The Nova system categorizes foods as minimally processed foods (MPFs), processed culinary ingredients (PCIs), processed foods (PFs), and ultra-processed foods (UPFs). High UPF consumption is linked to adverse health outcomes in older adults. Traditional Food Frequency Questionnaires (FFQs) often fail to capture processing differences. This study evaluated the reproducibility and relative validity of a Nova-based FFQ (NFFQ-Elderly) in Italian healthy older adults aged ≥65 years. Methods: A total of 111 older adults (73.7 ± 5.9 years; 56.8% women) completed the NFFQ-Elderly twice (4–6 weeks interval). Relative validity was compared with a three-day weighed food record. Foods were categorized by Nova groups and analyzed for absolute intake, energy and weight percentages. Pearson correlation (r), intraclass correlation coefficients (ICCs), and Bland–Altman plots were used. Results: Reproducibility was satisfactory for MPFs (r = 0.75; ICC = 0.74), UPFs (r = 0.87; ICC = 0.85), and PFs (r ≈ 0.73; ICC ≈ 0.66–0.67). Relative validity was moderate for MPFs (r = 0.57; ICC = 0.53) and UPFs (r = 0.48; ICC ≈ 0.37), but lower for PCIs. Accuracy generally improved when intakes were expressed as percentages of total energy or weight. Bland–Altman analyses showed limited mean bias for MPFs and PFs, but higher variability for PCIs and absolute energy intake. Conclusions: The NFFQ-Elderly appears to be a suitable tool for ranking older adults according to their relative intake of MPFs and UPFs. Estimates for PCIs are less reliable, indicating caution when interpreting absolute intake values.

This study examined the relationship between postdiagnostic UPF consumption and mortality and assessed whether chronic disease biomarkers explain this association. Methods: We longitudinally analyzed 802 men and women with a previous cancer diagnosis (any site) recruited in the Moli-sani Study (2005–2010). Food intake data were collected at baseline and assessed an average of 8.4 years after diagnosis. UPF was defined according to the Nova classification and calculated as a weight ratio. The Mediterranean diet score (MDS) was used to assess overall diet quality. Inflammatory, metabolic, and cardiovascular biomarkers were analyzed as potential explanatory factors on the basis of change-in-estimate. Results: During a median follow-up of 14.6 years, a total of 281 deaths occurred. Multivariable (including the MDS)-adjusted HRs of higher UPF consumption for all-cause and cancer mortality were 1.48 [95% confidence interval (CI), 1.07–2.03 for highest vs. lowest third] and 1.57 (95% CI, 1–2.47), respectively. Taken together, inflammation markers and resting heart rate levels attenuated the associations of UPF with all-cause mortality by approximately 40%. Conclusion: Higher consumption of UPFs after cancer diagnosis was associated with increased all-cause and cancer mortality rates, irrespective of diet quality. Part of these associations was explained by elevated levels of inflammation markers and resting heart rate. Impact: These findings contribute to the growing body of evidence linking UPFs to adverse health outcomes in cancer survivors. Further research is needed to identify specific mechanisms underlying this relationship and explore potential dietary interventions for improving long-term survival.

Its 2025 update refined quantitative targets and emphasised the need for regional adaptation. This study compares the EAT-Lancet reference diets, the FAO/WHO Sustainable Healthy Diets, and Mediterranean dietary models represented by national guidelines from Italy, Spain, Greece, and Portugal, alongside the Italian Mediterranean Diet Pyramid proposed by the Italian Society of Human Nutrition. Quantitative recommendations were harmonised into weekly intakes, and qualitative features were systematically analysed. Overall, strong convergence was observed, particularly for whole grains, fruits, vegetables, and unsaturated fats. However, the EAT-Lancet model recommends higher intakes of legumes and nuts than the Mediterranean guidelines. Differences also emerged for dairy products and olive oil, reflecting regional adaptations. The Mediterranean Diet represents a culturally grounded and feasible model for translating planetary health principles into practice.

Methods Dietary data were assessed using a 188-item FFQ. Participants (n = 22,495) were classified as abstainers, former drinkers, moderate drinkers according to national guidelines (≤250 mL/d men; ≤125 mL/d women) or Mediterranean Diet (MD) (125–500 mL/d men; 62.5–250 mL/d women), and heavy drinkers (>500 mL/d men; >250 mL/d women). Biological age (BA) was estimated with a deep neural network using 36 circulating biomarkers, and Δage (BA–chronological age) served as an index of biological aging. Results In men, wine consumption, at doses defined moderate by a current MD Score, was associated with slower biological aging (Δage β = −0.39; 95%CI: −0.78, −0.01 vs. abstainers). Dose–response analyses showed a J-shaped curve, with the slowest Δage at ∼170 mL/d (Δage = −0.34 years; 95%CI: −0.66, −0.03). Overall ethanol intake, including all alcoholic beverages consumed, was neutral at moderate levels and associated with faster biological aging at higher doses. Conclusion Moderate wine consumption, but not overall ethanol intake, may contribute to slower biological aging in men.
