  • The Epidemiology and Socioeconomic Associates of Pulmonary Aspiration and Foreign Body in the Airway in the Middle East and North Africa Region From 1990 to 2021: A Descriptive Epidemiological Study

    Background and Aims Pulmonary aspiration and foreign body in the airway (PAFBA) is one of the medical emergencies, which can mostly affect children and geriatrics. Reporting the epidemiology of this condition is necessary to plan preventive and controlling measures. Our objective was to investigate the burden of PAFBA in the Middle East and North Africa (MENA) region and its 21 countries from 1990 to 2021, with age and sex patterns. Moreover, the relationship with the socio‐demographic index (SDI) and the PAFBA burden was evaluated. Methods We extracted incidence, prevalence, disability‐adjusted life years (DALYs), and death counts and rates of PAFBA for MENA and countries from the Global Burden of Disease 2021 study. The association with SDI was investigated using the smoothing spline model. The raw frequencies and age‐standardized rates were represented with 95% uncertainty intervals (UIs). Results In 2021, age‐standardized PAFBA rates per 100,000 in the MENA region were as follows: incidence at 15.0 (11.8, 19.1), prevalence at 23.8 (19.7, 28.6), DALYs at 38.5 (23.0, 47.5), and mortality at 0.6 (0.4, 0.8), all showing declines since 1990. Lebanon and Afghanistan reported the highest age‐standardized incidence and prevalence rates, while Afghanistan and Sudan had the highest DALY and mortality rates. The under‐five age group showed the greatest burden in incidence, DALYs, and deaths, with males more affected than females. A negative association with SDI was observed. Conclusions Despite a decline in PAFBA burden over the past 32 years in the MENA region, mortality rates and DALYs remain high. Prioritizing public awareness programs and improving diagnostic and treatment methods are essential to further reduce this burden.
  • Atypical juxtaglomerular cell tumor in a young male with resistant hypertension and normal renin–aldosterone levels

    Juxtaglomerular cell tumor (JGCT), or reninoma, is a rare renin-secreting renal neoplasm that typically presents with severe hypertension, hypokalemia, and elevated renin and aldosterone levels. We describe an atypical case of a 21-year-old male with morbid obesity and resistant hypertension who exhibited normal biochemical findings despite the presence of a renal mass. He underwent partial nephrectomy, resulting in immediate normalization of blood pressure and subsequent regression of left ventricular hypertrophy. This case underscores the importance of considering JGCT in patients with refractory hypertension and a renal mass, even when the characteristic biochemical profile is absent.
  • Impact of Ultrasound-Guided Femoral Nerve Block on Ease of Positioning for Spinal Anesthesia in Patients with Femoral Fracture

    Background: Spinal anesthesia is commonly used for orthopedic surgeries. The most important issue to improve outcome is patient cooperation to have a proper position for reducing pain during spinal anesthesia procedure. However, any change in position is painful which leads to sympathetic activation, tachycardia and hypertension, which may put patients with ischemic heart disease at risk. Peripheral nerve block is a safe and reliable alternative to systemic analgesia. Using the ultrasound-guided method reduces local anesthetics’ amount and side effects.
 Methods: In this study, 53 patients who came to the operating room with femoral fracture as candidates for spinal anesthesia were enrolled and received ultrasound-guided Femoral Nerve Block (FNB) (10 ml Lidocaine 1.5%). Before and after receiving the nerve block, the patients were asked about the severity of their pain using the Numerical Rating Scale (NRS). Both groups of patients and physicians were asked about their satisfaction according to a 5-point Likert scale.
 Results: Mean pain intensity in the NRS score at the time of admission to the operating room was 7.6±0.8, which was reduced to 1.8±1.5 after blockade. In this study, 39 (73.5%) patients were very satisfied (Likert scale 5), 10 (18.8%) were satisfied (Likert scale 4), 2 (3.7%) had no opinion (Likert scale 3), 2 (3.7%) were not satisfied (Likert scale 2), and no one was very dissatisfied (Likert scale 1). There was significant difference in physician’s satisfaction.
 Conclusion: Ultrasound-guided nerve block is a safe and effective method to be used for positioning during spinal anesthesia procedure.
  • The Effect of Propofol Anesthesia on the Pain Severity anFrequency of Migraine Attacks in Patients with Chronic Migraine Headache over a Six Month Follow Up: An Observational Study

    Propofol is a short-acting intravenous anesthetic that is commonly used for induction and maintenance of anesthesia. Subanesthetic low doses of propofol has also been used to treat intractable migraine attacks in emergency wards with dramatic results. However, there is little information on the long-term efficacy of this drug in migraine headaches. The aim of this nonrandomized prospective observational study was to assess the effect of propofol anesthesia on the pain severity and frequency of migraine attacks in a 6-month follow-up period after anesthesia in patients with migraine headaches. The study was conducted on 51 known cases of migraine ranging in age from 21 to 66 years. Before anesthesia, patients completed a questionnaire including their characteristics, pain intensity of the headache using a visual analog scale, and a number of headache repetitions per month. All patients received propofol as the main anesthetic agent. At the end of anesthesia, the total amount of propofol usage was recorded. Patients were then followed up by telephone in the first, third, and sixth months after anesthesia, and the severity and frequency of the headache were recorded. Pain intensity or pain frequency significantly improved in 22 patients (43.1%), remained unchanged in 24 (47%), and worsened in 5 cases (9.8%) 6 months after anesthesia compared to before the anesthesia. In conclusion, since about half of the patients had significant improvement in the headache, propofol anesthesia may be considered as an acceptable anesthetic method in patients with migraine.
  • Comparison of epidural dexmedetomidine versus ropivacaine infusion effectiveness in preventing erection following reconstructive urethral surgery: A randomized clinical trial

