasize the importance of personalized treatment approaches according to AAO, although further studies are warranted to confirm these results.

    Irritable bowel syndrome (IBS) is a gut–brain axis chronic disorder, characterized by recurrent abdominal pain and altered bowel habits in the absence of organic pathology. Nutrition plays a central role in symptom management, yet no single dietary strategy has demonstrated universal effectiveness. This narrative review critically evaluates current nutritional approaches to IBS. The low-Fermentable Oligo-, Di-, Mono-saccharides and Polyols (FODMAP) diet is the most extensively studied and provides short-term symptom relief, but its long-term effects on microbiota diversity remain concerning. The Mediterranean diet, due to its anti-inflammatory and prebiotic properties, offers a sustainable, microbiota-friendly option; however, it has specific limitations in the context of IBS, particularly due to the adverse effects of certain FODMAP-rich foods. A gluten-free diet may benefit individuals with suspected non-celiac gluten sensitivity, although improvements are often attributed to fructan restriction and placebo and nocebo effects. Lactose-free diets are effective in patients with documented lactose intolerance, while a high-soluble-fiber diet is beneficial for constipation-predominant IBS. IgG-based elimination diets are emerging but remain controversial and require further validation. In this review, we present the 10 dietary commandments for IBS, pragmatic and easily retained recommendations. It advocates a personalized, flexible, and multidisciplinary management approach, avoiding rigidity and standardized protocols, with the aim of optimizing adherence, symptom mitigation, and health-related quality of life. Future research should aim to evaluate, in real-world clinical settings, the impact and applicability of the 10 dietary commandments for IBS in terms of symptom improvement and quality of life
  • High Rate of Antibody Response to Multiple Doses of the COVID-19 Vaccine in Liver Transplant Recipients: Analysis of Predictive Factors

    Background: The COVID-19 pandemic highlighted the vulnerability of immunocompromised individuals, including liver transplant recipients (LTRs), who often exhibit reduced vaccine immunogenicity. While initial vaccine doses and subsequent boosters improved immune response, LTRs were prioritized for vaccination. Studies have shown increased antibody response after each booster dose. Vaccine hesitancy, defined as delayed or refused vaccination despite availability, poses a public health challenge, often fueled by misinformation. This study aimed to evaluate anti-spike antibody responses in vaccinated LTRs after two initial doses and at least one booster, also assessing adherence to subsequent doses. Methods: We conducted a retrospective observational study at a transplant center in Milan, Italy, between January 2021 and December 2023. LTRs who had received four or more doses of mRNA vaccines (Pfizer or Moderna) were included. Anti-spike antibody levels were measured 60–80 days after each dose. Data on vaccination status were collected in January 2024. Statistical analysis was performed to compare antibody responses and identify predictive factors. Results: LTRs showed a significant increase in anti-spike antibody responses after the first booster compared to the second dose with a trend versus a further increase following the fourth dose in a subgroup of the patients receiving two booster doses. However, adherence to booster doses decreased over time. In LTRs, predictors of a weaker response after the second dose were chronic kidney disease and metabolic etiology at transplant. Conclusions: The study highlighted that in LTRs, multiple doses of the COVID-19 vaccine led to a continuous increase in anti-spike antibody responses. The progressive decline in adherence of LTRs “to further booster doses” should be related to the fact that after the spread of vaccination programs worldwide, COVID-19 is still a current infection, but it is much less severe than before.
