  • Intraoperative prevention of postoperative hypoparathyroidism

    ObjectiveMore than 30,000 thyroid surgeries are performed annually in the Russian Federation. The surgeries are relatively safe because of the prevention methods for postoperative complications. Currently, there is no single effective method of postoperative hypoparathyroidism prevention. This complication is frequently reported and may be health and life-threatening.AimWe aimed to estimate the effectiveness of the intraoperative ICG-angiography and intrathyroid injection of Brilliant Green for the prevention of postoperative hypoparathyroidism.Material and methodsOne hundred and forty-three thyroidectomies were performed. Patients were divided into three groups: intraoperative angiography was used in 24 cases; Brilliant Green was injected in 58 cases to identify parathyroid glands; the visual estimation of the parathyroid preservation was used in 61 cases. Calcium level was measured in all patients before and after surgery.ResultsCalcium level in the serum before and after surgery was 2.37±0.14 and 2.27±0.17 in Group 1, 2.38±0.16 and 2.21±0.16 in Group 2, and 2.39±0.17 and 2.18±0.19 in Group 3. Postoperative hypocalcemia was more prominent in the group with the visually estimated PTG than in the two other groups. The differences in postoperative calcium levels in Groups 1 and 3 were statistically different. Pre- and postoperative Parathormone levels were 6.2±0.4 in Group 1, 5.6±0.57 in Group 2, and 3.5±0.32 in Group 3. Postoperative levels significantly differed in Groups 1 and 3 (p<0.01) and in Groups 2 and 3 (p<0.05).ConclusionsICG-angiography and intrathyroid injection of the Brilliant Green are safe methods of identification and sparing of the parathyroid glands. The severity of hypocalcemia and hypoparathormonemia in Group 3 shows the necessity of finding new methods in endocrine surgery to improve patient outcomes.
  • The Role of Carotid Body in the Vital Signs’ Changes after Thyroid Surgery: An Experimental Study

    Aim: to investigate the underlying causes of altered vital signs in an animal model of post-thyroidectomy bleeding. Material and methods. We performed a series of in vivo animal studies simulating post-thyroidectomy hemorrhage and its effect on respiratory and cardiovascular system. Two series of tests were carried out in 20 Chinchilla rabbits under general anesthesia. The rabbits were intubated. The first experimental series involved 10 animals. Postthyroidectomy hemorrhage in this series was simulated by injections of autologous blood (5.0–10.0 ml) under the fascia of cervical neurovascular bundles. The pressure induced by post-thyroidectomy hemorrhage in the second series was simulated by mechanical compressions of the carotid body unilaterally or bilaterally. The minimum compression time reached 1 minute. The maximum compression time was 20 minutes. Blood pressure, heart rate, and respiratory rate were measured in all cases. Results. Mechanical compression of the carotid body resulted in decreased blood pressure, respiratory and heart rate values. Time to complete recovery of respiratory and cardiovascular function depended on the duration of compression. Conclusion. Respiratory and cardiovascular function can be suppressed by compression of the carotid body. Post-thyroidectomy hematomas must be evacuated immediately. Цель исследования: выяснить основные причины расстройств жизненно важных функций у больных кровотечениями в ложе щитовидной железы (ЩЖ) в послеоперационном периоде в эксперименте. Материал и методы. Исследование выполнено на 20 кроликах породы шиншилла. Выполняли моделирование послеоперационной гематомы и оценивали влияние гематомы на дыхательную и сердечно-сосудистую системы. Вмешательства на животных проводили под общей анестезией, все кролики были интубированы. Животные были разделены на 2 группы в зависимости от способа моделирования гематомы. 1-я группа – 10 особей, которым вводили собственную кровь (5,0–10,0 мл) под фасцию сосудисто-нервного пучка, 2-я группа – 10 особей, которым компрессионное воздействие послеоперационной гематомы осуществлялось с помощью наложения зажима на область сонного гломуса с одной и двух сторон. Минимальное время компрессии – 1 минута, максимальное – 20 минут. Оценивали частоту сердечных сокращений, артериальное давление и частоту дыхательных движений во всех наблюдениях. Результаты. Механическое сдавление сонного гломуса приводило к снижению АД, уменьшению частоты сердечных сокращения и дыхательных движений. Время восстановления жизненных функций зависело от длительности компрессии. Заключение. Сдавление синокаротидной рефлексогенной зоны приводит к нарушению витальных функций. Эвакуация гематомы из ложа удаленной ЩЖ должна выполняться незамедлительно.
  • The use of fluorescent angiography with indocyanine green in endocrine surgery - a future technology for identification of the parathyroid glands or a fleeting hobby?

    Рост числа пациентов, страдающих заболеваниями щитовидной железы (ЩЖ), неизбежно приводит к увеличению числа хирургических вмешательств на данном органе. Проблема возникновения послеоперационного гипопаратиреоза (ГПТ) в настоящее время продолжает быть актуальной, несмотря на различные методы идентификации околощитовидных желез (ОЩЖ). В последние годы в различных областях хирургии успешно применяется флуоресцентная ангиография с индоцианином зеленым, позволяющая надежно оценить степень перфузии различных органов и тканей. В настоящей статье рассматривается международный опыт использования вышеуказанной методики в эндокринной хирургии в качестве метода профилактики послеоперационного ГПТ в зависимости от активности интраоперационного кровоснабжения ОЩЖ. Показано, что использование флуоресцентной ангиографии с индоцианином зеленым является надежным методом для идентификации и сохранения ОЩЖ. Таким образом, данная технология может позволить свести к минимуму риск послеоперационного ГПТ. An increase in the number of patients with thyroid gland diseases leads to an increase of cases when thyroid surgery is indicated. Despite of the existence of various methods for identifying the parathyroid glands, the issue of postoperative hypoparathyroidism is relevant until nowadays. Recently ICG fluorescent angiography has been successfully introduced into various fields of surgery. It provides reliable assessment of perfusion in various organs and tissues. This article addresses the international experience in ICG fluorescent angiography in endocrine surgery for postoperative hypoparathyroidism prevention, depending on intraoperative blood flow activity in the parathyroid glands. It is shown that ICG fluorescent angiography is a reliable method of the parathyroid glands identification and preservation. This technology may thereby minimize the risk of postoperative hypoparathyroidism.