    Background: One of the most common devastating problems that occur after urethroplasty is erection, which causes surgical complications (fistula, wound dehiscence, and surgical graft failure) and the need for repairing the complications. We attempted to compare the effect of continuous epidural infusion of dexmedetomidine and ropivacaine as a post-surgical erection prevention strategy. Objectives: In this study, we aimed to compare the effect of dexmedetomidine and ropivacaine epidural infusion on the incidence of erection after reconstructive urethral surgery. Methods: An RCT was conducted on 45 patients who were scheduled for reconstructive urethral surgery. They were randomly divided into three groups: (1) control (n = 15), (2) epidural dexmedetomidine (n = 15), (3) and epidural ropivacaine (n = 15). The control group received oral medication after surgery according to the conventional method (cyproterone compound tablets 50 mg BD and diazepam tablets 2 mg TDS for a week) to prevent erection. The DEX group received dexmedetomidine as continuous epidural infusion, and the ROP group received ropivacaine in addition to the conventional method. The occurrence of erection during day and night was recorded separately until the seventh day after surgery. Due to the long-time interval between case selection, participants from different groups were not matched with each other. Results: The incidence of erection in the dexmedetomidine group was lower than that in the ropivacaine group per person (0.87) and significantly lower than in the control group (2.8 per person). Also, there was significantly less erection in the ropivacaine group (1.2 per person) than in the control group. Our study showed that erection after surgery significantly decreased with the continuous epidural infusion of dexmedetomidine and ropivacaine. Conclusions: Dexmedetomidine seems to have a significant preventive effect on erection after reconstructive urethral surgery.
  • The effect of stretching exercises and caffeine tablets on reducing headache after spinal anesthesia; A randomized clinical trial

    Introduction: We performed this study to compare the effect of Caffeine and stretching exercise on Post Dural Puncture Headache (PDHP). Methods: In this Randomized Clinical Trial, 160 candidates for elective inguinal hernia surgery or varicocele surgery in Shahid Modares Hospital in Tehran, were assigned into the following groups using a computer-generated randomization table: caffeine group received Caffeine tablet (200 mg TDS); exercise group received stretching exercise; caffeine combine exercise group received Caffeine tablet (200 mg TDS ), and stretching exercise and control group received placebo tablets. After the end of the surgery, up to 48 h, the Visual analog scale (VAS) was recorded every 6hours by the same clinician. Results: The occurrence of PDPH was less in the caffeine and exercise combined group than in the control group. Also, the headache was more severe in the control group than in the caffeine group. Furthermore, the need for rescue analgesics was more in the control group than in the caffeine group. Conclusion: caffeine and exercise combined had better outcomes than placebo regarding PDHP.
  • Magnesium gargle versus Ketamine gargle in postoperative sore throat pain; A randomized placebo-controlled clinical trial

    Background: A wide range of approaches have been tested for the prevention and treatment of postoperative sore throat pain (POST pain). This study attempted to compare the effects of gargling with Ketamine or Magnesium Sulfate on POST pain. Materials and Methods: In a randomized clinical trial, 60 patients scheduled for prone position laminectomy were randomly assigned into three groups: Ketamine (n=20), Magnesium (n=20), and Control (n=20). The Magnesium group received magnesium sulfate gargle (30 mg/kg in a total of 30 ml 5% Dextrose water), the Ketamine group received Ketamine gargle (0.5 mg/kg in a total of 30 ml 5% Dextrose water) and the control group received 30 ml 5% Dextrose water gargle; all these solutions were administered 10 min before anesthesia induction. Visual Analog Scale (VAS) for throat pain was recorded in the recovery room; immediately after arrival and then, at 2, 4, and 24 hours postoperatively in the ward. Would there be any VAS equal to or more than 3 of 10, rescue analgesics were administered immediately and their cumulative doses were recorded. Results: The incidence of complaint-free patients in the Ketamine group was significantly higher than in the other two groups. The incidence of sore throat with VAS≥3, mandating rescue analgesia, was significantly lower than the other two groups. patient satisfaction after surgery was significantly higher in the Ketamine group. Conclusion: Patients experiencing POST pain treated with “Ketamine gargle” had better results compared with “Magnesium Sulfate” or “Placebo” gargle.
  • Anesthesia considerations in surgical deep brain stimulation for tourette syndrome management: A case report

    Tourette’s syndrome (TS) is a neuro-behavioral disease associated by phonic and motor tics with a high frequency of psychiatric co-morbidities. For these cases, deep brain stimulation (DBS) is a developing neuro-modulated treatment option since the first report on a successful surgery in 1999. A male thirty-one years old (77 kg, 178 cm) with diagnosis of Gilles De La Tourette syndrome admitted to neurosurgery ward. His medication included Aripiprazole, pimozide, buspirone, clomipramine, citalopram, phenytoin, Desmopressin and Lithium. The patient underwent implanting DBS (Deep Brain Stimulator) surgery and battery implantation in two steps with two weeks interval. General anesthesia with considerations and according to behavior of disease and drug interactions was performed. The cause and symptoms may be due to central dopaminergic hyperactivity or anomalous dopamine neurotransmission and interventions and anesthesia should be done considering these abnormalities.
